Abstract

Chronic Kidney Disease (CKD) is a condition where there is structural or functional damage to the kidneys for more than 3 months with or without being followed by a decrease in the estimated Glomerular Filtration Rate (eGFR) and is progressive and irreversible. Hypertension is the most common comorbidity found in patients with CKD which can increase the progression of kidney damage. Objective: To determine the relationship between the stage of CKD and hypertension and the distribution of the frequency of patients with CKD based on age, sex, eGFR value, and hypertension. Methods: This research was an observational analytic study with a cross-sectional design. The research was conducted at the Medical Record Installation of RSUP Dr. M. Djamil from December 2022 - March 2023 used a purposive sampling technique and obtained 33 samples. The research data was secondary data in the form of blood pressure and serum creatinine and then analyzed using the Chi-Square test with a degree of confidence of 95% and p-value <0,05. Results: This study showed that most cases of CKD occurred in the age group 41-65 years (54,5%) and mostly found in men (57,6%). In this study, stage 5 CKD was the most common (72,7%) and the majority of patients with CKD suffered from hypertension (69,7%). The results of data analysis obtained p = 0,090 (p> 0.05). Conclusion: There was no significant relationship between the stage of CKD and hypertension (P=0,090) in adult patients at Dr. M. Djamil Padang in 2020–2021.

Abstract

Anemia remains a significant public health issue among female adolescents, particularly due to iron deficiency. The consumption of iron (Fe) tablets is a primary government-recommended intervention, yet adherence levels remain low. This study aimed to analyze the factors influencing Fe tablet consumption behavior among female adolescents in the working area of Nan Balimo Public Health Center, Solok City. A mixed methods approach with an explanatory sequential design was employed. A total of 293 female adolescents were selected using proportional random sampling and analyzed using chi-square tests and multiple logistic regression. Qualitative data were obtained through in-depth interviews with 11 informants. Multivariate analysis revealed four variables significantly associated with Fe tablet consumption behavior: knowledge, attitude, role of health workers, and media exposure (p ≤ 0.05), with the role of health workers emerging as the dominant factor (p = 0.002; POR = 2.655; 95% CI: 1.429–4.935). The qualitative findings supported these results, highlighting challenges such as irregular tablet distribution, monotonous education, weak monitoring, and suboptimal coordination, all contributing to a stagnation in consumption rates, which declined from 96.6 % to 83.06 % over three consecutive quarters. Enhancing the role of health workers is a strategic key to improving Fe tablet adherence among female adolescents. Therefore, it is essential for health centers and health authorities to strengthen communication skills, counseling, and health worker support capacity, while also ensuring the availability of engaging educational media and intensive monitoring.

Abstract

Premature rupture of membranes is the most common complication of pregnancy. The incidence of premature rupture of membranes in pregnancy ranged from 6% to 10%, and 20% of these cases occur before 37 weeks gestation. The incidence of premature rupture of membranes in Indonesia ranges from 4.5% to 7.6% of all pregnancies. This research was conducted to determine the cross-sectional differences in the blood plasma levels of vitamin C in term pregnancy premature rupture of membranes with blood plasma levels of vitamin C in term pregnancy without premature rupture of membranes in M. Jam- il Padang hospital, Achmad Muchtar Bukittinggi hospital, and Pariaman Hospital. There are significant differences in vitamin C blood plasma levels in term pregnancy with premature rupture of membranes and term pregnancy without premature rupture of membranes ( P < 0.05). Mean levels of vitamin C in blood plasma at term pregnancy with premature rupture of membranes lower than in the blood plasma levels of vitamin C in term pregnancy without premature rupture of membranes.Keywords: Premature rupture of membrane in aterm, blood plasma levels of vitamin C

Abstract

West Pasaman District and Agam District are filariasis-endemic areas in West Sumatra with prevalence of 12.40 per 100,000 cases and 11.27 per 100,000 cases respectively. This study aimed to determine risk factors associated with the prevalence of filariasis and mapping the vulnerability of the area in West Sumatra. A case control study design was conducted in the West Pasaman District and Agam District. The study used a sample of 74 cases and 74 controls taken by simple random sampling for the case and purposive random sampling for control. Data analysis used univariate, bivariate, multivariate and spatial. The results in Agam District showed that variables associated relation with filariasis (p value < 0.05) were the level of knowledge, plantations, the paddy, the ceiling of the house. The most dominant risk factor was knowledge. Type of vector that was found was Culex. While in West Pasaman District, the variable associated with filariasis (p value < 0.05) was the use of mosquito nets, the habit of dressing, marshes, and ceiling. The most dominant risk factor was the ceiling of the house.

Abstract

Optimal nutrition significantly impacts human health, particularly for primary school-aged children. Malnutrition manifests in various health issues, such as stunting, underweight, micronutrient deficiencies, and obesity. The prevalence of malnutrition among school-aged children in Indonesia, including Batam City, is rising yearly. This study aimed to analyse factors affecting the nutritional needs of elementary school children in Batam City. The study was conducted over one month and employed a qualitative approach. Participants included 39 respondents: 13 children, 11 parents, 5 peers, 5 teachers, and 5 school health centre officers, selected purposively. Data collection used open-ended responses through in-depth interviews. Findings revealed limited knowledge among children about balanced nutrition. All informants emphasized the importance of consuming diverse foods to meet nutritional needs, with children showing a positive attitude toward trying various foods. The respondents also highlighted the importance of avoiding impulsive snacking. Moreover, the study identified a positive relationship between physical activity and cognitive focus in learning, underscoring the need for regular physical activity. Programs addressing the specific nutritional challenges faced by children in Batam could enhance awareness and encourage healthy eating habits among elementary students.

Abstract

Indonesia’s aging population presents an increasing demand for accessible, community-based health services. However, the utilization of Rumah Kasih, a community health program for older adults, remains suboptimal. Objective to examine the reinforcing factors influencing its utilization, including support from healthcare workers, university students, community health workers (CHWs), village heads, family members, and peers. A quantitative, cross-sectional study was conducted involving 192 elderly respondents with total sampling. Data were collected using questionnaires and analyzed using chi-square tests and multivariate logistic regression. The bivariate analysis indicated that all support factors were significantly associated with service utilization (p < 0.05). Multivariate analysis identified healthcare worker support as the most influential predictor (OR = 48.346; p < 0.001), followed by student involvement (OR = 5.259; p = 0.005). Although family support was significant in the bivariate analysis, it was not retained in the multivariate model. CHW and peer support yielded high odds ratios but lacked statistical significance, possibly due to data variability or multicollinearity. These findings highlight the critical role of professional engagement and student participation in enhancing elderly involvement in community health programs. Strengthening these factors, along with targeted strategies to involve social actors, may improve the effectiveness and long-term sustainability of elderly care services.

Abstract

Anxiety in pregnant women is a common emotional condition that occurs during pregnancy, characterized by excessive feelings of anxiety, worry, or fear. Pregnancy anxiety itself is experienced by many pregnant women due to lack of knowledge, readiness and support from family, community and environment. Excessive anxiety during pregnancy can lead to an increased incidence of uterine atony, infection, maternal fatigue, and shock, while in infants it can increase the risk of preterm birth and low birth weight (LBW). This study aims to examine the relationship between family support and maternal readiness with the anxiety level of pregnant women before delivery. This study used a quantitative research design with a cross-sectional approach, conducted in the working area of Andalas Padang Community Health Center. The sample consisted of 59 third-trimester pregnant women, selected through Simple Random sampling. Data collection was carried out using a questionnaire. The results of this study showed that the percentage of pregnant women who received family support and experienced less severe anxiety was 52.2% (p=0.048), including analysis of this data using the Chi-Square Test. Furthermore, it was found that pregnant women who were less prepared for childbirth experienced severe anxiety at a rate of 50.0% (p=0.002). In conclusion, there is a relationship between family support and maternal readiness with anxiety levels before delivery in the working area of Andalas Padang Community Health Center.

Abstract

Ketidakpatuhan pasien tuberkulosis dalam pengobatan merupakan penyebab terpenting kegagalan pengobatan dan menjadi hambatan untuk mencapai kesembuhan Tujuan penelitian ini yaitu untuk mengetahui faktor-faktor yang berhubungan dengan kepatuhan berobat pasien tuberkulosis di Puskesmas Andalas. Penelitian ini adalah penelitian analitik observasional dengan rancangan cross sectional. Teknik pengambilan sampel menggunakan total sampling dengan jumlah sampel sebanyak 53 responden. Sampel penelitian adalah pasien tuberkulosis sensitif obat yang berusia diatas 18 tahun dan memiliki kartu TB-01. Pengumpulan data dilakukan dengan wawancara terstruktur menggunakan kuesioner. Analisis data menggunakan uji  Chi-square. Analisis 53 sampel didapatkan sebanyak 43 responden (86,8%) patuh dalam pengobatan, sedangkan sebanyak 7 responden (13,2%) tidak patuh dalam pengobatan TB paru. Hasil penelitian menunjukkan variabel yang berhubungan dengan kepatuhan berobat adalah efek samping obat (p=0,015) dan peran pengawas minum obat (p=0,001), dan variabel yang tidak berhubungan adalah umur (p=0,419), jenis kelamin (p=0,223), tingkat pendidikan (p=0,443), pekerjaan (p=1,000), lama pengobatan (p=0,661) dan jarak rumah (p=0,451). Kesimpulan penelitian ini didapatkan faktor-faktor yang berhubungan dengan kepatuhan berobat pasien TB yaitu, efek samping selama pengobatan dan peran pengawas minum obat

Abstract

One of the national indicators of hospital quality is compliance with doctor's visit time, the achievement of the National Quality Index (INM) at the inpatient installation of Tk. III dr. Reksodiwiryo Padang Hospital in 2023 is 67%. This value is below the quality indicator standards that have been regulated by the Minister of Health Regulation No. 30 of 2022. This study is aimed at analyzing the adherence to doctor's visit time at the inpatient installation of Tk. III Dr. Reksodiwiryo Padang Hospital. The research used the mixed method method, the first stage was quantitative research with stratified random sampling techniques, then qualitative research was carried out with in-depth interviews with 14 informants, field observation and document review. Compliance with the doctor's visit time at the inpatient installation of Tk. III dr. Reksodiwiryo Padang Hospital based on research was obtained at 40%. The causes of non-compliance with doctor's visit time are the lack of the number of organic/permanent specialist doctors and the absence of an alarm to remind the doctor's visit time, the absence of clear regulations related to decrees, SOPs, and doctor's visit policies, the absence of a clear doctor's visit schedule, in addition to the DPJP's lack of understanding of the Regulation of the Minister of Health on visit time and the lack of optimal follow-up from monitoring and evaluating the compliance of doctor's visit time in inpatient installations.

Abstract

Type 2 Diabetes Mellitus (T2DM) is a metabolic disease that can cause various complications, one of which is cognitive dysfunction. Specific characteristics in T2DM patients can be risk factors for cognitive dysfunction, such as sex, age, employment status, education level, duration of diagnosis, comorbidity status, and degree of treatment adherence. Objective: To determine the association between the characteristics of Type 2 DM patients and cognitive dysfunction. Methods: This study used an observational analytical method with a cross-sectional study design conducted on 104 Type 2 DM patients at the Andalas Community Health Center, Padang, who met the inclusion and exclusion criteria. This study utilized the Montreal Cognitive Assessment-Indonesia (MoCA-Indonesia) questionnaire to evaluate the cognitive level of patients. This study was conducted from January to October 2024. Results: The Chi-square test showed a significant association between employment status (p = 0.038) and education level (p = 0.046) and cognitive dysfunction. Meanwhile, there is no significant association between sex (p = 1.000), age (p = 0.456), duration of diagnosis (p = 1.000), comorbidity status (p = 1.000), and degree of treatment adherence (p = 0.188) with cognitive dysfunction. Conclusion: Employment status and education level are significantly associated with cognitive impairment. Keywords: cognitive dysfunction, T2DM, patient characteristics
