Abstract

The rapid spread of the Covid-19 virus has impacted economic, social, environmental, and health challenges. One of them is maternal and child health services, such as reducing contraceptive use by 10% in some low and middle-income countries (LMIC) to prevent and control COVID-19. Quality family planning services affect satisfaction and correlate with continuity of family planning use. This study aims to determine the relationship between family planning acceptor satisfaction and the quality of family planning services during the COVID-19 pandemic. This study used quantitative research with a cross-sectional. The study population was women of child-bearing age (WCBA) aged 15-49 years who received non-natural family planning services in Sleman Regency, Yogyakarta, starting in March 2020. The sampling technique used was consecutive sampling. All research data were collected online (Google Forms). The total sample size was 171 respondents. The analysis used was the Chi-Square test and logistic regression. The results showed that the satisfaction of family planning acceptors with the quality of family planning services during the COVID-19 pandemic in Sleman Regency was 93.6%. There is a significant relationship between the satisfaction of family planning acceptors and the quality of family planning services in tangible, reliability, responsiveness, assurance, and empathy (p < 0.05). There was no association between family planning acceptor satisfaction with age, occupation, education, and health facilities. The quality of family planning services with the tangible dimension was 8.376 times more likely to result in family planning acceptor satisfaction (p=0.026; 95% CI=1.284-54.631). Strengthening cooperation between health service providers, health workers, and all relevant stakeholders is needed to maintain the quality of family planning services, and ensuring the availability and access to family planning services in the community is a strategy for family planning services in the new normal of the coming era.

Abstract

Latar Belakang: Pandemi COVID-19 telah mengganggu sistem kesehatan di seluruh dunia dan salah satu pelayanan kesehatan yang terdampak pandemi COVID-19 adalah terganggunya sistem pelayanan imunisasi. Perubahan prioritas layanan yang difokuskan pada pelayanan pasien COVID-19, adanya kebijakan physical distancing dan pada akhirnya menyebabkan penurunan promosi dan layanan imunisasi, kurangnya permintaan imunisasi karena orangtua tidak membawa bayi ke pelayanan kesehatan, hal ini menyebabkan ketidaktepatan pemberian imunisasi pada bayi. Tujuan: Menganalisis faktor-faktor yang berhubungan dengan ketepatan pemberian imunisasi dasar pada masa pandemi COVID-19 di Puskesmas Kasihan II Bantul. Metode: Penelitian cross-sectional dilakukan melalui survei offline pada Juni 2021-Juli 2021 terhadap 56 responden di Puskesmas Kasihan II Bantul dengan mematuhi protokol kesehatan. Teknik pengambilan sampel menggunakan total sampling dengan menggunakan kuesioner yang sudah tervalidasi. Hasil: Faktor yang berhubungan dengan ketepatan pemberian imunisasi dasar adalah pengetahuan, sikap, pendidikan, pekerjaan, pendapatan dan dukungan keluarga. Faktor yang paling dominan menyebabkan ketepatan pemberian imunisasi adalah pengetahuan (0,032). Simpulan: Dapat mengoptimalisasi kegiatan sosialisasi terkait pentingnya ketepatan pemberian imunisasi dasar pada anak dengan menggunakan media visual pada saat kegiatan imunisasi berlangsung guna meningkatkan cakupan imunisasi dan ketepatan jadwal pemberian imunisasi..

Abstract

Data from the Basic Health Research survey (Riskesdas) in 2013 showed that 33.3% deliveries in Indonesia occurred outside health facilities. Culture and gender influenced the decision-making process regarding place of delivery and birth attendants. A qualitative longitudinal study with an ethnography study design was conducted to explore the socio-cultural context and women’s autonomy in the dynamics of decision making regarding  place of delivery and birth attendants. This study was conducted in the working area of Muara Kaman Health Center, Kutai Kertanegara, East Kalimantan. In-depth interviews with 17 pregnant women were conducted since the first or second trimester of pregnancy until childbirth. Data were analyzed using thematic analysis. Nine informants delivered at the health facilityand eight informants chose home delivery.Those who delivered at the health facility made their own decision. Nevertheless some informants who were autonomous still chose homebirth, either assisted by  midwives, TBAs, or both. Women whose choice was decided by others (husbands, parents and TBAs), all gave birth at home assisted by TBAs. Women’s autonomy needs to be strengthened by improved knowledge, practice of delivery plan and also increase family support to enable women to choose health facilities as place for delivery.

Abstract

Background: Reproductive health and premarital services are important aspects of marriage preparation to prevent health hazards for the bride and groom. There are still many prospective brides who have limited knowledge about reproductive health and the benefits of premarital screening and counseling. Low utilization of premarital services can increase the risk of pregnancy complications, sexually transmitted infections, as well as early marriage, which poses a high risk to maternal and child health. This study aims to explore the experiences of prospective brides related to reproductive health and premarital services through a systematic review using the scoping review method.Method: This study employed a scoping review using the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) Flowchart approach. The literature search was performed in the main databases such as PubMed, ScienceDirect, and Wiley, using keywords determined based on the PEOS (Population, Exposure, Outcome, and Study Design) framework. The literature review was conducted on scientific publications in English, primary research articles or reviews relevant to the same theme, as well as studies published within the last 10 years. In addition, it was analyzed thematically to identify patterns of key findings.Result: The level of knowledge of future husbands and wives’ reproductive health and premarital services still varies, with most individuals having limited understanding before undergoing educational interventions. Interventions such as premarital courses, counseling, and brochure-based counseling are effective in improving the knowledge and attitudes of future husbands and wives. However, there are barriers to the implementation of premarital services, such as limited access, social stigma, and a lack of rules regulating obligatory premarital examinations. Therefore, stronger policies are needed from the government, as well as ongoing education in schools and universities to increase public awareness of the importance of reproductive health before marriage.

Abstract

Background: Unintended pregnancy (UP) among adolescents represents a global issue with profound psychological, social, and emotional consequences. Adolescents experiencing UP often face social stigma, family pressure, and uncertainty about their future, which can lead to prolonged psychological distress. This study aims to explore the experiences of stress and the resilience strategies employed by adolescents dealing with UP.Subjects and Method: This research employs a scoping review methodology based on the Arksey and O’Malley framework, with the population consisting of adolescents, teenage girls, and young mothers. The concepts included stress, stress experience, resilience strategies, and coping mechanisms, and the context was applicable across various settings. Literature was sourced from databases such as PubMed and ScienceDirect. Inclusion criteria comprised scholarly publications in English or Indonesian, primary research articles or relevant reviews addressing similar themes, and studies published within the last 10 years to ensure relevance. The PRISMA Flowchart illustrated the selection process, while the critical appraisal of the selected articles followed the Joanna Briggs Institute (JBI) guidelines.Results: UP is a significant contributor to psychological stress among adolescents, driven by social stigma, isolation, and a lack of emotional support. Nevertheless, resilience emerges as a crucial factor in managing such stress. Protective factors such as support from family and peers and emotional regulation training are pivotal in enhancing adolescent resilience.Conclusion: Community-based interventions focusing on fostering resilience through the development of psychological skills and stress management strategies are essential for supporting the mental health of pregnant adolescents.

Abstract

Hubungan seksual sebelum nikah pada remaja merupakan masalah yang serius, berhubungan dengan peningkatan penularan penyakit menular seksual, mempunyai pasangan lebih dari satu, dan kehamilan dini. Suatu kerangka kerja model perilaku terintegrasi (Integrated Behavioral Model, IBM) digunakan untuk menilai berbagai faktor prediktor hubungan seksual prematur pada remaja. Tujuan penelitian ini adalah mengeksplorasi inisiasi hubungan seksual sebelum nikah pada remaja level 10 dan 11 berdasarkan kerangka kerja IBM, meliputi komunikasi tentang seks kelompok peers, orang tua, paparan perilaku pornografi, kepercayaan normatif, agen personal, dan keinginan hubungan seksual. Metode yang digunakan adalah menyertakan 626 responden dalam survei awal. Responden adalah siswa sekolah menengah atas level 10 – 11 di kota Denpasar. Data dikumpulkan dengan kuesioner laporan sendiri khususnya prediktor inisiasi hubungan seksual sebelum menikah. Penelitian ini menemukan bahwa pajanan pornografi, perilaku langsung dan tidak langsung berhubungan secara signifikan dengan inisiasi hubungan seksual sebelum nikah (nilai p < 0,05). Remaja laki-laki tampaknya melakukan lebih banyak aktivitas seksual daripada remaja perempuan. Penelitian ini berimplikasi terhadap pemahaman perilaku langsung dan pajanan pornografi mungkin digunakan dalam meningkatkan program kesehatan dan kesehatan remaja. Premarital sexual inisiation on adolescence is a serious problem, associated with increased transmition sexually transmitted disease/STD, had having more partners, and early pregnancy. An Integrated Behavioral Model (IBM) framework used to assess predictors of premarital sexual on adolescents. The purpose of this research is to explore predictors of premarital sexual inisiation in adolescents grade 10 and 11 based on IBM framework, includes: communication about sex with peers, parents, pornography exposure, attitude, normative belief, personal agency, and intention to have sex. Method that used is 626 respondent included in earlier survey, and respondent were students of senior high school grade 10 – 11 in Denpasar City. Data collected with self reported questionaire particularly predictor of premarital sexual initiation. The result found that pornography exposure, indirectly attitude, and directly attitude were significantly associated with premarital sexual initiation (p < 0,05). Male adolescents engage in more sexual activity like premarital sexual inisiation than female adolescents. This study has implications for understanding how directly attitude and pornography exposure may be used in intervention to promoting adolescents health program and adolescents ressiliency.

Abstract

Demam berdarah dengue (DBD) menimbulkan syok dan kematian. Penderita DBD di Yogyakarta sebagian besar usia 1 - 12 tahun dengan DBD parah. Asupan vitamin D rendah diasumsikan penyebab DBD parah. Asumsi ini perlu dibuktikan dengan menganalisis pengaruh asupan Vitamin D dan keparahan DBD. Rancangan penelitian adalah studi kasus kontrol. Penelitian di bangsal rawat inap anak dan instalasi catatan medik RS Jogja dan RSUP Dr. Sardjito. Kasus adalah anak usia 1 - 14 tahun dengan DBD grade III & IV, kontrolnya DBD grade I & II. Data asupan vitamin D diambil dengan food frequency questionnaire (FFQ). Variabel luar adalah indeks massa tubuh (IMT), usia, status penyakit kronis dan intensitas terpapar matahari pagi. Analisis dengan uji-t dan regresi logistik. Hasil penelitian didapatkan 60 kasus dan 60 kontrol tanpa matching. Cut-off point asupan vitamin D berdasarkan ROC curve adalah 2,7 µg/hari. Penderita DBD parah rata-rata asupan vitamin D 1,10 kali lebih sedikit dibandingkan DBD tidak parah. Rata-rata asupan vitamin D lebih rendah 1 µg/day bersama IMT ≥18,75 kg/m2, penyakit kronis, dan kurang terpapar matahari pagi berpengaruh pada DBD parah (OR=0,47; 95% CI: 0,32-0,71). Cukup Asupan vitamin D disarankan untuk menghindari keparahan DBD. Penyakit kronis dan berat badan lebih perlu menjadi perhatian tenaga medis sebagai kewaspadaan dini terjadinya shock. Dengue hemorrhagic fever (DHF) lead to shock and death. Patient in Yogyakarta mostly aged 1 - 12 years old with severe dengue. A low vitamin D intake is assumed to be the cause of severe dengue. This assumption needs to be proved by analyzing the effect of vitamin D intake and severity of DHF. Study design was a case control study. Research on children’s wards and medical record installation in hospital. Cases were children with DHF grade III and IV, the control of DHF grade I & II. Data vitamin D intake was obtained by FFQ. Outer variables: BMI, age, chronic diseases and intensity morning sun exposure. Analysis by t-test and logistic regression. The results showed 60 cases and 60 controls without matching. Cut-off point vitamin D intake based ROC curve was 2.7 µg/day. Patients with severe dengue average vitamin D intake of 1.10 times less than not severe dengue. The average vitamin D intake lower 1 µg/day with a BMI ³ 18.75 kg/m2, chronic disease, and less exposed to the morning sun effect on severe dengue (OR = 0.47, 95% CI: 0.32 to 0.71). Sufficient vitamin D intake is recommended to avoid the dengue severity. Chronic diseases and more weight should be a concern of medical personnel as early warning shock occurrence.

Abstract

Purpose: This research was to identify the relationship between body mass index pre-pregnancy and the incidence of hypertension in pregnancy in Bantul District in 2020. Methods: This research used a retrospective cohort design based on secondary data from an online maternal cohort with SIPIA Bantul in the Bantul district in 2020. The effect size was calculated using Relative Risk (RR) from bivariate chi-square, multivariate binomial regression, and stratification by Mantel-Haenszel. The total sample of this research was 670 pregnant women. Results: Bivariate analysis results showed that there was body mass index pre-pregnancy (RR: 1.45; CI 95%: 1.220 – 1.730; p-value=0,00), mother age (RR: 1.47; CI 95%: 1.243 – 1.745; p-value=0,00), upper arm circumference (RR: 1.45; CI 95%; 1.194 – 1.761; p-value=0,001), and gestational weight gain (RR: 1,34; CI 95%: 1.123 – 1.598; p-value=0,002) have significant association with hypertension in pregnancy in Bantul District 2020. The multivariate analysis showed that body mass index pre-pregnancy and mother's age were the most significant variables with hypertension in Bantul District 2020. The stratification analysis showed that maternal age, upper arm circumference, gestational weight gain, and parity were not confounding. Still, there were modifying effects between body mass index pre-pregnancy and the incidence of hypertension in pregnancy in Bantul District in 2020. Conclusion: Pregnant women who are overweight and obese can increase the incidence of hypertension in pregnancy in Bantul District 2020. Collaboration with nutritionist workers needs to be done to reduce the incidence of hypertension in pregnancy in Bantul District.

Abstract

Purpose: The first timely antenatal care can detect complications during early pregnancy, such as hemorrhage, anemia, diabetes, and other infectious diseases. The province of Papua has the three lowest antenatal care K1 coverage areas, at 56.02%, and Jayapura city has an antenatal care K1 coverage of 51.49%. Methods: This research employed a mixed-methods approach using a sequential explanatory design. In the first stage, a quantitative approach was conducted using a cross-sectional research design. In the second stage, a qualitative approach was applied through in-depth interviews. Results: Multivariable analysis showed that unintended pregnancies were 2.8 times more likely to result in delayed first antenatal care visits compared to planned pregnancies (AOR 2.62; 95% CI: 1.23–6.25), after controlling for partner/spouse support, exposure to mass media, gestational interval, and education level. Interview findings revealed that other contributing factors to delayed first antenatal care visits included low awareness about the importance of timely care, being too busy, and a lack of knowledge regarding the benefits of antenatal care. Conclusion: The importance of family planning counseling for reproductive women is to manage and plan their pregnancies.

Abstract

Purpose: One of the key barriers to exclusive breastfeeding is the increasing number of mothers who work outside the home. This study aimed to analyze the relationship between maternal employment and exclusive breastfeeding in Indonesia using data from the Indonesian Family Life Survey (IFLS) 4 and 5. Methods: This study employed an analytic observational design with a cross-sectional approach. The sample consisted of women aged 15–49 years who were never married and currently breastfeeding. Data from IFLS 4 and 5 were analyzed using Chi-square tests and logistic regression to examine the relationship between maternal employment status and exclusive breastfeeding. Results: The percentage of exclusive breastfeeding increased by approximately 10% between 2007 and 2014 (from 10.4% to 22%). During the same period, the percentage of working mothers also increased by about 20% (from 70.3% to 95.2%). In 2007, the highest proportion of breastfeeding working mothers was among private employees (13.5%), while in 2014, it was among those working in informal agricultural jobs (39.3%). There was no statistically significant relationship between maternal employment status in general and exclusive breastfeeding (p>0.05). However, there was a significant association between knowledge of exclusive breastfeeding and exclusive breastfeeding practices (p<0.05). Conclusion: Exclusive breastfeeding rates in Indonesia are affected by maternal knowledge about exclusive breastfeeding.
