Abstract

Lingkungan tidak akan terlepas dari kehidupan manusia. Perubahan di dalamnya mempengaruhi faktor kesehatan, sosial, ekonomi, dan budaya. Lingkungan yang menghasilkan particulate matter berasal dari hasil alam dan kegiatan manusia. Salah satu kegiatan manusia yang dihasilkan adalah pembakaran dari aktivitas merokok terutama di ruang tertutup. Pengaruh kesehatan adanya particulate matter adalah potensi terjadinya gangguan pernapasan. Monitoring kualitas udara diperlukan sebagai evaluasi kondisi lingkungan. Pengukuran kualitas udara dilakukan di Kota Surabaya dengan pembagian yang proporsional yaitu Surabaya pusat, Surabaya utara, Surabaya selatan, Surabaya barat, dan Surabaya timur. Pengukuran secara kuantitatif untuk megukur konsentrasi  particulate matter berukuran kurang dari 2,5 micron (PM2,5). Pengukuran  dilakukan di beberapa tempat di Kota Surabaya pada tahun 2015 menunjukkan bahwa sebesar 53% konsentrasi PM2,5 melebihi ambang batas yang ditetapkan WHO yaitu > 25µg/m3. Tempat umum merupakan tempat yang terbanyak dengan PM2,5 yang melebihi ambang batas. Pada tempat umum sebagai tempat hiburan tertutup dan adanya aktivitas merokok konsentrasi PM2,5 melebihi ambang batas yang ditetapkan yaitu 18 kali lebih besar dari standar. Potensi gangguan kesehatan tidak hanya diaami oleh perokok aktif namun dapat berdampak pada perokok pasif baik secondary smoker atau tertiary smoker. Diperlukan adanya kebijakan yang melindungi masyarakat seluruhnya terhadap potensi gangguan kesehatan yang ditimbulkan dari aktivitas merokok. Kebijakan dengan penegakan smoke-free area di tempat umum dan juga pada  sarana yang lain yang telah ditetapkan oleh pemerintah dengan adanya undang – undang atau peraturan.

karena pencemaran baik pencemaran udara, air, dan tanah. Pencemaran udara dengan sumber polutan primer salah satunya adalah partikel debu. Partikel debu sering disebut Particulate Matter (PM) dengan berbagai ukuran. Penggolongan dibagi menjadi Total Suspended Particulate (TSP) ; PM10 ; dan PM2,5 yang berasal dari sumber organik dan anorganik. Parameter dalam mengukur paparan lingkungan yang dapat menimbulkan masalah kesehatan bagi manusia adalah pengukuran konsentrasi debu dan ukuran partikel 1-3 mikron akan masuk langsung sampai permukaan jaringan dalam paru - paru.Hasil pengukuran pada tujuh sarana menunjukkan rata – rata kadar PM2.5 melebihi konsentrasi standar (> 25 µg/m3) dan kadar tertinggi di sarana umum (75,7 µg/m3), kemudian di sarana kegiatan anak (41,9 µg/m3). Selanjutnya tertinggi ketiga di sarana transportasi (36,3 µg/m3) dan keempat di sarana kesehatan (33,1 µg/m3) serta tertinggi kelima di sarana pendidikan (25,9 µg/m3). Diantara ketujuh sarana maka sarana ibadah dan tempat kerja menunjukkan hasil pemeriksaan dibawah standart.

Abstract

  Stroke is a non-communicable disease that causes death in the top ranks in urban areas which around 28.5% in Indonesia at 2011. Strokes begin at the productive age. This study aims to determine factors associated with knowledge about stroke in workers of educational institutions in Surabaya. This type of research is an observation analytic study using a cross-sectional study design. The questionnaire was given to 142 respondents. Dependent variables in this study were knowledge of stroke risk factors, level of initial stroke recognition, and how to handle it. The independent variables are age, sex, and level of education. Data analysis using chi-square test and spearman test. The results showed a level of knowledge of stroke risk factors 78.9% of respondents in the level of "good" knowledge about stroke risk factors, the level of recognition of early symptoms of the Face, Arm, Speech, and Time (FAST) method of respondents mostly in the category of "not good", and 63.4% of respondents had knowing how to handle it properly. The analysis showed that there was a significant relationship between sex with the level of knowledge of stroke risk factors (p = 0.020) and between the level of education with the level of recognition of early symptoms of stroke (p = 0.006; r = 0.229). The conclusion of this study is that respondents in the level of knowledge in both categories regarding stroke risk factors, how to do the treatment is correct, the health service chosen is also correct that is the hospital, the duration of treatment is less than 3 hours and only the level of recognition of early symptoms of stroke still not well known.

Abstract

Increased prevalence of overweight and obesity is associated with an increased incidence of type 2 Diabetes Mellitus (DM). Abdominal obesity is an important risk factor for the onset of DM. This study aims to determine the relationship between body mass index, abdominal circumference, and the ratio of hip waist circumference with blood sugar levels. The study design was cross-sectional. The study population was members of Integrated Health Posts for the elderly (Posyandu lansia) aged 45 to 70 years in the working area of the Public health center (PHC) of Menur registered in the Posyandu activities in May 2013. The number of respondents was 60 respondents taken using the simple random sampling method. The variables studied were respondent characteristics, body mass index, abdominal circumference, hip waist circumference ratio, and fasting blood sugar levels. The results showed that respondents were overweight (38%), obesity (33.3%), abdominal obesity based on abdominal circumference (77.8%), abdominal obesity based on the hip waist circumference ratio (81.7%), and blood sugar levels of125 mg/dl (8.3%). Pearson correlation test shows that body mass index has a relationship with blood sugar levels (p = 0.007; r = 0.345), abdominal circumference has a relationship with blood sugar levels (p = 0.001; r = 0.424,) and hip waist circumference ratio is related to blood sugar levels (p = 0.002;r = 0.392). The abdominal circumference has the strongest relationship with blood sugar levels. Abdominal circumference measurements should be used in elderly Posyandu activities to detect individuals at high risk of diabetes.

Abstract

The government held a diphtheria sub-national immunization week in 2012 to overcome the problem of diphtheria outbreaks in East Java Province. Bangkalan District is the district with the high incidence of diphtheria in East Java Province. This study aims to analyze the determinants of diphtheria events in Bangkalan District after the diphtheria sub-national immunization week in 2012. This study uses a case-control study design. Case samples were taken from a total of 31 cases, and control samples were taken as many as 124 people spread in 25 case villages and25 control villages. The bivariate analysis a using chi-square test and simple logistic regression. Multivariate analysis using logistic regression. The results showed that based on the results of bivariate analysis, individual and household levels related to diphtheria events, namely the status of Diphtheria Pertussis Tetanus (DPT) immunization, sub-national immunization week status, age and mother's education. Multivariate analysis showed that factors related to the diphtheria incidence in Bangkalan District after the 2012 diphtheria sub-national immunization week were DPT immunization (p = 0.012; OR= 4.765), incomplete DPT immunization (p = 0.001; OR = 6.276), age 3- 7 years (p = 0.014; OR = 15.137), ages 7-15 years (p = 0.001; OR = 41.984), and are not immunized at the time of sub-national immunization week (p = 0.020; OR = 3.553). The conclusion of this study is the DPT immunization status, age and status of diphtheria sub-national immunization week were the dominant factors affecting the incidence of diphtheria in Bangkalan District.

Abstract

The Maternal Mortality Ratio (MMR) in Indonesia remains high, i.e approximately 359 per 100.000 life birth (IDHS 2012). MMR is an indicator of mother’s health, especially the risk of being death for a mother while pregnant and delivery. Mostly the majority of MMR is occurring in the first two days after delivery and care after giving birth services required to manage complication. Sidoarjo regency has high postpartum maternal mortality case, so it is necessary to study determinants influencing postpartum maternal mortality in that regency. This research aimed to analyze the determinants that influence postpartum maternal mortality. This research was an observational research using case control study. Number of samples was 21 cases and 43 controls. Data were analyzed by univariate analysis, bivariate analysis with chi-square test, and multivariate analysis with multiple logistic regressions. The result showed that the determinants which influence postpartum maternal mortality according to multivariate analysis were pre-eclampsia / eclampsia (OR = 20,98; 95%CI : 2,250 – 323,416; p = 0,008) and delivery complication (OR = 5,47; 95%CI=1,356 – 22,022; p = 0,017). Probability of mother to have risk of postpartum maternal mortality with all those risk factors above was 92,9%. This research recommended are need to detect early sign of pregnancy, delivery, and post delivery complication, especially danger sign of pre-eclampsia/eclampsia, referral preparation, and pregnancy planning.

Abstract

The Directly Observed Treatment, Short-course (DOTS) Strategy is a tuberculosis control program. This study aims to evaluate the implementation of the DOTS program in Public Health Center (PHC) of Tanah Kalikedinding whose results are associated with the achievement of tuberculosis indicators. This study uses a descriptive design with a population that is all lung specialist doctors, tuberculosis officers, laboratory workers, and pulmonary tuberculosis patients. Sampling technique with purposive sampling. Data collection by interview questionnaire and checklist. The variables of this study are case finding, treatment, driving and inhibiting factors, recording and reporting, and outcomes based on tuberculosis indicators. The results showed the Case Detection Rate (CDR) at 2013 was 112% meeting the national target ≥70%. This success was due to the selection of up to 87.5% in suspected tuberculosis (cough> 2 weeks) and all patients were examined according to the diagnosis flow in the guidelines of the Ministry of Health of the Republic of Indonesia, while the Success Rate (SR) at 2013 was 65.5% under the target ≥85%. This is due to the fact that there are still patients who do not have a Drug Supervisor (PMO). The reinforcing  factor is routine counseling by staff at the Public health center (PHC). The inhibiting factor is the distance to the PHC which most of the patients are more than 1 km (65.6%). Recording and reporting using an electronic system and reported online. The conclusion of the study is the need for drug swallowing supervision to improve the success of the treatment is expected that all tuberculosis patients have PMO.

Abstract

Hypertension is a disease that causes morbidity and mortality in 20-50% of total deaths. Obesity and smoking are risk factors for hypertension. This study aims to determine the relationship between obesity, and smoking on the incidence of hypertension. The study was conducted with a cross-sectional design involving 75 people and was taken using simple randomization techniques from patients seeking treatment at the cardiology clinic in Public Hospital of Haji Surabaya in May 2014. The variables studied were gender, obesity, and smoking behavior. Smoking behavior that was studied consisted of smoking history, use of filters, duration of smoking, number of cigarettes, and types of cigarettes. Data were analyzed using Chi-Square test. The incidence of hypertension found in this study was 45 people (60%). 64.4% of people with hypertension are women. The analysis showed a significant relationship between obesity and the incidence of hypertension (p = 0.014) with a strong relationship of 0.299. History of smoking (p = 0.211), use of filters (p = 0.378), duration of smoking (p = 1,000), smoking criteria (p = 0.848) and types of cigarettes (p = 0.673) showed no significant relationship. The conclusion of this study is obesity is related to the incidence of hypertension.

Abstract

Stroke is a cerebrovascular disease that affects paralysis. Family support is something that needs to be considered in the handling of stroke patients because it is very instrumental in the compliance of patients undergoing rehabilitation to prevent stroke recurrent. Characteristics of stroke patients can also affect patient compliance in undergoing rehabilitation. The purpose of this study was to determine the relationship between the characteristics of stroke patients and family support with adherence to undergo rehabilitation in the Medical Rehabilitation Unit of the Haji Public Hospital in Surabaya. This research was an analytic observational with cross-sectional design. Samples were taken with a total population technique, as many as 22 respondents. The independent variable in this study was family support. The dependent variable is rehabilitation compliance. The results of the study are presented in the form of frequency distribution and calculate the strength of the relationship by looking at the coefficient Phi. The results of this study are that there is a strong relationship between family support and adherence to undergo rehabilitation (r = 0.582). There was a weak relationship between age (r = -0.027), sex (r = 0.092), level of education (r = -0.295), occupation (r = 0.098), and marital status (r = 0.319) and adherence to rehabilitation. Based on the results of the study, it can be concluded that family support can affect the compliance of stroke patients in undergoing rehabilitation.

Abstract

The epidemiological surveillance system of Dengue Hemorrhagic Fever (DHF) currently underway at the Mataram City Health Office has not been able to provide information on the incidence of DHF based on risk factors. Data processing and analysis are also still done manually. The purpose of this study is to develop a database of risk factors for DHF events. This research is action research. This research was conducted at the Mataram City Health Office in West Nusa Tenggara (NTB) province from April 2014 to August 2014. The informants in this study consisted of 3 people, which are the Head of The Disease Prevention and Control Program (P2P), DHF Prevention and Control Program (P2DHF) officers, and Disease Surveillance Officer. The type of data used is primary and secondary data. The database design includes logical and physical design. In the logical design, data normalization, making relationships between data, describing Entity Relationship Diagrams (ERD), and proceed to a physical design by making a database prototype using Epi Info software application. The trial was conducted involving 2 (two) informants. Evaluation of the trial of the surveillance database for risk factors for dengue events, namely by assessing the level of ease, speed, accuracy, and completeness of the resulting data. The results of this study are the existence of a new DHF risk factor surveillance database that can be used easily, quickly, and can produce more accurate information.
