Abstract

Hypertension with comorbid type 2 diabetes mellitus is a chronic condition strongly associated with an increased risk of cardiovascular complications, such as coronary heart disease, stroke, and vascular damage. This condition requires pharmacological therapy that is not only clinically effective but also cost-effective in terms of healthcare services. In clinical practice, statins such as simvastatin and atorvastatin are frequently used as lipid-lowering therapy to help reduce the risk of cardiovascular complications in patients with metabolic diseases. However, differences in treatment costs and clinical outcomes between these two medications make pharmacoeconomic evaluation crucial in determining the most cost-effective therapeutic option. This study aims to analyze the cost-effectiveness of simvastatin and atorvastatin use in hypertensive patients with type 2 diabetes mellitus comorbidity from the perspective of healthcare providers at KHZ Musthafa Regional General Hospital, Tasikmalaya. This study employed a retrospective cross-sectional design based on medical records and patient billing data from January 2024 to December 2025. Using purposive sampling, 57 patients who met the inclusion criteria were included in the study. Cost-effectiveness analysis was performed by calculating direct medical costs, therapeutic effectiveness based on the average length of hospital stay (LOS), the Average Cost-Effectiveness Ratio (ACER), and the Incremental Cost-Effectiveness Ratio (ICER). The results showed that the group of patients receiving simvastatin had lower direct medical costs, namely Rp4,269,569, compared to the atorvastatin group at Rp5,902,031. In addition, the simvastatin group also showed a shorter average length of stay, namely 3.8 days, while the atorvastatin group reached 4.7 days. The ICER for simvastatin was recorded at Rp1,123,570 per day, which is lower than that of atorvastatin at Rp1,255,751 per day. Based on these results, simvastatin is considered more cost-effective than atorvastatin in hypertensive patients with type 2 diabetes mellitus, making it a potentially more efficient therapeutic option for hospitals.

Abstract

Tuberkulosis (TB) hingga saat ini masih menjadi salah satu penyakit infeksi yang memberikan dampak besar terhadap kesehatan masyarakat di berbagai belahan dunia. Selain mengganggu kondisi fisik, penyakit ini juga dapat menurunkan kualitas hidup penderitanya, terutama pada individu yang memiliki kebiasaan merokok. Penelitian ini bertujuan untuk menganalisis hubungan antara intensitas merokok dan kualitas hidup pasien TB paru yang menjalani pengobatan di RSUD Kota Banjar. Penelitian ini menggunakan metode observasional analitik dengan desain potong lintang. Intensitas merokok diukur menggunakan kuesioner Global Adult Tobacco Survey (GATS) dan dihitung berdasarkan Indeks Brinkman, sedangkan kualitas hidup dinilai menggunakan instrumen Functional Assessment of Chronic Illness Therapy–Tuberculosis (FACIT-TB). Analisis data dilakukan menggunakan uji korelasi Spearman. Sebanyak 42 pasien TB paru perokok aktif yang memenuhi kriteria penelitian dilibatkan pada periode Januari–Maret 2026. Hasil penelitian menunjukkan bahwa perokok sedang merupakan kelompok terbanyak, yaitu 19 responden (45%), diikuti perokok berat sebanyak 17 responden (40%). Sebagian besar responden memiliki kualitas hidup kategori buruk sebanyak 19 orang (45%) dan sangat buruk sebanyak 13 orang (31%), terutama pada domain fisik dan emosional. Hasil analisis menunjukkan adanya hubungan yang signifikan antara intensitas merokok dan kualitas hidup pasien TB paru (p=0,001; r=-0,487). Nilai korelasi tersebut menunjukkan bahwa semakin tinggi intensitas merokok, semakin rendah kualitas hidup pasien. Temuan ini menegaskan pentingnya integrasi program berhenti merokok dalam pelayanan TB guna meningkatkan kualitas hidup pasien.Kata kunci: intensitas merokok, kualitas hidup, tuberkulosis paruAssociation of Smoking Intensity on Quality of Life Among Patients with Pulmonary TuberculosisAbstractTuberculosis (TB) remains one of the infectious diseases that poses a major public health burden worldwide. In addition to affecting physical health, the disease can also reduce patients’ quality of life, particularly among individuals with smoking habits. This study aimed to analyze the relationship between smoking intensity and quality of life among pulmonary TB patients receiving treatment at RSUD Kota Banjar. An analytical observational method with a cross-sectional design was employed. Smoking intensity was assessed using the Global Adult Tobacco Survey (GATS) questionnaire and categorized based on the Brinkman Index, while quality of life was evaluated using the Functional Assessment of Chronic Illness Therapy–Tuberculosis (FACIT-TB) instrument. Data were analyzed using Spearman’s correlation test. A total of 42 active-smoking pulmonary TB patients who met the inclusion criteria were enrolled between January and March 2026. The results showed that moderate smokers constituted the largest group, accounting for 19 respondents (45%), followed by heavy smokers with 17 respondents (40%). Most respondents had poor quality of life (19 patients, 45%) and very poor quality of life (13 patients, 31%), particularly in the physical and emotional domains. Statistical analysis revealed a significant relationship between smoking intensity and quality of life among pulmonary TB patients (p=0.001; r=-0.487). The negative correlation indicates that higher smoking intensity is associated with lower quality of life. These findings highlight the importance of integrating smoking cessation programs into TB care to improve patients’ quality of life.Keywords: pulmonary tuberculosis, quality of life, smoking intensity

Abstract

Meningkatnya kasus hiperurisemia yang berkaitan dengan stres oksidatif mendorong pemanfaatan bahan alami yang memiliki aktivitas antioksidan sekaligus antihiperurisemia. Salah satu bahan alam yang berpotensi dikembangkan adalah rumput laut Eucheuma spinosum karena mengandung berbagai senyawa bioaktif yang berkhasiat. Penelitian ini bertujuan untuk mengetahui aktivitas antioksidan dan antihiperurisemia ekstrak etanol Eucheuma spinosum serta menentukan dosis yang paling efektif dalam menurunkan kadar asam urat. Tahapan penelitian meliputi ekstraksi menggunakan etanol 96%, skrining fitokimia, pengujian aktivitas antioksidan dengan metode DPPH, serta pengujian antihiperurisemia pada mencit yang diinduksi kalium oksonat. Hasil skrining fitokimia menunjukkan bahwa ekstrak mengandung alkaloid, flavonoid, tanin, saponin, dan fenol. Uji aktivitas antioksidan menunjukkan nilai IC₅₀ sebesar 52,219 ppm yang termasuk kategori antioksidan kuat. Pada pengujian in vivo, ekstrak terbukti mampu menurunkan kadar asam urat secara signifikan dengan efek yang meningkat seiring kenaikan dosis. Dosis paling efektif diperoleh pada pemberian 120 mg/20g BB mencit dengan persentase penurunan sebesar 32,71%, meskipun aktivitasnya masih berada di bawah allopurinol dengan dosis 100 mg sebagai kontrol positif dengan persentase penurunan sebesar 35,16%. Berdasarkan hasil tersebut, ekstrak Eucheuma spinosum memiliki potensi untuk dikembangkan sebagai bahan alami dalam membantu terapi hiperurisemia. Namun demikian, penelitian lanjutan masih diperlukan untuk meningkatkan efektivitas ekstrak dan mengkaji mekanisme kerjanya secara lebih mendalam.Kata kunci: antioksidan, antihiperurisemia, DPPH, Eucheuma spinosum, kalium oksonat, stres oksidatif Evaluation of the Antioxidant Activity of Eucheuma spinosum Extract by the DPPH Assay and Its Antihyperuricemic Activity in Male White MiceAbstractThe rising prevalence of hyperuricemia associated with oxidative stress has driven the exploration of natural substances with antioxidant and antihyperuricemic properties. Eucheuma spinosum seaweed is recognized for its bioactive compounds that may serve as potential natural therapeutic agents. This study aimed to evaluate the antioxidant and antihyperuricemic activities of the ethanol extract of Eucheuma spinosum and to identify the most effective dose in reducing uric acid levels. The study procedures included extraction using 96% ethanol, phytochemical screening, antioxidant activity assessment using the DPPH method, and antihyperuricemic testing in potassium oxonate-induced mice. Phytochemical analysis indicated the presence of alkaloids, flavonoids, tannins, saponins, and phenolic compounds in the extract. The antioxidant assay showed that the extract possessed strong antioxidant activity with an IC₅₀ value of 52.219 ppm. In the in vivo experiment, the extract significantly decreased uric acid levels in a dose-dependent manner. The most effective dose was observed at 120 mg/20 g of body weight in mice, resulting in a 32.71% reduction, although its activity was still lower than that of allopurinol at a dose of 100 mg, which served as the positive control and resulted in a 35.16% reduction. These findings suggest that Eucheuma spinosum extract has promising potential as a natural agent for hyperuricemia management. Nevertheless, further studies are required to optimize its effectiveness and to elucidate its mechanisms of action more comprehensively.Keywords: antioxidant, antihyperuricemic, DPPH, Eucheuma spinosum, oxidative stress, potassium oxonate
