Abstract

Leprosy is a chronic disease that attacks the skin, peripheral nerves, to other body organs which can cause permanent disability. Sampang is an area with the highest average prevalence rate (PR) of leprosy cases in the period 2010-2014 which has a proportion of level II disability of 13%, while the standard set is no more than 5%. The purpose of this study was to analyze the effect of health services on the incidence of leprosy level II disability. This type of research is observational-analytic research with a case-control design. This research was conducted in 8 working areas of the Public health centers (PHC) in Sampang District. The sample was Multi Basiler (MB) type of leprosy which consisted of 33 people with level II disabilities as a case group and 33 people with leprosy who had level 0 or level I disabilities as a control group. Sampling is done using a simple random sampling technique. Data analysis was performed simultaneously with the logistic regression test. The results showed that the passive case finding method (OR = 7,334; 95% CI = 1,643–32,739), very late diagnosis time (OR = 15,267; 95% CI = 1,447–161,071), lack of Prevention of Disability (POD) (OR = 7,016; 95 % CI = 1,574–31,274) and lack of counseling (OR = 9,154; 95% CI = 1,786–46,906) influenced the incidence of level II leprosy disability. The conclusion of this study is that the case-finding method, diagnosis time, POD, and counseling affect the incidence of level II leprosy disability in patients. It is recommended to actively increase case finding and improve counseling activities.

Abstract

The default treatment is related to the patient's regularity in taking medication. Irregular drug consumption, the leprosy bacteria can become resistant or immune to Multi-Drug Therapy. The purpose of this study was to determine the indicators and formulate a predictive index formula for the default incidence of leprosy treatment. This study uses a case-control design. The sample size in this study was 62 respondents. The sampling technique uses a simple random sampling technique. Data collection techniques were carried out with interviews with the help of questionnaires. The results showed that the variables that became indicator candidates (p <0.25) were income (p = 0.013), access to health services (p = 0.022), the role of health workers (p = 0.032), knowledge (p = 0.001), and family support (p = 0.023), perception of disease (p = 0.098), leprosy reaction (p = 0.198). The seven variables are indicators of the incidence of leprosy treatment defaults, namely the role of health workers, family support, knowledge, and income. Formula predictive index of the default occurrence of treatment of people with leprosy type multibacilary is (-1,666 + 1,451 * Income (low) + 1,989 * Role of Health Officers (less involved) + 1,338 * Knowledge (less) + 1,543 * Family Support (less support)). This index has an accuracy rate of 79%. This index is expected to be used as a measurement tool to help health workers in screening leprosy patients who are currently undergoing treatment, so that early on it can be known which sufferers have the potential to default treatment.

Abstract

Stroke is a non-communicable disease that is influenced by many risk factors. Some risk factors for stroke have similarities with the criteria for metabolic syndrome, namely abdominal obesity, glucose intolerance, reduction of High-Density Lipoprotein (HDL) cholesterol, increased triglyceride levels, and insulin resistance. This study aims to determine the relationship between metabolic syndrome and the incidence of stroke. This type of research is analytic observational with a case-control design. The case group is stroke patients who are undergoing hospitalization at the Regional Public Hospital of Dr. M. Soewandhie Surabaya. The case group was 32 people and the control group was 32 people, randomly selected. The variables studied were age, sex, metabolic syndrome the first criteria, metabolic syndrome the second criterion, and metabolic syndrome the third criterion. The results showed that the majority of stroke patients in the age range of 51-75 years (78%). Chi-Square test results showed a significant relationship between the first criteria metabolic syndrome (abdominal obesity, triglyceride levels, and HDL cholesterol levels) p = 0.01 OR = 6.82 (95% Cl = 1.23-68.17) ) and the second criterion of metabolic syndrome (HDL cholesterol level, blood pressure, and fasting glucose level) p = 0.007 OR = 5.80 (95% Cl = 1.30-35.15) with the incidence of stroke. It is expected that people routinely carry out lipid profile checks and waist circumference measurements to detect metabolic syndrome and also the risk of stroke.

Abstract

Background: The prevalence of hypertension in Indonesia was 25,80% in 2013 found in people aged above 18 years old. One of the risk factors is obesity. Purpose: This study aimed to analyze the relationship between the demographical characteristic and central obesity with the risk of hypertension in the health center of Sidoarjo. Methods: This analytical observational study design was cross sectional. There were 50 respondents selected through accidental sampling. The independent variables of this study were respondents’ characteristic and central obesity status. However, the dependent variable was hypertension. Overall, respondents were divided into two groups based on sex (female and male) and age (> 59 years old and ≤ 59 years old). Data were analyzed with chi-square statistical analysis. Results: Respondents with central obesity had a higher risk to get hypertension, with majority male respondents aged above 59 years old. Conclusion: There was a correlation between age and hypertension (p = 0,01), sex and hypertension (p = 0,04), also between central obesity status and hypertension (p = 0,01).

Abstract

Background: Smoking is a major risk factor that causes lung cancer. The high number of active smokers in developing countries is found in several countries; one of those countries is Indonesia. Purpose: The purpose of the study was to describe the degree of smoking based on brinkman index on lung cancer patients. Methods: It was a descriptive study. It employed cross sectional research design. The population in this study included lung cancer patients in Outpatient and Inpatient Dr. Soetomo Regional Public Hospital, Surabaya. The independent variable in this study was the degree of smoking. Whereas, the dependent variable in this study was lung cancer. Accidental sampling was used as the sampling technique of the study. The total sample were 31 respondents. The instrument used for collecting data was questionnaire containing the risk factors of lung cancer in November 2018. The categorization of smoking degrees used the Brinkman Index measurement. Data were processed by univariate analysis which was presented in the form of a frequency distribution table between variables. Results: Almost all of the respondents were on the age of > 40 years (90.30%) and male (96.80%). The majority of respondents were senior high school graduates (38.70%), and had other jobs (32.20%). Almost half of the respondents were classified into heavy smoking degree according to the brinkman index (48.40%). Conclusions: Some of cancer patients in Dr. Soetomo Regional Public Hospital Surabaya has a smoking degree of heavy category.

Abstract

Background: Coronary heart disease (CHD) is a non-communicable disease that causes the highest mortality in the world, including in Indonesia. Risk factors for CHD are divided into modifiable and non- modifiable risk factors. Purpose: This study aims to discover the description of risk factors that are modifiable in coronary heart disease patients at Dr Soetomo Regional Public Hospital. Methods: It was a descriptive observational study with cross sectional study design. The study population was coronary heart disease patients who were doing outpatient treatment at the Integrated Heart Service Center (PPJT) of Dr. Soetomo Regional Public Hospital. The number of study sample was 72 respondents using accidental sampling technique. Data sources used are primary data using questionnaires and secondary data using medical record. Data were collected during November 2018. The location of this study was Dr. Soetomo Regional Public Hospital. The data analysis technique chosen was univariate analysis and presented in the form of a frequency distribution table. Results: The results of the study showed that the characteristics of respondents were mostly between 56-65 years old (43,05%) and male (70,84%). Risk factors found on the respondents were smoking (84,72%), hypertension (72,22%), hyperlipidemia (68,05%), diabetes mellitus (81,94%) and poor physical activity (77,77%). Conclusion: Modifiable risk factor that was mostly found on coronary heart patients was smoking, while least one was hiperlipidemia.

Abstract

Background: Green tobacco sickness (GTS) is a type of disease that is still not widely understood, but is often experienced by tobacco farmers. The duration and frequency of contact, the work procedures associated with wet tobacco, and individual vulnerability are risk factors for GTS. Purpose: This study aimed to measure the factors associated with incidences of GTS in tobacco farmers in Jember, Indonesia, to contribute to the prevention of GTS. Method: The research design used case-control. This research was conducted in Jember District, within a group of tobacco farmers. The case group included farmers who experienced GTS and the control group included farmers who did not experience GTS. The study measured age, sex, nutritional status, passive smoking status, alcohol consumption status, subjective complaints, individual hygiene, Occupational Health and Safety (OHS) attitudes, OHS actions, use of personal protective equipment, principal occupation, extra work, type of tobacco leaf, length of service, length of work-rest period, and workload. The study population were tobacco farmers. The sample size was determined using the Lemeshow sampling formula for case-control. The sampling technique was simple random sampling. Data analysis was conducted using chi-squared tests. Results: Variables related to GTS cases in tobacco farmers were sex (p = 0.01) and nutritional status (p = 0.03). There were no significant occupational factors. Conclusion: Factors that influence GTS are sex and nutritional status.

Abstract

Background: Stroke is a disease condition caused by the sudden stopping of the blood supply to the brain, either because of a blockage or ruptured blood vessels. The prevalence of stroke in Indonesia based on Basic health research in 2013 was 7‰ and increased in 2018 to 10.9‰. Purpose: This research aims to determine the effect of smoking behaviour on the prevalence of stroke in Surabaya, Indonesia. Methods: It was a cross-sectional study. The study population included all visitors to nerve poly at Haji Hospital Surabaya. Data were collected from respondents through interviews and patient medical record data from August to September 2019. Univariate and bivariate analyses were done to identify the association between smoking and stroke incidence. Results: A total of 187 people participated in the study. The majority of them were aged ≥ 55 years (78.16%), with 55.17% being male. There is 32.18% active smoker and 28.74% passive smoker for stroke patients. There is an association of the type of non-filter cigarettes with the prevalence of stroke p = 0.01, PR = 4.02; 95% Cl = 1.38-11.67), and duration of exposure ≥30 years (p = 0.01, PR = 7.84; 95% Cl = 2.75-22.32) Conclusion: there is an influence of types of non-filter cigarettes and exposure time ≥ 30 years with the incidence of stroke in the city of Surabaya.

Abstract

Background: Hypertension is still a major health problem in the world. One of the factors causing hypertension is exposure to cigarette smoke. Purpose: The aim of this study was to analyze the relationship between cigarette smoke exposure and the incidence of hypertension in housewives in houses in Dampit village. Methods: This research adopted an analytical observational approach. This was a cross-sectional design study. The population for this study was all housewives in Dampit village in March 2020. A total number of 115 housewives was selected using the cluster proportional random sampling technique. The dependent variable was the incidence of hypertension; the independent variables were the duration of cigarette smoke exposure in houses, the number of instances contact was made by being in close proximity to smokers, and the contact duration with smokers. Data collection was done by interviewing respondents using a questionnaire. Data analysis was carried out using frequency tables and Epi Info. Results: The study indicated that there was a relationship between the duration of cigarette smoke exposure (over 33 years) in houses with the incidence of hypertension in housewives (p=0.01; OR=3.52; 95%CI=1.53–8.05). Similarly, there was a relationship established between the number of occasions contact was made by being in close proximity to smokers every day (over three times every day) (p=0.01; OR=36; 95%CI=7.64–168.76) and the contact duration with smokers (p=0.01; OR=8.09; 95%CI=3.13–20.87) with the incidence of hypertension in housewives. Conclusion: There was a link between the duration of cigarette smoke exposure in houses, the number of instances of being in close proximity to smokers every day, and the contact duration with smokers with the incidence of hypertension in housewives in Dampit village.

Abstract

Background: The earthquake that hit the Jailolo sub-district in 2015 caused massive damage and loss. This catastrophic event affected not only impacted the local government's economy but also affected many communities, households and individuals living in these communities. Purpose: Aim of this study is to assess the economic resilience of communities in the Jailolo sub-district in response to earthquakes. Methods: This research was based on a descriptive observational study and employed a survey method to assess the economic resilience of communities in the Jailolo sub-district. The study was conducted in five villages, namely Tedeng, Payo, Saria, Matui, and Buku Maadu. The cut-off point for each indicator was classified as very high criteria (>1.05), high (0.95–1.05), moderate (0.85–0.94), low (0.74–0.84), and very low (≤0.73). Results: The proportion of community home ownership was found to be 100% (Resilience Factor Index (RFI)=1.67). The proportion of community work was 33.75% (RFI=0.68). The proportion of dual-income sources of communities in the Jailolo sub-district was 50.89% (RFI=1.02). The proportion of community income that exceeded the provincial minimum wage (PMW) was 8.71% (RFI=0.10). Based on the results of these indicators, the economic resilience of people in the Jailolo sub-district, which was obtained by considering the average RFI of each indicator, was 0.86. Conclusion: Community economic resilience in the Jailolo sub-district was found to be in the medium category. The highest and lowest resilience factors resulted from home ownership and income, respectively.
