Abstract

Kombinasi ekstrak bulbus bawang putih (Allium sativum Linn.) dan rimpang kunyit (Curcumma domestica Val.) dapat digunakan sebagai obat antidiabetes oral pada penderita diabetes melitus (DM) tipe 2, dan secara klinis telah terbukti dapat menurunkan kadar glukosa darah dengan dosis 2,4 g/hari. Penelitian klinis dilakukan untuk melihat efek antidiabetes kombinasi ekstrak dibandingkan dengan antidiabetik oral, glibenklamid. Subjek adalah usia >35 tahun dengan DM tipe 2 yang berobat ke poliklinik Penyakit Dalam dan Endokrin Rumah Sakit Hasan Sadikin Bandung periode November 2007–Agustus 2008 dan telah mendapat terapi gizi medis selama 2 minggu. Penelitian dilakukan secara paralel, acak, dan tersamar ganda. Penggunaan kombinasi ekstrak menunjukkan penurunan kadar glukosa darah puasa rata-rata 9,25 mg/dL, glukosa darah 2 jam postprandial (PP) 22,25 mg/dL, HbA1c 1,30%, serta insulin 12,57 mg/dL bila dibandingkan dengan baseline glibenklamid rata-rata kadar glukosa darah puasa 72,37 mg/dL, glukosa darah 2 jam PP 114,25 mg/dL, dan HbA1c 4,12%, tetapi meningkatkan insulin 3,34 mg/ dL. Kombinasi ekstrak tidak mempengaruhi fungsi hati, ginjal, dan profil hematologi. Kesimpulannya kombinasi ekstrak memiliki efek antidiabetes tetapi efek yang ditimbulkan tidak sebaik glibenklamid. [MKB. 2011;43(1):26–34].Kata kunci: Bawang putih, kunyit, diabetes melitus tipe 2, glibenklamid, glukosa darah Antidiabetic Effect of Garlic Extract (Allium sativum Linn.) and Curcumin Extract (Curcumma domestica Val.) Combination Compared to Glibenclamide in Type 2 Diabetes MellitusThe combination of garlic (Allium sativum Linn.) and curcumin extract (Curcumma domestica Val.) can be used as an oral antidiabetic in type 2 diabetes mellitus (DM) patients. The clinical trial has shown that the extract can decrease blood glucose at a dose of 2.4 g/day. This clinical trial  was conducted to explore the antidiabetic effect of garlic and curcumin extract combination compared to oral antidiabetics, glibenclamide. The subjects were >35 years old patients with type 2 DM who visited the internal and endocrine clinic of Dr. Hasan Sadikin Hospital Bandung and has been treated with medical nutrition therapy for 2 weeks during the period of November 2007–December 2008. This study was a parallel, randomized and double blind study. The garlic and curcumin extract combination decreased the mean value of fasting blood glucose 9.25 mg/dL, 2h PP blood glucose 22.25 mg/dL, HbA1c 1,30% and insulin 12.57 mg/dL  compared to baseline whereas glibenclamide decreased the mean value of fasting blood glucose to 72.37 mg/dL, 2h PP 114,25 mg/dL, HbA1c 4.12% and increased the insulin to 3.34 mg/dL. In conclusion, the extract combination has antidiabetic effect even though it is lower than the glibenclamide. Abstract

Gastrointestinal toxicity due to non-steroid anti-inflammatory drugs can be inhibited by the compounds that have antioxidant activity. Quercetin is a flavonoid that has antioxidant activity and protection effect against gastric ulcer. Chlorination of quercetin enhanced the antioxidant activity. This study aims to obtain the chlorinated derivative of quercetin and examine the protection effect against acetosal-induced gastric ulcer. Chlorination was done by the addition of chlorine at room temperature. Ulcer induction was carried out on rats by oral administration of acetosal. Incidences of gastric ulcer were determined by macroscopic and microscopic observation. Chlorination of quercetin with chlorine gas produced 6-chloroqueretin as major product. The protection effect against acetosal-induced gastric ulcer of this compound was higher than quercetin.Key words : quercetin, chlorination, gastric ulcer, NSAIDs

Abstract

The anti-inflammatory effectof dominant compound of mahkota dewa fruit (Phaleria macrocarpaScheff. Boerl), a hydroxyl benzophenon glucoside,to Wistar  female  rats  (using  λ-karagenan  as  inflammatory  inducer)  has  been investigated.   The inflammation was measured by pletismometer.  The test has been used physiology NaCl solution as a control andNaproxen at a dose of 22.5 mg/kg body weight as a standard. Anti-inflammatorytest was showed that the decreased  inflammation  of  the  isolate  at  a  dose  of  22.5  mg/kg  body  has  twice lower than the standard.Key words:  Phaleria  macrocarpa,  anti-inflammatory  activity,  hydroxyl  benzophenon glucoside

Abstract

Microorganisms were isolated from soil taken from Tangkuban Perahu mountain. Five strains were investigated in this study, designated TP1, TP2, TP3, TP4, and TP5, respectively. Morphological, biochemical and molecular identifications were conducted for all five strains. These isolates were shown to be closely related to Nocardia sp. YIM 65630 (90%), Streptomyces galbus (99%), Aspergillus unguis (86%), Paecilomyces marquandii (100%) and Nocardia niigatensis (95%), respectively. Production of antibacterial, antifungal and anticancer metabolites was done by fermentation. Screening for bioactivity of five isolates was done by testing the fermentation broth against resistant and pathogenic bacteria, fungi and T47D breast cancer cell line. TP2 strain showed the best bioactivity; the metabolite was purified by extraction with ethyl acetate. Antibacterial, antifungal and anticancer activities from the ethyl acetate extract of TP2 strain were tested by agar diffusion, microdilution and MTT. The extract was shown to be active against methicillin resistant Staphylococcus, methicillin sensitive Staphylococcus aureus, methicillin resistant coagulase negative Staphylococcus, vancomycin resistant Enterococcus, Escherichia coli, Microsforum gypseum with the minimum inhibitory concentration (μg/mL) and diameter of inhibition (mm): 150, 35; 150, 30; 300, 35; 300, 35; 300, 29; 4.7, 36, respectively. The IC50 value of the T47D cell line was 457 μg/mL.

Abstract

The aim of the study was to determine the antioxidant and antihyperuricemia activity of ethyl acetate extract of snake fruit (Salacca edulis Reinw.) var. Bongkok. The research methods used in this study comprised  of three stages. First stage, the isolation processes, consist ed of maceration, fractionation, and purification using several techniques of chromatography. The chemical structures of the isolated compounds were determined based on UV, IR, 1-D NMR, and 2-D NMR spectral data. The ethyl acetate extract of snake fruit var. Bongkok isolated was a new compound 2-methylester-1-H-pyrolle-4- carboxylic acid. In the second stage the antioxidant activity of the extract and the isolated compounds were measured by 1,1 diphenol  (DPPH) method. The antioxidant activity of the extracts and the isolated compounds were expressed as IC50, The ethyl acetate extracts at concentrations of 0.2, 2, 20, 200, 400, and 2000 µg/mL showed inhibition of 9.67, 4.47, 41.89, 96.06, 82.54, and 90.60 % respectively, with an IC50 of 1.6 µg/mL. Ascorbic acid standards at the same concentration range showed an  IC50 of 0.54 µg/mL. Meanwhile, at the same concentrations the 2-methylester-1-H-pyrolle-4-carboxylic acid showed free radical inhibition of 17.48, 21.48, 18.14, 31.87, and 62.34 % respectively, with an  IC50 of 3.27  µg/mL. During the third stage,  the antihyperuricemic properties of the extracts and the isolated compound were examinated in vitro using inhibition of xanthin oxidase method. The ethyl acetate extracts at concentrations of 0.01, 0.02, 0.2, 2, and 2000  µg/mL showed xanthin oxidase inhibition of 49.24, 49.58, 50.28 and 52.26 % respectively, with an  IC50 of 24.75 µg/mL. At the same concentrations, the 2-methylester-1-H-pyrolle-4- carboxylic acid, showed xanthin oxidase inhibition of 27.7, 30.5, 37.3, 50.27 and 50.55 % respectively, with an IC50 of 48.86 µg/mL.  Allopurinol as a standard drug showed an IC50 of 0.92 µg/mL.

Abstract

Hisbiscus surattensis L the local name “tamoenju” is utilized by ethnic Sumari (Central Sulawesi) as an antidiabetic herbal, but de nitive studies on ef cacy and safety are lacking. Therefore, this study was aimed to investigate the antidiabetic activity of ethanolic extract of the leaves and fractions using in vitro and in vivo models. Ethanol extract (EE) of the leaves of H. surattensis L was prepared by maceration. The ethanol extract was separated into three fractions by liquid/liquid extractions to yield n-hexane (HF), ethyl acetate (EAF) and water fraction (WF). Firstly, the extract and fractions were tested for its inhibitory activity on α-glucosidase in vitro. The results showed IC50 EE, HF and WF more than 5000 ppm while the IC50 fraction of EAF 3888.34 ppm. Then, oral glucose tolerance test (OGTT) of the extract/fractions were carried out in normal mice. On OGTT, the ethanol extract, EAF, and WF at doses of 150 and 300 mg/kg BW were able to inhibit increasing the level of glucose in the blood. The results showed that the extract and fraction of H. surattensis L leaves in vitro had no effect as α-glucosidase inhibitor but in vivo had the hypoglycemic effect especially on the fraction of ethyl acetate and the water fraction. Abstract

Dementia is increasing in the world which is a major cause of disability and dependence in the elderly. This causes the elderly can not do their daily activities so often live in a nursing home. It is important to know the factors associated with dementia to prevent and treat dementia with appropriate interventions. The objective of this study was to identify the demographic factors and disease history associated with dementia among elderly in nursing homes. The research method was cross sectional study. Sample were recruited from three nursing homes located in Bandung and Garut using purposive sampling technique for a-3 month period (n=163). Data were collected using questionnaire consisting of demographic data, disease history, and MMSE (Mini Mental State Examination). The analysis of data was performed using chi-square test, fisher test, and logistic regression analysis. In term of its association with dementia, low education had the higher odd ratio (OR: 5.90, 95% CI: 2.02-17.20, p=0.001) than unmarried status (OR: 4.78, 95% CI: 1.23-18.52, p=0.024) and stroke (OR: 0.23, 95% CI: 0.06-0.88, p=0.032). However, diabetes mellitus was identified as confounding variable (OR: 0.10, 95%CI: 0.01-1.01, p=0.051). In conclusion, low education, unmarried status, stroke, and diabetes mellitus were predictor factors of dementia among elderly in nursing homes. It is recommended to include effective treatment could be in the form of health education about management of stroke and diabetes, physical activity, improvement of nutritional adequate, and social activities to prevent loneliness.

Abstract

Zebrafish (Danio rerio) merupakan jenis ikan tropis berukuran kecil yang banyak ditemukandi negara India dan Asia Selatan. Hingga saat ini, zebrafish telah digunakan sebagai organismemodel untuk pengembangan berbagai model penyakit, studi toksikologi, studi neurobiologi,gangguan-gangguan metabolisme, dan pengembangan genetika. Beberapa keunggulan dari penggunaanzebrafish antara lain mekanisme adaptasi terhadap lingkungan, naluri yang cukup bervariasi,pola sirkadian, dan berbagai macam mekanisme adaptasi lainnya. Secara genetika, zebrafishjuga memiliki persamaan gen yang menyerupai manusia. Persamaan gen ini merupakan prosesevolusi yang terjadi selama ratusan juta tahun yang lalu. Sebagai tambahan, zebrafish juga memilikipersamaan-persamaan lainnya dengan manusia antara lain yaitu pada sistem saluran pencernaan,jaringan adiposa viseral, sistem otot rangka, dan sistem organ lainnya. Persamaan gen, sistemsaluran pencernaan, jaringan adiposa viseral, sistem otot rangka, dan sistem organ lainnya menjadikanlandasan utama untuk pengembangan dan eksplorasi berbagai macam model penyakit padamanusia. Beberapa model yang dapat diaplikasikan pada model zebrafish antara lain model obesitas,diabetes mellitus, gangguan jantung atau kardiovaskular, gangguan ginjal, gangguan lipid, penyakitperlemakan hati, dan beberapa model lainnya.

Abstract

Lebah madu merupakan golongan serangga yang dapat menghasilkan madu. Produk madu ini dihasilkan dari nectar yang diolah oleh lebah madu sebagai cadangan makanan bagi koloni lebah madu. Komposisi kimia yang terkandung pada madu menjadikan produk alam ini memiliki manfaat yang banyak. Salah satunya adalah manfaat untuk meningkatkan stamina. Peningkatan aktivitas dapat menurunkan stamina dan menimbulkan kelelahan. Penelitian ini bertujuan untuk melihat perbedaan efek anti lelah dari tiga jenis madu yang banyak digunakan di Indonesia, yaitu madu Apis mellifera, Apis cerana, dan Trigona sp. Pengujian anti lelah dilakukan dengan mengukur efek anti lelah secara fisik dan biokimia terhadap hewan coba mencit jantan dengan menggunakan metode Weight-loaded Forced Swimming Test (WFST). Madu digunakan pada pengujian dengan pemberian oral menggunakan dosis 10,40 g/kg yang dilanjutkan dengan pengamatan waktu aktivitas serta uji biokimia darah dengan parameter glukosa darah, asam laktat, serta glikogen pada otot dan hati. Hasil penelitian menunjukkan bahwa ketiga jenis madu yang diujikan dapat memberikan efek anti lelah dibanding kelompok kontrol, dimana madu Apis cerana menunjukkan hasil yang lebih baik dari dua kelompok sampel lainnya.

Abstract

Kanker merupakan penyakit yang ditandai dengan pembelahan sel yang tidak normal, tidak terkendali, dan dapat membentuk massa tumor, bersifat invasif, serta dapat mengalami metastasis. Sampai saat ini belum ada terapi yang dapat memberikan efikasi dan keamanan yang memuaskan. Pada penelitian ini, telah diuji efek khemopreventif dari ekstrak etanol lempuyang wangi (Zingiber aromaticum) terhadap mencit yang diinduksi kanker menggunakan DMBA. Pada kelompok yang diberi ekstrak lempuyang wangi, jumlah kanker yang terjadi pada akhir minggu ke-10 adalah sebanyak satu kanker, dan pada kelompok obat pembanding ditemukan 0 kanker, sedangkan pada kelompok kontrol ditemukan 33 kanker. Meskipun masih ditemukan kanker pada kelompok ekstrak uji, persentase kematian yang terjadi pada kelompok tersebut hanya sebesar 20%, lebih kecil dibandingkan dengan kelompok obat pembanding yaitu 60%, dan kelompok kontrol 60%.
