Abstract

Beberapa studi uji klinis yang meneliti efikasi asam folat untuk mencegah kejadian stroke pada pasien hiperhomosisteinemia dengan penyakit kardiovaskular memberikan simpulan yang inkonklusif. Tujuan laporan kasus berbasis bukti ini untuk menilai apakah suplementasi asam folat dapat menurunkan risiko stroke pada pasien dengan riwayat jantung koroner dengan hiperhomosisteinemia. Pencarian komprehensif melalui beberapa basis data online. Telaah kritis menggunakan alat penilaian dari Centre of Evidence Based Medicine (CEBM). Diperoleh dua studi kajian sistematik yang sesuai kriteria seleksi. Hasil telaah kritis mendapatkan bahwa kedua studi tersebut memiliki validitas baik. Analisis menunjukkan bahwa suplementasi asam folat dapat menurunkan kejadian stroke pada pasien dengan riwayat penyakit kardiovaskular. Many studies investigating the efficacy of folic acid in hyperhomocysteineemia patients with cardiovascular disease for stroke prevention has been inconclusive. This evidence-based case report was to assess whether folic acid supplementation could reduce stroke risk in patients with a history of coronary heart disease with hyperhomocysteinemia. We performed comprehensive literature search in several online databases. Critical reviews use the assessment tool from the Center of Evidence Based Medicine (CEBM). Two systematic reviews met our selection criteria. Critical appraisals had shown that both studies have good validity. Analysis showed that folic acid supplementation could reduce the incidence of stroke in patients with history of cardiovascular diseases.

Abstract

Beberapa studi efikasi suplementasi vitamin D pada diabetes melitus gestasional hasilnya masih bertentangan. Laporan kasus berbasis bukti ini bertujuan untuk konfirmasi efikasi suplementasi vitamin D terhadap kontrol glikemik pada wanita hamil dengan diabetes melitus gestasional dan hipovitaminosis D. Pencarian artikel komprehensif melalui PubMed dan Scopus. Studi yang dipilih adalah studi dengan desain randomized controlled trial atau systematic review pada wanita hamil dengan diabetes melitus gestasional dan hipovitaminosis D membandingkan terapi standar obat anti-diabetes (OAD) dengan obat terapi standar dan suplementasi vitamin D. Telaah kritis sesuai perangkat dari Center for Evidence Based Medicine (CEBM). Diperoleh 3 studi systematic review yang memenuhi kriteria eligibilitas. Dua studi menunjukkan perbedaan kadar glukosa darah puasa dan HbA1C yang bermakna secara statistik dan klinis pada kelompok yang mendapat suplementasi vitamin D dibandingkan kelompok kontrol. Satu studi hasilnya tidak bermakna. Results of recent studies on the use of vitamin D supplementation in gestational diabetes mellitus were conflicting. This evidence-based case report aimed to confirm the efficacy of vitamin D supplementation on glycemic control in pregnant women with gestational diabetes mellitus and hypovitaminosis D. A comprehensive search for articles in PubMed and Scopus included randomized controlled trial or systematic reviews in gestational diabetes mellitus and hypovitaminosis D comparing standard anti-diabetic drug therapy (OAD) with standard therapeutic drugs and vitamin D supplementation. Critical appraisals were done using the tools from Center for Evidence Based Medicine (CEBM). Three systematic reviews met the eligibility criteria. Two studies showed statistically and clinically significant differences in fasting blood glucose and HbA1C level in vitamin D supplementation group compared to the control group, while the result of the third study was not significant.

Abstract

This study aimed to assess the accuracy of the party receiving information and giving consent in the hospital. This type of research is observational and was conducted at Public Hospital of Banten Province (RSUD Banten) and Dr. Drajat Prawiranegara Public Hospital in 2024. The minimum sample size used was 94 samples taken using consecutive sampling methods. This study used a checklist as a record to check the completeness of the informed consent form and approval sheet by the SOP and applicable legal basis. Data analysis is descriptive of the accuracy of the party receiving information and giving consent. The study observed 28 pieces of information and consent being received and given by the right party, while the wrong party signed 66 (70.2%) consents for medical procedures. The study's focus on the accuracy of the party receiving information and giving consent in the hospital is a contribution to a monitor process of informed consent in hospital according to principles of patient’s competency

Abstract

The decreasing of sensitivity or resistance to tamoxifen occured after long-term treatment in breast cancer. One of the major factor in tamoxifen resistance is over expression of efflux transporter P-glycoprotein (P-gp) and Breast Cancer Resistance Protein (BCRP). Curcumin has known as inhibitor of P-gp and BCRP. The addition of curcumin to the tamoxifen resistant cells is expected to increase the sensitivity of breast cancer cells to tamoxifen. This study aim to know the effect of curcumin in increasing the cell sensitivity to tamoxifen through inhibition of P-gp and BCRP transporter efflux. MCF-7 breast cancer cell line was induced with tamoxifen 1 µM for 10 passage (MCF-7(T)), then cell viability and mRNA expression of P-gp and BCRP were analyzed. To the MCF-7(T) cells, curcumin was given at of 5/10/20 µM with or without tamoxifen for 5 days and cell viability and mRNA expression of P-gp and BCRP were analyzed on day 5th. As positive control, verapamil 50 µM was used as P-gp inhibitor, ritonavir 15 µM and nelfinavir 15 µM were used as BCRP inhibitor. The results showed that MCF-7(T) cells sensitivity to tamoxifen decreased with 11.8 times, the cell viability increased 10.82 fold and mRNA expression of P-gp and BCRP increased 4.04 fold. Then after administration of curcumin with or without tamoxifen for 5 days, the cell viability and the mRNA expression of P-gp and BCRP decreased. As conclusion, curcumin increased the sensitivity of MCF-7(T) to tamoxifen characterized by the decreasing of cell viability and mRNA expression of P-gp and BCRP. However, the administration of combination of curcumin with tamoxifen was more potent than just curcumin. The increased sensitivity was estimated at least partly through the inhibition of P-gp and BCRP mRNA expression by curcumin

Abstract

Objective: This is to compare peripheral and central serum levels of Anti-Mullerian hormone (AMH) in experimental animals for predicting ovarian reserve. Methods: This is an experimental study involving 20 female Sprague-Dawley rats aged 8–10 weeks with normal estrus cycles as the young control group and 5 female rats aged 28–30 weeks as the old control group (n = 5/group): the young control group, the old control group, the 1x cisplatin group, the 2x cisplatin group, and the 3x cisplatin group. After treatment, tissue collection, histological staining, and blood collection through the retro-orbital bleeding (ROB) and heart were performed. Subsequently, measurements of ovarian weight, follicle counting, and levels of AMH and Follicle-stimulating hormone (FSH) were conducted.Results: The serum AMH levels from ROB in the young control, 1x cisplatin, 2x cisplatin, 3x cisplatin, and the old control groups were 1151, 1818, 2782.96, 1381.352, and 1544 ng/mL, respectively. Cisplatin 2x group was significantly (p<0.005) higher compared to the young control. The average concentrations of serum AMH in the ROB and heart were higher in the 2x cisplatin group compared to the other groups. Meanwhile, cisplatin 3x group decreased in level due to the burn out phenomenon. Conclusion: AMH is a preferred marker compared to FSH. Blood collection through the ROB is considered a less invasive alternative technique in the treatment group, requiring serial observation. Keywords: AMH, follicle pool, ovarian aging, ROB
