CFR with arate of more than or equal to 50% indicates a high share ofdeaths. Jember District Health Office implemented a screeningprogram for childbearing women’s TT status in early 2010.However, not until the end of 2010 there were already 6 casesand 3 deaths due to TN (CFR = 50%). Thescreening implementation for childbearing women’s TT statusby the respondents had not been in accordance with the twooperational procedures. The results of the screening programshowed that five health centers were still experiencing thesame problem, i.e., not identified TT status of all women andunmet target coverage of T5 childbearing women and T2 pluspregnant women. Conclusion: Technically, some obstacles in the implementationof the screening program are still present; therefore, there is aneed for conducting training procedures for the personnel ofthe screening program for childbearing women’s TT statusand conducting an evaluation for the program periodically andcontinuously. Keywords: evaluation, tetanus toxoid, childbearing women

Methods: This research is a descriptive analytic research using secondary data from the report of HIV/AIDS program in Jember Regency at 2012. Collected data is analyzed descriptively with table and narration. Result: The impacts of the Regency Decree on closing the transition social service place for the sex worker in Puger, Jember district include: 1) The prostitution closure and the transition social service place closure lead to increased and widespread of illegal sex worker in Jember district, 2) Health coaching and behavior change communication to the sex worker become harder to conduct, and 3) the number of HIV/ AIDS cases each year might be higher. In 2004, the number of HIV/AIDS cases is 1 and it increased annually until in 2012 when the cumulative amount cases of HIV/AIDS in Jember district reach 822. According to the data, the recorded case according to the highest risk factor is sexually transmitted through heterosexual transmission which is 685 cases (83.3 %). This suggests that the transmission occurred from the sex worker to the consumer and also from the consumer to the sex worker. The transmission to the housewives would also be increased, followed by the mother-to-baby transmission. Conclusion: Jember district Government needs to conduct an evaluation review of this closure regulation (the Regency Decree Number 188.45/39/012/2007).

Abstract

Increased HIV prevalence in gay populations is a warning that needs to get government attention. AIDS prevention programs in gay populations have been implementing various methods to change high-risk behavior. However, HIV prevalence in gay populations continues to rise. The Indonesian Constitution affirms human rights, including the right to life, the right against discrimination, and other fundamental rights protected by the state. The research results showed that the fulfillment of the right to health in gay is not going well or less effective. It was evidenced by the percentage of gay that reaches out to health services was still limited. Gays were reluctant to access health services because of the lack of confidentiality and privacy of the services of health workers, the general public, and the limited facilities. Besides, stigma and discrimination are still often received both from health workers and families and communities. They worried other gays would know the result of the test of VCT. There is a need for standardization of services at all subdistrict health centers and hospitals providing VCT services for gay in Jember.
