ABSTRAKLatar Belakang: Permasalahan pengelolaan obat ditandai keluhan dari Puskesmas karena tidak terpenuhinya permintaan obat sehingga menimbulkan kekosongan obat. Penelitian ini bertujuan mengevaluasi pengelolaan obat pada tahap perencanaan dan pengadaan di UPTD Farmasi Dinas Kesehatan Kabupaten Mukomuko. Penelitian ini menggunakan pendekatan mix method study dengan sampel item obat di LPLPO, informan berdasarkan kecukupan dan kesesuaian, jumlah 18 informan. Tempat penelitian di UPTD Farmasi Dinas Kesehatan Kabupaten Mukomuko. Hasil penelitian kuantitatif berdasarkan metode analisis ABC-VEN dengan matriks ABC-VEN perencanaan pengadaan obat tahun 2021 belum tepat dan sesuai dengan kebutuhan.Metode: Penelitian kualitatif komponen input ditemukan kekurangan SDM dan belum membentuk TPOT, pembiayaan belum mencukupi, kebijakan mengacu pada perundang-undangan berlaku, sumber data lengkap, keterlambatan Puskesmas menyampaikan data, keakuratan data diragukan. Sarana prasarana sudah mencukupi. Komponen proses, perencanaan belum mengacu pada Kepmenkes Nomor 1121/MENKES/SK/XII/2008, pengadaan mengacu pada peraturan perundangan berlaku.Hasil : Komponen out put indikator standar : persentase kesesuaian pengadaan dengan kenyataan pakai untuk masing-masing item obat 124,8%, frekuensi pengadaan tiap item obat 1 kali setahun, frekuensi kurang lengkapnya surat pesanan/kontrak tidak ditemukan, frekuensi tertundanya pembayaran tidak ada. Indikator belum standar : kesesuaian item obat dengan DOEN 88,4 % dan Fornas TK 1 73,3%, ketepatan perencanaan 10,6%, persentase dana tersedia dengan keseluruhan dana dibutuhkan 39,1%, persentase alokasi dana pengadaan obat 1,3%, pengadaan obat esensial sebesar 85,3 %, pengadaan obat generik 98,7 %.Simpulan: Untuk optimalnya perencanaan dan pengadaan obat perlu peningkatan kapasitas SDM, tersedianya kerangka acuan kegiatan pendidikan pelatihan, monev yang optimal dan pembentukan TPOT.Kata Kunci : Evaluasi, Perencanaan, Pengadaan, Obat ABSTRACT Background: Problems with drug management were marked by complaints from the Puskesmas because the demand for drugs was not fulfilled, resulting in drug shortages. The infrastructure is sufficient. The process component, planning does not refer to Kepmenkes Number 1121/MENKES/SK/XII/2008, procurement refers to applicable laws and regulations. Results: Components of standard indicator output: percentage of conformity of procurement with actual use for each drug item 124.8%, frequency of procurement of each drug item once a year, frequency of incomplete orders/contracts not found, frequency of delayed payments absent. Non-standard indicators: compatibility of drug items with NLEM 88.4% and Fornas Kindergarten 1 73.3%, accuracy of planning 10.6%, percentage of available funds with all required funds 39.1%, percentage of drug procurement fund allocation 1.3%, procurement of essential drugs 85.3%, procurement of generic drugs 98.7%. Conclusion:To optimize the planning and procurement of drugs, it is necessary to increase the capacity of human resources, provide activity framework for education and training, optimize monitoring and evaluation and establish TPOT.Keywords: Evaluation, Planning, Procurement, Medicine
