Abstract

Abstract

Abstract Acute postoperative pain following long bone fractures is a common clinical problem that may hinder the healing process and delay early patient mobilization. This study aimed to analyze the effectiveness of Progressive Muscle Relaxation (PMR) techniques in reducing the intensity of acute postoperative pain in patients with long bone fractures. A randomized controlled trial with a pretest–posttest control group design was employed. The sample consisted of 72 respondents, who were divided into an intervention group (NSAIDs + PMR) and a control group (NSAIDs only). Univariate analysis showed that the mean pain intensity in the intervention group decreased from 68.2 ± 8.5 mm before the intervention to 32.4 ± 7.6 mm after the intervention, while in the control group, mean pain intensity decreased from 67.9 ± 8.1 mm to 48.6 ± 8.9 mm. Bivariate analysis using the paired t-test demonstrated a statistically significant reduction in pain intensity before and after treatment in both the intervention group (p = 0.000) and the control group (p = 0.001). Furthermore, the independent t-test revealed a significant difference in post-intervention pain intensity between the intervention and control groups (p = 0.000). These findings indicate that Progressive Muscle Relaxation is effective as a complementary therapy in reducing acute postoperative pain following long bone fractures and is recommended for integration into postoperative nursing care. Abstract

Avascular Necrosis (ANV) is a pathological condition caused by impaired blood supply to the subchondral femur bone, causing necrosis and deformity of bone tissue. This condition causes severe pain, joint stiffness, and impaired physical mobility. Chronic avascular necrosis requires surgery to stabilize physical function. Total Hip Replacement surgery is one of the surgical options to relieve pain and restore joint function in patients with avascular necrosis. Nursing care for ANV patients is very important in managing pain, preventing complications, and promoting recovery. Through comprehensive nursing care, nurses have a significant impact on the physical and emotional health of patients, helping them live healthier and more functional lives. This article aims to implement nursing care for avascular necrosis patients undergoing surgery at Haji Adam Malik General Hospital, Medan. This study is a case report of a 63-year-old patient who had suffered from avascular necrosis for 10 years and underwent Total Hip Replacement surgery. Assessment data were collected through physical examination and supplemented with laboratory and radiology results. The postoperative diagnoses that are the priority in this patient are acute pain, impaired physical mobility, impaired skin integrity, and self-care deficit. Nurses focus on the criteria for treatment outcomes in reducing pain levels, increasing joint range of motion and physical mobility, improving skin integrity, and improving self-care. The intervention results are pain control that allows earlier mobilization and can be a reference for professional nurses in improving the quality of service, especially for avascular necrosis patients undergoing Total Hip Replacement (THR) surgery.

Abstract

Diabetes mellitus is a chronic degenerative disease characterized by increased blood glucose levels that exceed normal limits. Complications often experienced by patients with diabetes mellitus are wounds. Wounds that are difficult to heal can result in amputation. Banana leaves have antioxidant activity and biological functions including anti-diabetic and have been proven to be a source of bioactive compounds that function to stop or inhibit the growth of bacteria and fungi that can cause infection in wound tissue. Objective: to analyze the effectiveness of banana leaves as a dressing in the wound healing process in diabetic patients. The study was a quasi-experiment with a pre-post control design. Thirty-two were applied in this study, 16 were intervention group and 16 were control group. Data were collected using the Bates-Jensen Wound Assessment Tool and analyzed using the Wilcoxon signed-rank test. There is a significant difference in wound healing in the intervention group before and after the use of banana leaves as a dressing (p<0.001). The results of this study can be a basis for nurses in applying or implementing banana leaves as wound dressings for the healing process of diabetic wounds, both chronic and acute wounds, and these results can support nurses in carrying out independent nursing care.

Abstract

Percutaneous Coronary Intervention (PCI) is one of the most effective actions for patients with Acute Coronary Syndrome (ACS), which can improve prognosis and minimize hospitalization time. To optimize medical therapy programs, PCI must be complemented with further preventive measures, such as cardiovascular rehabilitation, including lifestyle modification and risk stratification. This study aims to analyze the relationship between phase I cardiovascular rehabilitation, risk stratification, and length of hospitalization in post-PCI patients. The study design was a retrospective cohort sourced from electronic medical record data. Respondents were divided into groups: those exposed and those not exposed to phase I cardiovascular rehabilitation. Risk stratification and length of hospitalization were studied as study outcomes. Among 201 patients who were followed up during hospitalization, high-risk stratification was more in the unexposed group: exposed group (92.2%: 90.8%; RR 0.85, 95%CI 0.340-2.104). Length of hospitalization ≥3 days was more in the unexposed group: exposed group (63.1%: 59.2%; RR 0.904 95%CI (0.639-1.279). Length of hospitalization ≥3 days was more in the high stratification group: low stratification (63.6%: 35.3%; RR 0.555 95%CI (0.289-1.066). Phase I cardiovascular rehabilitation was not associated with risk stratification and length of hospitalization. Patient risk stratification was related to the length of hospitalization of post-PCI patients. Phase I cardiovascular rehabilitation is an integral part of medical therapy that can reduce adverse events in post-PCI patients.
