Abstract

Background: Osteoarthritis (OA) is a major cause of disability in the elderly. OA can cause chronic pain, disability, and the patient's quality of life. This study aimed to determine the biopsychosocial factors affecting functional disability and depression in patients with OA in Surakarta, Central Java.Subjects and Method: This was an analytic and observational study with a case-control design. The study was conducted at Dr. Moewardi Hospital and Dr. Soeharso Orthopedic Hospital, Surakarta, Central Java, from January to February 2018. A sample of 200 OA patients was selected by simple random sampling. The dependent variables were functional disability and depression. The independent variables were age, sex, grade of OA, duration, body mass index (BMI), history of injury, pain level, family support, and peer support. Depression data were measured by Beck’s Depression Inventory (BDI). Functional disability data were measured by The Western Ontario and McMaster University Arthritis Index (WOMAC). The other variables were collected by questionnaire. The data were analyzed by path analysis.Results: The risk of depression increased with higher BMI (b= 0.23; SE= 0.10; p= 0.021), pain level (b= 0.16; SE= 0.07; p= 0.022), duration (year) (b= 0.27; SE= 0.12; p= 0.025), and poor functional disability (b= 0.19; SE= 0.04; p<0.001). Depression decreased with stronger family support (b= -0.05; SE= 0.03; p= 0.031) and stronger peer support (b= -0.09; SE= 0.05; p= 0.001). The risk of functional disability increased with female sex (b= 3.36; SE= 1.21; p= 0.006), age (b= 0.14; SE= 0.06; p= 0.016), BMI (b= 0.31; SE= 0.17; p= 0.070), had experienced injury (b= 2.27; SE= 1.16; p= 0.051), higher grade of OA (b= 1.32; SE= 0.65; p= 0.041), and higher pain level (b= 0.41; SE= 0.12; p<0.001). The risk of functional disability decreased with stronger family support (b= -0.16; SE= 0.04; p<0.001) and stronger peer support (b= -0.29; SE= 0.08; p<0.001).Conclusion: Depression in OA patients is affected by BMI, pain level, duration (year), functional disability family support, and peer support. Abstract

Background: Socioeconomic factors are recognized as key contributors to the risk of depression among adults and older populations. However, socioeconomic status (SES) is often measured using single indicators, which may not capture its multidimensional nature. Moreover, the mechanisms through which socioeconomic conditions influence depression remain insufficiently explored. This study aimed to examine the pathways linking socioeconomic inequalities to depression using a multidimensional composite measure of SES. Subjects and Method: A cross-sectional analysis was conducted using data from the 2023 Indonesian Health Survey, a nationally representative population-based survey. SES was constructed using principal component analysis (PCA) based on wealth index, education level, employment status, participation in government social protection programs, and provincial-level income inequality (Gini index). Path analysis was performed to assess direct and indirect relationships between SES, physical activity, comorbidity, and depression. All analyses were conducted using STATA 17. Results: Higher SES was associated with a lower risk of depression. Significant pathways were observed from SES to depression both directly (β= -0.06; 95% CI= -0.07 to -0.06; p <0.001) and indirectly through comorbidity (β= -0.03; 95% CI= -0.03 to 0.02; p <0.001). Conclusion: Socioeconomic inequalities play a critical role in shaping depression risk through multiple pathways.

Abstract

Background: Osteoarthritis (OA) is a degenerative joint disease associated with damage to joint cartilage, where a complex joint degradation process occurs, consisting of a repair process in the cartilage, bone, and synovium, followed by a secondary component of the inflammatory process. The primary cause of osteoarthritis is still not known for certain, but there are several risk factors that play a role, namely: age, gender, genetics, obesity, metabolic diseases, and other factors. This study aimed to examine the effect of theraband exercise on reducing knee pain in OA patients. Subjects and Method: This was a quasy experiment study conducted at Dr. Soedjono Hospital, in Magelang, Central Java, from May to June 2023. A sample of 33 knee osteoarthritis patients was selected using purposive sampling. Intervention group (n= 17) received Theraband exercise and control group (n= 16) received usual care. The dependent variable was knee pain. The independent variable was Theraband exercise. Knee pain was measured using a visual analogue scale (VAS). The data were tested using independent t test. Results: After intervention, mean of VAS score in the intervention group (Mean= 5.25; SD= 1.57) was slightly higher than control group (Mean= 5.13; SD= 1.75), but it was statistically non-significant (p= 0.848). Conclusion: Mean of VAS score in the intervention group is slightly higher than control group, but it is statistically non-significant. Keywords: theraband exercise, pain, osteoarthritis knee. Abstract

Background: The foot is a part of the body that function is to support the body weight and to maintain balance while standing, but the foot also has many problems, one of which is flat foot, a type of flat foot caused by the flattening of the bone's curvature, which is the lack of arches on the soles of the feet. The weakness of the plantaris ligament, posterior and anterior tibial tendons, and intrinsic muscles, which maintain the medial longitudinal arch, are the cause of this foot deformity that can have an impact on health. Flat feet that occur in the community are often ignored and considered to be normal. Poor foot conditions will affect the fulcrum, one of the foot conditions that can affect the fulcrum is flat foot. When a child has the flat foot, the resulting fulcrum is also going to be bad, it will affect the balance. Purpose: To ascertain how flat foot relates to both static and dynamic balance. Method: A cross-sectional approach, specifically focusing on observational study. The study took place from June 2024, in MI Muhmmadiyah Gonilan, Surakarta. The independent variable was flat foot the dependent variables is a static balance and dynamic balance. Using purposive sampling and the Slovin formula, 108 participants were selected. Univariate statistical analysis was conducted, bivariate data analysis using Spearman rank test. Results: The correlation coefficient value of 0.645 falls into the strong relationship category, while the correlation coefficient value of 0.207 is classified as a weak relationship. Additionally, the significance value of <0.05 indicates a statistically significant relationship between flat foot and both static and dynamic balance. Conclusion: The analysis showed that while there was a correlation between flat feet and both types of balance, the degree of association was different. Flat feet had a very weak relationship with dynamic balance, while its relationship with static balance was stronger and more significant. Thus, it can be said that flat feet have more influence on static balance than dynamic balance.

Abstract

Background: An intervertebral disc's nucleus pulposus or annulus fibrosus may protrude in part or in its entirety in a condition known as herniated nucleus pulposus (HNP). This can put pressure on the spinal canal or nerve roots through the annulus fibrosus's tear. Over 90% of HNP instances take place in the disc between the L4-L5 lumbar vertebrae or the L5-S1 sacral vertebrae, which is the disc between the fourth and fifth lumbar (L4-L5) vertebrae, which means that pressure is exerted on the L4, L5 and S1 nerves, causing local low back pain and radicular pain in the lower limbs, especially in the back of the lower leg and in the back of the foot. Purpose: To determine the effect of core stability exercise on reducing pain and increasing functional ability in LBP EC HNP patients. Method: A quantitative research method in the form of Quasy Experimental with a Two Group Pretest-Posttest design, namely a control group and a group of 20 respondents who experienced LBP EC HNP. The research instrument uses the ODI questionnaire and Visual Analog Scale (VAS). The data obtained were then analyzed using the Wilcoxon test and the Mann Whitney test. Results: There is a difference in the influence of ODI and Motion Pain between group 1 control and group 2 treatment (p <0.05). While in the pressure pain value (p > 0.05) then the decision was taken to fail to reject H0 which means there is no difference in the influence of pressure pain between group 1 control and group 2 treatment. Conclusion: There is an effect of Core Stability Exercise on reducing pain and increasing the ability of functional activities of daily living in patients with Lumbar Hernia Nucleus Pulposus.

Abstract

Latar Belakang: Prosedur Double Valve Replacement (DVR) dengan sternotomi sering kali memicu komplikasi respirasi dan penurunan kapasitas fungsional yang memerlukan penanganan rehabilitasi dini. Tujuan: Penelitian ini bertujuan mengevaluasi efek fisioterapi Fase I terhadap fungsi respirasi dan kapasitas fisik pasien pasca-DVR. Metode: Studi kasus ini meneliti satu pasien laki-laki berusia 20 tahun di ICCU menggunakan intervensi breathing exercise, myofascial release, dan mobilisasi bertahap berbasis FITT, dengan alat ukur ekspansi toraks dan ICU Mobility Scale (IMS). Hasil:Terapi selama tiga sesi meningkatkan ekspansi toraks dan meningkatkan skor IMS secara signifikan dari skala 2 menjadi 9 tanpa mengganggu stabilitas hemodinamik. Kesimpulan: Intervensi fisioterapi dini secara aman mengakselerasi kemandirian fungsional serta mengoptimalkan ventilasi paru pasien pasca-DVR.
