competitive nation by 2045, the nursing workforce plays a pivotal role in realizing this ambition (Efendi, 2024). Nurses form the backbone of the healthcare system, ensuring the delivery of quality care to communities across the archipelago (WHO, 2021). While Indonesia currently has a temporary surplus of nurses, this presents a strategic opportunity to compete in the global nursing labor market (Efendi et al., 2024). However, succeeding in this competitive arena requires critical investments in areas such as language proficiency, readiness to adapt to diverse healthcare systems, and mental preparation for working overseas (Pradipta et al., 2023).A resilient and globally competitive nursing workforce can drive transformative health sector reforms, directly contributing to the realization of Golden Indonesia 2045. Nurses trained for international roles can also return to Indonesia, bringing back valuable skills and experiences to strengthen the local healthcare system. This cyclical benefit underscores the importance of policies facilitating international mobility, such as streamlined licensing and certification processes. Additionally, partnerships with foreign governments and institutions are critical to ensure ethical recruitment practices and the protection of migrant nurses' welfare.To support Golden Indonesia 2045, the government and stakeholders must prioritize comprehensive strategies to strengthen the nursing workforce. Key initiatives should include scaling up nurse education and training programs to align with international needs, enhancing workforce planning to address global demand, and establishing robust career development pathways to retain skilled nurses. By addressing these challenges, Indonesia can build a resilient and globally competitive nursing workforce, capable of advancing health sector reforms, contributing to human capital development, and supporting the nation's overarching goal of inclusive and sustainable progress.

Abstract

Background: Type 2 Diabetes Mellitus (T2DM) presents a chronic health challenge that extends beyond the patients, significantly affecting family members who assume caregiving responsibilities. Family caregivers often experience emotional, physical, and psychological burdens, particularly in low-resource settings. However, the interplay between family communication, decision-making, caregiver burden, and self-efficacy, and how these factors jointly influence caregiver quality of life, remains insufficiently explored, particularly in culturally specific and resource-limited settings.Purpose: This study aimed to examine the structural relationships among family communication, shared decision-making, caregiver burden, self-efficacy, and their influence on the quality of life of family caregivers of individuals with T2DM.Methods: A cross-sectional study was conducted involving 327 family caregivers recruited from 16 primary health centers using cluster random sampling. Data were collected using validated instruments to measure family communication, decision-making, caregiver burden, self-efficacy, and quality of life. Structural Equation Modeling (SEM) was used to analyze both direct and indirect relationships among the variables.Results: The findings revealed that family communication (β = 0.35) and decision-making (β = 0.42) had significant positive effects on self-efficacy, whereas caregiver burden (β = -0.33) had a significant negative effect. Self-efficacy, in turn, significantly improved caregivers’ quality of life (β = 0.51). Furthermore, self-efficacy mediated the indirect effects of family communication, decision-making, and caregiver burden on quality of life. The model explained 49% of the variance in self-efficacy and 64% of the variance in caregiver quality of life.Conclusion: Self-efficacy plays a central role in enhancing caregiver well-being and quality of life. Interventions that promote open family communication, shared decision-making, and burden reduction are essential. Nursing implications include the need for culturally sensitive, family-centered nursing interventions to enhance caregiver self-efficacy through communication training, psychosocial support, and empowerment in decision-making. Abstract

Background: Low birth weight (LBW) is a major global health concern because of its strong association with infant mortality, morbidity, and impaired long-term development. The determinants of LBW among neonates in rural areas of Indonesia remain underexplored. Objective: To examine the prevalence of LBW and determine its associated factors among neonates in rural areas of Indonesia.   Methods: A cross-sectional study used secondary data sources from the 2017 Indonesian Demographic and Health Survey. A total of 6,701 mothers who lived in rural areas were included. Explanatory variables were maternal age, maternal education, smoking status, parity, birth interval, twin history, antenatal care (ANC), husband support, wealth quintile, region of residence, and complications during pregnancy. The outcome variable was the prevalence of LBW. Data were analyzed using bivariate analysis with a Chi-square test (χ2) and multivariable logistic regression. Results: The prevalence of LBW was 6.65%. First birth [AOR = 1.486; 95% CI: 1.126-1.959], twin history [AOR = 27.165; 95% CI = 13.006-56.738], fewer than four ANC visits [AOR = 2.193; 95% CI = 1.519-3.164], and complications during pregnancy [AOR = 1.890; 95% CI = 1.427-2.503] were significantly associated with the prevalence of LBW. Conclusion: This study revealed the prevalence of LBW among neonates in rural areas of Indonesia. First birth, twin history, ANC visits, and complications during pregnancy were significantly associated with LBW. It is suggested that health professionals should strengthen the quality of antenatal care and improve health promotion and education during pregnancy for rural mothers to reduce the prevalence of LBW.

Abstract

Introduction: Substance abuse among adolescents, particularly street children, is a global public health concern influenced by demographic and social factors. While substance abuse in school settings has been widely studied, evidence focusing on street children remains limited. This review aims to evaluate strategies for reducing substance abuse among street children, with an emphasis on interventions affecting physical, mental, and social well-being. Method: This systematic review followed PRISMA 2020 guidelines and was registered in PROSPERO. Literature searches were conducted in four databases (Scopus, Web of Science, PubMed, and ProQuest), yielding 8,774 records. After screening and eligibility assessment, eight studies published between 2016 and 2025 were included. Study quality was appraised using the JBI critical appraisal tool, and findings were synthesized narratively. Results: Eight studies involving homeless or street-involved adolescents aged 10–19 years were reviewed. Commonly reported substance use included alcohol, cannabis, and inhalants. Interventions such as motivational interviewing, psychological reinforcement, and strength-based approaches were effective in reducing risky behaviors and increasing readiness for change. Conclusion: Motivational interviewing combined with community-based psychosocial support is effective in addressing substance abuse among high-risk adolescents. Sustainable interventions should be holistic and adapted to local cultural and structural contexts.

Abstract

The risk of heart disease and stroke can be reduced with adequate cardiovascular disease prevention, as outlined in the American Heart Association's Life's Essential 8 guidelines for modifiable health behaviors. This review examines the role of Community Health Workers (CHWs) in CVD prevention trials across the United States. In the 24 clinical trials identified, our review emphasized the effectiveness of CHWs in improving health behaviors and outcomes, particularly for underserved populations with limited access to health care. CHWs were actively involved in implementing interventions, providing culturally sensitive education, offering health coaching, and supporting lifestyle modifications, such as increased physical activity and medication adherence. It is noteworthy that most studies focused on heart disease. Beyond the CHW's role in delivering behavioral interventions, training for community health workers (CHWs) generally includes disease-specific knowledge and communication skills. CHW training programs vary widely in scope and standards, with unclear role definitions and inadequate collaboration. To improve CHW-led preventive health care, it is recommended to develop a standardized training framework, define CHW responsibilities in clinical and research collaborations, and build sustainable community-academic partnerships. This action could significantly enhance the role of CHWs in reducing cardiovascular disease (CVD) disparities, thereby promoting more equitable health care.

Abstract

Introduction: The growing elderly population in Indonesia raises health challenges, including chronic diseases and cognitive decline. Informal caregivers play a key role, yet limited knowledge and digital literacy reduce care quality. Method: The CITRA (Innovative Caregiver for the Elderly) program was conducted from July to September 2025 in Pacar Kembang, Surabaya. Twenty informal caregivers participated in health education, Android-based CITRA app training, and door-to-door mentoring. Evaluation used pre- and post-tests analyzed with the Wilcoxon test. Results: Caregivers’ knowledge improved significantly. In the first session, scores rose from 65 ± 10.13 to 82.75 ± 6.58 (p=0.000). In the second, scores increased from 71.5 ± 9.44 to 87.5 ± 8.57 (p=0.005). Skills in basic health checks and app utilization also improved. The CITRA app served as an accessible educational tool, though currently limited to Android devices. Discussion: Capacity improvement supports the active ageing framework and aligns with evidence that technology-based interventions effectively empower families in elderly care. Conclusion: CITRA successfully enhanced caregiver knowledge, skills, and provided a replicable digital innovation to improve elderly quality of life in Indonesia.
