Abstract

Latar belakang: Seksio sesarea dilakukan atas indikasi medis dan persetujuan pasien. Proses pengambilan keputusan terkadang menghadapi kendala dan dilemma.Tujuan: Menganalisis kelengkapan dan kepatuhan pengisian lembar informed consent dan aspek bioetika pemberian informed consent pada SC elektif.Metode: Menggunakan metode campur dengan pendekatan explanatory sequential design. Data kuantitatif untuk menilai kelengkapan dan kepatuhan pengisian lembar informed consent diperoleh dari rekam medik. Data kualitatif untuk menilai penerapan aspek bioetik diperoleh dari wawancara terstruktur dengan dokter Obgin, dokter umum jaga, bidan, dan pasien pasca SC.Hasil dan Pembahasan: Kelengkapan dan kepatuhan pengisian lembar informed consent adalah 72,5% dan 71,8%. Kedisiplinan dan perubahan rekam medik elektronik menjadi kendala pengisian formulir informed consent. Pemberian informed consent terkendala ketidakhadiran orang terpenting yang akan mengambil keputusan, kurangnya pemahaman dan pengetahuan pasien, dan pasien peserta asuransi kesehatan sehingga memunculkan dilema. Implementasi informed consent berdasarkan prinsip bioetika, otonomi, beneficence, non- maleficence, dan justice, digunakan untuk menyelesaikan dilema etik dalam pengambilan keputusan.Kesimpulan: Kelengkapan dan kepatuhan pengisian lembar informed consent belum mencapai 100%. Memilih prinsip bioetika yang lebih dominan nilai dan prioritasnya dengan analisis prima facie menjadi solusi untuk menyelesaikan dilema etik dalam pengambilan keputusan pada SC elektif.

Abstract

Background: Patients with mental diseases with low self-concept are more likely to engage in maladaptive behavior, isolate themselves, and relapse. This study aimed to examine the effectiveness of deep breathing exercise and community mental health nursing (CMHN) training in improving the self-concept quality of patients with mental disorders. Methods: The study used a quantitative quasi-experimental approach with a non-equivalent control group. A total of 235 individuals were randomly selected from ten primary health facilities in Bantul, Yogyakarta, including 120 in the intervention group (combination of deep breathing exercises and CMHN training) and 115 in the control group (self-directed study only). Eligible participants were cooperative patients with mental disorders in the maintenance or health promotion phase, while those unwilling to follow the training protocol were excluded. Self-concept served as the dependent variable, measured using a self-concept questionnaire. Data were analyzed using paired t-tests in SPSS, with statistical significance set at p < 0.05. Results: The intervention group showed a significant increase in mean self-concept scores, from 105.2 pretest to 110.0 posttest (p = 0.004), while no significant change was observed in the control group (p = 0.092). Conclusion: Integrating deep breathing exercises with CMHN training improves the self-concept of individuals with mental illnesses during the maintenance and health promotion stages.

Abstract

Background: Indonesia reduced maternal mortality from 390 per 100,000 live births in 1994 to 228 in 2007, but this rose again to 305 in 2015. In Banyumas Regency, maternal mortality declined from 2014 to 2020 but increased again from 2021 to 2023. Purpose: This study aims to identify social determinants affecting maternal mortality trends in Banyumas from 2014 to 2023. Methods: A qualitative study was conducted following the Standards for Reporting Qualitative Research (SRQR). Data collection involved in-depth interviews with 45 respondents—including health office staff, hospital informants, midwives, and health cadres—and a Focus Group Discussion. Interviews and discussions were guided by semi-structured protocols to explore participants’ views on maternal health issues. Results: Most respondents had over three years of experience and were aged 40–50. Key themes emerged: challenges in maternal and neonatal health services, referral refusal influenced by sociocultural factors, emergency training for health workers, collaboration and tiered referral systems, and hospital standardization. Discussion: The five key determinants influencing maternal mortality rate in Banyumas Regency were identified: human resource adequacy in maternal care, continuity of emergency training, accessibility of referral systems and collaborations, and hospital standardization. Healthcare personnel shortages, especially post-Covid-19, also combined with inadequate emergency training, may affect the healthcare quality. Sociocultural beliefs and poor referral coordination lead to treatment delays, particularly in rural areas. Inconsistencies in hospital standardization further exacerbate access inequities. While maternal mortality showed a declining trend from 2014 to 2023 (excluding pandemic years), systemic challenges remain. Strengthening referral systems, improving hospital standards, continuous training, and culturally sensitive public education are crucial. Conclusion: Social determinants influencing maternal mortality trends include the adequacy of human resources, ongoing emergency maternal and neonatal training, accessible and integrated referral systems, consistent hospital accreditation standards (PONEK), and sociocultural factors. Addressing these areas is essential to improving maternal health outcomes in Banyumas

Abstract

Anemia remains a major public health problem among women of reproductive age worldwide, particularly in developing countries such as Indonesia. Limitations of conventional anemia management, including adherence issues and side effects of iron supplementation, have encouraged interest in complementary therapies such as wet cupping therapy (WCT). This study aimed to systematically map existing empirical evidence on the effects of wet cupping therapy on hemoglobin levels and hematological parameters in women through a scoping review approach. The review was conducted following the PRISMA-ScR guidelines. Literature searches were performed across multiple databases, including PubMed, Scopus, Google Scholar, and institutional repositories, resulting in 312 identified records. After screening and eligibility assessment, six studies published between 2016 and 2023 were included. The selected studies employed diverse designs, including experimental clinical trials, quasi-experimental, pre–post, cross-sectional, prospective cohort, and retrospective observational studies, with sample sizes ranging from 17 to 364 female participants. Across all studies, wet cupping therapy was associated with either maintenance or improvement of hemoglobin levels, as well as favorable changes in iron, total iron-binding capacity, and other biochemical parameters. No evidence indicated that WCT caused anemia when administered appropriately. Overall, the findings suggest that wet cupping therapy may serve as a safe and potentially beneficial complementary intervention for improving hematological status in women. However, further research using randomized controlled trials and standardized intervention protocols is required to establish its clinical effectiveness and inform evidence-based practice.

Abstract

Despite the World Health Organization's continued advocacy for the adoption of Maternal and Perinatal Death Surveillance and Response (MPDSR) as a systematic approach to uncovering and addressing the root causes of maternal and perinatal mortality, death rates within these populations remain alarmingly high in many regions. The effectiveness of MPDSR varies widely across settings, often limited by weak health information systems, reporting delays, blame-oriented cultures, and poor follow-up of audit recommendations. This scoping review aims to examine the practical application of MPDSR across various real-world contexts, with a particular emphasis on the mechanisms for identifying, reporting, evaluating, and responding to maternal and perinatal deaths, as well as the factors that enable or hinder these processes. Peer-reviewed publications from the past decade were systematically retrieved from major databases, including PubMed, Wiley, and ScienceDirect. The review identified key barriers such as shortages of skilled personnel, incomplete documentation, and limited financial and monitoring resources, while effective MPDSR was facilitated by community engagement, clear reporting systems, and supportive policies. These findings highlight the need to strengthen workforce capacity, data systems, and governance to reduce preventable maternal and neonatal deaths.
