Abstract

Background: Osteoarthritis (OA) is a degenerative disease that often causes pain, disability and limited mobility, especially in the elderly. Total Knee Replacement (TKR) is one of the preferred surgical interventions in severe OA to reduce pain and improve joint function. Objective: This case report aims to evaluate nursing care in a 76-year-old female patient with OA genu grade III postoperative TKR sinistra accompanied by acute pain, impaired physical mobility, and risk of infection. Methods: This study is a case report using interview techniques, observation, physical examination, and documentation study. Nursing management was carried out based on the Indonesian Nursing Diagnosis Standards (SDKI), Indonesian Nursing Outcome Standards (SLKI), and Indonesian Nursing Intervention Standards (SIKI). Prioritized nursing diagnoses include acute pain, impaired physical mobility, and risk of infection. Results: After 3x24 hours of intervention, there was a decrease in the patient's pain scale from 5 to 3 (0-10 scale). Left lower limb muscle strength increased, with active range of motion. The surgical wound remained dry with no sign of infection, and the patient was able to sit on the edge of the bed with minimal assistance. Conclusion: The application of nursing care based on SDKI, SLKI, and SIKI showed significant results in improving the patient's condition. The Evidence-Based Nursing (EBN) approach contributed greatly to the recovery of patients after TKR surgery. This intervention can be a reference for nurses in designing nursing care in similar cases.

Abstract

Avascular Necrosis (ANV) is a pathological condition caused by impaired blood supply to the subchondral femur bone, causing necrosis and deformity of bone tissue. This condition causes severe pain, joint stiffness, and impaired physical mobility. Chronic avascular necrosis requires surgery to stabilize physical function. Total Hip Replacement surgery is one of the surgical options to relieve pain and restore joint function in patients with avascular necrosis. Nursing care for ANV patients is very important in managing pain, preventing complications, and promoting recovery. Through comprehensive nursing care, nurses have a significant impact on the physical and emotional health of patients, helping them live healthier and more functional lives. This article aims to implement nursing care for avascular necrosis patients undergoing surgery at Haji Adam Malik General Hospital, Medan. This study is a case report of a 63-year-old patient who had suffered from avascular necrosis for 10 years and underwent Total Hip Replacement surgery. Assessment data were collected through physical examination and supplemented with laboratory and radiology results. The postoperative diagnoses that are the priority in this patient are acute pain, impaired physical mobility, impaired skin integrity, and self-care deficit. Nurses focus on the criteria for treatment outcomes in reducing pain levels, increasing joint range of motion and physical mobility, improving skin integrity, and improving self-care. The intervention results are pain control that allows earlier mobilization and can be a reference for professional nurses in improving the quality of service, especially for avascular necrosis patients undergoing Total Hip Replacement (THR) surgery.

Abstract

Percutaneous Coronary Intervention (PCI) is one of the most effective actions for patients with Acute Coronary Syndrome (ACS), which can improve prognosis and minimize hospitalization time. To optimize medical therapy programs, PCI must be complemented with further preventive measures, such as cardiovascular rehabilitation, including lifestyle modification and risk stratification. This study aims to analyze the relationship between phase I cardiovascular rehabilitation, risk stratification, and length of hospitalization in post-PCI patients. The study design was a retrospective cohort sourced from electronic medical record data. Respondents were divided into groups: those exposed and those not exposed to phase I cardiovascular rehabilitation. Risk stratification and length of hospitalization were studied as study outcomes. Among 201 patients who were followed up during hospitalization, high-risk stratification was more in the unexposed group: exposed group (92.2%: 90.8%; RR 0.85, 95%CI 0.340-2.104). Length of hospitalization ≥3 days was more in the unexposed group: exposed group (63.1%: 59.2%; RR 0.904 95%CI (0.639-1.279). Length of hospitalization ≥3 days was more in the high stratification group: low stratification (63.6%: 35.3%; RR 0.555 95%CI (0.289-1.066). Phase I cardiovascular rehabilitation was not associated with risk stratification and length of hospitalization. Patient risk stratification was related to the length of hospitalization of post-PCI patients. Phase I cardiovascular rehabilitation is an integral part of medical therapy that can reduce adverse events in post-PCI patients.

Abstract

Fracture is one of the problems in health care centers around the world. The condition of the tissue around the fracture will experience damage to its integrity so that it can cause pain, trauma, joint stiffness, and musculoskeletal disorders. Pain is a sensory or emotional experience related to tissue damage. Sharp and stabbing pain is felt due to bone infection due to muscle spasms or pressure on sensory nerves. Non-pharmacological therapies include cold compresses that can reduce pain, reduce inflammatory responses, and reduce blood flow and reduce edema. This therapy is more effectiveand easy to do independently by patients. Cold pack for compresses are also useful for reducing muscle tension and reducing swelling so that patients are more comfortable and relaxed. This study aims to assess the difference in the duration of cold compresses using cold packs on reducing the intensity of acute pain in fracture patients at the Haji Adam Malik Central General Hospital. The research method is Quasi-Experimental with a pretest, posttest group design approach. Sampling using the consecutive sampling technique so that 66 respondents were obtained. The research instrument used the Numeric Rating Scale. The data were analyzed using the Wilcoxon and Mann Whitney U tests. The results showed that the mean value for the 20 minute compress group was 23.85, while the mean value for the 30 minute compress group was 43.15 with a p value <0.05. It can be concluded that giving a cold compress for 30 minutes is much more effective than giving a cold compress for 20 minutes. It is hoped that nurses in providing nursing care can apply a cold compress for 30 minutes using a cold pack as an independent nursing action in managing fracture pain in the hospital.
