Abstract

Background:The rising prevalence of Type 2 Diabetes Mellitus (T2D) has become a significant global health burden, with profound implications for healthcare systems and affected individuals. Strengthening primary healthcare services by integrating health and community-based interventions is critical for improving health outcomes. This rapid systematic review aimed to evaluate the effectiveness of community-based interventions in enhancing health outcomes for patients with T2D. Objectives:To evaluate the effectiveness of community-based interventions in managing T2D, focusing on their impact on health outcomes Methods:This systematic review adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Data collection involved five major databases: Scopus, Cochrane Library, PubMed, Web of Science, and Clinical Key, covering publications from January 2013 to May 2024. Eligible studies included randomized controlled trials, mixed-methods research, clinical trials, prospective studies, experimental repeated-measures designs, and quasi-experimental designs. The risk of bias in the included studies was assessed using the RevMan 5.4.1 software. Article selection, risk of bias evaluation, data extraction, and synthesis were conducted independently by the authors, with discrepancies resolved through discussion and consensus. Results:Out of 4.463 identified articles, 13 met the eligibility criteria for inclusion in the review. The community-based intervention strategies identified in these studies involved three primary approaches: family-centered interventions, peer support, and community volunteer programs. Conclusion:This systematic review concludes that community-based interventions are effective in improving health outcomes for individuals with T2D. Integrating and strengthening health and community-based collaborations within the broader healthcare system is crucial for achieving optimal health outcomes. Nurses are encouraged to collaborate with community-based networks to bridge the gap between healthcare providers and communities, thereby enhancing public health outcomes. Abstract

Diare merupakan penyebab kematian nomor dua pada Balita di dunia. Karena itu, penting untuk mengetahui faktor risiko kejadian diare pada Balita. Tujuan dari penelitian ini adalah untuk mengetahui faktor risiko kejadian diare pada Balita usia 6-59 bulan di Pulau Sumatera. Penelitian ini menggunakan data sekunder Indonesian Family Life Survey (IFLS) 2014 yang dikumpulkan oleh Research And Development (RAND) Corporation dengan sampel sebanyak 1315 anak-anak di Pulau Sumatera. Penelitian ini menggunakan desain cross-sectional dan data dianalisis bivariat dengan uji statistik chi-square dan multivariat dengan uji statistik regresi logistik ganda. Hasil analisis bivariat menunjukkan bahwa variabel umur anak (Pv=0,006; OR=1,525), Jenis kelamin (Pv=0,019; OR=1,433), dan Imunisasi campak (Pv=0,005; OR=1,531) merupakan faktor resiko diare pada Balita. Hasil Analisis multivariat menunjukkan bahwa jenis kelamin merupakan faktor risiko yang paling dominan dengan diare (Pv=0,018; OR=1,427). Jenis kelamin merupakan faktor risiko paling dominan dengan diare sehingga upaya untuk memberikan asupan gizi yang optimal pada anak, menjaga hygiene dan sanitasi lingkungan yang baik, pola asuh yang baik, pemberian imunisasi sangat penting untuk mempertahankan status gizi anak agar normal dan terhindar dari diare.

Abstract

Menurut World Health Organization (WHO) tahun 2018, 24% dari populasi dunia terinfeksi kecacingan. Di Indonesia, kecacingan masih menjadi masalah kesehatan masyarakat dengan prevalensi kecacingan pada siswa sebesar 60-80%. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan kejadian kecacingan pada siswa SD terpilih di Kecamatan Cihara, Kabupaten Lebak Tahun 2020. Penelitian ini menggunakan desain studi cross sectional dengan total sampling sebanyak 152 siswa. Hasil peneltian menunjukkan hubungan yang bermakna antara status gizi (Pv=0,006; OR=3,117), pendidikan ibu (Pv=0,039; OR=2,647), pendidikan ayah (Pv=0,031; OR=2,392), pekerjaan ayah (Pv=0,004; OR=3,123), penghasilan (Pv=0,046; OR=3,067), kebiasaan mencuci tangan (Pv=0,000; OR=9,556), kebiasaan buang air besar sembarangan (BABS) (Pv=0,000; OR=29,450), mengkonsumsi sayur mentah (Pv=0,013; OR=2,698), kebersihan kuku (Pv=0,034; OR=2,824), pemakaian alas kaki (Pv=0,011; OR=3,405), kepemilikan jamban (Pv=0,000; OR=31,333), sumber air minum (Pv=0,029; OR=8,824), sumber air bersih untuk masak dan minum (Pv=0,000; OR=8,095), konsumsi obat cacing dalam 1 tahun terakhir (Pv=0,000; OR=5,354) dengan kejadian kecacingan pada anak sekolah. Perlunya peningkatan promotif, preventif serta kuratif terkait infeksi cacing. Selain itu, masyarakat pun perlu meningkatkan personal hygiene dan menjaga sanitasi lingkungan rumah agar terhindar dari kontaminasi telur cacing.

Abstract

Anemia pada ibu hamil merupakan permasalahan kesehatan yang berdampak luas terhadap kesejahteraan ibu dan perkembangan janin, khususnya di negara berkembang seperti Indonesia. Data Riset Kesehatan Dasar (Riskesdas) tahun 2018 menunjukkan bahwa prevalensi anemia pada ibu hamil di Indonesia mencapai 48,9%. Kondisi anemia selama kehamilan dapat meningkatkan risiko komplikasi serius, seperti kelahiran prematur, bayi dengan berat badan lahir rendah, hingga meningkatnya angka morbiditas dan mortalitas ibu serta bayi. Salah satu upaya preventif yang direkomendasikan adalah konsumsi tablet tambah darah (TTD) yang mengandung zat besi dan asam folat. Namun, tingkat kepatuhan ibu hamil dalam mengonsumsi TTD masih menjadi kendala yang berkontribusi terhadap rendahnya kadar hemoglobin dan meningkatnya kejadian anemia. Penelitian ini bertujuan untuk mengevaluasi hubungan antara kepatuhan konsumsi TTD dengan kadar hemoglobin pada ibu hamil di Kecamatan Sawangan, Kota Depok. Metode penelitian yang digunakan adalah kuantitatif dengan desain cross-sectional, melibatkan 225 ibu hamil sebagai sampel penelitian. Analisis data dilakukan menggunakan Uji T-independent dan Uji Chi Square untuk mengidentifikasi hubungan antara kepatuhan konsumsi TTD dengan kadar hemoglobin serta kejadian anemia. Hasil penelitian mengungkapkan bahwa ibu hamil yang patuh mengonsumsi TTD memiliki kadar hemoglobin yang lebih tinggi dibandingkan dengan kelompok yang tidak patuh, dengan perbedaan yang signifikan secara statistik (p = 0,005). Hasil penelitian ini menegaskan bahwa kepatuhan dalam mengonsumsi TTD berperan penting dalam pencegahan anemia selama kehamilan. Oleh karena itu, diperlukan strategi efektif untuk meningkatkan kepatuhan ibu hamil, seperti edukasi kesehatan yang lebih optimal, pemantauan berkala oleh tenaga medis, serta dukungan keluarga agar konsumsi TTD dapat dilakukan secara konsisten guna menurunkan angka kejadian anemia pada ibu hamil.

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a global health problem with an increasing prevalence. Most related studies focus on Diabetes Self-Management (DSM) in urban populations or general contexts, leaving a gap in understanding the specific factors affecting DSM among rural T2DM patients. Therefore, there is limited understanding of the factors that influence DSM in rural T2DM patients.Purpose: This study aimed to investigate DSM in rural T2DM patients and its associated factorsMethods: This study employed a cross-sectional design. The samples involved 146 patients with T2DM from the community health center in Ogan Komering Ulu Regency, Indonesia, selected using a simple random sampling technique. Diabetes Knowledge Questionnaire (DKQ), Diabetes Management Self-Efficacy Scale (DMSES), Hensarling Diabetes Family Support Scale (HDFSS), and Diabetes Self-Management Instrument (DSMI) were used for data collection. The t-test, ANOVA, Pearson correlation, and multiple linear regression were used to analyze the data.Results: The study found a significant relationship between gender (p=0.001), knowledge (p=0.001), self-efficacy (p=0.001), and family support (p=0.032) with DSM in rural T2DM patients. However, the multiple regression analysis revealed that self-efficacy (β=0.392, p=0.001), gender (β=0.283, p=0.001), and duration of DM (β=-0.189, p=0.012) significantly affected  DSM in patients with T2DM in rural areas, explaining  28 percent of the variance  (R2=0.280).Conclusion: The results indicated a significant relationship between gender, knowledge, self-efficacy, and family support with DSM in T2DM patients in rural areas, with self-efficacy being the most dominant factor affecting DSM. This study recommends that community nurses can play a pivotal role in designing and implementing interventions enhancing self-efficacy. Additionally, the involvement of PROLANIS, collaborating with key community figures, is crucial to increasing community participation and support for effective diabetes self-management.

Abstract

Food safety culture (FSC) can be defined as how a group or organization of individuals achieve food safety in their thoughts and behaviors. Extending FSC implementation to downstream industries can assure quality and safety in the food supply chain. However, food distributors often lack a thorough understanding of good warehouse practices (GWP) and good distribution practices (GDP), even though both are essential to ensure the quality and safety of food. The inspection report by Indonesian Food and Drug Authority (Badan Pengawas Obat dan Makanan – BPOM) from 2017−2021, more than 30% of distribution facilities did not conform to the standard. This study evaluated employees' perception of FSC, analyze the Strengths, Weaknesses, Opportunities, and Threats of FSC implementation, and develop strategies for implementation in a food distribution company (PT XYZ). The research employed quantitative and qualitative methods. The quantitative study involved a questionnaire completed by 350 respondents, revealing that 82.13% of the employees perceived a positive perception of FSC. Meanwhile, the qualitative study included a focus group discussion with 21 informants, that identified key weaknesses and threats to FCS implementation. The Internal Factor Evaluation−External Factor Evaluation (IFE−EFE) analysis identified seven strengths, 11 weaknesses, seven opportunities, eight threats, and nine alternative strategies for the implementation of food safety culture at PT XYZ. The strategy with the highest total attractive score (TAS) of 6.91, based on quantitative strategic planning matrix (QSPM), was the periodic implementation of “Gemba program” to monitor food safety practices.

Abstract

In Indonesia, the prevalence of hypertension has increased significantly, from 25.8% in 2013 to 30.8% in 2023. The proportion of controlled hypertension reached only 19.8% in the productive age group and 17.7% in the elderly. This large number of uncontrolled hypertension cases contributes significantly to the increased risk of cardiovascular disease. The objective of this study was to ascertain the correlation between consumption patterns, physical activity, and obesity status with blood pressure in hypertensive patients in the work area of the Kemirimuka Village Health Center, located in Depok City. This is a quantitative study using a cross-sectional design. The sampling method used was non-probability total sampling, with a sample size of 178 respondents who met the inclusion criteria. The data collected were analyzed univariate and bivariate using the Chi-Square statistical test. The results showed that most of the hypertensive patients, 66.3%, had uncontrolled blood pressure. Statistical test results showed a significant inverse relationship between age and blood pressure (p=0.002 and OR=0.356). In addition, there was a significant relationship between the frequency of junk food consumption and blood pressure (p= 0.017 and OR= 2.269). While the variables gender (p=1.000), frequency of coffee consumption (p=0.154), physical activity (p=0.583), BMI (p=0.594) and BF% (p=0.517) were not associated with blood pressure in hypertensive patients. In conclusion, the elderly group had a 2.81 times lower risk of uncontrolled blood pressure than the productive age group, and frequent consumption of junk food was associated with a 2.269 times increased risk of uncontrolled blood pressure. Education about hypertension, with an emphasis on weight management and healthy eating, should be provided to all age groups. Keywords: Hypertension, Blood pressure, dietary patterns, physical activity, obesity ABSTRACT Di Indonesia prevalensi hipertensi meningkat signifikan, dari 25,8% di tahun 2013 meningkat menjadi 30,8% ditahun 2023. Proporsi hipertensi yang terkendali hanya mencapai 19,8% pada kelompok usia produktif dan 17,7% pada kelompok usia lanjut. Banyaknya jumlah kasus hipertensi yang tidak terkendali ini berkontribusi besar terhadap meningkatnya risiko penyakit kardiovaskular. Tujuan penelitian ini untuk mengetahui hubungan pola konsumsi, aktivitas fisik dan status obesitas dengan tekanan darah pada pasien hipertensi di wilalayah kerja Puskesmas Kelurahan Kemirimuka Kota Depok. Penelitian ini bersifat kuantitatif, dengan menggunakan desain cross-sectional. Metode pengambilan sampel yang digunakan adalah non-probability total sampling, dengan jumlah sampel sebanyak 178 responden yang memenuhi kriteria inklusi. Data yang terkumpul dianalisis secara univariat dan bivariat dengan menggunakan uji statistik Chi-square.  Hasil penelitian menunjukkan bahwa sebagian besar pasien hipertensi 66,3% memiliki tekanan darah tidak terkendali. Hasil analisis menunjukkan hubungan terbalik yang signifikan antara usia dan tekanan darah (p=0.002 and OR=0.356). Selain itu, terdapat hubungan yang signifikan antara frekuensi konsumsi junk food dengan tekanan darah (p= 0.017 and OR= 2.269). Sedangkan variabel jenis kelamin (p=1,000), frekuensi konsumsi kopi (p=0,154), aktivitas fisik (p=0,583), indeks massa tubuh (IMT) (p=0,594) dan persen lemak tubuh (PLT) (p=0,517) tidak berhubungan dengan tekanan darah pada pasien hipertensi. Kesimpulannya, kelompok usia lanjut memiliki risiko 2,81 kali lebih rendah untuk memiliki tekanan darah yang tidak terkontrol dibandingkan kelompok usia produktif, dan seringnya konsumsi junk food ditemukan berhubungan dengan peningkatan risiko 2,269 kali lebih tinggi untuk memiliki tekanan darah yang tidak terkontrol.  Edukasi tentang hipertensi, dengan penekanan pada manajemen berat badan dan pola makan sehat, harus diberikan kepada semua kelompok usia. Kata Kunci: Hipertensi, Tekanan darah, pola makan, aktivitas fisik, obesitas

Abstract

Diabetes, a leading cause of 6.7 million deaths in 2021, poses a significant challenge despite existing interventions. Non-adherence to treatment remains a barrier to diabetes management. However, a comprehensive instrument to assess medication adherence determinants in diabetes patients’ population in Indonesia with low-medium literacy levels and following the sociocultural characteristics of Indonesian society has been lacking. This study aimed to develop and evaluate a valid and reliable instrument for measuring medication adherence in type 2 diabetes mellitus patients based on the Borg and Gall model. Through the input of an expert panel, a valid and reliable instrument was developed, which comprised 21 questions and encompassed all medication adherence determinants, with a CVR and CVI value of 1 and a final Cronbach’s alpha value of 0.731. This instrument is still being tested and needs to be implemented in the right and wider population to obtain more accurate results.

Abstract

Dewasa ini pola makan modern sering dihubungkan dengan meningkatnya kadar kolesterol di dalam darah. Tingginya kolesterol darah dapat memicu munculnya penyakit degeneratif seperti stroke dan penyakit jantung koroner. Jenis makanan yang diduga berpengaruh terhadap timbulnya penyakit degeneratif yaitu makanan yang mengandung asam lemak jenuh dan asam lemak trans. Asam lemak trans memiliki pengaruh hampir 2 kali lipat dalam meningkatkan rasio K-LDL/K-HDL dibandingkan dengan asam lemak jenuh. Perubahan pada rasio kolesterol total/HDL-K atau K-LDL/K-HDL merupakan prediktor CHD (Coronary Heart Disease). Nowadays modern meal pattern is related to the increase of cholesterol level in the blood. The high blood cholesterol can lead to degenerative diseases such as stroke and coronary heart disease. Foods that suspected to influence the occurrence of degenerative diseases are saturated fatty acid and trans fatty acid. Trans fatty acid can influence almost twice higher in increasing ratio of K-LDL/K-HDL compared to saturated fatty acid. The change in ratio of total cholesterol/ HDL-K or K-LDL/K-HDL is predictor of coronary heart disease.

Abstract

The COVID-19 pandemic has significantly impacted adolescents' unhealthy eating habits and sedentary lifestyles, leading to an increase in blood pressure and obesity rate. High blood pressure among adolescents is an early indicator of adult hypertension, but this condition has received less attention recently. Therefore, this study aimed to determine the differences in dietary changes among high school students with normal and high blood pressure before and during the COVID-19 pandemic in West Lampung District, Indonesia. A cross-sectional design was used, and the sample population consisted of 167 students aged 14-19 years. The results showed that 43 participants had elevated blood pressure and stage 1 hypertension, accounting for 25.7% of the total population. Among these participants, 14 (32.6%) were found to be overweight and obese. Based on the results, students with high blood pressure were less likely to improve their dietary choices during the COVID-19 pandemic than those with normal blood pressure.
