Abstract

Background: Oral microbiota has been suggested to have a role in the etiopathogenesis of oral cancer; however, the oral microbiota diversity in patients with oral cancer compared to healthy individuals remains unclear. Purpose: This scoping review aimed to provide an overview of the current evidence regarding the oral microbiota composition colonized in oral cancer patients and its comparison with healthy individuals. Reviews: This study was conducted according to the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines. Eligible studies were searched in PubMed, Scopus, Web of Science, and ScienceDirect databases from January 2015 to March 2022. A total of 20 relevant studies were included according to the inclusion and exclusion criteria, including 14 cross-sectional studies and 6 cohort studies. All studies have identified various oral bacteria, but only one study has detected viruses and parasites diversity. A variety of oral microbiota found were 6 phyla of bacteria, 6 phyla of viruses, 7 phyla of fungi, and 7 phyla of parasitic. Seventeen studies proved that oral microbiota compositions were statistically significant differences compared to healthy controls, but not in 3 studies. Conclusion: The majority of studies showed various oral bacteria in oral cancer patients which were statistically significant difference compared to healthy controls. This study indicates the need for more research to evaluate viruses and parasites composition and diversity in oral cancer patients. Moreover, future research should focus to clarify whether the changes of oral microbial composition as a community may play a critical role in the etiopathogenesis of oral cancer.

Abstract

Background: Vitamin D has several roles, namely physiological function of calcium and bone metabolism, cell growth and differentiation, immune and cardiovascular function. Vitamin D deficiency can cause the risk of cancer. Head and neck cancer is one of the cancers that occur due to vitamin D deficiency in the body. This literature review was to asses and evaluate the impact and benefits of vitamin D supplementation for head and neck cancer.Discussion: A study found an association between vitamin D supplementation and a low risk of recurrence in conditions of high total vitamin D levels. Another study found a significant increase in quality of life (QOL). The primary mechanism of vitamin D action is mediated through binding of either 1,25(OH)2D3 (active form) or 25(OH)D (less active form) to the VDR, which is a member of the nuclear receptor superfamily of steroid and thyroid hormones with gene-regulatory and consequent anti-proliferative properties.Conclusions: Vitamin D supplementation provides a role in improving the condition of patients with head and neck cancer. Both in terms of suppressing recurrence and in terms of increasing quality of life

Abstract

Background: Estrogen plays a key role in human physiological processes. Polymorphisms of estrogen receptors have been implicated in the development of numerous diseases. The aim of this study was to evaluate the frequency of ERα gene Pvull and Xbal polymorphisms and assessing their association with CD4+ T-cell counts in HIV-infected children on highly active antiretroviral therapy.Methods: CD4+ T cell counts were determined using the FACS count system. ERα PvuII and XbaI polymorphisms were analyzed by PCR-RFLP.Results: This study enrolled 34 HIV-infected children on HAART. The frequencies of the PvuII and XbaI gene polymorphisms were PP 41,2%, Pp 26,5%, pp 32,4% and XX 35,3%, Xx 17,6%, xx 47,1% respectively. CD4+ T-cell counts were significantly associated with XbaI polymorphisms (p<0.05), but not PvuII polymorphisms (p>0.01).Discussion: Host genetic factor polymorphism is an important determinant of HIV  disease progression and treatment response. The ERα Pvull and Xbal polymorphisms can increase risk for the development of HIV-related complication,including oral diseases.Conclusion: The ERα gene XbaI polymorphism was significantly associated with CD4+ T-cell counts. It may explain the role of estrogen in the regulation of HIV replication. Studying human genetic variation in HIV-infected individuals is important to guide a new therapeutic approach.

Abstract

Background: Microcystic lymphatic malformation (MLM) is an abnormal congenital growth of lymphatic vessels. It may occur in any part of the human body, with 75 % cases found in the head and neck, including in oral cavity. This case report aimed to describe a case of MLM in a child who was successfully treated non-operatively with antibiotics and corticosteroid.Case Report: A 4-year-old child came to Oral Medicine Clinic of Dr. Hasan Sadikin General Hospital, Bandung, with a chief complaint lesions of the tongue since a year ago, accompanied by bleeding in the affected areas but not pain. Patient gave history of recurrent episodes of the symptoms since 7 months old. Result : Ultrasonography was performed and diagnosis of microcystic lymphatic malformation was established. Systemic oral corticosteroid, antibiotics, and multivitamins were given and the lesions showed significant improvement after 2-weeks treatment. Conclusion: Therapy of MLM with antibiotics and systemic oral corticosteroid significantly improved patient’s condition, and ultrasonography may be used as a diagnostic modality in MLM diagnosis.

Abstract

Background: Oral lichen planus (OLP) is a chronic autoimmune disease sprinkled by T cells on the oral mucosal surface. The goal of OLP treatment is to eliminate erythema, ulceration and relieve symptoms. Corticosteroids are the first line in the treatment of OLP, either systemically or topically. The problem arises when using topical steroids, namely the time of topical steroid attachment to the oral mucosa, especially in cases of OLP with clinical desquamative gingivitis. Some studies suggest that the use of individual gingival tray can overcome this problem. Objective: To provide information on topical steroid use in OLP cases with desquamative gingivitis clinical signs.Case Management: Women, age 31, complain that pain with burning on the lips and mouth is aggravated by spicy food. Intraoral examination shows irregular white plaques on the buccal, labial and dorsal mucosa of the tongue accompanied by diffuse erythema in the anterior gingiva of the upper and lower jaws. Patients diagnosed with OLP. Lesions improve 3 months after topical steroid administration, except lesions on the gingiva. Gingival individual tray is then used to obtain adequate attachment of topical steroids on the gingiva surface. The gingiva showed significant improvement after a month later.Conclusion: The use of topical steroid concoctions applied to the gingival individual tray is effective in treating OLP lesions especially with clinical signs of desquamative gingivitis.

Abstract

Background: Oral mucositis (OM) is common in people undergoing radiotherapy, chemotherapy, or a combination of both. Pharmacological and low-level laser therapy (LLLT) are potential treatments to reduce pain and accelerate the healing process. Purpose: The aim of this case report is to describe the effectiveness of combining pharmacological therapy and LLLT for OM. Case: A 54-year-old man with post-hemi glossectomy diagnosed with tongue cancer was referred from the radio-oncology department with complaints of painful sores all over the oral cavity after receiving radiotherapy. The canker sores worsened when eating but felt more comfortable when drinking cold water. Intraoral examination revealed erythematous lesions accompanied by fibrinous pseudomembranes and yellowish-white plaques, which could be scraped off, leaving erythematous areas with pain. These lesions were present on the upper and lower labial mucosa and the right and left buccal mucosa. The diagnosis was grade-3 OM, according to the World Health Organization (WHO) criteria. Case management: Pharmacological therapy included 0.025% hyaluronic acid gargles three times a day and the use of LLLT with a wavelength of 976 nm. Conclusion: The combination of pharmacological therapy with LLLT was effective in maintaining and improving the condition of the oral mucosa by reducing pain and ultimately improving the patient’s quality of life.

Abstract

Recurrent intraoral herpetic (RIH) and recurrent aphthous stomatitis (RAS) are common oral disorders and these diagnosis are often mistaken for one another. The confusion associated with developing an accurate diagnosis is somewhat understandable since these two very different lesions share some common characteristics. Misdiagnosis may result in effective treatment, thereby worsening the lesion. We reported 4 cases to evaluate these similarities and differences, includes the patients history, information regarding initiating factors, frequency of lesions, relieving factors, aggravating factors, the physical examination, and the results of any indicated tests, which are important data to the diagnostic process of these two lesions. By considering these data, an accurate diagnosis of theses two common oral disordes can be developed. It’s concluded that RIH and RAS have some similarities and differences, which are very important to developing an accurate diagnosis and the treatment plan, therefore the dental and oral health professional must be knowledgeable in defferentiating between RIH and RAS.

Abstract

Introduction: Herpes simplex virus (HSV) and herpes zoster virus (HZV) are two members of the herpes virus family that can manifest in oral mucosa. Following primary infection, the viruses remain latent and may reactivate if the immune system becomes compromised. The purpose of this study was to know the prevalence and the site of manifestation of HSV and HZV infection in RSUP Hasan Sadikin Bandung from January 2007 until December 2011. Method: The method of this study was descriptive retrospective using secondary data and literature study. Data were taken from medical records of patients in RSUP Hasan Sadikin Bandung, from January 2007 until December 2011. The results were then presented in table and graphs. Result: The study’s result showed the prevalence of HSV infection from 2007-2011 was 6% while the prevalence of HZV infection was 46%. HSV and HZV infection occurred 41% at head and neck, 36% at trunk, 13% at extremities, 7% generalized, and 3% unspecified. The systemic diseases involved in these herpes viruses found in 20.3% patients. Conclusion: The conclusion of this study were the prevalence of HSV infection was low, while the prevalence of HZV infection was high. HSV infection frequently affected at head and neck, and HZV infection frequently affected the trunk. Systemic diseases involved in these herpes viruses were malignancies, neoplasm, internal organ disorders, skin diseases, blood disorder, and nerve disorder.

Abstract

Introduction: Oral mucositis is an inflammatory reaction of oral mucous membrane that often appears in cancer patients due to the chemotherapeutic agents, such as 5-fluorouracil (5-FU). The aim of this study was to describe the characteristic patients who receive 5-FU and had oral mucositis. Methods: This study was conducted on 41 patients with cancer receiving 5-FU chemotherapy at Dr Hasan Sadikin Hospital Bandung. The data was retrieved through interviews to find out patient’s characteristic; nutritional status examination by using body mass index measurement; and oral examination. Severity level was determined by using National Cancer Institute’s Common Toxicity Criteria scale, and the level of pain was measured by Numeric Pain Intensity Rating scale. Results: This research have shown 60,98% patient with cancer had received 5-FU chemotherapy treatment, and 44% with poor nutritional status (underweight). Oral mucositis was only found at non-keratinised mucous. The finding of this study was patients that receiving 5-FU chemotherapy treatment diagnosed with oral mucositis was on the 1st stadium (52%) and the 2nd stadium (44%) with the level of pain was on the mild level (48%) and moderate level (32%).Conclusion: Oral mucositis was found on patients with cancer that received 5-FU chemotherapy with a variety of characteristics, nutritional statuses, locations, levels of severity and pain.
