Abstract

Studi ini bertujuan mengetahui perubahan perilaku dan tingkat kemandirian keluarga sesudah diberikan intervensi modifikasi perilaku dengan hypnocaring (Moluca). Desain studi ini mengunakan studi kasus. Subyek penelitian adalah keluarga dewasa yang menderita hipertensi intervensi dilakukan selama 8 minggu melalui modifikasi perilaku dengan hypnocaring. Teknik pengambilan sampel dengan purposiv sampling. Peneliti mengamati kasus selama 6 bulan dengan mengambil data secara kuantitatif dan mengobservasi perubahan perilaku  objek yang diberikan intervensi. Hasil intervensi peningkatan perilaku: pengetahuan meningkat menjadi 80%, sikap  menjadi 70%  dan keterampilan menjadi 60%. Peningkatan kemandirian keluarga, terdapat 6 keluarga mencapai mandiri 3 dan 4 keluarga mencapai mandiri 4.

Abstract

Older people’s health care is carried out by involving the family as a support group. If done optimally, the support provided will help maintain the health of older people. The purpose of the study was to analyze the determinant factors (family support and burden) on the quality and satisfaction of life the older people. This descriptive correlational research was the second stage of previous research related to the identification of caregivers' burden when caring for older people. A sample of 104 respondents was taken through simple random sampling. The results showed that the family support and family burden affected the older people's quality of life (< 0.05), but did not affect the older people's life satisfaction (> 0.05). Family support and burdens affect the QoL for older people, which is due to the family's ability in caring for older people and the need for long-term services at home. Recommendations for health workers, especially nurses, are to focus not only on the elderly but also to the family's physical, psychological, and financial burdens to increase family support.   Abstrak Faktor Determinan dari Kualitas dan Tingkat Kepuasan Hidup Lansia. Lansia yang sehat dapat dicapai dengan melibatkan seluruh anggota keluarga sebagai support group dalam merawat lansia. Karenanya, perawatan yang optimal oleh keluarga dapat menjaga kesehatan lansia itu sendiri. Studi ini bertujuan untuk menganalisis faktor determinan (dukungan keluarga dan beban keluarga) pada kualitas hidup dan kepuasan hidup pada lansia. Penelitian deskriptif korelasional ini merupakan studi tahap kedua dari penelitian sebelumnya, yaitu studi identifikasi masalah pada beban yang dihadapi oleh caregiver. Sebanyak 104 responden sebagai sampel untuk studi ini dipilih melalui teknik simple random sampling. Penelitian ini menunjukan bahwa faktor dukungan dan beban keluarga mempengaruhi kualitas hidup lansia (< 0,05), namun tidak memengaruhi kepuasan hidup pada lansia (> 0,05). Dukungan keluarga yang memengaruhi kualitas hidup lansia terjadi karena faktor kemampuan keluarga itu sendiri dalam merawat lansia dan faktor kebutuhan jangka panjang dalam merawat lansia itu sendiri. Rekomendasi untuk para tenaga kesehatan, khususnya perawat, adalah untuk fokus tidak hanya pada lansia namun juga pada keluarga dari aspek beban fisik, mental dan finansial, untuk meningkatkan dukungan keluarga. Abstract

The factors associated with changes in work patterns and working hours due to rotating shifts have an effect on the increased risk of health problems in workers. Manufacturing industry workers, specifically those on rotating shift schedules, are at a high risk of various health problems, such as cardiovascular diseases, circadian rhythm problems, social life problems, and stress. These health problems may be worsened by poor lifestyle habits, such as smoking, unhealthy diet, and infrequent physical activity. This research aimed to explore the experience of 12 manufacturing workers on rotating shift schedules in Greater Jakarta, Indonesia. Through a phenomenological approach, this qualitative study employed 12 participants selected from manufacturing industry shift workers. The participants were selected through purposive sampling whom met the inclusion criteria, namely working in three rotating shift patterns (morning, afternoon, and night shift), aged 20–50 years old, having at least three years of experience in shift work, and able to communicate well. Selection was done with the assistance of the supervisors of the participants working in the manufacturing industry. Thematic analysis yielded three themes: the reasons for working shifts, the effects of shift work, and efforts made to maintain health during working shifts. The findings of this study imply the need for occupational health nursing services as the main intervention at the primary and secondary prevention levels. Occupational health nurses provide occupational health nurs-ing services in the workplace in accordance with the nursing intervention model of fatigue management.   Abstrak Suara Pekerja Manufaktur Indonesia dalam Sistem Shift Berputar. Dalam sistem kerja shift, faktor-faktor seperti perubahan pola kerja dan jam kerja dapat berdampak pada masalah kesehatan para pekerjanya. Pekerja shift manufaktur berisiko memiliki berbagai masalah kesehatan, seperti penyakit kardiovaskuler, gangguan irama sirkadian, gangguan pola kehidupan sosial, stres, didukung oleh perilaku pekerja seperti merokok, diet yang buruk serta aktivitas olah raga yang jarang dilakukan. Penelitian ini bertujuan untuk mengeksplorasi pengalaman para 12 pekerja di sector manufaktur di Indonesia, khususnya di daerah Jakarta dan sekitarnya. Peneliti menggunakan metode penelitian kualitatif dengan pendekatan fenomenologi. Partisipan berjumlah 12 didapatkan melalui teknik purposive sampling yang memenuhi kriteria inklusi: menggunakan sistem kerja rotasi tiga shift (pagi, sore, dan malam), berusia 20–50 tahun, memiliki pengalaman bekerja sistem rotasi shift minimal tiga tahun, dan mampu berkomunikasi dengan baik. Proses pemilihan partisipan dilakukan peneliti bersama key person, yaitu supervisor/leader dari beberapa pabrik manufaktur. Analisis tematik yang dilakukan menghasilkan tiga tema: alasan bekerja shift, dampak dari kerja shift, dan upaya para pekerja dalam menjaga status kesehatan. Temuan dari penelitian ini dapat menjadi implikasi bagi kesadaran terhadap perlunya tenaga kesehatan atau perawat di area kerja atau sektor industri sebagai upaya intervensi utama dan sekunder dalam pencegahan kecelakaan kerja untuk para pekerja dan keluarga pekerja melalui model keperawatan manajemen kelelahan kerja (MARI-KERJA). Abstract

Hypertension as Non-Communicable Diseases (NCD) is the largest contributor to the causes of death in the world. Complications of hypertension account for the majority of deaths globally. Uncontrolled health behavior in patients with hypertension is closely related to an increase in morbidity rates due to complications from hypertension each year. Medication adherence is one of an effort to form positive health behaviors to prevent hypertension complications. This study aims to identify the relationship between health behavior with medication adherence in hypertensive clients in Cipayung District, East Jakarta. This study using cross sectional design, approach with 109 participants, who were selected by purposive sampling, are choosen primary hypertensive clients of middle adulthood. The results of the chi square test showed that there was a relationship between health behavior and medication adherence (p value: 0.024) 90% CI. The results of the study it can be concluded that the more positive the health behavior, the higher the adherence to taking medication in hypertensive clients. Community nurses have an important role in modifying the behavior of hypertensive patients with a variety of approaches both as individuals, families, and the public can maintain their health well-being. Abstract

Health and functional status are standard measurements in older adult’s services that showed physical and mental condition. However, institutionalized older adults with several adjustments and their limited source might cause compromised health and functional status. The quasi-experimental study aimed to investigate the effect of MiRaDaSia (nurses and caregivers joined the partnership model) on health and functional status among institutionalized older adults in Jakarta. It included 106 participants as intervention groups and 106 participants as control groups, who selected by multistage random sampling. We used the SF-12 and Barthel index to measure older-adults health and functional status. MiRaDaSia model was implemented for twelve weeks. Generally, there has been an increase in the mean of health and functional status after the intervention. There were significant improvements in functional condition between each group (p=0,001); however, mean difference oh health status show the significant increase only on six weeks following the intervention. MiRaDaSia can be implemented as a practical model to enhance services among institutionalized older-adults by professional’s staff as it encourages partnership among the nurse, caregiver, and the institutionalized older-adults. Future research may consider the effectiveness of the model in private institutional, with widening variation of older adults and caregivers’ characteristics as well as the different working environment of the institution. Abstrak Model Kemitraan Perawat, Caregiver, dan Lansia (Miradasia) Dalam Meningkatkan Status Kesehatan Dan Fungsional Lansia di Panti. Status kesehatan dan fungsional merupakan pengukuran standar yang harus dilakukan dalam menilai pelayanan kesehatan lansia yang meliputi pengkajian sampai evaluasi. Kedua pengukuran tersebut pada akhirnya menggambarkan kondisi fisik dan mental lansia. Namun, kondisi lansia yang berada di panti dengan berbagai permasalahan kesehatan dan keterbatasan sumber daya dapat menimbulkan gangguan pada status kesehatan dan fungsional lansia. Penelitian ini bertujuan untuk mengetahui pengaruh pelaksanaan model praktik Kemitraan antara perawat, caregiver, dan lansia (MiRaDaSia) terhadap status kesehatan dan fungsional pada lansia yang tinggal di panti wilayah Jakarta. Penelitian dengan desain quasi-eksperimental melibatkan 2 kelompok yang terbagi menjadi 106 partisipan di kelompok intervensi serta 106 partisipan di kelompol kontrol. Pemilihan sampel dilakukan melalui multistage random sampling dengan alat pengukuran berupa SF-12 dan Barthel index untuk melihat staus kesehatan dan fungsional lansia. Model praktik keperawatan MiRaDaSia diimplementasikan selama 12 minggu pada kelompok intervensi. Secara umum, hasil penelitian menunjukkan bahwa terdapat peningkatan status kesehatan maupun fungsional status setelah intervensi model MiRaDaSia. Status fungsional secara signifikan mengalami peningkatan antara kelompok perlakuan (p=0,001) namun, beda reratastatus kesahatan menunjukkan peningkatan signifikan pada pengukuran 6 minggu setelah intervensi. terdapat peningkatan rerata status kesehatan setelah intervensi. Status kesehatan dan fungsional pada lansia dipengaruhi Model praktik keperawatan MiRaDaSia dapat diimplementasikan sebagai model praktik untuk meningkatkan pelayanan lansia oleh petugas maupuun tenaga profesional pada setting panti, karena memberikan penguatan pada kemitraan antara perawat, caregiver, dan lansia. Penelitian yang akan datang sebaiknya perlu mempertimbangkan penerapan model prakting di setting panti swasta, dengan variasi karakteristik lansia dan caregiver yang lebih banyak maupun lingkungan kerja institusi, untuk mengetahui lebih jauh tentang efektivitas model. Abstract

Purpose: This research aims at discovering the relationship between nursing care supervision done by chief nurse and the implementation of Standard Operational Procedure of parenteral drug giving at RSUD Solok. Method: The research method is descriptive analytic with cross sectional design. It was done from July until August 2014 by using questionnaire and observation to 96 nurses in charge. The sample was taken by using proportional sampling from each in-patient room. The data were analyzed by using chi-square and logistic regression analysis. Result: The result shows that there was a correlation between Head of Room supervision, which includes planning with p value=0,004, implementation with p value=0,001, evaluation and feed back with p value=0,012 and documentation with p value= 0,037, towards the implementation of Standard Operational Procedure of parenteral drug administration. The dominant variable of the implementation of Standard Operational Procedure of parenteral drug is supervision, with p value = 0,001 and OR=4,14. Conclusion: From the result, it can be concluded that chief nurse supervision has a meaningful correlation towards the implementation of Standard Operational Procedure of parenteral drug.

Abstract

Background:  Understanding the barriers and support that family caregivers face when caring for older adults with dementia is critical for improving the quality of  dementia care. However, research in this area remains limited in Indonesia. Purpose: This study aimed to explore the barriers and support for family caregivers in caring for older adults with dementia in Indonesia.Methods: A descriptive phenomenological study was conducted among 15 family caregivers who cared for older adults with dementia. Purposive sampling was used to recruit the participants. Inclusion criteria included primary family caregivers who provided care for four hours per day for at least six months, were proficient in Indonesian, assisted older adults in daily activities, and were capable of expressing their experiences. Data were collected using in-depth semi-structured interviews and analyzed using the Colaizzi method.  Results: This study found that family caregivers faced barriers during the care of older adults with dementia, which have decreased the quality of care. These barriers included inadequate knowledge and skills of family caregivers regarding dementia, stigma, lack of knowledge and awareness in the community, limited availability of healthcare services, and healthcare professionals’ lack of knowledge about dementia. On the other hand, support received by family caregivers within the family, support from other family caregivers, and education provided by NGOs about caring for someone with dementia have shown a positive impact in reducing the burden on caregivers and improving the quality of care. Therefore, appropriate support is essential for family caregivers caring for older adults with dementia.Conclusion: This study showed that barriers originating from family caregivers, community awareness, access to health services, and barriers from health workers have impacted poor quality of care, while the support of families, fellow family caregivers, and NGOs have contributed to good quality of care. Further studies are needed to explore the perspectives of family caregivers representing different cultures in Indonesia.

Abstract

Background: Adolescents face increased vulnerability to HIV/AIDS due to factors such as insufficient knowledge and awareness, stigma, and restricted access to information. The Indonesian government initiated a peer education program through the Planning Generation Program (GenRe) ambassadors. These ambassadors are tasked with educating their peers about HIV/AIDS. However, their experiences are largely underexplored. Objective: This study aimed to explore the GenRe ambassadors’ experience in improving adolescents’ HIV/AIDS prevention behavior. Methods: This study employed a qualitative phenomenological approach, with in-depth and semi-structured interviews of GenRe ambassadors who had served for at least three to six months within the South Jakarta Health Sub-Department, Indonesia. Purposive sampling and data saturation techniques were utilized. Data were collected from 13 May to 31 August 2022, and Colaizzi’s steps were used to guide the data analysis. Results: Nineteen GenRe ambassadors participated, predominantly female university students ages 18–22. Six themes were developed: 1) the impact of GenRe ambassadors’ roles in improving HIV/AIDS prevention behavior, 2) activities to improve HIV/AIDS prevention behavior, 3) enhancement of self-concept as GenRe ambassadors, 4) obstacles to activities for improving HIV/AIDS prevention behavior, 5) support for activities to improve HIV/AIDS prevention behavior, and 6) hope for improving the GenRe ambassador program’s implementation. Conclusion: GenRe ambassadors are influencers who play a critical role in promoting HIV/AIDS prevention behaviors and fostering positive change in the broader youth community. Despite their positive self-concept, they encounter challenges in fulfilling their roles. Utilizing their insights, community health nurses can create effective peer-led HIV/AIDS prevention programs and contribute to a model of health promotion for adolescents. Leveraging the role of GenRe ambassadors as peer educators is recommended to enhance HIV/AIDS prevention efforts among adolescents.

Abstract

Latar Belakang: Sistem jenjang karier memberikan penghargaan kepada perawat berdasarkan pendidikan, dan kompetensi perawat. Metode: Penelitian ini melibatkan 6 orang partisipan yang diambil dengan teknik purposive sampling. Hasil: Hasil penelitian mengidentifikasi 5 tema utama yaitu 1) Pengembangan Profesional Berkelanjutan, 2) Sosialisasi Sistem Jenjang Karir, 3) Implementasi Jenjang Karir Belum Sesuai Dengan Beban Kerja Dan Kewenangan Klinis Perawat, 4) Dukungan Manajemen dan Pemimpin dalam Sistem Jenjang Karir Keperawatan, 5) Asuhan Keperawatan Profesional. Rekomendasi: Rekomendasi dari hasil penelitian ini diharapkan Bagi Pimpinan/kepala ruangan diharapkan dapat meningkatkan program supervisi untuk dapat mengkawal kompetensi klinis yang sesuai dengan jenjang karir masing-masing perawat, sehingga perawat pelaksana dapat melaksanakan kompetensi klinis sesuai dengan kewenangan klinisnya

Abstract

Teenagers have big influence on clean and health lifestyles such as no smoking lifestyle. That is why; the family function has an important role on controlling the smoking behavior of the teenagers. This is a descriptive correlation research with the cross Sectional approach to find out the family function influence that divided into 5 family functions; affective, social function, economic function, reproductive function, and health function correlated with the smoking behavior of SMA/SMK  students in Gombong, Kebumen Regency, Central Java. There are 220 teenagers as the samples in this research. They are still in Senior High School (Vocational High School/SMK and General High School/SMA) and they are smokers. The sampling technique was done by using the Cluster Random Sampling. The data analyses used were univariate, bivariate, with Chi Square test and multivariate with the regression logistic test. The research found that the family function influenced the smoking behavior of the students in SMK/SMA in Gombong are social function (p value =0.000), economic function (p value =0.003), reproductive function (p value =0.004) and affective function (p value =0.008). Meanwhile the most dominant family functions variable that influenced the smoking behavior of SMK/SMA students were social and family economic functions. Because of this reason, it is necessary for the parents or family to have a strong commitment to apply their function as the family to control their teenage children and they can also consider their teenage children development characteristics. Community Nurse has a role on giving suggestion to the parents so that they can do their exact function as the family and give the right and needed information for their teenage children appropriate with their children development stages.   Keywords; family function, teenage smokers.
