Abstract

Improving service quality is a primary priority in hospital management. Service quality can be improved by reducing the percentage of non-value added activities through the Lean Hospital approach. This study aims to identify activities and analyze the root causes of critical waste in the drug distribution and utilization processes at the Outpatient Pharmacy Installation of Karanganyar Regency Hospital. This study is a non-experimental research with a qualitative descriptive design. Critical waste was obtained through the distribution of a waste weighting questionnaire assessed by all personnel involved in the drug distribution and utilization processes. The results show that the Value Stream Mapping calculation for the drug distribution process obtained a lead time of 147.41 minutes and a VAR value of 36%, while in the drug utilization process, compounded prescription service obtained a lead time of 128.53 minutes and a VAR of 24%, and non-compounded prescription service obtained a lead time of 75.8 minutes and a VAR of 26%. The critical waste questionnaire calculation using the Borda method in the drug distribution process showed overproduction 60%, inventory 53.33%, and waiting 43.33%, while in the drug utilization process, waiting 43.75%, overproduction 42.85%, and defect 39.70%. The 5S method, a Lean method, was used to eliminate waste in the service process at the Outpatient Pharmacy Installation of Karanganyar Regency Hospital.

Abstract

Antioxidants are substances needed by the body to neutralize free radicals and prevent damage caused by free radicals. Moringa leaves contain flavanoids, alkaloids, saponins and tannins that are antioxidants because of their ability to capture free radicals. This study aims to determine the effect of moringa leaf fractions on spatial memory ability using the Radial Arm Maze method, a decrease in malondialdehyde levels and inhibition of damage to the number of hippocampal pyramidal cells in CA1 & CA3 areas. Moringa leaves were extracted using the maceration method with 70% ethanol solvent. Then the ethanol extract of Moringa leaves was fractionated using the liquid-liquid method with n-hexane, ethyl acetate and water as solvents. Next, the mice were subjected to induction treatment with lead acetate, then observed for 21 days with the Radial Arm Maze, then the mice underwent surgery and their brains were removed for MDA level testing and histopathological observation. Data were analyzed statistically using one-way ANOVA (p<0.05) followed by Tukey's test. Based on the results obtained, the ethyl acetate fraction, n-hexane fraction and water fraction can influence the improvement of spatial memory by reducing latency time and type B errors, reducing malondialdehyde levels with TBARS and inhibiting damage to CA1 and CA3 hippocampal pyramidal cells which are characterized by pknotic neuron nuclei (shriveled), dark, solid, and vacuolized.

Abstract

Tanaman benalu mangga memiliki kandungan senyawa seperti alkaloid, flavonoid, terpenoid, dan  polifenol yang mana kandungan tersebut berpotensi sebagai antiinflamasi, antioksidan, antikanker, dan immunomodulator. Penelitian ini bertujuan untuk mengetahui aktivitas pada indeks fagositosis dari ekstrak etanol daun benalu mangga pada mencit yang diinduksi karbon. Penelitian ini menggunakan 25 ekor mencit jantan yang dikelompokkan menjadi 5 kelompok yaitu kelompok kontrol negatif (CMC Na), kelompok kontrol positif (methylprednisolon) dengan dosis 1,04 mg/kg BB mencit, dan kelompok perlakuan ekstrak dosis 420 mg/kg BB, 840 mg/kg BB, dan 1.680 mg/kg BB. Mencit diberi perlakuan selama 7 hari peroral dan pada hari ke-8 diinduksi karbon secara intravena melalui ekor mencit. Metode yang digunakan adalah metode bersihan karbon dengan parameter indeks fagositosis mencit dan persentase indeks organ limpa. Hasil penelitian menunjukkan bahwa ekstrak etanol daun benalu mangga memiliki aktivitas penurunan pada indeks fagositosis dan persentase indeks organ limfa. Hal ini menunjukkan bahwa ekstrak etanol daun benalu mangga memiliki aktivitas dengan penurunan indeks dan persentase indeks organ limfa dengan dosis efektif 840 mg/kgBB yang sebanding dengan kontrol positif.

Abstract

Latar Belakang: Pengendalian persediaan obat yang tidak optimal menyebabkan kekosongan atau kelebihan stok di rumah sakit, sehingga diperlukan sistem analisis yang komprehensif untuk meningkatkan efisiensi biaya dan mutu pelayanan. Tujuan: Menganalisis pengendalian persediaan obat menggunakan metode ABC-VEN, EOQ, ROP, ITOR dan merumuskan strategi perbaikan melalui analisis SWOT di RSUD dr.Soehadi Prijonegoro Sragen tahun 2021. Metode: Penelitian deskriptif dengan data kualitatif dan kuantitatif yang dikumpulkan secara retrospektif periode Januari-Desember 2021. Hasil: Analisis ABC-VEN mengidentifikasi 68 item kategori AE bernilai Rp6.095.836.400 yang memerlukan prioritas pengendalian ketat. EOQ menunjukkan jumlah pemesanan optimum tertinggi 113.332 unit dengan ROP bervariasi hingga 1.271 unit. Nilai ITOR sebesar 4,34 kali/tahun menunjukkan perputaran obat masih di bawah standar ideal 8-12 kali. Matriks IFE (3,04) dan EFE (2,81) menempatkan rumah sakit pada kuadran IV dengan strategi pengembangan. Simpulan: Implementasi metode EOQ-ROP pada kategori AE berpotensi menghemat 25% biaya persediaan. Saran: Diperlukan integrasi sistem informasi manajemen, monitoring ITOR bulanan, dan pelatihan berkala staf farmasi
