Abstract

Various terms have been used to define coping in caregivers of older people with dementia with varied abstract concepts that are widely used in nursing and other disciplines, such as psychology, medicine, and education. A strong understanding of the concept of caregiver coping is essential as a basis for providing appropriate nursing care. This concept analysis aims to clarify the concept of coping in caregivers of older people with dementia and develop the operational definition, attributes, antecedents, and consequences of this concept. This study utilizes Walker and Avant’s concept analysis method. The analysis shows that coping attributes of caregivers of older people with dementia include cognitive and behavioral efforts in providing care, stress adaptation, and use of resources on oneself and the environment. Antecedents of coping include characteristics of caregivers, stressors, and caregivers’ assessment of an event. Meanwhile, the consequences include positive impacts and negative impacts. This analysis can help nurses understand the concept and determine appropriate assessments and interventions for caregivers of older people with dementia. It is suggested that nurses be aware of caregivers’ characteristics, stressors, and assessment of an event. Further research is needed to investigate the need for intervention on the consequences of coping in caregivers of older people with dementia. Abstrak: Berbagai istilah digunakan untuk mendefinisikan koping caregiver lansia dengan demensia  dengan konsep abstrak yang bervariasi luas yang  digunakan di semua bidang keperawatan, serta dalam disiplin ilmu lain seperti psikologi, kedokteran, dan pendidikan. Pemahaman yang kuat tentang konsep koping caregiver lansia dengan demensia penting dalam keperawatan untuk dapat menjadi dasar dalam melakukan asuhan keperawatan yang tepat. Tujuan dari analisa konsep ini adalah untuk memperjelas konsep Koping caregiver dan mengembangkan definisi operasional dari konsep koping caregiver lansia dengan demensia, atribut , Antesenden dan konsekuensi dari konsep koping caregiver lansia dengan demensia. Metode yang digunakan adalah dengan Pendekatan analisis konsep Walker dan Avant. Hasil analisis menunjukkan bahwa atribut koping caregiver lansia dengan demensia meliputi Upaya kognitif dan perilaku, selama melakukan perawatan, adaptasi terhadap stress, dan Penggunaan sumber pada diri dan lingkungan. Anteseden untuk koping caregiver lansia meliputi karakteristik caregiver, stresor dan penilaian caregiver terhadap suatu peristiwa. Konsekuensinya meliputi dampak positif dan dampak negative. Analisis ini membantu perawat untuk memahami konsep dan menentukan pengkajian dan intervensi yang tepat bagi caregiver lansia dengan demensia. Disarankan bahwa perawat harus menyadari karakteristik caregiver, stresor dan penilaian caregiver terhadap suatu peristiwa. Penelitian lebih lanjut diperlukan untuk menganalisis kebutuhan intervensi  terhadap konsekuensi dari koping caregiver lansia dengan demensia.

Abstract

Introduction: Discharge planning is transferring a patient to a different level of care to improve continuity of care by bridging the gap between hospital and community care. This study aimed to explore the perspectives of healthcare professionals involved in discharge planning for patients with mental illness. Method: This research used qualitative methods with a phenomenological approach. Ten participants were selected using a purposive sampling method, with inclusion criteria healthcare professionals involved in the discharge planning process for patients in psychiatric hospitals, had worked in the service for at least 5 years, and provided direct or indirect services to patients. Data were collected through in-depth interviews using interview guides, recording devices, and field notes. Data were analyzed using thematic analysis. Results: The study identified four themes related to discharge planning preparation for patients with mental illness perceptions of roles and experiences in discharge planning, internal and external barriers in discharge planning, internal and external support in discharge planning and expectations for the continuity of discharge planning. Conclusions:  The findings indicate that there are still significant disparities in the way that discharge planning for patients with mental illness is implemented in different mental health service settings. These study findings are intended to serve as a further source of information to develop a discharge planning system that integrates patients with mental illnesses from the hospital into the community.

Abstract

Berbagai istilah digunakan dalam mendefinisikan manajemen stres lansia dengan hipertensi dengan konsep abstrak yang bervariasi digunakan di berbagai bidang termasuk keperawatan, psikologi, kedokteran dan pendidikan. Pemahaman yang kuat tentang konsep manajemen stres pada lansia hipertensi penting dalam keperawatan untuk dapat menjadi dasar dalam melakukan asuhan keperawatan yang tepat. Tujuan dari analisis konsep ini adalah untuk memperjelas konsep manajemen stres dan mengembangkan definisi operasional dari konsep manajemen stres pada lansia dengan hipertensi, atribut, antesenden, dan konsekuensi dari konsep manajemen stres lansia dengan hipertensi. Metode yang digunakan adalah pendekatan analisis konsep dari Walker dan Avant. Hasil analisis menunjukan bahwa atribut manajemen stres meliputi strategi mengelola stres, koping adaptif, dan penggunaan sumber dari diri dan lingkungan. Antesenden untuk manajemen stres lansia dengan hipertensi meliputi karakteristik lansia, sumber stres, dan penilaian terhadap stres. Konsekuensi manajemen stres meliputi penurunan tekanan darah, stres menurun, kemampuan koping meningkat, dan kualitas hidup menjadi baik. Analisis ini membantu perawat untuk memahami konsep dan menentukan pengkajian dan intervensi yang tepat bagi lansia dengan hipertensi. Disarankan agar perawat harus memahami karakteritik lansia, sumber stres, penilaian lansia terhadap stresor, koping yang digunakan, serta sumber daya dari diri dan lingkungan yang dapat digunakan untuk mengatasi stres. Penelitian lebih lanjut diperlukan dalam mengembangkan intervensi untuk meningkatkan koping adaptif pada lansia dengan hipertensi terhadap konsekuensi dari manajemen stres lansia dengan hipertensi.

Abstract

This study aims to examine the effectiveness of implementing online CBT-I on older adults with sleep disorders. The method used is a Systematic Review guiding a search of 4 databases, ScienceDirect, Scopus, PubMed, and UI Lib, in October 2023 with article publication years starting from 2019-2023 to identify all relevant journal articles. The search technique follows the PRISMA flow. The research results showed that online CBT-I was proven to be effective in treating insomnia sleep disorders in the elderly. CBT-I can even treat secondary disorders due to insomnia, such as depression and anxiety. In conclusion, Online CBT-I can be implemented in health services provided that there is adequate computer and internet accessibility. Keywords: Online CBT, Insomnia, Sleep Disorders, Elderly

Abstract

Hypertension is a non-communicable disease with an increasing prevalence every year and is one of the causes of death due to complications that occur in adulthood. Stress management is a part of hypertension management. The purpose of this study is to provide an overview of the implementation of evidence-based interventions (evidence based practice) guided imagination interventions in family nursing care to reduce blood pressure and stress perception in adults and adult families. The method used is a case study method with a fact-based nursing practice approach in 10 adult families with hypertension problems in Depok City. The measuring instrument used to measure the perception of stress is the Perceived Stress Scale (PSS). The results showed an increase in family knowledge, attitudes and skill in caring for hypertensive adults with values of 30.3%, 34,25% and 33%. The decrease in TDS from 156±15,05 to 148 ±13,16 and TDD from 101±11,00 to 95±9,78 and the average stress perception from 5,0 to 2.4. The guided imagery nursing intervention had an effect on decreasing blood pressure (p value < 0.05) and stress perception (p value < 0.05) Guided imagery intervention can be an effective intervention option in managing the health of family members with hypertension.

Abstract

Type 2 diabetes mellitus is a common metabolic disorder that often occurs in the elderly. One key component in diabetes management is physical activity. Various studies have been conducted to examine the effectiveness of physical exercise on glycemic control and it has been found that physical exercise can improve insulin sensitivity and glycemic control. The research aimed to identify the impact of resistance exercise on HbA1c in elderly individuals with diabetes. Research method with systematic review on ProQuest, Scopus, PubMed, EBSCO Host, Clinical Key Nursing, and Science Direct databases. The inclusion criteria were articles published between 2018 and 2023 in the English language. A total of 7,812 articles were found based on the keywords (Older adult OR elderly OR aged OR Older People OR Older person) AND (type 2 diabetes OR diabetes OR diabetes mellitus OR Noninsulin Dependent Diabetes Mellitus OR NIDDM OR Diabetes Mellitus Type II) AND ("resistance exercise" OR "resistance training" OR "Strength Training") AND (HbA1c OR Glycated Hemoglobin OR Hemoglobin A1c OR glycemic index), and after screening, 9 articles were included in the final analysis. Resistance exercise has a significant impact on glycemic control when performed using a circuit or progressive approach with proper supervision. Resistance training can be applied in diabetes management in the elderly and should be conducted circuit (a series of exercises performed sequentially) or progressive (gradual increase in intensity or load of exercises) approaches, as well as supervision.

Abstract

Background: The elderly are one of the age groups that are easily affected by diabetes mellitus due to the decline in organ function and unhealthy lifestyles. Self-care management of diabetes is crucial for helping to control the blood glucose levels of individuals with diabetes mellitus. Self-care management can be carried out through education, discussions, and the practice of physical exercise to regulate blood glucose levels. The implementation of self-care management interventions is considered effective when conducted in groups among the elderly.Purpose: This research aims to observe changes in the knowledge and attitudes of the elderly, as well as changes in their independence skills.Methods: The implementation of the Diabetes Self-Management intervention was carried out on 33 elderly individuals with diabetes who were organized into one group. The intervention was conducted over 12-14 weeks, which included education, simulation, and chair-based exercise (CBE) training performed once a week consecutively.. This research uses a pre-post group design with t-test analysis to determine changes in the self-care behavior of group members.Results: There was an increase in the average score of 1.54 in the knowledge domain, and an average score of 5.18 in the attitude domain. Additionally, there was a decrease in the average blood glucose level of 61.61 mg/dL in the elderly group with diabetes mellitus. Other improvements were also observed in the changes in the Functional Independence variables for motoric and cognitive functions.Conclusion: Diabetes self-management intervention has become one of the effective strategies that can be implemented in elderly groups with diabetes in the community.

Abstract

Introduction: This study aimed to explore lived experiences as evidence-based in developing health services for hypertensive older adults living alone. Methods: This study employed a qualitative phenomenological approach, with in-depth and semi-structured interviews of hypertensive older adults living alone in Kendari City Southeast Sulawesi. Purposive sampling was utilized. 15 participating older adults, predominantly female, and with ages ranging from 61-80.  Data were collected from April until July 2022, and Colaizzi’s steps were used to guide the data analysis. Results: Three themes were identified: 1) health changes in the older adults living alone consisting of physical, emotional, social, and spiritual changes; 2) coping mechanisms consisting of emotion-focused coping, biobehavior-focused coping, social-focused coping, and spiritual-focused coping; and 3) healthcare needs consisting of health information needs, regular health checks, group activities, and spiritual activities. Conclusions: This research highlights the needs of hypertensive older adults living alone to adapt to their health changes and requiring holistic hypertensive care as well as regular health services. Recommendations in this study include developing a holistic hypertensive care program consisting of physical, psychological, social, and spiritual activities. Further research needs to explore the effectiveness of holistic hypertension care on blood pressure, stress, coping, and quality of life of older adults with hypertension by comparing older adults living alone with older adults living with family, and qualitative research using ethnographic methods to determine the adaptation process of the older adults living alone according to culture.

Abstract

Background:The rising prevalence of Type 2 Diabetes Mellitus (T2D) has become a significant global health burden, with profound implications for healthcare systems and affected individuals. Strengthening primary healthcare services by integrating health and community-based interventions is critical for improving health outcomes. This rapid systematic review aimed to evaluate the effectiveness of community-based interventions in enhancing health outcomes for patients with T2D. Objectives:To evaluate the effectiveness of community-based interventions in managing T2D, focusing on their impact on health outcomes Methods:This systematic review adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Data collection involved five major databases: Scopus, Cochrane Library, PubMed, Web of Science, and Clinical Key, covering publications from January 2013 to May 2024. Eligible studies included randomized controlled trials, mixed-methods research, clinical trials, prospective studies, experimental repeated-measures designs, and quasi-experimental designs. The risk of bias in the included studies was assessed using the RevMan 5.4.1 software. Article selection, risk of bias evaluation, data extraction, and synthesis were conducted independently by the authors, with discrepancies resolved through discussion and consensus. Results:Out of 4.463 identified articles, 13 met the eligibility criteria for inclusion in the review. The community-based intervention strategies identified in these studies involved three primary approaches: family-centered interventions, peer support, and community volunteer programs. Conclusion:This systematic review concludes that community-based interventions are effective in improving health outcomes for individuals with T2D. Integrating and strengthening health and community-based collaborations within the broader healthcare system is crucial for achieving optimal health outcomes. Nurses are encouraged to collaborate with community-based networks to bridge the gap between healthcare providers and communities, thereby enhancing public health outcomes. Abstract

Falls in the elderly are a serious problem because they can cause physical injuries, such as fractures, head trauma, and even death. Preventing falls in the elderly requires a multidimensional approach that is not only focused on the individual, but also involves the family as the main support unit. This review aims to identify family nursing-based fall prevention interventions. This systematic review method follows the PRISMA framework with article searches through Cinahl, ProQuest, Pubmed, ScienceDirect, Google scholar, and the Cochrane database of systematic reviews. The findings of this review revealed that of the 541 articles identified in the initial stage, nine articles met the eligibility criteria and were retained for further in-depth analysis. The results of the analysis provide insights into fall prevention interventions including; health education, mindfulness and psychoeducation, Otago exercise program, family involvement, fall risk assessment, and home modification. These findings have important implications for community health nurses in providing family nursing care, through empowering the family as the main caregiver in managing fall risk at home.
