Abstract

Introduction: COVID-19 disease has become a comprehensive world issue and has been declared a significant threat to global health. Interleukin-6 (IL-6) is an important inflammatory marker and one of the triggers of the cytokine storm in COVID-19, where increased levels can be an independent predictor of COVID-19 mortality. This study aimed to observe the effect of upper arm and breathing exercises on IL-6 levels in severe COVID-19 patients. Methods: The study design was quasi-experimental, with blood tests conducted before and after the examination. A total of 20 patients with confirmed COVID-19 were involved, divided into intervention and control groups. Results: Blood tests to determine baseline IL-6 levels were performed in all patients. Patients from the intervention group were given upper arm and breathing exercises for ten days, twice a day, via video tutorials on mobile phones, while patients from the control group did not receive any exercises. Patients from the intervention group obtained mean pre- and post-exercise IL-6 levels of 42.38 ± 48.48 and 16.78 ± 18.29, respectively (p = 0.005). Conclusion: Upper arm and breathing exercises showed significant changes in IL-6 levels in severe COVID-19 patients.

Abstract

Background: Stroke is the third most common disease after heart disease and cancer and the main cause of disability worldwide. The most common type of stroke, which accounts for around 85%-87% of cases, is ischemic stroke (IS). This report is to examine the benefits of citicoline for IS patients. Case: A 63-year-old male was admitted with a diagnosis of IS and electrolyte disturbances. General treatment is semi-Fowler’s position, oxygen, and maintaining fluid and nutritional intake. Specific management is RL infusion 20 gtt/minute, candesartan 1x16 mg, clopidogrel 1x75 mg, omeprazole 40 mg/24 h intravenous (IV), and citicoline 500 mg/12 h IV. The patient showed clinical improvement. Method: Articles in PubMed, Cochrane, and Google Scholar in February-April 2022 were searched to find relevant studies to be evaluated with inclusion and exclusion criteria. The full text was obtained and critically reviewed with the evidence-based medicine guidelines. The results are limited to systematic reviews and meta-analyses. Results: Five studies were deemed valid, important, and applicable. However, the quality of these studies varies. Conclusion: All studies show that citicoline use could potentially improve clinical outcomes in IS patients.

Abstract

Background: Choline-containing phospholipids, citicoline, and choline alphoscerate have been proposed as adjuvants in acute stroke treatment. An evidence-based case report (EBCR) was conducted to examine the benefits of citicoline in hemorrhagic stroke (SH) patients. Method: Search for articles published from October 2019-2023 from PubMed, Cochrane, Google Scholar, and ProQuest sources using the keywords “Citicoline”, “Hemorrhagic Stroke”, “Choline”, and “Intracerebral Hemorrhage”. The search results were evaluated using inclusion and exclusion criteria. Selected full-text studies were evaluated. The final selection results were critically reviewed based on validity, importance, and applicability by the three authors. Results: One study was considered valid, important, and applicable. The quality of this study is very high based on the literature review. Conclusion: The study shows that citicoline has the potential as an adjuvant therapy to improve clinical outcomes in SH patients, although it does not sufficiently improve neurological or functional outcomes.

Abstract

ABSTRACT Background: Indonesia ranked 2nd around the world for TB cases with case population continue increasing. In 2018, Shorter Treatment Regimen (STR) was introduced as a new regimen for treating MDR-TB patient. The aim of this study is to determine factors associated with treatment outcome of MDR-TB patients treated with Shorter Treatment Regimen Methods: This is a descriptive-analytic study using cross sectional design which was conducted at Adam Malik Hospital Medan. Subject was 150 patients with drug-resistant Pulmonary TB at MDR-TB polyclinics according to inclusion and exclusion criteria Results: MDR-TB patients that treated with STR was mostly < 50 years old (65.2%) and 107 subjects (70.4%) were male. The majority of subjects with comorbidity were 94 subjects (61.8%); 43 subjects (28.3%) with DM, 5 subjects (3.3%) with CHF, 3 subjects (2.0%) with HIV, 1 subject (0.7%) with DM & CHF, and 1 subject (0.7%) with DM & HIV. When evaluated the patients treatment outcome, 47.4% were cured, 6.6% were failed, 34.8% were default and 11.2% were death. A chi square test was conducted to assess association between age with treatment outcome. Age was significantly associated with treatment outcome (p=0.038) but gender and comorbidity were not associated with treatment outcome with p-value 0.152 and 0.497 (p>0,05) respectively. Conclusions: There is a significant association between age and treatment outcome but no significant association between gender and comorbidity with treatment outcome. ABSTRAK Latar Belakang: Indonesia menduduki peringkat ke-2 dunia untuk kasus TBC dengan populasi kasus yang terus meningkat. Pada tahun 2018, Shorter Treatment Regimen (STR) diperkenalkan sebagai rejimen baru untuk mengobati pasien TB-MDR. Tujuan dari penelitian ini adalah untuk mengetahui faktor-faktor yang berkorelasi dengan hasil pengobatan pasien TB-MDR yang diobati dengan Regimen STR. Metode: Penelitian ini merupakan penelitian deskriptif-analitik dengan desain cross-sectional yang dilakukan di Rumah Sakit Adam Malik Medan. Subyek penelitian adalah 150 pasien TB Paru Resisten Obat di poliklinik TB MDR sesuai kriteria inklusi dan eksklusi. Hasil: Pasien TB-MDR yang diobati dengan STR sebagian besar berusia <50 tahun (65,2%) dan berjenis kelamin laki-laki sebanyak 107 subjek (70,4%). Mayoritas subjek dengan penyakit penyerta sebanyak 94 orang (61,8%); 43 orang (28,3%) DM, 5 orang (3,3%) CHF, 3 orang (2,0%) HIV, 1 orang (0,7%) DM & CHF, dan 1 orang (0,7%) DM & HIV. Hasil pengobatan adalah 47,4% sembuh, 6,6% gagal, 34,8% putus berobat dan 11,2% meninggal. Uji chi square dilakukan untuk menilai hubungan antara usia dengan hasil pengobatan dan didapatkan nilai p = 0,038 yang menunjukkan adanya hubungan yang signifikan namun berdasarkan jenis kelamin dan penyakit penyerta ditemukan nilai p masing-masing 0,152 dan 0,497 (p>0,05) yang menunjukkan tidak ada hubungan antara jenis kelamin dan penyakit penyerta terhadap hasil pengobatan. Kesimpulan: Terdapat hubungan yang signifikan antara usia dan hasil pengobatan namun tidak ada hubungan yang signifikan antara jenis kelamin dan penyakit penyerta dengan hasil pengobatan.
