Abstract

Angka kematian maternal di Indonesia masih masuk dalam kategori tinggi bila dibandingkan dengan negara ASEAN lainnya. Secara khusus di kecamatan GatakKabupaten Sukoharjo Jawa Tengah, angka kematian maternal di tahun 2022 sebesar 3 kematian dari 628 kelahiran hidup, dan di bulan Juli 2023 sudah terdapat 2 kematian ibu terkait kehamilan. Upaya pencegahan primer telah dilakukan melalui penggunaan skrining keberadaan ibu hamil risiko tinggi dengan skoring Pudji Rochjati, dalam kegiatan SABU- SABU (Satu ibu hamil satu ibu kader). Meskipun demikian, dalam pengumpulan data dilakukan secara manual dan berjenjang membuat data terkumpul secara utuh dalam waktu satu bulan. Waktu satu bulan ini membuat keputusan penting untuk intervensi dini lebih lambat dilakukan. Solusi yang ditawarkan adalah pengembangan aplikasi digital sederhana yang dapat dioperasikan oleh kader kesehatan, tetapi dapat terkoleksi oleh pengelola program kesehatan ibu dan anak di Puskesmas secara real time, sehingga keputusan intervensi yang dilakukan dapat dilakukan secara responsif. Di akhir kegiatan pengabdian telah tersusun aplikasi SI-Bulan (Sistem Informasi Ibu Hamil Lancar Bersalin). Aplikasi SI-Bulan sebagai alat bantu digital, dan waktu yang diperlukan untuk pelaporan bisa didapatkan saat itu juga.

Abstract

Meskipun Rekam Medis Elektronik (RME) berpotensi meningkatkan efisiensi dan keselamatan pasien, hambatan implementasi sering kali menghalangi tercapainya kualitas layanan yang optimal. Studi ini bertujuan mengidentifikasi, mengkategorikan, dan mensintesis hambatan utama yang membatasi efektivitas RME dalam meningkatkan kualitas layanan rumah sakit. Tinjauan literatur sistematis dilakukan mengikuti pedoman PRISMA 2020 dengan menganalisis 28 studi empiris yang diterbitkan dalam bahasa Inggris antara tahun 2020 hingga 2024 dari basis data Scopus dan PubMed. Sintesis naratif mengidentifikasi delapan tantangan utama, yaitu masalah kegunaan dan beban kerja; kesenjangan pelatihan; risiko interoperabilitas dan keamanan; tata kelola yang lemah; kendala infrastruktur; resistensi adopsi; manajemen perubahan yang tidak memadai; serta implikasi terhadap keselamatan pasien. Oleh karena itu, efektivitas implementasi RME bergantung pada strategi multidimensi di luar aspek teknis. Penguatan tata kelola, jaminan interoperabilitas, pelatihan berkelanjutan, serta kepemimpinan yang kuat dan manajemen perubahan proaktif sangat penting untuk mengatasi resistensi budaya dan memaksimalkan kontribusi RME terhadap kualitas layanan rumah sakit.   Despite the potential of Electronic Medical Records (EMR) to enhance hospital efficiency and patient safety, implementation barriers often hinder optimal service quality. This study identifies, categorizes, and synthesizes the primary barriers limiting EMR effectiveness in improving hospital service quality. A systematic literature review was conducted following the PRISMA 2020 guidelines, analyzing twenty-eight peer-reviewed empirical studies published in English between 2020 and 2024 retrieved from Scopus and PubMed. The narrative synthesis revealed eight dominant themes posing major challenges: usability and workload issues; training gaps; interoperability and security risks; weak governance; infrastructure constraints; resistance to adoption; insufficient change management; and negative implications for patient safety. Consequently, the effectiveness of EMR implementation depends on multidimensional strategies beyond technical capabilities. Strengthening governance, ensuring secure interoperability, and providing continuous user training, alongside strong leadership and proactive change management, are essential to overcome cultural resistance and maximize EMR contributions to hospital service quality.

Abstract

Benign Paroxysmal Positional Vertigo (BPPV) is the most common cause of peripheral vertigo, characterized by brief episodes of vertigo due to otoconia displacement. Although most previous studies have focused on intrinsic factors such as age, gender, osteoporosis, and metabolic disorders, evidence regarding the role of environmental factors, particularly occupational noise exposure, is limited. Chronic noise has the potential to affect vestibular function through both sensory and vascular mechanisms. This study aims to narratively review the effect of occupational noise exposure on the risk of BPPV by integrating clinical, epidemiological, and experimental findings. The method used is a literature-based narrative review of the PubMed, Scopus, Web of Science, and Google Scholar databases without year restrictions, using the keywords "BPPV", "occupational noise exposure", "vestibular dysfunction", "VEMP", and "otoconia displacement". The search results obtained 25 relevant articles linking BPPV to otolith, hormonal, vascular, lifestyle factors, and occupational noise exposure. The results indicate that chronic noise can cause sensory damage (otoconia and vestibular hair cells), vascular disorders (hypertension, cardiovascular disorders, and inner ear microvascular circulation disorders), and exacerbate lifestyle comorbidities (sedentary lifestyle, osteoporosis, hypertension, diabetes). The discussion confirms that these multifactorial mechanisms explain the susceptibility of industrial workers to BPPV despite normal hearing function. The conclusion of this study is that workplace noise exposure has been shown to play a significant role as a risk factor for BPPV, therefore, prevention strategies, vestibular health monitoring, and healthy lifestyle interventions need to be optimized in occupational health programs.

Abstract

Furunkel hidung, infeksi akut folikel rambut di vestibulum nasi yang utamanya disebabkan Staphylococcus aureus, berisiko serius karena kedekatannya dengan vena fasialis dan sinus kavernosus, memicu komplikasi seperti trombosis sinus kavernosus. Studi terkini menyoroti faktor lingkungan seperti polusi PM2.5 dan debu industri yang melemahkan barier mukosa hidung serta menginduksi disbiosis mikrobiota. Tinjauan naratif ini mensintesis literatur 2020-2025 untuk menguraikan interaksi ini.​ Penelitian bertujuan menguraikan kontribusi risiko lingkungan terhadap furunkel hidung melalui bukti klinis, epidemiologis, dan eksperimental, fokus pada interaksi host-mikrobiota-lingkungan. Menggunakan metode narrative review kualitatif, pencarian dilakukan di PubMed, Scopus, Google Scholar, dan ScienceDirect dengan kata kunci seperti "furunkel hidung" dan "faktor risiko lingkungan". Populasi adalah artikel peer-reviewed (2020-2025); sampel studi memenuhi kriteria inklusi (etiology, patogenesis). Instrumen pencarian literatur elektronik; analisis data sintesis naratif dan pengelompokan tematik. Hasil menunjukkan PM2.5 memicu disbiosis nasal, stres oksidatif, dan gangguan clearance mukosilier, bersinergi dengan faktor host seperti diabetes meningkatkan dominasi S. aureus dan kekambuhan. Kesimpulannya, pencegahan memerlukan pendekatan holistik di luar antibiotik, termasuk pengendalian polusi, edukasi higienis, dan modulasi mikrobiota untuk kurangi komplikasi.

Abstract

Penelitian ini bertujuan untuk mengevaluasi efisiensi dan efektivitas biaya penggunaan metode C-ARM dan Non C-ARM dalam penanganan fraktur radius di RSU Islam Boyolali. Penelitian ini menggunakan metode Activity-Based Costing (ABC) untuk menghitung biaya berdasarkan aktivitas yang terlibat dalam prosedur medis dan Cost Effectiveness Analysis (CEA) untuk membandingkan biaya dan hasil klinis dari kedua metode tersebut. Sampel yang digunakan periode 2023-2025 dengan 69 pasien non C-ARM dan 100 pasien C-ARM. Hasil penelitian menunjukkan bahwa penggunaan C-ARM memiliki biaya yang lebih tinggi dibandingkan dengan Non C-ARM, terutama pada biaya tindakan operasi dan radiologi, namun C-ARM menawarkan efisiensi waktu yang lebih baik serta hasil klinis yang lebih optimal. Hal ini membuat RSU Islam Boyolali perlu mempertimbangkan penggunaan C-ARM untuk kasus-kasus yang memerlukan akurasi tinggi meskipun dengan biaya yang lebih tinggi, sementara Non C-ARM dapat digunakan untuk kasus yang lebih sederhana dengan efisiensi biaya yang lebih baik. Penelitian ini memberikan kontribusi terhadap pengelolaan sumber daya rumah sakit melalui pendekatan yang lebih terperinci dalam analisis biaya dan efektivitas, sehingga mendukung pengambilan keputusan yang lebih baik dalam penetapan tarif dan pemilihan metode pengobatan yang optimal.

Abstract

This study investigates how job demands and job rewards influence hospital service quality, and whether employee satisfaction mediates these relationships among staff at Prof. Dr. R. Soeharso Orthopedic Hospital, Surakarta. Using an explanatory quantitative design, Likert-scale questionnaires were collected from 235 medical and non-medical employees (n=235) through voluntary response sampling and analyzed with PLS-SEM (SmartPLS) to test a model comprising job demands (X1), job rewards (X2), employee satisfaction (Z), and service quality (Y), operationalized with demand, reward, satisfaction, and SERVQUAL-based service quality indicators. The measurement model met adequacy criteria (AVE>0.50; composite reliability=0.875–0.935; Cronbach’s alpha>0.80) and showed acceptable collinearity (VIF<5). In the structural model, job demands had a negative but non-significant direct effect on service quality (β=-0.025; t=0.415; p>0.05), while job rewards had a positive, significant direct effect (β=0.281; t=3.405; p=0.001). Job demands significantly reduced employee satisfaction (β=-0.198; t=3.455; p=0.001), whereas job rewards strongly increased satisfaction (β=0.759; t=26.836; p<0.001). Employee satisfaction positively predicted service quality (β=0.389; t=4.210; p<0.001) and mediated both exogenous variables’ effects on service quality; effect-size results indicated a large contribution of rewards to satisfaction (f²=1.564). The model explained 63.3% of the variance in employee satisfaction (R²=0.633; Q²=0.439) and 40.5% in service quality (R²=0.405; Q²=0.280). Theoretically, the findings extend job demands–resources logic in a hospital context by evidencing satisfaction as a key mechanism linking work conditions to perceived service quality. Practically, hospital managers should calibrate workloads and institutionalize fair financial and non-financial rewards to sustain satisfaction and improve service delivery. Overall, the study shows that reward strengthening and demand management are strategic levers for consistently high-quality hospital services.

Abstract

Latar Belakang: Rumah sakit merupakan penghasil limbah medis terbesar yang berisiko tinggi terhadap kesehatan dan lingkungan jika tidak dikelola dengan tepat. Meskipun regulasi nasional telah menetapkan standar ketat, efektivitas pelaksanaannya masih bervariasi antara rumah sakit pemerintah dan swasta karena perbedaan kebijakan, sumber daya, dan tantangan operasional. Penelitian ini bertujuan menganalisis sistem, membandingkan efektivitas, serta mengidentifikasi faktor pendukung dan penghambat pengelolaan limbah padat medis di rumah sakit. Metode Penelitian: Penelitian kuantitatif dengan pendekatan analitik. Pengumpulan data dilakukan melalui observasi, kuesioner, dan telaah dokumen terkait pengelolaan limbah padat medis. Analisis data dilakukan secara deskriptif dan inferensial untuk menilai tingkat efektivitas pengelolaan limbah serta hubungan antarvariabel yang diteliti. Hasil Penelitian: Berdasarkan hasil analisis statistic menunjukkan pengetahuan responden tergolong baik dengan (median 7,75/9). Abstract

Purpose: This study aims to examine the adoption, hospital integration, and revenue implications of telemedicine, with specific emphasis on Indonesia where hospital-based telemedicine services remain less developed than commercial platforms. The review focuses on identifying key drivers of user acceptance, organizational readiness, and financial outcomes to support strategic telemedicine development in hospitals.Methodology: A systematic review was conducted using a comprehensive search of open-access empirical studies published from 2020 to 2025. Twenty-six eligible articles were identified and analyzed. Data were synthesized thematically across three domains: factors influencing user adoption, hospital-level implementation processes, and the economic impact of telemedicine services. The review integrates both global and Indonesian evidence to provide a comparative perspective.Results: Findings show that perceived usefulness, trust, and digital readiness consistently shape patient and provider adoption of telemedicine. Successful hospital integration is supported by adequate infrastructure, clear governance, and alignment with clinical workflows. Several studies report cost-effectiveness and increased service utilization, indicating potential for enhanced hospital revenue, although economic evidence from Indonesian hospitals is still emerging.Applications/Originality/Value: This review offers practical insights for policymakers and hospital administrators planning long-term institutionalization of telemedicine. It is original in combining global and Indonesian contexts while emphasizing the financial sustainability of hospital-based telemedicine services.

Abstract

Type 2 diabetes mellitus (T2DM) is increasingly conceptualized as a complex psychoneuroendocrine disorder. While acute physiological stress typically induces hypercortisolemia, chronic diabetes distress may lead to allostatic overload and Hypothalamic-Pituitary-Adrenal (HPA) axis blunting. The compensatory role of oxytocin, a stress-buffering neuropeptide, remains underexplored in this specific clinical phenotype. This study aimed to characterize the neuroendocrine phenotype of patients with uncontrolled T2DM and severe distress, specifically investigating the synchronization between serum cortisol and oxytocin as a marker of homeostatic regulation. In this analytic cross-sectional study, 86 patients with uncontrolled T2DM (HbA1c greater than or equal to 7.0%) and severe diabetes distress (Diabetes Distress Scale-17 mean score greater than or equal to 3.0) were recruited via purposive sampling. Strict exclusion criteria were applied for exogenous steroid use and renal failure to ensure biological validity. Fasting morning serum cortisol and oxytocin were analyzed using Enzyme-Linked Immunosorbent Assays (ELISA). Statistical analysis utilized Spearman’s rank correlation and multivariate linear regression, controlling for Body Mass Index and age. The cohort exhibited severe metabolic dysregulation (Mean HbA1c: 10.03 ± 2.10%) and psychological distress (Mean DDS: 4.65 ± 0.35). Paradoxically, mean morning cortisol was low-normal (170.32 ± 135.43 nmol/L), suggestive of HPA axis blunting rather than the expected hypercortisolemia. A robust positive correlation was observed between cortisol and oxytocin (r = 0.555, p < 0.001). Multivariate regression confirmed cortisol as a significant independent predictor of oxytocin levels (b = 0.521, p < 0.001), independent of metabolic confounders. In conclusion, patients with severe diabetes distress display a distinct phenotype of HPA axis exhaustion coupled with synchronized oxytocinergic activity. This suggests a preserved reactive mobilization mechanism where oxytocin is upregulated in tandem with adrenal output to buffer chronic allostatic load. These findings highlight the potential of the oxytocinergic system as a therapeutic target in diabetes burnout.

Abstract

ABSTRAK Pada era digitalisasi saat ini, kepemimpinan dan tantangan digitalisasi sering menjadi permasalah banyak rumah sakit  Indonesia. Terkait permasalahan tersebut maka perlu dilakukan kegiatan pengabdian masyarakat dengan tema yang dilaksanakan untuk menjawab permasalahan kepemimpinan dan tantangan digitalisasi yang dihadapi RS PKU Karanganyar. Program pengabdian masyarakat ini bertujuan memperkuat kompetensi manajerial dan kepemimpinan kepala unit melalui pengembangan keterampilan kepemimpinan transformatif, komunikasi efektif, pengambilan keputusan berbasis data, serta sinergi multidisiplin. Pengabdian yang dilakukan menggunakan metode workshop, Focus Group Discussion (FGD), penyusunan modul pelatihan, serta pengembangan Standart Operational Procedure  SOP terkait kepemimpinan dan digitalisasi rumah sakit. Hasil kegiatan menunjukkan peningkatan signifikan pada pengetahuan dan keterampilan peserta. Peningkatan yang signifikan salah satunya terlihat dari kenaikan skor post-test dibandingkan skor pre test. Selain itu, peningkatan kualitas diskusi dan tersusunnya SOP merupakan luaran yang dapat dihasilkan pada kegiatan ini. Keberlanjutan kegiatan salah satunya dapat dilihat dari implementasi serta monitoring penggunaan SOP terkait kepemimpinan dan digitalisasi di rumah sakit. Kegiatan ini juga menghasilkan forum kolaborasi lintas unit yang memperkuat koordinasi berkelanjutan. Pada evaluasi akhir dapat disimpulkan bahwa program berhasil mencapai tujuan penguatan kepemimpinan serta mendorong budaya kerja kolaboratif. ABSTRACT In today's era of digitalisation, leadership and the challenges of digitalisation are often issues in Indonesian hospitals. In relation to these issues, it is necessary to carry out community service activities with themes that address the leadership and digitalisation challenges faced by PKU Karanganyar Hospital. This community service programme aims to strengthen the managerial and leadership competencies of unit heads through the development of transformative leadership skills, effective communication, data-based decision making, and multidisciplinary synergy. The community service was carried out using workshops, focus group discussions (FGDs), the development of training modules, and the development of standart operasional procedure (SOPs) related to hospital leadership and digitalisation. The results of the activity showed a significant increase in participants' understanding and skills. One of the significant improvements was seen in the increase in post-test scores compared to pre-test scores. In addition, the improvement in the quality of discussions and the formulation of SOPs were the outputs that could be produced in this activity. The sustainability of the activity can be seen from the implementation and monitoring of the use of SOPs related to leadership and digitalisation in hospitals. This activity also resulted in a cross-unit collaboration forum that strengthened ongoing coordination. The final evaluation concluded that the programme successfully achieved its objectives of strengthening leadership and promoting a collaborative work culture.
