Abstract

The number of cases of COVID-19 in various countries has led to soaring needs in the health care system and has resulted in overcrowding of hospitals, so that when the wider community has to stay at home to take care of themselves, the opposite happens to health workers who work, they still have to serving COVID-19 patients. This study aims to analyze the preparedness model carried out by the Medan Marelan Hospital in the face of the COVID-19 spike. This study used a cross sectional study design. The population in this study were all employees of the Royal Prima Marelan Hospital Medan with a total of 113 people, the sample was taken using quota sampling, where at the Royal Prima Marelan Hospital there were as many as 60 people, the data analysis used Univariate, Bivariate and Multivariate. The independent variables that have an effect in this study are the attitude of preparedness variable, management planning and management evaluation, and the variables that have no effect in this study are the knowledge of Covid-19 variable, management implementation, and management follow-up. The independent variable in this study that has the most influence on hospital preparedness in dealing with the COVID-19 spike is the management evaluation variable.

Abstract

Tuberculosis as a disease caused by Mycobacterium tuberculosis. Tuberculosis can attack the lungs and can attack all parts of the body. Just as Pneumonia is a respiratory infection that most often causes death in infants and toddlers. The purpose of this study was to evaluate the cost of Pulmonary TB and Pneumonia therapy for the period July 2020 to July 2021 Based on INA-CBGs at RSU Royal Prima Medan. This type of research is descriptive with a qualitative approach. The sample in this study were informants consisting of officers in the INA-CBGs tariff coding section, pulmonary specialists, administrative management staff/staff and patients who were treating pulmonary tuberculosis and pneumonia. The determination of informants was carried out by purposive sampling. Analysis of research data using source triangulation, method triangulation and time triangulation. The results of the study show that the direct costs of patients for pulmonary TB therapy are based on the INA-CBGs rates, these costs are distributed into several costs such as registration fees, consultation fees, laboratory fees, drug costs and X-ray fees. Indirect costs incurred for pulmonary TB therapy patients and indirect costs incurred for pneumonia therapy patients who undergo therapy at RSU Royal Prima Medan. This fee is the sum of transportation costs, food costs, losses due to leave, patient care costs, and drug costs that are not available at the hospital. The duration of therapy for pulmonary TB patients at RSU Royal Prima Medan was in the majority > 5 days in 23 (65.7%) patients, and 1-5 days in 12 (34.3%) patients. The majority of Pneumonia patients at RSU Royal Prima Medan spent 1-5 days in 24 (68.6%) patients, and > 5 days in 11 (31.4%) patients.

Abstract

Community-based noncommunicable disease (NCD) surveillance data are increasingly utilized as secondary sources in public health research. Variations in dataset structure, variable nomenclature, data types, and coding systems across surveillance periods frequently impede data integration and subsequent analyses. This study developed a harmonized community-based NCD surveillance dataset using secondary data collected at UPTD Puskesmas Johar Baru, Jakarta, Indonesia, from 2019 to 2021. The methodology included dataset inventory, variable mapping, standardization of variable names, data types, and coding systems, data cleaning, derivation of analytical variables, and dataset integration using Python with the Pandas and NumPy libraries. Three surveillance datasets, comprising 196,949 observations, were successfully integrated. The harmonization process identified 158 unique variables, which were standardized into 17 analytical variables while preserving all observations. The resulting dataset offers a consistent and interoperable structure suitable for epidemiological studies, health program evaluation, and other public health research. Furthermore, the harmonization framework established in this study may serve as a practical reference for standardizing multi-period health surveillance datasets and promoting the broader use of secondary health data in future public health research

Abstract

Carpal Tunnel Syndrome (CTS) is a neuropathy caused by compression of the median nerve within the carpal tunnel; it is frequently experienced by workers engaged in repetitive activities, including computer users. This condition is classified as a Musculoskeletal Disorder (MSD) that can reduce workers' productivity and quality of life. This study aimed to analyze the relationship between risk factors—specifically age, gender, BMI, work duration, and work posture—and the incidence of CTS among UNPRI employees. The theoretical framework was based on concepts of occupational ergonomics, individual and occupational risk factors, and CDC BMI classifications. A cross-sectional analytical design was employed, involving 55 respondents selected via purposive sampling. Primary data were collected using validated questionnaires, BMI measurements, and work posture assessments, while secondary data were obtained from institutional records. Data analysis was performed using the Chi-Square test via SPSS. The results showed a CTS prevalence of 9.1%. Statistical tests indicated no significant association between CTS complaints and age (p=0.643), gender (p=0.327), BMI (p=0.526), work duration (p=1.000), or work posture (p=0.622). It is concluded that none of the five studied risk factors showed a significant association with the incidence of CTS; this may be attributed to the predominance of a younger age group and other factors not examined in this study.

Abstract

Emergency Department Length of Stay (EDLOS) is an important indicator of emergency department service efficiency and patient flow. Prolonged EDLOS may reflect delays in clinical assessment, diagnostic processes, and treatment decisions. This study aimed to analyze factors associated with EDLOS efficiency among non-trauma patients at the Emergency Department of RSU Royal Prima Medan. An analytic observational study with a retrospective approach was conducted using secondary data from medical records of 334 non-trauma patients treated in April 2026. Data were analyzed using univariate, bivariate, and multivariate analyses with binary logistic regression using the enter method. The results showed that 171 patients (51.2%) had efficient EDLOS (≤4 hours), while 163 patients (48.8%) had inefficient EDLOS (>4 hours). Bivariate analysis demonstrated significant associations between arrival time (p=0.049), triage level (p=0.045), diagnostic tests (p=0.032), and Turnaround Time (TAT) (p=0.045) with EDLOS. Age, sex, and disposition status were not significantly associated with EDLOS. Multivariate analysis identified TAT as the only significant factor, with inefficient TAT increasing the odds of inefficient EDLOS by 3.705 times (aOR=3.705; 95% CI: 1.044–13.146; p=0.043). Improving diagnostic TAT should therefore be prioritized to enhance emergency department efficiency and reduce prolonged EDLOS.
