Abstract

Background: Congestive heart failure is a progressive disease that causes a decreased quality of life, increases the need for medical expenses, and has a long treatment time. The purpose of this study is to analyze the cost of treatment and see the utility index in patients with congestive heart failure at RSUD Dr. Moewardi Surakarta. Method: The research method used a cross-sectional design, data were collected retrospectively from medical records, financial data, and pharmaceutical installations to see data on direct medical costs and utility data using the EQ-5D-5L questionnaire during March-May. The subjects of the study were 78 outpatient heart failure patients at RSUD Dr. Moewardi Surakarta with inclusion criteria, namely heart failure patients aged > 40 years and complete medical record data including age, sex, comorbidities, degree of severity, and drug therapy. Results: Data were analyzed using the Man Whitney U test and the Kruskal Walis H test. The results of this study showed direct medical costs, namely pharmacy (Rp. 2,837,328.00), service fees (Rp. 150,000.00), medical procedures (Rp. 89,717.95), and administrative fees (Rp. 10,000.00). Total direct medical costs Rp. 3,087,045.95. The results of the EQ-5D-5L questionnaire based on each dimension were 78.2% no problems with walking ability, 97.4% no problems with self-care, 59.0% no problems with activities, 34.6% little had problems with pain, and 57.7% had no problems with anxiety/depression. The utility index value is 0.803. Conclusion: The conclusion is that the average direct medical cost is Rp. 3,087,045.95 and the utility index value is 0.803.

Abstract

Kejadian diabetes melitus di masyarakat Indonesia semakin meningkat, khususnya pada kelompok usia dewasa akibat gaya hidup dan pola konsumsi yang tidak sehat. Penggunaan herbal lokal sebagai alternatif terapi menjadi perhatian, salah satunya Kunyit Hitam (Curcuma caesia) yang potensial sebagai antidiabetes. Namun, pemanfaatan kunyit hitam di Dusun Belembem belum optimal karena minimnya pengetahuan dan keterampilan dalam pengolahan. Kegiatan pengabdian ini bertujuan untuk meningkatkan pemahaman ibu-ibu RT 02 Belembem dalam memanfaatkan kunyit hitam sebagai herbal antidiabetes melalui pemaparan materi berbasis presentasi dan diskusi terbuka untuk mendorong literasi kesehatan yang mandiri. Hasil pelaksanaan menunjukkan tingginya minat peserta terhadap pemanfaatan kunyit hitam, dengan mayoritas mengaku baru mengetahui manfaatnya setelah edukasi. Kegiatan ini berdampak signifikan dalam meningkatkan pengetahuan dan kesiapan ibu-ibu dalam menjaga kesehatan keluarga menggunakan tanaman obat keluarga. Model pemberdayaan komunitas terbukti efektif dalam memperkuat pemanfaatan kearifan lokal untuk kesehatan keluarga serta mendukung ekonomi rumah tangga.

Abstract

Komplikasi hipertensi sering terjadi salah satunya pada DM tipe 2. Dalam mencapai penurunan tekanan darah diperlukan terapi obat kombinasi dengan menggunakan ACEI-CCB dan ARB-CCB. Terapi obat kombinasi efektif dalam mengontrol tekanan darah, namun mampu membuat beban biaya pengobatan meningkat. Tujuan penelitian ini memiliki tujuan yakni mencari tahu gambaran obat kombinasi ACEI-CCB dan ARB-CCB pada pasien hipertensi dengan DM tipe 2, dalam rangka mengetahui keefektifan biaya, serta untuk mengetahui rencana perbaikan masalah cost-effectiveness pada lingkup rumah sakit. Penelitian berjenis observasional dengan pengambilan datanya dilakukan dengan cara retrospektif. Biaya yang dianalisis yaitu mencakup biaya terapi obat, biaya penunjang, biaya obat tambahan, dan biaya laboratorium, kemudian melakukan rencana perbaikan untuk cost-effectivenes dengan menggunakan metode analisis SWOT. Didapatkan sampel penelitian ini sebanyak 90 pasien, pengguna kelompok terapi ACEI-CCB sebanyak 38 pasien dan kelompok terapi ARB-CCB sebanyak 52 pasien. Hasil penelitian analisis rata-rata biaya pengobatan kombinasi antihipertensi pasien komplikasi DM Tipe 2 didapatkan pada kombinasi ACEI-CCB sebesar Rp. 635.663, sedangkan pada kombinasi ARB-CCB sebesar Rp. 570.314. Biaya penggunaan obat kombinasi yang paling efektif adalah kombinasi ACEI-CCB dengan nilai efektivitas 44,74% dan dengan nilai ACER sebesar Rp. 14.209 dan ICER yaitu Rp. 7.970,96. Hasil analisis SWOT dengan matriks IE ini berada pada sel I sehingga dapat diindikasikan rumah sakit yang peneliti jadikan lokasi penelitian ada pada posisi grow and build atau tumbuh dan membangun, sehingga strategi yang dapat diterapkan yaitu meliputi penetrasi, pengembangan dan integrase.

Abstract

Penyakit jantung iskemik (IHD) adalah suatu kondisi medis jantung tidak mendapatkan oksigen dan nutrisi yang cukup karena adanya penyempitan atau sumbatan pada arteri koroner. Efek vasodilatasi pada IHD dapat membantu mengatasi beberapa komplikasi yang timbul dari penyakit ini. Buah okra (Abelmoschus esculentus) merupakan tanaman yang berpotensi memberikan efek vasodilatasi karena mengandung kuersetin. Tujuan penelitian ini adalah menguji aktivitas ekstrak buah okra (Abelmoschus esculentus) dan dosis efektifnya dalam memberikan efek vasodilatasi. Buah okra diekstraksi dengan metode maserasi menggunakan pelarut etanol 70%. Penelitian ini menggunakan hewan uji tikus putih (Rattus novergicus) jantan galur Sprague Dawley dengan variasi tiga dosis ekstrak buah okra yaitu dosis 75 mg/kg bb, 150 mg/kg bb, dan 300 mg/kg bb. Digunakan kontrol positif Isosorbide mononitrat dan kontrol negatif CMC 0,5%. Aktifitas vasodilatasi ekstrak buah okra diamati melalui parameter kadar nitrit oxide. Data hasil pengamatan dari masing-masing parameter dianalisis dengan menggunakan one way ANOVA. Hasil penelitian menunjukan bahwa ekstrak etanol buah okra (Abelmoschus esculentus) dapat memberikan aktivitas efek vasodilatasi terhadap tikus putih (Rattus novergicus) jantan galur Sprague Dawley. Ekstrak buah okra yang memiliki dosis paling efektif adalah dosis 75 mg/kg bb tikus.

Abstract

Kegiatan pengabdian masyarakat bertema “Pendampingan Pelatihan Pembuatan Teh Celup Kunyit Hitam (Curcuma caesia) sebagai Pencegahan Penyakit Diabetes Melitus” bertujuan meningkatkan pengetahuan dan keterampilan warga RT 02, Dusun Blembem, Plesungan, Karanganyar, Jawa Tengah, dalam memanfaatkan kunyit hitam untuk menurunkan kadar glukosa darah. Dilaksanakan pada 30 Mei 2025 pukul 08.00–10.00 WIB di rumah ketua RT, kegiatan ini melibatkan 30 ibu-ibu PKK. Persiapan meliputi perizinan kepala desa, survei lokasi, dan sosialisasi. Metode demonstrasi dan contoh digunakan untuk penyuluhan manfaat kunyit hitam sebagai antidiabetik, antiinflamasi, dan antioksidan, serta praktik pembuatan teh celup. Peserta antusias mengikuti sesi, praktik, dan mencoba seduhan teh. Kegiatan berlangsung lancar, meningkatkan pemahaman tentang obat tradisional dan keterampilan membuat produk herbal sederhana, berpotensi sebagai solusi kesehatan dan produk lokal.

Abstract

Introduction: Flavonoid compounds and vitamin C from noni fruit have the potential to be antioxidants. The unpleasant odor, soft texture, and unpleasant taste of noni fruit are minimized through a fractionation process and formulated into a gel preparation, which is expected to increase the acceptance of noni fruit in the community. Objective: to determine the optimal gel formula of the most active fraction of noni fruit with variations of HPMC, carbopol 940, and propylene glycol through critical parameters of viscosity, pH, and antioxidant activity using the Simplex Lattice Design method. Method: The extraction used is maceration and continued with a fractionation process. Physical properties tests include organoleptic, homogeneity, viscosity, adhesiveness, spreadability, and pH. Determination of the IC50 value of antioxidants with DPPH using a UV-Vis spectrophotometer. The statistical results of the data obtained were processed using ANOVA, Wilcoxon, and the T-test. Results: showed that the noni fruit fraction that had the best antioxidant activity was the ethyl acetate fraction (20.35±0.18 ppm), variations of 13 gel formula compositions with the most optimal viscosity, pH, and IC50 responses were carbopol 940:HPMC 8060-M: propylene glycol (1.491:0.509:8.000). Variations in the combination of HPMC 8060-M, carbopol 940, and propylene glycol in the gel preparation of the most active noni fruit fraction affected the critical parameters of viscosity, pH, and antioxidant activity. Conclusion: The ethyl acetate fraction has the best antioxidant activity and the optimal gel formula composition is carbopol 940:HPMC 8060-M:propylene glycol (1.491:0.509:8.000) which is predicted to have a very high desirability value (0.917) and has been validated.

Abstract

Hypertension prevalence in Indonesia reached 34.4%. Chronic Disease Management Program or Program Pengelolaan Penyakit Kronis (PROLANIS) was one of the preventive and promotive strategies from BPJS, carried out to reduce or prevent complications of chronic diseases, including hypertension. Aim of this study was to determine education influence using leaflet media on the level of knowledge, compliance and therapy outcomes in prolanis hypertension patients at X Clinic, Surakarta. Method of this study used a quasi-experimental with pre-test and post-test design with 40 patients in each control and treatment group. Knowledge level assessed using HK-LS questionnaire, compliance assessed using MARS questionnaire, and therapy outcomes collected from medical record. The results of this study showed that education using leaflet media influenced level of knowledge with a value of p = 0.014, level of taking medication adherence with a value of p = 0.015 and results of systolic blood pressure therapy with a value of p = 0.000. There were an influence of education using leaflet media on the level of knowledge, compliance and therapy coutcome for prolanis hypertension patients at X Clinic, Surakarta.

Abstract

Public Health Center (PHC) was a health center that needed and demanded by the community, accordingly availability and quality of medicines at PHC should met the needs. Purpose of this study were to determine the level of conformity and specify selection and procurement strategies for pharmaceutical service standards in Kediri's PHC based on priority scale of problem using Hanlon analysis. Design of this study was a non-experimental study, with data were analyzed quantitative and qualitative, collected using questionnaire instrument by conducting interviews, while results of this study was data in the form of counting results from respondent’s answers. The research results indicate that the procurement compliance level at the Public Health Centers in Kediri City has one problematic indicator, namely the percentage of procurement compliance with the actual usage for each drug item. Meanwhile, three indicators that are in compliance include the frequency of procurement for each drug item, the frequency of invoice errors, and the frequency of delayed payments by Puskesmas in relation to the established timelines.

Abstract

Quality of pharmaceutical services can be measured by the performance of the Blitar General Hospital Pharmacy Installation which focuses on customer growth rates. This study aims to determine the growth rate of outpatient and inpatient customers of the Blitar General Hospital Pharmacy Installation from 2021 to 2023. This research uses a qualitative descriptive approach. Data were collected through in-depth interviews, observations, and document studies, which were then analyzed using a qualitative approach. The results showed that the total customer growth rate increased over time; in 2021, there were 12,786 customers; in 2022, there were 44,833 customers; while in 2023, there were 58,850 customers

Abstract

Medication management is an important factor in ensuring the level of drug availability. The aim of this research is to analyze and provide strategies for improving drug management in the Pharmaceutical Installation of Dr. Oen Kandang Sapi Hospital Surakarta using the Hanlon method. This research uses a descriptive design, with data taken retrospectively and concurrently. Data was obtained from the selection, procurement, distribution, and use stages. In the selection stage, the results obtained were drug suitability with the National Formulary (65.14%) and suitability with the Hospital Formulary (75.07%); Procurement stage: frequency of procurement per year (96 times), frequency of delayed payments from hospitals (0 time). In the distribution stage, the percentage of expired date medicines damaged (0.15%), the accuracy of the data on the number of medicines on the stock card was (95.78%), and the percentage of dead stock (3.61%). Stage of use: number of items per prescription sheet (3.18 sheets), percentage of antibiotic drug prescriptions (8.53%), percentage of injection drug prescriptions (29.86%), the average time to serve prescriptions (non-concocted 8.88 minutes and concocted 17.52 minutes), the percentage of drugs that can be submitted (99.95%), percentage of drugs that are fully labeled (100%). Percentage of prescriptions with generic names (49.26%). The results of analysis using the Hanlon method show that drug management in the Pharmacy Installation of Dr. Oen Kandang Sapi Hospital Surakarta is quite good but needs to be improved continuously.
