Abstract

Pendahuluan: Pengambilan keputusan dalam keluarga tergantung dari dominasi atau keseimbangan kekuasaan di dalam keluarga tersebut. Banyak negara berkembang menganggap kedudukan perempuan lebih rendah. Hal ini membuat efek banyak wanita menyerahkan seluruh keputusan mengenai kesehatan reproduksinya pada pasangannya. Penggunaan AKDR di Kabupaten Kebumen masih rendah hanya 8%. Tujuan: tujuan dari penelitian ini adalah untuk mengetahui relasi kuasa dalam pengambilan keputusan dengan keputusan penggunaan kontrasepsi dalam rahim dari perspektif istri. Metode Penelitian: Penelitian ini merupakan jenis penelitian observasional dengan rancangan cross sectional dengan pendekatan mix methode. Populasi penelitian adalah seluruh pasangan usia subur di wilayah Kecamatan Pejagoan yang menggunakan kontrasepsi modern yang berjumlah 8001. Subjek penelitian sebanyak 158. Teknik pengambilan sampel penelitian kuantitatif menggunakan proportionate random sampling. Analisis data menggunakan analisis chi-square Hasil: Analisa bivariat menunjukkan hubungan antara variabel relasi kuasa dalam pengambilan keputusan dengan penggunaan AKDR (p<0,05). Kesimpulan: Terdapat hubungan antara relasi kuasa dalam pengambilan keputusan penggunaan kontrasepsi.

Abstract

Dietary pattern and anemia with the occurrence of LBW babies: Indonesian Life Family Survey (IFLS 5)Background: Nutritional problems occur throughout the human life cycle. Maternal, infant, and neonatal mortality rates are critical national growth indicators. The most common cause of death in neonates is babies with low birth weight (LBW). Objective: This study examines the association between diet, anemia, and the incidence of LBW in Indonesia using the Indonesia Family Life Survey (IFLS) wave 5. Methods: This observational study used IFLS 5 secondary data. The study population is women of reproductive age (15-49 years), married, had their last child born alive, and their weight weighed. The independent variables were diet and anemia, while the dependent was LBW. Effect modifiers were age, parity, education, employment, pregnancy complications, consumption of iron tablets, and residence. Results: The total number of respondents was 2,368, with an LBW incidence of 8.7%. The percentage of non-diverse diets is 72.3%, more than diverse diets. There was a significant relationship between diverse dietary patterns and LBW. A less varied diet provides a 1.32 more substantial risk of giving birth to an LBW baby compared to mothers with a diverse diet. However, there was no relationship between anemia and LBW. Further analysis showed two food groups significantly associated with LBW: nuts and meat and fish. Conclusions: A diverse diet lowers the risk of LBW but is not significantly related. Efforts are needed to improve community nutrition through a varied and balanced diet. Social factors related to LBW are mothers' education and employment.

Abstract

Background: The increasing prevalence of adolescent pregnancy is a concern, as it affects 17% of the world’s population. Adolescent pregnancy could potentially have significant psychological impacts, including shameful, fearness, depression, and even suicidal thoughts. Purpose: The aim of this scoping review was to analyze the psychological effects of adolescent pregnancy in developing countries based on evidences published previously. Method: This scoping review was using Arksey and O’Malley’s approach. PEOS framework was used for identifying review question. The literature search was conducted by using three primary databases: PubMed, ScienceDirect, and Wiley Online Library. Gray literature was searched through Google Scholar, and included evidences were critically appraised by using JBI and MMAT critical appraisal tools. Results: Based on the search results of 542 articles, 7 were selected. This review focuses on the psychological impact of teenage marriage, including depression, stress, low self-efficacy in becoming parents, stigma or social pressure, and uncertainty about the future. Adolescent pregnancy leading to a negative impact on mental health, depression, stress, and low self-confidence among prospective parents. These issues arise due to a lack of psychological readiness to face the challenges of marriage, lack of family support, marital conflict, community stigmatization, ostracism, and shame. Conclusion: Adolescent pregnancy has adverse psychological impact for mother, thus it  is necessary to initiate cross-sectoral work collaboration to provide health and social services as well as reducing stigma within family, peer, and community environment.

Abstract

Background: Midwifery educational institutions have an important role in the scoring power of professional andqualified midwives. To be able to produce a professional midwife as expected, then it takes a model of effective andinnovative learning.Objective: To know the effectiveess of reflective learning on Student Academy Midwifery Yogyakarta.Method: This type of research survey with descriptive study. The research at Academy of Midwifery in Yogyakarta.Sample student research level I, II, and III. Sampling using a minimum sample size. Variable independent variable research, namely is the reflective learning. The dependent variable is the attitude of professional midwiferystudents.Result: The result showed statistical significance (P <0.05) between the independent variable (reflective learning)with the dependent variable (professional attitude) with a prevalence ratio 2.13 95% CI (1.62 to 2.78).Conclusion: Students who did reflective learning finely will have a chance to do a good professional attitude.Keywords: Reflective learning, professional attitude, midwifery student

Abstract

Centella asiatica (C. asiatica) or gotu kola has been used traditionally in many Asian countries as herbal medicines for many conditions, such as headache, asthma and memory enhancement. Centella asiatica has been also widely investigated for its neuroprotective effects in animal disease model including epilepsy, Alzheimer’s and Parkinson’s diseases. A brief review of neurological effects studies of C. asiatica in animal model was reported. Eligible studies published through December 2020 from PubMed and EMBASE journal data base were collected. Any keywords related to C. asiatica or gota kola AND neurological conditions were used. The results showed that several experimental studies concerning the neuroprotective properties of C. asiatica have been reported. Phytochemical studies reported that C. asiatica contained many bioactive compounds in which triterpenoids are the most identified. Furthermore, the triterpenoids have proven to have neuroprotective effect due to their effects of anti-inflammatory, antioxidant, improvement of mitochondrial dysfunctions, and increased brain-derived neurotrophic factors. Further studies are needed to investigate the neuroprotective mechanisms of C. asiatica. In conclusion, C. asiatica might have potential as an alternative medicine for neurological conditions, such as stroke, epilepsy, Alzheimer’s and Parkinson’s disease.

Abstract

Post-stroke cognitive impairment involves memory, visuoconstructional, and spatial dysfunctions. Centella asiatica L. Urban. and Curcuma longa L. are both well-known herbs in South and South-East Asia countries that are believed to possess neuroprotective properties as memory enhancers. This study aimed to investigate the effects of C. asiatica L. Urban. and C. longa L. extracts combination in improving memory performance in stroke model rats and its acute toxicity. Twenty-five Wistar rats underwent transient bilateral common carotid artery occlusion. Y-maze pre-test was performed 24 h after the occlusion. The rats were then divided into five groups i.e. Group 1 received NaCMC dilution, Group 2 received donepezil 0. 7 mg/kg BW/day, and Groups 3, 4, and 5 received the extracts combination with dose of 59; 118; and 236 mg/kg BW/day, respectively. Y-maze post-test was performed 24 h after the last dose had been given. Spontaneous alternation behavior was used as the indicator of working memory measurement. The fixed-dose method according to OECD Guideline was used to estimate the LD50 in acute toxicity testing on Wistar rats. There was a significant difference in the delta spontaneous alternation percentage among groups tested (p<0. 05). Group 1 had significant difference with any other group (compared to Groups 2 to 5; p<0.05). No significant difference could be found between groups of the extracts combination and donepezil group suggesting that the effect is not more inferior nor dose-dependent in improving memory performance. The extracts combination at a single dose of 2000 mg/kg BW did not show related signs of toxicity or mortality in any of the rats during the 14-day observation period. In conclusion, the C. asiatica L. Urban. and C. longa L. extracts combination can improve performance of memory on stroke model animal. According to Globally Harmonized Classification System, the extracts combination can be classified as Category 5/unclassified.

Abstract

Background: Three delays models in referring the mother to health facility are the most important determinants in maternal mortality. Based on the culture in West Sumba, most of the people still follow patrilinear patrimony. This culture limiting women to make decisions such as the decision to determine where to get health services, plan the number of children and bird spacing. The first model should be immediately prevented from causing the next delay which is late in making a family decision and too late to recognize danger sign in pregnancy that affects maternal mortality. Purpose: The purpose of this study was to explore the role of decision-makers in delayed maternal referrals. Methods: Qualitative research using the phenomenological design on 5 mothers who experienced a delay in referral at Padediwatu Primary Health Care. Data collected with in-depth interviews. Result: Decision maker in maternal referring was dominated by the husband. Obstacles in the referral process such as not having a vehicle, long distances and difficult road access, low economic status, and the culture that depend on others so maternal health status with complications is not a priority for the husband. Husband's work as a farmer and sailor that keeps the husband away from home or near to his wife for long periods was affected by the decision-making process of maternal referral. Conclusion: Decision-making in maternal referrals still emphasizes patrilinierculture, husbands have an important role in making maternal referral decision.

Abstract

Latar belakang: Seksio sesarea dilakukan atas indikasi medis dan persetujuan pasien. Proses pengambilan keputusan terkadang menghadapi kendala dan dilemma.Tujuan: Menganalisis kelengkapan dan kepatuhan pengisian lembar informed consent dan aspek bioetika pemberian informed consent pada SC elektif.Metode: Menggunakan metode campur dengan pendekatan explanatory sequential design. Data kuantitatif untuk menilai kelengkapan dan kepatuhan pengisian lembar informed consent diperoleh dari rekam medik. Data kualitatif untuk menilai penerapan aspek bioetik diperoleh dari wawancara terstruktur dengan dokter Obgin, dokter umum jaga, bidan, dan pasien pasca SC.Hasil dan Pembahasan: Kelengkapan dan kepatuhan pengisian lembar informed consent adalah 72,5% dan 71,8%. Kedisiplinan dan perubahan rekam medik elektronik menjadi kendala pengisian formulir informed consent. Pemberian informed consent terkendala ketidakhadiran orang terpenting yang akan mengambil keputusan, kurangnya pemahaman dan pengetahuan pasien, dan pasien peserta asuransi kesehatan sehingga memunculkan dilema. Implementasi informed consent berdasarkan prinsip bioetika, otonomi, beneficence, non- maleficence, dan justice, digunakan untuk menyelesaikan dilema etik dalam pengambilan keputusan.Kesimpulan: Kelengkapan dan kepatuhan pengisian lembar informed consent belum mencapai 100%. Memilih prinsip bioetika yang lebih dominan nilai dan prioritasnya dengan analisis prima facie menjadi solusi untuk menyelesaikan dilema etik dalam pengambilan keputusan pada SC elektif.

Abstract

Background: Patients with mental diseases with low self-concept are more likely to engage in maladaptive behavior, isolate themselves, and relapse. This study aimed to examine the effectiveness of deep breathing exercise and community mental health nursing (CMHN) training in improving the self-concept quality of patients with mental disorders. Methods: The study used a quantitative quasi-experimental approach with a non-equivalent control group. A total of 235 individuals were randomly selected from ten primary health facilities in Bantul, Yogyakarta, including 120 in the intervention group (combination of deep breathing exercises and CMHN training) and 115 in the control group (self-directed study only). Eligible participants were cooperative patients with mental disorders in the maintenance or health promotion phase, while those unwilling to follow the training protocol were excluded. Self-concept served as the dependent variable, measured using a self-concept questionnaire. Data were analyzed using paired t-tests in SPSS, with statistical significance set at p < 0.05. Results: The intervention group showed a significant increase in mean self-concept scores, from 105.2 pretest to 110.0 posttest (p = 0.004), while no significant change was observed in the control group (p = 0.092). Conclusion: Integrating deep breathing exercises with CMHN training improves the self-concept of individuals with mental illnesses during the maintenance and health promotion stages.

Abstract

Background: Indonesia reduced maternal mortality from 390 per 100,000 live births in 1994 to 228 in 2007, but this rose again to 305 in 2015. In Banyumas Regency, maternal mortality declined from 2014 to 2020 but increased again from 2021 to 2023. Purpose: This study aims to identify social determinants affecting maternal mortality trends in Banyumas from 2014 to 2023. Methods: A qualitative study was conducted following the Standards for Reporting Qualitative Research (SRQR). Data collection involved in-depth interviews with 45 respondents—including health office staff, hospital informants, midwives, and health cadres—and a Focus Group Discussion. Interviews and discussions were guided by semi-structured protocols to explore participants’ views on maternal health issues. Results: Most respondents had over three years of experience and were aged 40–50. Key themes emerged: challenges in maternal and neonatal health services, referral refusal influenced by sociocultural factors, emergency training for health workers, collaboration and tiered referral systems, and hospital standardization. Discussion: The five key determinants influencing maternal mortality rate in Banyumas Regency were identified: human resource adequacy in maternal care, continuity of emergency training, accessibility of referral systems and collaborations, and hospital standardization. Healthcare personnel shortages, especially post-Covid-19, also combined with inadequate emergency training, may affect the healthcare quality. Sociocultural beliefs and poor referral coordination lead to treatment delays, particularly in rural areas. Inconsistencies in hospital standardization further exacerbate access inequities. While maternal mortality showed a declining trend from 2014 to 2023 (excluding pandemic years), systemic challenges remain. Strengthening referral systems, improving hospital standards, continuous training, and culturally sensitive public education are crucial. Conclusion: Social determinants influencing maternal mortality trends include the adequacy of human resources, ongoing emergency maternal and neonatal training, accessible and integrated referral systems, consistent hospital accreditation standards (PONEK), and sociocultural factors. Addressing these areas is essential to improving maternal health outcomes in Banyumas
