Abstract

Stunting masih menjadi masalah kesehatan masyarakat di Indonesia dengan prevalensi tinggi pada anak balita dan penurunan yang relatif lambat. Faktor penyebab mencakup gizi buruk, infeksi berulang, rendahnya pendidikan, kondisi sosial ekonomi, serta keterbatasan akses layanan kesehatan. Di Kecamatan Gatak, Sukoharjo, masalah stunting juga dipengaruhi oleh perilaku makan anak, termasuk fenomena Gerakan Tutup Mulut (GTM) yang berdampak pada status gizi dan tumbuh kembang balita. Kader posyandu memiliki peran strategis dalam deteksi dini dan pendampingan keluarga melalui edukasi serta konseling. Program pengabdian masyarakat di Desa Trangsan dilakukan dengan pelatihan pengukuran antropometri, edukasi status gizi, serta strategi penanganan GTM, diikuti oleh 31 kader posyandu. Hasil evaluasi menunjukkan peningkatan signifikan pengetahuan kader: pada tema status gizi nilai rata-rata meningkat dari 80,00 menjadi 90,32 (p=0,007), dan pada tema GTM dari 60,00 menjadi 87,74 (p=0,000). Pelatihan ini terbukti efektif meningkatkan kapasitas kader posyandu sebagai garda terdepan pencegahan stunting di masyarakat.

Abstract

Kinerja karyawan dapat menunjang organisasi dalam mencapai tujuannya. Rumah sakit merupakan salah satu organisasi yang memer-lukan kinerja karyawan yang baik sehingga terwujud peningkatan efek-tivitas, efisiensi, maupun kualitas yang tinggi dalam menjalankan pela-yanan terhadap pasien. Penelitian ini merupakan studi kuantitatif yang bertujuan untuk menyelidiki dampak komitmen dan budaya organisasi terhadap kinerja karyawan, dengan mempertimbangkan Organizational Citizenship Behavior (OCB) sebagai mediator. Pengumpulan data dilakukan secara primer melalui kuesioner yang diisi oleh responden, dan pengujian hipotesis memanfaatkan metode Structural Equation Model berbasis Partial Least Squares (PLS) serta bootstrap resampling. Temuan penelitian ini mengindikasikan adanya pengaruh langsung dan signifikan dari komitmen organisasi dan OCB terhadap kinerja karyawan. Namun, budaya organisasi tidak secara langsung memengaruhi kinerja karyawan secara signifikan. Lebih lanjut, OCB ternyata menjadi penghubung penting, memediasi hubungan antara komitmen organisasi serta budaya organisasi dengan kinerja karyawan. Untuk mencapai kualitas pelayanan rumah sakit yang lebih baik, temuan ini menggarisbawahi perlunya peningkatan komitmen organisasi, penguatan budaya organisasi yang positif, dan pendorong perilaku OCB. Inisiatif-inisiatif ini akan berkontribusi pada peningkatan kinerja karyawan secara menyeluruh.

Abstract

This study evaluates the effects of service quality, pricing, location, and promotion on decision-making regarding taking laboratory examinations in a clinical lab clinic in Solo City. Given the rapid pace of development in both technology science and healthcare needs, these factors must be understood to improve the competitiveness and performance of clinical laboratories. SEM data was collected from 100 respondents who were examined at the Solo City Clinical Laboratory using Smart PLS software. The results support the hypothesis that service quality, pricing, and promotion have a significant impact on patient trust, influencing their brand decision-making process. But on trust and judgment, that doesn't appear to be a factor. This study underscores the need for a holistic approach to patient engagement, recognizing the interrelated nature of these elements in constructing trust and decisions. Our study does offer rich perspectives that can help clinical laboratories develop best practices, especially in resource-limited settings that would Strive to provide universal access to high-quality diagnostic services. Generalization to other healthcare contexts and longitudinal studies of the relationships with patient loyalty/retention are warranted in future research.

Abstract

This study aims to systematically review the role of hospital image in building patient loyalty through a Systematic Literature Review (SLR) and bibliometric analysis approach. Articles were searched using a combination of Boolean keywords for the period 2020–2025. The screening process followed the PRISMA protocol, resulting in 30 articles that met the final criteria for thematic and bibliometric analysis using VOSviewer. The results showed four main clusters: (1) the relationship between hospital image, service quality, and satisfaction; (2) the reputation of doctors and institutions as determinants of trust; (3) brand image and patient experience as predictors of revisit intention; and (4) trust and service quality as the main mediating mechanisms. Publication trends increased rapidly after 2023, with the focus shifting from classic satisfaction-based models to a new paradigm emphasizing patient experience and trust. This study provides conceptual and practical contributions for hospitals in managing their image, strengthening their digital reputation, and building patient loyalty in a sustainable manner through services oriented towards patient experience and emotional value.

Abstract

Type 2 diabetes mellitus (T2DM) is increasingly conceptualized as a complex psychoneuroendocrine disorder. While acute physiological stress typically induces hypercortisolemia, chronic diabetes distress may lead to allostatic overload and Hypothalamic-Pituitary-Adrenal (HPA) axis blunting. The compensatory role of oxytocin, a stress-buffering neuropeptide, remains underexplored in this specific clinical phenotype. This study aimed to characterize the neuroendocrine phenotype of patients with uncontrolled T2DM and severe distress, specifically investigating the synchronization between serum cortisol and oxytocin as a marker of homeostatic regulation. In this analytic cross-sectional study, 86 patients with uncontrolled T2DM (HbA1c greater than or equal to 7.0%) and severe diabetes distress (Diabetes Distress Scale-17 mean score greater than or equal to 3.0) were recruited via purposive sampling. Strict exclusion criteria were applied for exogenous steroid use and renal failure to ensure biological validity. Fasting morning serum cortisol and oxytocin were analyzed using Enzyme-Linked Immunosorbent Assays (ELISA). Statistical analysis utilized Spearman’s rank correlation and multivariate linear regression, controlling for Body Mass Index and age. The cohort exhibited severe metabolic dysregulation (Mean HbA1c: 10.03 ± 2.10%) and psychological distress (Mean DDS: 4.65 ± 0.35). Paradoxically, mean morning cortisol was low-normal (170.32 ± 135.43 nmol/L), suggestive of HPA axis blunting rather than the expected hypercortisolemia. A robust positive correlation was observed between cortisol and oxytocin (r = 0.555, p < 0.001). Multivariate regression confirmed cortisol as a significant independent predictor of oxytocin levels (b = 0.521, p < 0.001), independent of metabolic confounders. In conclusion, patients with severe diabetes distress display a distinct phenotype of HPA axis exhaustion coupled with synchronized oxytocinergic activity. This suggests a preserved reactive mobilization mechanism where oxytocin is upregulated in tandem with adrenal output to buffer chronic allostatic load. These findings highlight the potential of the oxytocinergic system as a therapeutic target in diabetes burnout.

Abstract

Breast cancer remains the most common malignancy among women worldwide, and environmental exposures may contribute to breast carcinogenesis through inflammatory pathways such as NLR, PLR, and interferon-γ. This study aimed to examine the association between agricultural environmental exposure, represented by rice field area, and inflammatory biomarkers including the Neutrophil-to-Lymphocyte Ratio, Platelet-to-Lymphocyte Ratio, and Interferon-γ in breast cancer. A case–control method was applied to 128 histopathologically confirmed samples consisting of 100 malignant (Invasive Carcinoma NST) and 28 benign (Fibroadenoma Mammae) breast tumors; hematologic indices were derived from preoperative blood tests, IFN-γ expression was analyzed immunohistochemically, and rice field proportions near participants’ residences were obtained from national statistics, with data analyzed using Chi-square and multivariate logistic regression. The results showed that agricultural exposure (rice field ≥60%) was not significantly associated with breast cancer (p = 0.703), whereas elevated NLR (OR = 4.89, p = 0.014), PLR (OR = 12.13, p = 0.017), and IFN-γ expression ≥20% (aOR = 23.94, p = 0.049) were significantly associated with malignancy. In conclusion, although agricultural land area was not a direct risk factor, inflammatory and immune biomarkers—particularly IFN-γ—served as strong predictors of breast cancer, indicating their potential utility in risk stratification for agrarian populations.
