Abstract

Pendahuluan: Terdapat sejumlah virus yang dapat menyebabkan infeksi daerah perioral, mulut, dan orofaring. Virus dapat menginisiasi infeksi, menyebar ke seluruh tubuh, dan bereplikasi berdasarkan sifat dan karakteristik virulensinya. Penyakit akibat virus dapat menyebabkan kerusakan sel secara langsung maupun dapat berupa reaksi sekunder. Virus yang menyerang rongga mulut dapat bermanifestasi diantaranya seperti vesikula dan ulser. Penelitian ini bertujuan untuk mengetahui prevalensi dan pola penyakit infeksi virus rongga mulut di RSUP Dr. Hasan Sadikin Bandung tahun 2013-2017. Metode: Penelitian deskriptif dengan menggambarkan prevalensi dan pola penyakit infeksi rongga mulut. Data yang dikumpulkan merupakan data sekunder berbentuk rekam medis pasien. Pengambilan sampel dilakukan dengan cara purposive sampling, dengan kriteria inklusi yaitu pasien dengan diagnosis infeksi virus rongga mulut di RSUP Dr. Hasan Sadikin Bandung tahun 2013-2017. Hasil: Penelitian ini menunjukkan total pasien rawat inap berjumlah 742 orang, didapatkan 21 pasien dengan infeksi virus rongga mulut, yang paling banyak ditemukan yaitu HSV-1 (66,67%), diikuti oleh infeksi HSV-2 (4,76%), CMV (9,52%), dan VZV sebanyak (19,05%). Sedangkan pada pasien rawat jalan yang berjumlah 531 orang didapatkan 58 pasien dengan infeksi virus rongga mulut, infeksi HSV-1 juga paling banyak ditemukan (91,38%), diikuti oleh infeksi CMV (1,72%), dan VZV (6,90%). Simpulan: Prevalensi infeksi virus rongga mulut sebanyak 2,83% pada pasien rawat inap dan sebanyak 10,92% pada pasien rawat jalan, dengan jumlah tertinggi yaitu infeksi HSV-1.Kata kunci: Infeksi virus, orofasial, famili herpes virus ABSTRACTIntroduction: Several viruses can cause infection of the perioral region, mouth, and oropharynx. Viruses can initiate infections, spread throughout the body, and replicate based on their virulence characteristics. Viral diseases can cause cell damage directly or can trigger a secondary reaction. Viruses that attack the oral cavity can manifest, such as vesicles and ulcers. This study was aimed to determine the prevalence and patterns of oral viral infection in Dr Hasan Sadikin Hospital Bandung in 2013-2017. Methods: Descriptive study was performed by describing the prevalence and pattern of oral infections. Data collected was secondary data in the form of patient medical records. Sampling was carried out by purposive sampling, with inclusion criteria, namely patients with a diagnosis of oral cavity virus infection in Dr Hasan Sadikin Hospital Bandung in 2013-2017. Results: This study showed a total of 742 inpatients, 21 patients with oral cavity infections, the most common were HSV-1 (66.67%), followed by HSV-2 infection (4.76%), CMV (9.52%), and VZV infections (19.05%). Whereas in total 531 outpatients, 58 patients were found with oral cavity infections, HSV-1 infections were also found the most (91.38%), followed by CMV (1.72%), and VZV infections (6.90%). Conclusion: The prevalence of oral cavity infection is 2.83% in inpatients and as many as 10.92% in outpatients, with the highest number being HSV-1 infection.Keywords: Viral infection, orofacial, herpes virus family

Abstract

Pendahuluan: Saliva merupakan zat kaya biomarker yang sering digunakan sebagai indikator kesehatan tubuh. Saliva menunjukkan keadaan infeksi, status hormonal, imunologi, nutrisi, metabolisme, bahkan keganasan. Saat ini perkembangan alat diagnostik mengarah pada nanoteknologi, yang populer karena menawarkan prosedur non-invasif, akurat, dan efisien. Evaluasi terhadap deteksi penyakit mulut menggunakan saliva sebagai biomarker, khususnya yang menggunakan teknologi nano perlu dilakukan guna mengetahui sejauh mana metode tersebut membantu dalam ketepatan diagnosis penyakit mulut. Penelitian ini bertujuan untuk membahas peran biomarker saliva dalam mendeteksi penyakit mulut dengan metode nanoteknologi. Metode: Jenis penelitian systematic review. Pencarian research gap menggunakan strategi PICO. PubMed database digunakan untuk pencarian artikel penelitian, dan kata kunci yang digunakan berdasarkan MeSH. Pelaporan hasil riset systematic review menggunakan panduan PRISMA. Penilaian kualitas artikel dianalisis menggunakan QUADAS. Hasil: Deteksi penyakit mulut menggunakan biomarker saliva merupakan alternatif metode non-invasif, selain itu, dengan teknologi nano memperlihatkan hasil yang akurat dalam waktu singkat. Penyakit gigi dan mulut, baik kelainan genetik maupun patogenik, bersifat lokal maupun sistemik, dapat berkembang menjadi kelainan yang mengancam jiwa. Kondisi ini membutuhkan investigasi yang cepat, salah satu caranya adalah dengan mengoptimalkan peran pemeriksaan biomarker saliva menggunakan nanoteknologi. Simpulan: Penerapan nanoteknologi menggunakan biomarker saliva merupakan pilihan metode yang menjanjikan sebagai alat diagnostik baik di riset maupun klinis, khususnya dalam deteksi penyakit mulut.Kata kunci: Alat diagnostik, biomarker, nanoteknologi, penyakit mulut, saliva. ABSTRACTIntroduction: Saliva is a biomarker-rich substance often used as a health indicator of the human body. Saliva shows the state of infection, hormonal status, immunology, nutrition, metabolism, even malignancy. Currently, the development of diagnostic tools leads to nanotechnology, which is popular due to the non-invasive, accurate, and efficient procedures. Evaluation of the detection of oral diseases using saliva as a biomarker, especially those using nanotechnology, needs to be done to determine how this method helps in the accuracy of the diagnosis of oral diseases. This study was aimed to discuss the role of salivary biomarkers in the detection of oral diseases using nanotechnology methods. Methods: type of research is systematic review. Exploration for research gaps were using the PICO strategy, research articles were collected from Pubmed database, and keywords were determined based on MeSH. The results were reported using the PRISMA guidelines, and the quality of the articles was analysed using QUADAS. Results: Detection of oral diseases using salivary biomarkers was an alternative non-invasive method. Additionally, along with nanotechnology enable to show accurate results in a short time. Oral diseases, both genetic and pathogenic, occur locally or systemically and can develop into a life-threatening condition. This situation requires rapid investigation; one of the ways is by optimising the role of salivary biomarker assessment using nanotechnology. Conclusion: The application of nanotechnology using salivary biomarkers is a promising method option as a diagnostic tool in both research and clinical utility, especially in the detection of oral diseases. Abstract

Penelitian ini bertujuan untuk mengetahui gambaran umum tentang statuskesehatan gigi dan mulut pada murid-murid kelas I, II, dan III Sekolah Dasaryang berada di sekitar Klinik Kerja Mahasiswa FKG Unpad dengan melihatprevalensi karies dan indeks def-nya. Jenis penelitian ini adalah deskriptif.Sampel penelitian sebanyak 333 orang murid, yaitu murid-murid kelas I, II, danIII dari 4 buah Sekolah Dasar diambil secara acak, yang berlokasi di sekitar klinikkerja mahasiswa FKG Unpad. Hasil penelitian menunjukkan prevalensi kariesadalah sebesar 99,9% yang berarti hampir semua anak pada penelitian inimenderita karies, sedangkan indeks def adalah 10,2 artinya pada setiap anakyang diperiksa terdapat 10,2 gigi yang mengalami karies, ditambal, dan sudahdicabut atau diindikasikan untuk pencabutan. Abstract

chronic autoimmune disease with blisters and erosions in the skin and or mucous membranes as its clinical manifestation. The ruptured blisters, followed by erosions and ulcerations, in oral mucous can be painful and interferes the oral function. Spices and seasoning, rich in thiols and isothiocyanates group (garlic, red chillies, black pepper, coriander and cumin seeds), able to trigger the recurrence of pemphigus vulgaris. This case report discussed the allergenic food to tigger the oral lesions in a 49 years old male patient with the diagnosis pemphigus vulgaris who was referred from The Dermatolgy Department. Since about 4 days before admitted to the hospital, the patient complained of painful ulcerations and blisters in the oral cavity that appeared after consuming the spices and seasoning food. Oral lesions caused difficulty in speaking, eating, and drinking, thus led to weight loss. The patient was uncooperative during treatment. Extraoral examination revealed painful erosions and crust on lips which tend to bleed easily. Intraoral examination revealed painful multiple erosions which interfered oral function. The oral lesions showed improvement after being treated using topical corticosteroid, topical antiseptic, antifungal suspension, and multivitamin. Pemphigus vulgaris caused by acantholysis due to autoantibodies IgG against desmogleins 1 and or 3. Tiol and isothiocyanates compound in spices and seasonings of food is one of the triggering factor of recurrence in oral lesions of pemphigus vulgaris

Abstract

Oral mucositis is an inflammatory process and ulcerative of the oral mucosa due to chemotherapy and/or radiotherapy. The incidence and severity of oral mucositis might be influenced by a variety of risk factors, related to the host and treatment of the cancer. This aim of the case report is to evaluate risk factors that affect the severity of oral mucositis in two patients with nasopharyngeal cancer undergoing chemotherapy and/or radiotherapy. The first case, oral mucositis grade III and oral candidiasis to a 54-year old woman undergoing 16 times radiotherapy for nasopharyngeal cancer stage IV. The second case, oral mucositis grade I and oral candidiasis were found in a 55-year old man suffering from nasopharyngeal cancer stage IV who has been treated 10 times with chemotherapy and radiotherapy. Both patients had a similar age range and cancer stage. The differences of severity oral mucositis in both cases were suspected due to the host factors-related that are gender and nutritional status. In addition, intra-oral condition (poor oral hygiene, xerostomia), smoking habits and patient compliance may also affect the severity of oral mucositis in the second case. Treatment factors such as the type, dose and duration of chemotherapy t, might also affect the severity in both cases. Both patients were given chlorhexidine gluconate 0.2%, nystatin oral suspension, vitamin B12, and folic acid for treating oral mucositis. In conclusion, the understanding of risk factors oral mucositis is deemed necessary to control the severity and to provide an appropriate management to improve the quality of patients’ life.

Abstract

Background: Vitamin D has several roles, namely physiological function of calcium and bone metabolism, cell growth and differentiation, immune and cardiovascular function. Vitamin D deficiency can cause the risk of cancer. Head and neck cancer is one of the cancers that occur due to vitamin D deficiency in the body. This literature review was to asses and evaluate the impact and benefits of vitamin D supplementation for head and neck cancer.Discussion: A study found an association between vitamin D supplementation and a low risk of recurrence in conditions of high total vitamin D levels. Another study found a significant increase in quality of life (QOL). The primary mechanism of vitamin D action is mediated through binding of either 1,25(OH)2D3 (active form) or 25(OH)D (less active form) to the VDR, which is a member of the nuclear receptor superfamily of steroid and thyroid hormones with gene-regulatory and consequent anti-proliferative properties.Conclusions: Vitamin D supplementation provides a role in improving the condition of patients with head and neck cancer. Both in terms of suppressing recurrence and in terms of increasing quality of life

Abstract

Background: Estrogen plays a key role in human physiological processes. Polymorphisms of estrogen receptors have been implicated in the development of numerous diseases. The aim of this study was to evaluate the frequency of ERα gene Pvull and Xbal polymorphisms and assessing their association with CD4+ T-cell counts in HIV-infected children on highly active antiretroviral therapy.Methods: CD4+ T cell counts were determined using the FACS count system. ERα PvuII and XbaI polymorphisms were analyzed by PCR-RFLP.Results: This study enrolled 34 HIV-infected children on HAART. The frequencies of the PvuII and XbaI gene polymorphisms were PP 41,2%, Pp 26,5%, pp 32,4% and XX 35,3%, Xx 17,6%, xx 47,1% respectively. CD4+ T-cell counts were significantly associated with XbaI polymorphisms (p<0.05), but not PvuII polymorphisms (p>0.01).Discussion: Host genetic factor polymorphism is an important determinant of HIV  disease progression and treatment response. The ERα Pvull and Xbal polymorphisms can increase risk for the development of HIV-related complication,including oral diseases.Conclusion: The ERα gene XbaI polymorphism was significantly associated with CD4+ T-cell counts. It may explain the role of estrogen in the regulation of HIV replication. Studying human genetic variation in HIV-infected individuals is important to guide a new therapeutic approach.

Abstract

Background: Microcystic lymphatic malformation (MLM) is an abnormal congenital growth of lymphatic vessels. It may occur in any part of the human body, with 75 % cases found in the head and neck, including in oral cavity. This case report aimed to describe a case of MLM in a child who was successfully treated non-operatively with antibiotics and corticosteroid.Case Report: A 4-year-old child came to Oral Medicine Clinic of Dr. Hasan Sadikin General Hospital, Bandung, with a chief complaint lesions of the tongue since a year ago, accompanied by bleeding in the affected areas but not pain. Patient gave history of recurrent episodes of the symptoms since 7 months old. Result : Ultrasonography was performed and diagnosis of microcystic lymphatic malformation was established. Systemic oral corticosteroid, antibiotics, and multivitamins were given and the lesions showed significant improvement after 2-weeks treatment. Conclusion: Therapy of MLM with antibiotics and systemic oral corticosteroid significantly improved patient’s condition, and ultrasonography may be used as a diagnostic modality in MLM diagnosis.

Abstract

Background: Oral lichen planus (OLP) is a chronic autoimmune disease sprinkled by T cells on the oral mucosal surface. The goal of OLP treatment is to eliminate erythema, ulceration and relieve symptoms. Corticosteroids are the first line in the treatment of OLP, either systemically or topically. The problem arises when using topical steroids, namely the time of topical steroid attachment to the oral mucosa, especially in cases of OLP with clinical desquamative gingivitis. Some studies suggest that the use of individual gingival tray can overcome this problem. Objective: To provide information on topical steroid use in OLP cases with desquamative gingivitis clinical signs.Case Management: Women, age 31, complain that pain with burning on the lips and mouth is aggravated by spicy food. Intraoral examination shows irregular white plaques on the buccal, labial and dorsal mucosa of the tongue accompanied by diffuse erythema in the anterior gingiva of the upper and lower jaws. Patients diagnosed with OLP. Lesions improve 3 months after topical steroid administration, except lesions on the gingiva. Gingival individual tray is then used to obtain adequate attachment of topical steroids on the gingiva surface. The gingiva showed significant improvement after a month later.Conclusion: The use of topical steroid concoctions applied to the gingival individual tray is effective in treating OLP lesions especially with clinical signs of desquamative gingivitis.
