Abstract

Introduction: Pelvic organ prolapse (POP) is a gynaecological disorder with a rising prevalence, significantly affecting quality of life. While the pathogenesis of POP is multifactorial, alterations in the extracellular matrix (ECM) have been implicated, particularly the role of transforming growth factor-beta (TGF-β) in ECM remodelling. However, the precise relationship between TGF-β and POP remains unclear. Methods: A systematic review was conducted following PRISMA guidelines. Relevant studies were identified from PubMed, Scopus, Web of Science, and the Cochrane Library. Inclusion criteria focused on studies investigating TGF-β expression in women with POP. Data extraction included study design, patient demographics, TGF-β detection methods, and key findings. Results: Analysis of nine studies revealed mixed findings. Four studies reported a significant reduction in TGF-β1 expression in POP patients, suggesting impaired ECM regulation via the TGF-β1/Smad3 pathway. Three studies found no significant differences in TGF-β levels between POP and control groups, while one study linked increased TGF-β1 levels to severe prolapse, possibly as a compensatory response. Variations in methodology, sample size, and patient characteristics contributed to inconsistencies across studies. Conclusion: Current evidence suggests that TGF-β1 plays a role in POP pathogenesis, with lower levels potentially weakening ECM integrity and higher levels in advanced cases reflecting a failed reparative response. Further research is needed to explore TGF-β interactions with other biomarkers and assess its potential as a therapeutic target.

Abstract

Empathy is a crucial component for healthcare professionals to build relationships with patients and improve the quality of care. However, it is often overlooked among laboratory personnel who tend to focus more on technical aspects. Individual and organizational factors are suspected to contribute to decreased empathy among laboratory staff. This study aimed to identify the factors associated with empathy among staff at the Central Laboratory Installation of M. Djamil Hospital Padang. This study employed a Mixed Method design using a Concurrent Embedded model involving 53 laboratory staff. Data were collected through questionnaires, in-depth interviews, observations, and document reviews. Quantitative data were analyzed using chi-square tests and logistic regression, with statistical significance set at p < 0.05. The results showed that the majority of respondents were aged 30–42 years (52.8%), female (77.4%), held a diploma degree (81.1%), had ≥5 years of work experience (84.9%), and had low empathy levels (56.6%). There were significant associations between age (p=0.043), education level (p=0.042), organizational support (p=0.005), workload (p=0.003), work stress (p=0.045), and work motivation (p=0.023) with empathy levels. Meanwhile, gender, length of service, and seminar/training attendance were not significantly associated with empathy. Multivariate analysis revealed that organizational support, workload, and work stress were the key factors associated with empathy, with organizational support emerging as the most dominant factor (p=0.004). Qualitative data supported these findings, indicating that high workload, lack of organizational support, and performance pressure were major barriers to demonstrating empathy toward patients. In conclusion, laboratory staff empathy is influenced by age, education, organizational support, workload, job stress, and work motivation, with organizational support being the most dominant factor.

Abstract

Thyroid carcinoma is the most common malignancy found in the endocrine glands and accounts for 1% of all malignancies. The average incidence of thyroid carcinoma cases according to the National Cancer Institute (NCI) from 2012-2017 was 14.3 per 100,000 total population per year, with an average mortality rate of 0.5 per 100,000 population per year. In Indonesia in 2000, thyroid carcinoma ranked ninth out of the ten most common carcinomas (4.3%) and covered 90% of all endocrine carcinomas. Based on medical record data at Dr. M. Djamil Padang General Hospital, the incidence of thyroid carcinoma in 2015 was 373 people and increased in 2016 to 850 people. Based on the problems described above, and there has not been much research on Thyroglobulin levels as a prognostic factor for recurrence of thyroid carcinoma in Indonesia, especially Padang. Encourage the author to conduct research on differentiated thyroglobulin levels after total thyroidectomy at Dr. M. Djamil Padang General Hospital.

Abstract

Latar Belakang: Defisiensi vitamin D telah menjadi masalah kesehatan dunia dan berkembang pesat selama beberapa tahun terakhir ini. Defisiensi vitamin D merupakan salah satu faktor risiko penyakit kardiovaskular. Indeks aterogenik plasma yang dirumuskan sebagai log10(TG/HDL) merupakan penanda aterogenesitas untuk memperkirakan risiko penyakit kardiovaskular. Defisiensi vitamin D dan nilai indeks aterogenik plasma yang tinggi sering ditemukan pada penyandang obes. Objektif: Penelitian ini bertujuan untuk mengetahui korelasi antara kadar vitamin D dengan indeks aterogenik plasma pada mahasiswa penyandang obes. Metode: Penelitian ini merupakan penelitian analitik observasional dengan desain cross sectional yang dilakukan di Fakultas Kedokteran Universitas Andalas dan Laboratorium Sentral RSUP Dr. M. Djamil Padang dari bulan Agustus 2020 hingga April 2021. Subjek penelitian ini adalah 30 mahasiswa angkatan 2017-2018 penyandang obes di Fakultas Kedokteran Universitas Andalas, yang terdiri dari 16 orang laki-laki dan 14 orang perempuan. Hasil: Hasil penelitian didapatkan rerata umur subjek 19,57±0,86 tahun dan rerata IMT 32,53±4,13 kg/m2. Rerata kadar vitamin D subjek 13,90±4,10 ng/mL dan rerata indeks aterogenik plasma subjek -0,025±0,237. Analisis bivariat didapatkan korelasi negatif sangat lemah dan tidak bermakna antara kadar vitamin D dengan indeks aterogenik plasma (r= -0,129; p= 0,498). Kesimpulan: Tidak terdapat korelasi yang bermakna antara kadar vitamin D dengan indeks aterogenik plasma pada mahasiswa penyandang obes.

Abstract

Quantitative real-time polymerase chain reaction (qRT-PCR) is being explored as an affordable alternative to flow cytometry for monitoring CD4+ T lymphocytes in HIV-infected patients, but detailed design and validation of CD4-specific primers are rarely reported. To design a CD4-specific primer-probe set for qRT-PCR, assess its in silico specificity, and determine the optimal annealing temperature. A forward primer, reverse primer, and hydrolysis probe targeting CD4 transcript were designed from published CD4 gene sequences. Specificity was evaluated using NCBI Primer-BLAST against the Human mRNA RefSeq database. Optimal annealing temperature was determined using an 8-point thermal gradient (54.0-66.0°C) on a Bio-Rad CFX96 system with SensiFAST™ Probe No-ROX chemistry, comparing Cq values across the gradient. Results: Primer-BLAST confirmed specificity of the forward (20nt, Tm 57.42°C, GC 50.00%) and reverse (24nt, Tm 57.69°C, GC 41.67%) primers, with low self-complementarity (3.00 and 2.00) and no 3' self-complementarity. Cq values were tightly clustered (23.25-23.76) across the gradient; with the lowest Cq (23.25) at 54.8°C. The CD4-specific primer-probe set showed confirmed specificity and robust tolerance across a wide annealing temperature range, with an optimum of 54.8°C, supporting its use as a foundational tool for CD4+ T lymphocyte monitoring where flow cytometry is inaccessible.
