Abstract

Excessive secretion of MMP-9 is one of the cause of COPD is which can cause undesirable lung tissue degradation. Increased expression of MMP-9 is associated with increased gene promoter activity. The purpose of this study was to determine the role of mettaloproteinase-9 matrix gene polymorphisms with COPD events. This was a case-control study. With a total sample of 60 patients consisting of 30 COPD patients and 30 non-COPD patients. Samples were amplified by PCR (Polymerase Chain Reaction) The results of DNA amplification were digested with SphI restriction enzyme to see the allele variant of MMP-9 C-1562T. Visualization of restriction results with 2% agarose gel electrophoresis will show 2 bands on the T allele (188bp, 247bp), 1 band on the C allele (435bp). The results obtained were homozygous CC variants in COPD 20 (66.67%) and non-COPD 21 (70%). Heterozygous variant of CT 10 (33.33%) in COPD and 9 (30%) in non-COPD. TT variant is not found at all. Based on the results obtained by the most CC homozygous variants in COPD (66.67%) and non-COPD (70%) and no association was found between MMP-9 polymorphism and COPD events (p> 0.05). Based on the results of this study, there is no significant relationship that we can conclude.

Abstract

Background: Military soldiers are army personnel who are required to have good physical endurance and optimal fitness, include. So, it's needed physical exercise, include to be able to improve lung function. One parameter that can reflect physical fitness are lung function and m VO2max. This study aims to determine whether there is a relationship between physical exercise in the form of 2-mile jogging with increased lung function and VO2max of military soldiers Kodam 1 Bukit Barisan Methods: This research is an experimental study carried out in February-April 2019 against the TNI soldiers of the Bukit Barisan Military District 1. The research subjects were divided into two groups, group 1 who underwent regular 2-mile jogging exercises 3-5 times a week with a duration of 25 minutes for 2 months, and group 2 who underwent non-routine jogging exercises (less than 3 times a week). Lung function was assessed by spirometry. Results: As many as 68 military soldiers were involved in this study which were divided into two groups, 38 people in the group 1 who routinely trained and 30 people in group 2 that did not routinely practice. Group 1 VEP1 values were higher than group 2 (p=0.03). No significant differences in terms of KVP, MVV and VO2max in both groups. Lung function of smokers were lower than non-smokers, but not statistically significant. Conclusion: There were significant differences in VEP1 values between military soldiers who routinely and did not routinely undergo 2-mile jogging exercises.

Abstract

Background: Based on the 2019 Global Initiative for Chronic Obstructive Lung Disease (GOLD), treatment for group B stable COPD patients is a long-acting beta-2 agonist (LABA) or long-acting anticholinergic (LAMA). Some studies experienced that LAMA was better than LABA but the opposite was also found in several studies. COPD patients often experience a decline in physical activity which causes a decrease in quality of life. We aimed to compare the effect of LABA or LAMA on quality of life in group B stable COPD patients.Methods: This was a study with case series design conducted on 50 COPD patients divided into two groups. The first group consisted of patients who used LABA for at least 3 months. The second group used LAMA for at least 3 months. All subjects filled in the SGRQ. The calculation of SGRQ was carried out using Microsoft Excel Calculator SGRQ.Results: A total 41 men and 9 women were enrolled in this study. There was no significant difference between the use of indacaterol monotherapy and tiotropium monotherapy on the quality of life although tiotropium showed a better quality of life (76%) than indacaterol monotherapy (64%).Conclusion: There was no significant difference in the use of indacaterol and tiotropium on the quality of life although in this case tiotropium showed better results (P=0.538).

Abstract

Coronavirus Disease 2019 (COVID-19) is concerning health issue that the world, particularly Indonesia, deals with. The number of COVID-19 cases in Indonesia is growing. The role of the family in providing care to the COVID-19 patient with mild and moderate symptoms during home quarantine should be given a serious concern to prevent virus transmission to other family members. This study aims to into the family’s experience in Medan while caring for a family member infected with COVID-19 during a home quarantine. The purposive sampling technique is employed to select the 10 participants for this qualitative with phenomenological design research. And indepth interview is conducted as a data collection method, and then the data are analyzed using the Collaizzi method. The findings address six issues that the family experiences while providing health care to the COVID-19 infected family member during home quarantine : family burdens, family healthcare, support, the impact on families of COVID-19 patients, family barriers and family expectation. The family caring for the COVID-19 patient experiences several challenges and seeks support from other family members to obtain information for the patients recovery.

Abstract

Drug-resistant TB threatens global TB care and prevention. It remains a major public health problem in many countries. Nurses are front line personnel who have contact with patients, and because of their role as other medical care providers, in the prevention and monitoring of adverse drug reactions caused by Anti-TB Drugs to improve the quality of care. The aim of the study was to explore in depth the experiences of MDR TB patients taking anti-TB drugs. This type of research is qualitative with a descriptive phenomenological approach. There were 10 participants, selected by purposive sampling technique. Methods of data collection with in-depth interviews. The data were analyzed by the Colaizzi analysis method. The themes obtained were 7 themes, namely the psychological response of MDR TB patients, lifestyle of MDR TB patients undergoing treatment, side effects of anti-TB drugs that occur in MDR TB patients, efforts to overcome side effects, expectations of MDR TB patients, perceptions of MDR TB patients in consuming drugs. Anti-TB, and the support needed by MDR TB patients while taking Anti-TB drugs. Based on the results of the study, it is recommended that nurses provide nursing education related to the side effects of anti-TB drugs, treatment planning and information about things that may occur during treatment need to be emphasized so that patients and families can prepare physically and mentally to complete treatment.

Abstract

This systematic review aims to assess public perceptions, knowledge and attitudes in controlling transmission in the Covid-19 era. The method used is a systematic review by searching articles from the Proquest, Pubmed and EBSCO databases selected using the PRISMA diagram. The research results show that public perception of the Covid-19 pandemic is positive, public knowledge of Covid-19 and public attitudes in controlling the risk of transmission are good. Some people have followed all recommendations such as following lockdown guidelines and taking precautions such as washing hands frequently, using masks when leaving the house, maintaining distance and doing work from home. In conclusion, public perception of Covid-19 is positive, and public knowledge about Covid-19 and public attitudes in controlling the risk of transmission are good. Abstract

Malnutrisi mempunyai pengaruh negatif terhadap struktur, elastisitas, dan fungsi paru serta kekuatan, ketahanan otot pernafasan, dan akan berdampak pada penurunan kapasitas fungsional serta meningkatkan risiko mortalitas. Pemberian latihan ketahanan otot dan pemberian nutrisi pada pasien Penyakit Paru Obstruktif Kronis (PPOK) dapat meminimalisasi gejala, dan meningkatkan aktivitas sehari-hari. Studi klinis ini menggunakan desain quasi-experimental dengan satu kelompok pre-test post-test yang melibatkan 11 pasien PPOK stabil. Latihan selama 30 menit dilakukan empat kali seminggu selama satu bulan dengan tambahan asupan tujuh butir putih telur per latihan dalam diet. Responden mendapatkan instruksi latihan pada satu sesi di rumah sakit, setelah itu mereka harus melakukannya secara mandiri di rumah dan dipantau dengan telepon dan WhatsApp (WA) grup. Sebelum dan setelah penelitian selesai, massa otot kedua kelompok diukur menggunakan Carada Scan. Setelah diberikan latihan ketahanan ekstremitas atas, pada kelompok ini menunjukkan adanya peningkatan massa otot yang signifikan dari baseline 27,99±6,58 menjadi 30,0±7,23. Setelah menjalani program rehabilitasi paru, terdapat peningkatan massa otot yaitu menjadi 30,0±7,23. Kesimpulannya setelah dilakukan latihan ketahanan dan penambahan nutrisi putih telur dilaporkan bahwa terdapat manfaat yang signifikan terhadap pasien PPOK dengan bukti terdapatnya peningkatan massa otot. Abstract

Pasien PPOK yang mengalami atrofi dan disfungsi otot akan mengurangi pengambilan oksigen maksimal (VO2 maks) dalam jaringan. Berat badan dan penipisan massa bebas lemak (fat free mass) adalah masalah umum pada pasien PPOK dan terkait dengan kelemahan otot dan intoleransi latihan, kedua hal ini akan menyebabkan penurunan aktivitas dan kualitas hidup. Olahraga (latihan fisik) pada pasien PPOK memberikan efek yang baik terhadap toleransi latihan dan kualitas hidup. Tujuan dari penelitian ini adalah untuk menilai peran olahraga terhadap VO2 maks dan fat free mass pada pasien PPOK. Desain penelitian ini adalah eksperimental, dilakukan pada 20 pasien PPOK. Pasien melakukan latihan tubuh bagian atas dan bawah dengan latihan otot pernapasan dan sepeda statis, dua kali seminggu selama 4 minggu, mulai dari 5-20 menit/sesi. Pengukuran VO2 maks dihitung sebelum dan sesudah intervensi menggunakan Nury’s Formula® yang dirancang khusus untuk orang Indonesia (r = 0,68; p < 0,005), dengan mengkonversi jarak berjalan 6 menit. Fat free mass diukur dengan BIA (Bio Impendances analysis) pre dan pasca intervensi dan indeks massa bebas lemak dihitung sebagai FFM kg/m2. Analisis statistik dilakukan oleh T Test. Terdapat peningkatan VO2 maks diperoleh dengan tes berjalan 6 menit, dari rerata 9.24 menjadi 9.44 tetapi tidak signifikan secara statistik (p = 0,066). Pada FFM ada peningkatan tetapi tidak signifikan secara statistik pada indeks massa bebas lemak dari rerata 18.37 menjadi 18.44 (p < 0,689) setelah intervensi. Kejadian eksaserbasi tidak ditemukan ketika intervensi dilakukan. Latihan tubuh dapat meningkatkan VO2 maks dan fat free mass tetapi tidak signifikan secara statistik.Kata Kunci: Latihan tubuh, Formula Nury®, PPOK, VO2 maks, FFM

Abstract

This research aims to examine the relationship between self-efficacy and treatment compliance with the asthma control test in asthma patients. This type of research is quantitative using descriptive correlational methods. The number of samples in this study that met the inclusion criteria was 44 patients at the North Sumatra University Hospital, Medan, with the sampling technique used was consecutive sampling. The instruments used in this research were self-efficacy instruments, medication adherence instruments namely MMAS-8 and Asthma control test measurements by (ACT) by the American Lung Association (ALA) questionnaire. Data analysis in this research consists of Univariate analysis and Bivariate analysis using the Chi-Square statistical test. The research results showed that self-efficacy and the Asthma Control Test (ACT) obtained a value of p = 0.01 < 0.05 and the value of treatment compliance with the Asthma Control Test (ACT) obtained a value of p = 0.051 < 0.05. In conclusion, there is no significant relationship between self-efficacy and treatment compliance with the Asthma Control Test (ACT) in asthma patients. Abstract

This study aims to explore in depth the experiences of the people of South Aceh related to health behavior during the COVID-19 pandemic. The research method uses a descriptive phenomenological approach with an in-depth interview process, making interview transcripts and then analyzing the data using the Collaizi analysis method. The study results showed four themes: community efforts to prevent COVID-19, psychological disorders, activities carried out during the COVID-19 pandemic, and difficulties in implementing COVID-19 prevention efforts. The conclusion is that most of the community's behavior is positive, and the community has implemented prevention efforts well, even though some religious and traditional events have been carried out during the COVID-19 pandemic.
