Impact of Hormonal Therapy Regimens and Adverse Drug Reactions on Quality of Life in Breast Cancer Patients at Dr. Moewardi Regional General Hospital, Surakarta Luthfiyanti, Niken; Ichsan, Burhannudin

Abstract

Breast cancer remains the most common malignancy among women worldwide, and hormonal therapy is a cornerstone in the management of hormone receptor-positive cases, although its impact on adverse drug reactions (ADRs) and quality of life (QoL) remains a concern. This study aimed to evaluate the association between hormonal therapy patterns, ADRs, and QoL in breast cancer patients. An observational analytic study with a crosssectional design was conducted at Dr. Moewardi Regional General Hospital, Surakarta, from January to June 2025, involving 64 patients selected through consecutive sampling. Data were collected from medical records and structured interviews. QoL was assessed using the EORTC QLQ-C30 questionnaire, and ADRs were evaluated using the FACT-ES instrument. Bivariate and multivariate logistic regression analyses were performed. Most patients were aged <50 years (60.9%) and diagnosed at an advanced stage (57.8%). No significant associations were found between sociodemographic or clinical characteristics and type of therapy, survival duration, or ADR severity (all p>0.05). The most common ADRs were fatigue (51.6%) and hot flushes (50%). QoL assessment showed good functional status (mean = 79.65) and low symptom burden (mean = 25.06), but poor global health status (mean = 26.30). Cancer stage (p = 0.010) and type of therapy (p = 0.027) were significantly associated with QoL. Multivariate analysis confirmed that advanced-stage cancer (OR = 0.31; 95% CI: 0.14-0.68; p = 0.004) and combination therapy (OR = 0.44; 95% CI: 0.21-0.92; p = 0.024) were independent predictors of poorer QoL. These findings highlight the importance of individualized treatment strategies to optimize patient quality of life.

Abstract

The National Health Insurance (JKN) program is threatened with a deficit and potential default in 2026 due to inefficiency and the risk of fraud in healthcare facilities. In the context of JKN, which uses a package system (INA-CBG), Clinical Pathway (CP) is a strategic instrument within the JKN system that has dual benefits for both Payers (BPJS Kesehatan) and Providers (Hospitals). For BPJS Kesehatan, CP primarily functions to ensure service quality and prevent undertreatment fraud, the practice of reducing services to benefit hospitals. This prevention ultimately achieves cost efficiency in the JKN program by avoiding payment of complication claims and readmissions arising from substandard services. On the hospital side, CP helps control real cost effectiveness (cost) to maintain efficiency. High CP compliance can reduce inefficiencies (e.g., resource overutilization), thereby reducing the gap in hospital losses under the INA-CBG package system. This indirectly reduces the incentive for hospitals to commit fraud. This study aims to synthesize comprehensive evidence regarding the impact of CP compliance on cost efficiency (Hospital Cost) from a provider perspective and its impact on the sustainability of JKN. The method used was a Systematic Review based on the PRISMA framework, analyzing 28 articles from hospitals in Indonesia (2016-2025) that discussed the impact of CP on costs and/or Length of Stay (LOS) as well as the quality of clinical services. During the snowballing and abstract screening stages, we also found that compliance was an important variable measured by the studies we reviewed. The results show that the level of CP compliance in Indonesia remains low; 80% of hospitals have compliance below 50%, which triggers service variations and the risk of fraud. CP compliance is consistently positively correlated with reduced Hospital Costs (thus increasing internal hospital efficiency) and accelerated LOS. In addition to cost efficiency, CP has also been shown to improve the quality of clinical services by reducing complications (SSI: Surgical Site Infection) and reducing the risk of readmissions. In conclusion, CP is an essential dual solution; improving CP compliance is a fundamental key to balancing hospital cost efficiency without sacrificing quality, while simultaneously safeguarding the financial sustainability of the JKN Program from the burden of unnecessary complication claims.

Abstract

Hypertension is a non-communicable disease with high prevalence and a substantial risk of serious complications if not properly managed. Patients’ perceptions of hypertension play a crucial role in shaping health-related behaviors, particularly medication adherence and the adoption of healthy lifestyles. This study aims to explore and analyze patients’ perceptions of hypertension, the factors influencing these perceptions, and their implications for disease management and treatment adherence. This study employed a qualitative phenomenological design. Data were collected through semi-structured in-depth interviews with 17 patients diagnosed with hypertension who attended routine follow-up visits at Nogosari Primary Health Care Center, Boyolali, Central Java, Indonesia. Participants were selected until data saturation was achieved. Data were analyzed using thematic analysis. The findings showed that all patients perceived hypertension as a dangerous disease, mainly due to the risk of complications such as stroke and heart disease. However, patients’ levels of understanding regarding the definition, disease mechanisms, and complications of hypertension varied. Hypertension management, as perceived by patients, included dietary regulation, physical activity, stress management, and regular medication use, although inconsistencies in implementation were reported. Better perceptions of hypertension tended to encourage improved medication adherence and healthier lifestyle behaviors. This study highlights the importance of continuous health education to enhance patients’ understanding and management of hypertension.

Abstract

Diabetes mellitus merupakan penyakit kronik yang ditandai oleh hiperglikemia. Kepatuhan minum obat pasien diabetes mellitus berpengaruh terhadap outcome klinik dan kualitas hidup mereka. Tujuan penelitian ini adalah untuk mengetahui hubungan antara kepatuhan terhadap HbA1C dan kualitas hidup pasien diabetes. Penelitian memiliki desain crossectional study dengan melibatkan pasien diabetes RSI Purwodadi sebagai sampel. Data diambil menggunakan kuesioner ProMAS untuk kepatuhan dan EQ-5D-5L untuk kualitas hidup. Pengambilan dilakukan pada Maret-April 2023. Sampel yang memenuhi kriteria inklusi yaitu pasien terdiagnosa minimal selama 2 tahun, rutin menggunakan antidiabetes oral dan terdaftar sebagai peserta BPJS. Jumlah responden yang terlibat dalam penelitian sebanyak 79 orang yang didominasi perempuan (72%). Uji Chi-Square tidak terdapat hubungan antara profil responden dengan kepatuhan (p>0,05). Kepatuhan terhadap HbA1C dan kualitas hidup memiliki hubungan yang signifikan (p<0,05). Uji korelasi Spearman rho menunjukkan bahwa tingkat hubungan memiliki kekuatan sedang (>0,5). Kepatuhan terhadap HbA1Cdan kualitas hidup pasien diabetes mellitus memiliki hubungan yang signifikan.

Abstract

Background: The Chronic Disease Management Program (Program Pengelolaan Penyakit Kronis/PROLANIS) is a strategic initiative implemented in primary health care center to improve the management of chronic diseases, including diabetes mellitus. This program emphasizes comprehensive, continuous, and patient-centered care. However, variations in program implementation at the primary care level may influence its effectiveness, highlighting the need for in-depth evaluation. Methods: This qualitative case study was conducted at Nogosari Primary Health Care Center, Boyolali. Informants included 15 diabetes mellitus patients participating in PROLANIS and 2 health care providers. Data were collected through observation, in-depth interviews, and document review, then analyzed using content analysis. Results: Most PROLANIS components, such as medical consultations, health education, physical activity, health monitoring, and SMS reminders, were implemented according to guidelines and showed positive contributions. Supporting factors included comprehensive medical services, effective support systems, and peer support among participants. However, barriers remained, including limited access to services and medications, suboptimal service quality, low motivation, awareness, and adherence among some participants, and underutilized home visit activities. Overall, PROLANIS encouraged healthier lifestyles, improved medication adherence, and increased patient knowledge. Conclusions: PROLANIS demonstrates significant potential in supporting diabetes mellitus management in primary health care center. However, improvements are needed in terms of service accessibility, program personalization, and patient engagement to enhance program effectiveness and sustainability
