Abstract

Kecamatan Sidemen memiliki masalah anemia di mana kejadian anemia pada ibu hamil adalah 60,5%. Meskipun Puskesmas Sidemen telah melaksanakan program kesehatan ibu dan anak melalui pemberian tablet 90 Fe kepada ibu hamil, tetapi kejadian anemia dan masalah gizi lainnya masih tinggi. Ini mungkin karena anemia dan atau kekurangan gizi telah terjadi dalam periode prakonsepsi. Anemia dapat diatasi dengan pemberian tablet besi 200mg selama 90 hari. Hanya saja kepatuhan wanita dalam tablet besi masih sangat rendah yaitu sebesar 35%. Selain itu, variasi sistem pelayanan dari tenaga kesehatan spesialis dan bidan cukup besar dalam hal dosis dan waktu pemberian tablet besi. Ini tentu akan berdampak pada keberhasilan program pencegahan anemia untuk semua wanita hamil. Berdasarkan itu, program pencegahan anemia pada periode prakonsepsi perlu dilaksanakan sebagai alternatif dari program yang ada. Pengabdian ini dilakukan dengan memberikan informasi tentang pentingnya mengkonsumsi tablet besi selama kehamilan pada wanita yang berprasangka dan juga kepada petugas kesehatan tentang cara yang efektif dan dosis tablet zat besi. Hasil dari pengabdian menemukan bahwa kejadian anemia pada wanita selama periode prakonsepsi sangat tinggi, yaitu 40,2%. Ceramah tentang anemia meningkatkan pengetahuan wanita yang prasekognisi tentang anemia sebesar 60%. Dan hasil FGD pada bidan menemukan bahwa tidak ada program administrasi tablet besi pada kelompok wanita pra-konsepsi. Jadi disarankan untuk memberikan tablet besi pada wanita prasekognisi karena pencegahan lebih baik daripada pengobatan

Abstract

Objective: To determine the accuracy difference between CD4 percentage and absolute CD4 in predicting the viral load of HIV-infected pregnant women. Methods: This study is a diagnostic study involving 22 HIV-infected pregnant women who came for PMTCT at the Outpatient Clinic in Sanglah Hospital, from September 2011 until August 2012, who were randomly selected through consecutive sampling. Blood samples were collected to analyze the viral load, CD4, and complete blood count (CBC). Viral load examination was conducted using PCR in the molecular biology laboratory in the Faculty of Medicine University of Udayana. CD4 and CBC test was conducted in Sanglah Hospital Laboratory. Analysis was done with 2x2 table using SPSS for windows® version 17 to evaluate sensitivity, specificity and accuracy rate of CD4 percentage and absolute CD4 in predicting the viral load. Results: Data analysis shows that CD4 percentage had 75.0% sensitivity, 88.9% specificity, and accuracy of 86.4% for predicting the viral load in HIV-infected pregnant women. Meanwhile, absolute CD4 had 50.0% sensitivity, 77.8% specificity, and 72.7% accuracy. Chisquare test shows that there was no significant difference in the accuracy of CD4 percentage and absolute CD4 (p=0.457). Conclusion: CD4 percentage and absolute CD4 had high accuracy in predicting the viral load in HIV-infected pregnant women (86.4% and 72.7%). Abstract

Objective: To determine factors which are related to the number of antral follicles on infertile patients. Method: This cross sectional study was conducted in In-Vitro Fertilization (IVF) clinic of Graha Tunjung, Sanglah hospital, Bali. All fertile patients following the IVF program were calculated the number of antral follicles in both ovarian using transgene USG. This sample was recruited by random sampling from April 1st, 2001 to April 30th, 2011. We analyzed the data using Chi square test through SPSS for Windows 17.0 version. Result: Of 102 samples, the mean of patients’ age was 32.9% (SD 4.6) years old. From 72 patients (70.6%) experienced above 3 years of infertile period, the primary infertile was on 69 patients (67.7%). There was a relationship between patients’ age and the number of antral follicles significantly (prevalence ratio (PR) 1.41; 95% CI 1.11- 1.79). Meanwhile, the number of antral follicles and type of infertile (PR 1.02; 95% CI 0.76-1.37) also infertile period (PR 0.95; 95% CI 0.72-1.27) were not associated significantly. Conclusion: Patients’ age has an association with the number of antral follicles on IVF. Abstract

Objective: To determine the difference of HER2/neu expression in benign, borderline and malignant epithelial ovarian tumor. Method: An analytic cross-sectional study was performed on 49 samples of epithelial ovarian cases which were divided into three groups: twelve cases of benign epithelial ovarian tumor, 9 cases of borderline epithelial ovarian tumor and 28 cases of malignant epithelial ovarian tumor. Paraffin embedded tissue samples was cut, stained with HER2/neu imunohistochemistry and examined in Pathology Laboratory at Sanglah Hospital. The other data was collected from medical record samples. The data was analysed using One Way Anova test and Chi-square test with level of confidence α = 0.05. Result: There were 13 cases (46.43%) of the HER2/neu over expression in malignant epithelial ovarian tumors, 2 cases (22.22%) of the HER2/neu over expression in borderline epithelial ovarian tumors and 1 case (9.09%) of the HER2/neu over expression in benign epithelial ovarian tumors. With p = 0.048 (p < 0.05), there are differentiation of HER2/neu expression in benign, borderline and malignant epithelial ovarian tumors. Conclusion: In this study HER2/neu expression was different between benign, borderline and malignant epithelial ovarian tumors. Abstract

Abstract Objective: To know the relation ship between age, education and occupation on the pregnant women and the HIVstatus of the husband against the risk of being infected with HIV. Method: This was an unpaired case cont rol study perform ed in the Obstetrics and Gynecology Department of Sanglah Hospital, Denpasar, from October to November 2011. The subjects were pregnant women who were willing to particip ate in the research, which were grouped into HIV-infected case and control group. Diagnosis of HIV was based on rapid test The data was analyzed using ChiSquar e test with the help of SPSS version 17.0 Result: There was fifty subjects consisting of 25 subjects in the case group and 25 subjects in contr ol group we obtained that the risk of HIV-infected pregnant women in the old vs young age, low vs higher education, and occupations at risk vs not at risk is not significant in the two groups. Each Odds ratio is 0.35 (CI =95% 0.08-1.55; P = 0.16),0.85 (CI=95% 0.28-2.59; P =0.77), and 2.09 (CI=95% 0.1824.62; P =1.00). Meanwhile, the odds ratio of the husband HIV status was 12.67 (CI =95% 3.31-48.50; P =0.01). Conclusion: Husband s infected with HIVincreases the risk of HIV in pregnant women 12 times greater than if the husband was not infected with HIV. Wher eas, age, education, and th e occupation of mothers is not a risk factor for the occurrence of HIV infection in pregnant women. Abstract

Objective: To study the risk of Cervical Intraepithelial Neoplasia (CIN) at higher CD4 levels compared to low CD4 levels in HIV-infected women. Method: Case-control study of 50 HIV-infected women who meet the inclusion and exclusion criteria and attended Kerti Praja Foundation outpatient clinic, Denpasar, who were then divided into 2 groups, those with CD4 500/mm3 and those with CD4 <500/mm3. Both groups had Pap smear examination done. The collected data was analyzed with Chi-Square test. Odds ratio was calculated to determine the influence of CD4 levels on the incidence of CIN in women infected with HIV. Result: In this study, three women were found to have CIN out of the 25 women with CD4 500/mm3 and nine women out of the 25 women with CD4 <500/mm3 were positive for CIN. The risk of CIN in HIV-infected women with low CD4 levels was 4.125 times higher than the risk in high levels of CD4 (95% CI = 1.96 - 17.7, p=0.047). Conclusion: The incidence of CIN in HIV-infected women is associated with the level of immunity, as characterized by levels of CD4. Decreased levels of CD4 in HIV-infected women may increase the risk for CIN incidence by four-fold. Keywords: CD4, cervical intraepithelial neoplasia, HIV

Abstract

Objective: To prove the association of iron deficiency anemia, cystitis, and bacterial vaginosis as risk factors for preterm labor.Methods: This study was an observational case-control study conducted at the Polyclinic and IRD of Obstetrics and Gynecology Sanglah Denpasar General Hospital Center between September and December 2015. The sample selection was done by consecutive sampling with total sample as many as 35 case samples (women with preterm pregnancy) and 35 control samples (women with aterm pregnancy).Results: Pregnant mothers with iron deficiency anemia had 4 fold increased risk of developing preterm labor (OR = 4.04, 95% CI = 1.15 to 14.16, p = 0.023) compared to those without iron deficiency anemia. Pregnant mothers with cystitis had 4 fold increased risk of preterm labor (OR = 4.00, 95% CI = 1.25 to 12.80, p = 0.016) compared to those without cystitis. Pregnant mothers with bacterial vaginosis had 3 fold increased risk of preterm frequency (OR = 3.24, 95% CI = 1.22 to 8.63, p = 0.017) compared to those without bacterial vaginosis.Conclusion: Iron deficiency anemia, cystitis, and bacterial vaginosis increase the risk for developing preterm labor.[Indones J Obstet Gynecol 2017; 5-2: 73-76]Keywords: bacterial vaginosis, cystitis, iron deficiency anemia, preterm labor

Abstract

Objective: To evaluate the accuracy of hK6, HE4, and CA125 in predicting the malignancy of ovarian mass. Methods: The design of this study was cross-sectional. This study was conducted in the Obstetrics and Gynecology Clinic, Sanglah Hospital, Denpasar, between the period of September 2014 and August 2016. Samples were all patients with ovarian tumors who underwent surgery at Sanglah Hospital, Denpasar. Data analysis was performed using McNemar and chi square test in SPSS for windows version 17.0. Results: 22 samples were obtained. P > 0.05 value of age and parity variables indicated no differences between the two groups. There is no accuracy differences (sensitivity, specificity, positive predictive value, negative predictive value) of hK6 compared to histopathology examination in diagnosing ovarian cancer (p = 1). There is no accuracy differences (sensitivity, specificity, positive predictive value, negative predictive value) of HE4 compared to histopathology examination in diagnosing ovarian cancer (p = 1). There is no accuracy difference (sensitivity, specificity, positive predictive value, negative predictive value) of CA125 compared to histopathology examination in diagnosing ovarian cancer (p = 0.687). Conclusion: There was no accuracy differences (sensitivity, specificity, positive predictive value, negative predictive value) found between hK6, CA125, HE4 compared to histopathology examination in predicting ovarian cancer.
