Abstract

Introduction: Spirituality plays an important role in the physical health of patients with chronic diseases. This study aimed to explore the experiences of nurses in providing spiritual care to patients with heart failure (HF) in Indonesia. Method: This study employed a descriptive exploratory design with a qualitative approach. Thirteen nurses from the cardiology unit were selected using purposive sampling. Data were collected through semi-structured interviews and field notes. Thematic analysis was conducted to analyze the data. Results: Five major themes emerged, including perceptions regarding spiritual care, the importance of meeting the spiritual needs of patients, implementation of spiritual care, obstacles in providing spiritual care, and nurses’ hopes. Most participants associated spiritual care with religious practices. All participants believed that the spiritual needs of patients with HF must be met. However, spiritual care was still rarely implemented. Nurses commonly assisted patients in performing religious practices rather than addressing existential needs. The barriers to providing spiritual care originated from patients, nurses, and the availability of tools. The nurses hoped that specific personnel could provide spiritual care and that spiritual guidance activities in the hospital could be reactivated. Conclusions: Spiritual care remains a neglected aspect in the treatment of patients with HF. Facilitating religious practices is the primary intervention that fulfills the spiritual needs of patients with HF in Indonesia.

Abstract

Hypertension is a chronic non-communicable disease which, if not treated properly, will cause cardiovascular complications which are the cause of disability and death in communities throughout the world. Patient behavior that does not follow hypertension management guidelines is a major factor in the risk of cardiovascular complications in hypertensive patients in the community. The aim of this research is to determine the effectiveness of a health behavior module based on transcultural nursing in preventing cardiovascular complications and its impact on hypertensive patients in the community. The research design was a quasi experiment with a pre-post-test control group design. The sample size was 100 hypertension patients divided into 50 treatment groups and 50 control groups. The research sample was taken using a purposive sampling technique based on predetermined inclusion criteria. The research data were analyzed descriptively, homogeneity test using Levene Statistics, Wilcoxon pre-post difference test for each group, simultaneous pre-post difference test for both groups with Mann-Whitney, and correlation test with Spearman Rho. Results of the test for equality of respondent characteristics p-value = > 0.05, Wilcoxon test in the treatment group p-value = 0.000, control group p-value = > 0.05, Mann Whitney test pre test p-value > 0.05, post test < 0.05, and the Spearman rho test obtained a p-value < 0.05. Conclusion: The behavioral module intervention to prevent cardiovascular complications based on transcultural nursing has proven to be effective in increasing efforts to prevent cardiovascular complications in hypertensive patients, and efforts to prevent cardiovascular complications based on transcultural nursing have a significant relationship in reducing systolic, diastolic blood pressure and cholesterol levels in hypertensive patients in the community. It is hoped that this module can become a health promotion media that can be used to increase efforts to prevent cardiovascular complications in hypertensive patients in the community

Abstract

The bacterium Mycobacterium leprae causes leprosy, which causes peripheral neuropathy and prolonged disability that impacts a person's psychology, causing fear and anxiety. The aim of this review study was to find variables that impact on individuals' mental health and social stigma. Method: A systematic review searched for previous studies using cross-sectional or non-experimental designs published between 2019 and 2024 in five electronic databases (Scopus, Wos, Science Direct, and ProQuest). To transfer this view, the Center for Research and Dissemination and the Joanna Briggs Institute Guidelines were used to assess quality and the Prisma checklist. To assess research eligibility, titles, abstracts, full texts, and methodologies were evaluated. Data tabulation and narrative analysis of research results were conducted. Results: We found that seven journals met the requirements for inclusion in this observation. Any research on psychology and stigma addresses psychological impact and factors related to social stigma. Psychological impacts include knowledge, gender, culture, disability, economy, education, and lack of support systems, while stigma includes societal culture and the disability of leprosy. Conclusion: The results of this systematic review found many things that are risk factors for psychological disorders and stigma in leprosy patients including knowledge, gender, culture economy, education and lack of support system, disability which causes leprosy sufferers to experience discrimination, self-isolation, and misperceptions from society. Further research to understand this phenomenon is needed to reduce the global psychological impact and negative social stigma.

Abstract

Introduction: Adolescents living in orphanages experience a lack of self-confidence, withdrawal, lack of interest in learning, lack of communication, and a closed attitude so that it will have a negative impact on cognitive, emotional, and social development. This study aims to determine the effect of Therapeutic Group Therapy (TKT) on the development of adolescent self-identity in orphanages. Methods : The study design was a Quasy experimental pre-post test control group design. The population was 104 children living in orphanages A and B. The research sample was 30 intervention groups and 32 control groups that met the inclusion criteria obtained through purposive sampling. The independent variable is Therapeutic Group Therapy while the dependent variable is self-identity. Data obtained using a questionnaire and analyzed using the Wilcoxon Sign Ranks Test and Mann-Whitney U Test with a significance level âˆ” <0.05. Results : The results of the analysis of the intervention group showed the effect of TKT on self-identity (p = 0.0001). The results of the Mann-Whitney U Statistical Test of self-identity test showed a value of p = 0,0001 ( âˆ” <0.05) meaning that there was an effect of TKT on self-identity between the two groups. Conclusion : TKT is done by health workers can help identify identity status teenagers and can improve self-identity teenager who lived in the orphanage through health education.

Abstract

A growing body of literature sheds light on internet based interventions and digital technology use in mental illness. The purpose of this study is to provide an overview of internet based  interventions and digital technology use  in patients with schizophrenia or mental illness  and their documented outcomes of the medication adherence and relapse prevention. Methods: Databases searched for this systhematic review were Scopus, CINAHL, Science Direct, Sage, and Pubmed at the time of publication between 2016 and 2021 which was limited by journal source type. The search terms, truncated when relevant, were ("schizophrenia" OR psychosis OR "mental illness") AND (“internet based intervention” OR “online intervention” OR “mobile health” OR telemedicine OR telehealth”) AND (“medication adherence” OR “medication compliance”). Eleven peer reviewed intervention studies targeting adults with schizophrenia or another mental illness were reviewed and analyzed. The Joanna Briggs Institute Guideline was used for critical appraisal. Results: There are eight categories of interventions emerged: telephone intervention, video conference, mobile texting, texting combined telephone, smartphone intervention, texting combined smartphone intervention, web based intervention, and WeChat based intervention. Conclusions: Most of  the previous literatures stated that there were significant effects of  internet based intervention and digital technology use for improving  medical adherence of patients with schizophrenia or another mental illness.

Abstract

Introduction: People with schizophrenia need the role of the family as a caregiver. Objectives: The aim of this study was to describe the burden of the family as a caregiver for schizophrenia patients in an outpatient polyclinic setting, Menur Mental Health Hospital Surabaya. Methods: Total participants were 160 family caregivers who carry out routine check-ups for families with schizophrenia. The data was obtained using the Indonesian version of the Zarit Burden Interview (ZBI) questionnaire and Cronbach alpha (α) was 0.931. Descriptive statistics were used to describe and categorize the family caregiver burden. Results: The results showed that the majority of participants had burden score in the category no burden – low burdens: 64 participants (40.0%), the mild-moderate burden category was 84 participants (52.5%), the moderate-severe burden was 11 participants (6.9%), very severe burden category was only 1 participant (0.6%). In addition, the demographic data showed that most of the participants were adult (55%), and the majority of caregivers are female (54.4%), the majority of education was senior high school (51.9%), for and most of the participants were unemployed (45.6%). Conclusion: This study was needed to provide evidence for evolving intervention among family caregivers.

Abstract

Keluarga pasien gangguan jiwa kerap menghadapi beban psikologis yang berat, stres, serta kesiapan rendah dalam proses perawatan dan rehabilitasi anggota keluarga. Manajemen layanan rumah sakit yang komprehensif, termasuk pendekatan psikoterapi seperti Acceptance and Commitment Therapy (ACT), berpotensi meningkatkan kesiapan keluarga dalam mendukung pasien. Systematic review ini bertujuan menganalisis bukti ilmiah terkait penerapan ACT dalam konteks manajemen layanan rumah sakit untuk meningkatkan kesiapan keluarga pasien gangguan jiwa. Tinjauan dilakukan berdasarkan pedoman PRISMA dengan pencarian artikel pada PubMed, ScienceDirect, ProQuest, dan Google Scholar periode 2013–2023. Kata kunci yang digunakan meliputi: “Acceptance and Commitment Therapy”, “family readiness”, “mental health patients”, “hospital service management”, dan “psychiatric disorders”. Studi yang diinklusikan adalah penelitian kualitatif, kuantitatif, maupun campuran yang mengevaluasi efektivitas ACT pada keluarga pasien gangguan jiwa di rumah sakit. Dari 1.248 artikel yang diidentifikasi, 6 memenuhi kriteria inklusi. Temuan menunjukkan ACT meningkatkan kesiapan keluarga melalui penerimaan kondisi pasien, pengurangan stres, penguatan komitmen perawatan, serta peningkatan fleksibilitas psikologis. Integrasi ACT dalam protokol rumah sakit melalui edukasi keluarga, pelatihan tenaga kesehatan, dan pendampingan psikologis memperbaiki kualitas layanan serta dukungan sistemik. ACT terbukti efektif meningkatkan kesiapan keluarga pasien gangguan jiwa. Rekomendasi meliputi pelatihan staf, pengembangan modul ACT berbasis keluarga, dan integrasi dalam alur layanan rumah sakit psikiatri

Abstract

Latar Belakang: Peningkatan kasus gangguan jiwa dan tingginya angka kekambuhan ODGJ (orang dengan gangguan jiwa), serta kurangnya rehabilitasi memadai dan kompetensi perawat, menghambat kemandirian ODGJ belum mencapai kemandirian ekonomi menjadi permasalahan serius di Kota Surabaya Tujuan: Program ini bertujuan meningkatkan kualitas hidup dan kemandirian ekonomi ODGJ, serta memperkuat kapasitas perawat dalam pelayanan jiwa komprehensif. Metode: Pengabdian masyarakat dilakukan melalui edukasi dan pendampingan di dua lingkup: intervensi langsung pada pasien di Bengkel Sehat Mandiri Surabaya dan peningkatan kompetensi perawat di 63 Puskesmas Surabaya Hasil: Pasien menunjukkan adanya peningkatan keterampilan motivasi dan kemandirian serta peningkatan keterampilan produktif, partisipasi aktif kegiatan rehabilitasi tercermin dalam pembuatan frozen food, bantal sofa, dan sabun cuci piring, serta pelatihan kewirausahaan dan pemanfaatan e-commerce. Sementara itu, kompetensi perawat dalam pemulihan psikososial dan pencegahan relaps ODGJ di Puskesmas menunjukkan peningkatan pemahaman mengenai pemulihan psikososial dan pencegahan kekambuhan setelah diberikan edukasi. Kesimpulan: Program pengabdian ini berhasil memberikan dampak positif terhadap pemulihan penderita gangguan jiwa serta memperkuat kapasitas pelayanan kesehatan jiwa di tingkat komunitas secara holistik dan berkelanjutan serta intervensi holistik ini berhasil memberdayakan ODGJ secara ekonomi dan meningkatkan kapasitas tenaga kesehatan, berkontribusi signifikan pada peningkatan kualitas hidup ODGJ dan pelayanan jiwa masyarakat.

Abstract

Depression is a prevalent concern among the elderly population, potentially leading to suicidal ideation and behavior. Consequently, proactive measures are imperative to mitigate depression's impact. Community cadres, who offer voluntary assistance to healthcare professionals, play a crucial role in the early detection of depression. This study aims to evaluate the efficacy of a training program designed for cadres in identifying depression among elderly individuals residing in rural areas. This study utilized purposive random sampling to employ a pre-experimental design with a single-group pre-post intervention. 89 participants meeting the inclusion criteria—being mental health cadres and having completed comprehensive training—were randomly assigned. The intervention involved training participants in recognizing depression symptoms in the elderly. Data was collected using a personal information form and a questionnaire assessing the ability to detect early signs of depression in the elderly, administered before and after the intervention. Statistical analyses were conducted using SPSS-22, employing paired t-tests and Chi-square tests. The findings indicate a significant improvement in the cadres' ability to identify depression among rural elderly individuals following the intervention (p = 0.005). This study underscores the effectiveness of training programs in enhancing cadres' proficiency in recognizing depression among elderly rural residents. Consequently, we advocate for integrating regular mental health training sessions for community cadres within public health initiatives.

Abstract

Lansia dengan gangguan kognitif di menghadapi penurunan kualitas hidup akibat penurunan fungsi memori, perubahan perilaku, dan ketergantungan dalam aktivitas sehari-hari. Kondisi ini berdampak pada meningkatnya beban fisik, emosional, dan sosial bagi keluarga sebagai caregiver. Kurangnya pengetahuan mengenai perawatan lansia, stigma sosial, serta minimnya dukungan emosional dan akses informasi kesehatan menyebabkan caregiver mengalami isolasi sosial dan stres. Pengabdian kepada masyarakat ini bertujuan memberdayakan keluarga dan komunitas melalui pembentukan support group sebagai solusi untuk meningkatkan pengetahuan, keterampilan merawat, dan dukungan psikososial bagi caregiver. Metode pelaksanaan meliputi sosialisasi, edukasi gangguan kognitif, pelatihan perawatan lansia dan strategi koping, pembentukan support group, serta evaluasi melalui pre-test dan post-test, observasi, dan kuesioner. Hasil kegiatan menunjukkan adanya peningkatan pengetahuan caregiver, keterampilan perawatan, dan penguatan jejaring sosial, ditunjukkan dengan peningkatan skor pengetahuan dan kepuasan peserta terhadap dukungan emosional. Program ini berkontribusi pada peningkatan kualitas hidup lansia dan caregiver serta memperkuat kapasitas kader posyandu dalam keberlanjutan program. Temuan pengabdian ini menegaskan pentingnya intervensi berbasis komunitas sebagai strategi efektif mengatasi tantangan perawatan lansia dengan gangguan kognitif.
