Abstract

Introduction: Adolescents living in tourist areas are exposed to permissive, high-risk environments that increase their vulnerability to reproductive health problems. This study explored the experiences of adolescent girls, their families, and community leaders in maintaining reproductive health in tourism-dependent settings. Methods: A descriptive qualitative design was used with ten adolescent girls, eight family members, and two community leaders in an Indonesian tourist area. Participants were recruited through snowball sampling. Data were collected through in-depth interviews and field notes, audio-recorded, transcribed verbatim, and analyzed using Braun and Clarke’s six-phase thematic analysis. Results: The five themes of this research were (1) adolescents’ experiences of navigating sexual invitations, harassment, and pressure within tourism-driven environments; (2) disrupted daily routines and shifting academic priorities influenced by nightlife-oriented social interactions; (3) multi-layered strategies adolescents used to preserve their physical, social, emotional, and spiritual well-being; (4) intersecting barriers arising from personal hesitation, sociocultural norms, and structural limitations in accessing reproductive health services; and (5) adolescents’ aspirations for respectful, confidential, and youth-friendly reproductive health care that is accessible within tourism areas. Conclusions: Adolescent girls in tourist areas face interconnected reproductive health challenges that are influenced by individual behaviors, peer interactions, community exposure, and limited access to supportive health services. Strengthening family communication, improving adolescent-friendly services, and designing culturally sensitive, multilevel interventions are essential to enhance reproductive health outcomes among adolescents in tourism settings.

Abstract

Rectosigmoid cancer patients undergoing chemotherapy are at high risk of malnutrition due to metabolic alterations and treatment-related gastrointestinal side effects. Evidence on the implementation of standardized nutritional care in primary healthcare settings remains limited. This study aims to evaluate the provision of standardized nutritional care with a diet high in energy and protein and its impact on the intake, nutritional status, and clinical condition of rectosigmoid cancer patients undergoing chemotherapy. The case was a 50-year-old male with stage III rectosigmoid cancer undergoing chemotherapy, who presented with undernourished status, diarrhea, reduced appetite, dysphagia-related discomfort, and anemia. Nutritional assessment identified inadequate oral intake and increased energy and protein requirements. Standardized nutritional care was implemented using the Nutrition Care Process (NCP) framework, including an individualized high-energy, high-protein (HEHP) diet and nutrition education for the patient and family. Dietary intake was monitored over three consecutive days using 24-hour food recalls. The patient demonstrated improved tolerance to oral intake, increased protein consumption, stabilization of body weight, and no worsening of gastrointestinal symptoms during the intervention period. This case illustrates that standardized nutritional care using a high-energy, high-protein diet can support dietary intake and help maintain nutritional status in rectosigmoid cancer patients undergoing chemotherapy, even in a primary healthcare setting. Early and individualized nutritional intervention may play an important role in preventing further nutritional deterioration and supporting comprehensive cancer care.

Abstract

Background: Patient safety is a fundamental aspect of healthcare in hospital settings; however, implementing it remains challenging in nursing practice due to factors such as inadequate staffing, heavy workloads, poor communication, and limited resources. While previous studies have primarily focused on these structural and organizational barriers, there is limited understanding of nurses’ experiences in implementing patient safety programs, particularly in inpatient settings.Purpose: This study aimed to explore nurses’ experiences in implementing patient safety programs in hospital inpatient settings. Methods: A descriptive qualitative study was conducted among 22 ward nurses selected using purposive sampling. The inclusion criteria were nurses with at least one year of work experience and a minimum of a bachelor’s degree in nursing or equivalent professional nursing qualifications. Data were collected through focus group discussions (FGDs) and analyzed using thematic analysis. Results: Four main themes were identified: (1) understanding patient safety principles, (2) barriers to patient safety implementation, (3) leadership and organizational support for patient safety, and (4) technology as a facilitator of patient safety. Although nurses demonstrated an understanding of patient safety principles, they continued to encounter barriers, including variations in peer awareness of safety standards, workload constraints, and a blame culture. Furthermore, nurses reported anxiety and demotivation when reporting errors, highlighting the need for stronger leadership and organizational support. Finally, participants emphasized technology’s role as a key facilitator in achieving patient safety goals. Conclusion: Nurses’ commitment, patient safety–related workplace culture, and the availability of innovative technology remain critical factors influencing the implementation of patient safety programs. Communication barriers, limited managerial support, and challenges in sustaining patient safety practices persist. Therefore, hospital leadership should strengthen systems and policies, improve resource planning, enhance support and supervision, and provide continuous training to improve patient safety programs.
