Abstract

This study aims to design and develop a Distributed Denial of Service (DDoS) attack detection system based on Wavelet Decomposition capable of identifying network traffic anomalies in real-time. The main problem addressed is the high false positive rate in conventional detection methods, which often fail to distinguish between legitimate traffic bursts and actual attacks. Two primary data sources were used: the CICIDS2017 dataset and self-generated data representing controlled DDoS attack patterns. The proposed method applies Discrete Wavelet Transform (DWT) to decompose network traffic signals into amplitude and energy components. Detection is then performed using the Median Absolute Deviation (MAD) approach, optimized with three parameter search methods: Grid Search, Random Search, and Optuna. Experimental results indicate that the energy-based method with Optuna optimization achieves the best performance, with an accuracy of 98.6% on the CICIDS2017 dataset and 99.4% on the self-generated data, and error rates of 1.4% and 0.6%, respectively. This research contributes to enhancing the accuracy of DDoS detection systems with low computational overhead, making it suitable for large-scale network environments.

Abstract

Background: Schizophrenia is a chronic mental disorder characterized by psychotic symptoms such as hallucinations and delusions. Relapses in schizophrenia patients often occur even after undergoing pharmacological treatment. Cognitive Behaviour Therapy (CBT) has been reported to be effective in reducing relapses through cognitive restructuring and improving patients' coping with psychotic symptoms. This systematic review aims to assess the effectiveness of cognitive behavioral therapy (CBT) on reducing relapse rates in patients with schizophrenia, especially in hallucination. Methods: The systematic review was conducted using the JBI approach. The included articles were primary studies with an RCT or quasi-experimental design, published between 2015 and 2024, and available in full text. The selection process was conducted using the PRISMA method, and the final results consisted of 7 articles that met the inclusion criteria. Results: Most studies show that CBT is effective in reducing relapse, improving quality of life or social functioning, and reducing positive and negative psychotic symptoms. Culturally-based CBT and CBT by videoconference have been reported to expand access to intervention. The side effects of CBT are relatively low, but some studies report a decline in impact after therapy is discontinued. Conclusion: CBT is effective in preventing relapse in schizophrenia patients with hallucinations by reducing psychotic symptoms and improving patient coping. Further research is needed to evaluate the long-term effectiveness of CBT.

Abstract

Pencarian menggunakan English keywords: (1) drug abuse or drug dependence, (2) relapse prevention therapy, (3) problem solve therapy, (4) substance abuse relapse.

Abstract

Background: Diabetic foot ulcers (DFUs) remain a significant clinicalchallenge, requiring precise classification systems to aid prognosis and treatment planning. The Wagner classification is widely used but offers limited detail on specific wound characteristics, while the SHID (Suriadi, Haryanto, Imran, Defa) system provides a more comprehensive evaluation but lacks validation. Purpose: This study was designed to evaluate and compare the predictive validity of the Wagner and SHID classification systems in forecasting DFU healing outcomes. Methods: A prospective cohort study was conducted at Kitamura Clinic and Doctor Soedarso Pontianak Hospital between August 2021 and July 2022, involving 89 DFU patients. Both systems were evaluated based on sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and likelihood ratios. Predictive validity was determined using receiver operating characteristic (ROC) curve analysis and the Youden index. Results: A cut-off grade of >2 provided optimal predictive value for both systems. The SHID classification demonstrated a sensitivity of 92%, specificity of 62%, PPV of 27.5%, and NPV of 98%, while the Wagner classification showed a sensitivity of 58%, specificity of 77%, PPV of 28%, and NPV of 92.2%. Positive likelihood ratios were 2.4 for SHID and 2.5 for Wagner. ROC analysis yielded an area under the curve (AUC) of 0.786 (95% CI: 0.69–0.87) for SHID and 0.703 (95% CI: 0.60–0.80) for Wagner. The Youden index was higher for SHID (0.540) compared to Wagner (0.349). Conclusion: Although both systems are effective for predicting DFU healing within 12 weeks, SHID’s superior AUC and Youden index suggest greater clinical utility in screening and managing DFUs. Abstract

The high incidence of traffic accidents in urban areas necessitates rapid first-aid responses before professional healthcare providers arrive at the scene. Online motorcycle taxi (ojek online) drivers are community members who are frequently present at accident sites and have the potential to act as first responders; however, they generally lack adequate knowledge and skills in providing physical and psychological first aid. This community service program aimed to strengthen first-aid preparedness through the establishment of the ToS-ReAct To PPFA Task Force (Tolong Sesama – Responsive and Rapid Action for Physical and Psychological First Aid) among online motorcycle taxi drivers in Depok City. The program employed educational and training methods combining theory and practice, covering basic first-aid concepts, Psychological First Aid (PFA) using the 3L approach (Look, Listen, Link), and physical first-aid skills such as cardiopulmonary resuscitation (CPR), simple splinting, and bleeding control. Program evaluation was conducted through pre- and post-tests of knowledge, observation of practical skills, and participant feedback. The results demonstrated an increase in the mean knowledge score from 54 to 72, representing a 42.31% improvement. Furthermore, all participants (100%) were able to correctly demonstrate the components of the PFA 3L approach and physical first-aid skills. This program proved effective in enhancing the capacity of online motorcycle taxi drivers as community first responders and has the potential to strengthen local emergency preparedness systems.
