Abstract

This study aims to determine the efficacy of using antimicrobial surgical draping on surgical area contamination. The research method used is a narrative review of the database used. The search engine used is Google Scholar. Articles based on the criteria include, published between 2020-2024, there are full manuscripts, primary research (RCT, Cross-sectional, retrospective, and cohort) in Indonesian and English. The article database uses Science Direct, EBSCO-CINAHL, and Pubmed. The keywords used are surgical draping, antimicrobial, and contamination. The study results showed 33 articles (CINAHL), 90 articles from Science Direct, and 271 articles from Pubmed, but only nine articles were the focus of the study. This study concludes that antimicrobial drapes can prevent contamination in the surgical area by preventing transmission and antibacterial. Nurses are advised to conduct further research on the types and characteristics of microbes before using antimicrobial surgical drapes. Keywords: Antimicrobial, Surgical Draping, Contamination

Abstract

Ineffective airway clearance is a common problem in pulmonary tuberculosis patients. This condition occurs due to an infectious process that causes an inflammatory response and excessive secretion production. The mediastinal tumor experienced by the patient resulted in the patient's condition getting worse because the lump in the mediastinal area pressed on the airway so that phlegm/secretions were difficult to expel. This study aimed to describe the ineffective management of airway clearance in patients with pulmonary tuberculosis, pleural effusion, and mediastinal tumors. This study uses a descriptive observational research design in the form of a case report using a nursing care approach. The subject of this study was a 34 year old woman with a medical diagnosis of pulmonary tuberculosis, pleural effusion, and mediastinal tumor. The intervention carried out, namely managing airway clearance, was not effective. Some of the actions carried out include semi-fowler position, collaborative administration of 3 lpm oxygen, combivent and pulmicort nebulization 3 times/day, and suctioning. After carrying out nursing procedures for 3 x 24 hours, the patient said he felt more comfortable in the semi-Fowler's position, she felt the shortness of breath was reduced after nebulization but the shortness of breath became worse again sometime later, the patient said he had difficulty expelling phlegm so his shortness of breath did not improve, SaO2 100%, RR 18 times/minute, and wheezing (-). It can be concluded that the interventions can reduce symptoms but cannot solve the problem. Management of ineffective airway clearance in this case cannot be done independently by the nurse. Collaboration is needed by considering the need for surgery and pharmacological therapy.

Abstract

Studi kasus ini unik karena menunjukkan adanya efek spiritual emotional freedom technique dalam meningkatkan kualitas tidur pasien dengan wound dehiscence. Wound Dehiscence adalah komplikasi yang dapat terjadi pada pasien pasca bedah abdomen. Wound Dehiscence dapat muncul pada 4 – 14 hari pasca operasi dengan rata-rata pada hari ke 7. Angka kejadian wound dehiscence di dunia mencapai angka 0,4%-3,5%. Beberapa literature menunjukan bahwa wound dehiscence memiliki tingkat morbiditas dan mortalitas yang tinggi yaitu 3-35%. Wound dehiscence memiliki beberapa dampak pada pasien, salah satunya yang terjadi pada kasus ini adalah peningkatan stress yang berujung kepada penurunan kualitas tidur. Gangguan pola tidur bisa ditangani dengan terapi farmakologi dan non-farmakologi. Terapi Spiritual Emotional Freedom Technique adalah salah satu teknik non-farmakologi yang menggabungkan sistem energi tubuh (energy medicine) dan terapi spiritualitas dengan tapping di beberapa tititk tubuh tertentu. Terapi ini dapat meningkatkan kualitas tidur karena tapping yang dilakukan dapat menstimulasi peningkatan hipotalamic pituitary adrenal yang menghentikan respon alarm dari system saraf simpatis dan parasimpatis dan berganti menjadi respon relaksasi. Sikap pasrah kepada Allah SWT disertai hati yang ikhlas akan merangsang tubuh kita untuk mengalami relaksasi, pernafasan dan denyut jantung menjadi stabil, memperlancar skulasi darah yang mengalir dan akhirnya menghasilkan kondisi yang rileks dan mudah untuk memulai tidur.

Abstract

Fraktur tibia segmental terbuka merupakan kejadian fraktur yang jarang terjadi dan memiliki angka komplikasi yang tinggi, umumnya fraktur ini disebabkan oleh kecelakaan bermotor. Prinsip penganan fraktur yaitu dengan reposisi tulang agar terjadinya penyatuan kembali. Salah satu tindakan operatif yang dapat dilakukan yaitu dengan Open Reduction Internal Fixation (ORIF). Tujuan dilakukannya penelitian ini untuk mengetahui manajemen perawatan pada pasien dewasa pasca ORIF open cruris tiba segmental. Metode yang digunakan yaitu dengan pendekatan laporan kasus. Hasil diamati dari respon pasien terhadap pemberian intervensi manajemen nyeri, pencegahan infeksi, perawatan luka, dan juga pembidaian yang menunjang dalam pemulihan fraktur. Manajemen yang tepat dalam menanggulangi pasien pasca ORIF menjadi kunci dalam indikasi pemulangan pasien one day care surgery, sehingga perlu mendapatkan perlakuan yang tepat yang tak luput dari peran perawat. Perawatan pasien yang diberikan sejalan dengan prinsip operasi perawatan satu hari dan mendukung indikasi pemulangan pasien pasca operasi. Namun, intervensi tertentu khususnya dalam mobilisasi dini masih belum optimal.

Abstract

Hypertension is one of the cardiovascular diseases where there is a persistent increase insystolic and diastolic pressure. The long-term impact of hypertension is damage to variousvital organs such as the heart, brain, kidneys, etc. Therefore, hypertension patients must haveself-efficacy to prevent complications. The purpose of this study is to identify the self-efficacyprofile of patients with hypertension at one of the community health centers in Garut Regency.The research design are descriptive quantitative with sample size of 64 people throughpurposive sampling based on proportioned formula. Data collection was conducted using aquestionnaire about self-efficacy in hypertensive patients. The research results show that 35patients (55%) have low self-efficacy. Health centers are advised to develop strategies toimprove the self-efficacy of hypertensive patients to prevent complications.

Abstract

ABSTRACT Percutaneous coronary intervention (PCI) is the most common therapy for treating CAD through improved myocardial perfusion. The transition from hospital to home is a challenge for CAD patients undergoing PCI. After a brief stay in the hospital, patients have to take a lot of medicine every day, and adjust their lifestyle at home. The purpose of conducting this Evidence Based Practice is to determine the types of nursing interventions that can improve the level of treatment adherence in post PCI patients. The research method used in this review literature is a narrative review type with the title "Nursing Intervention "EKOFOL" in Post PCI Patients. Based on the seven articles analyzed, there are seven different interventions to improve treatment adherence of post-PCI patients. The first article uses education and counseling methods, the other six articles use the same method, namely education, counseling and follow up. The following are the interventions discussed from the seven articles analyzed. Based on the results of the study, it was concluded that there was treatment adherence in post-PCI patients with educational, counseling and follow-up interventions with duration and frequency adjusted back to patient needs. But in one article that has been analyzed, patient treatment adherence is considered quite low despite the intervention. Keywords: Nursing Interventions, Post PCI Patients, Treatment Adherence  ABSTRAK Percutaneous Coronary Intervention (PCI) adalah terapi yang paling umum untuk mengobati CAD melalui peningkatan perfusi miokard. Transisi dari rumah sakit ke rumah merupakan tantangan tersendiri bagi pasien CAD yang menjalani PCI. Setelah dirawat sebentar di rumah sakit, pasien harus meminum banyak obat setiap hari, dan menyesuaikan gaya hidup mereka di rumah. Tujuan melakukan Evidence Based Practice ini adalah untuk mengetahui jenis-jenis intervensi keperawatan yang dapat meningkatkan tingkat kepatuhan pengobatan pada pasien post PCI. Metode penelitian yang digunakan dalam literatur review ini berjenis narrative review dengan judul “Intervensi Keperawatan “EKOFOL” Pada Pasien Post PCI. Berdasarkan tujuh artikel yang dianalisis, terdapat tujuh intervensi yang berbeda untuk meningkatkan kepatuhan pengobatan pasien post PCI. Artikel pertama menggunakan metode edukasi dan konseling, keenam artikel lainnya metode yang sama yaitu edukasi, konseling dan follow up. Berikut merupakan intervensi yang dibahas dari ketujuh artikel yang dianalisis.  Berdasarkan hasil penelitian maka disimpulkan adanya kepatuhan pengobatan pada pasien post PCI dengan intervensi edukasi, konseling dan follow up dengan durasi dan frekuensi yang disesuaikan kembali pada kebutuhan pasien. Namun dalam satu artikel yang telah dianalisis, kepatuhan pengobatan pasien dianggap cukup rendah meskipun telah dilakukan intervensi Kata Kunci: Intervensi Keperawatan, Pasien Post PCI, Kepatuhan Pengobatan

Abstract

Introduction : Vasovagal reactions after blood donation cause dizziness, nausea, and even loss of consciousness, thus disrupting the donor's comfort and the donation process. Vasovagal reactions can reduce the donor's interest in donating blood again. In fact, blood donation not only provides benefits for people in need, but also provides benefits for the donor, namely stimulating the production of new blood cells, reducing the risk of coronary heart disease, and routine health checks every time they donate blood. To overcome this problem, interventions are needed to prevent vasovagal reactions. One of these interventions is fluid hydration and consumption of salty snacks 30 minutes before donation. This study aims to determine the effects of fluid hydration and consumption of salty snacks in preventing vasovagal reactions in first-time donors. Method: The design of this study was a case series on 5 cases of first donors with nursing problems of hypovolemia risk. Result: The results of this study indicate that providing fluid hydration and consuming salty snacks before donating blood can reduce the incidence of vasovagal reactions in cases. Conclusion: Providing fluid hydration and consuming salty snacks before donating blood is one option in developing a strategy for managing the risk of vasovagal reactions.

Abstract

Heart failure is a pathophysiological condition due to impaired heart function in pumping blood that is chronic and progressive, and contributes to high morbidity and mortality rates. Reducing these rates requires a treatment strategy that maintains the health and well-being of patients, one of which is through effective self-care management. In its implementation, the role of family is an important aspect as a source of non-medical support that can motivate patients to undergo treatment. This study aimed to analyze the relationship between family support and self-care practices among individuals with heart failure attending polyclinics in Garut City. A quantitative cross-sectional design was employed, involving 105 participants who were selected through purposive sampling. Data were collected using a family support questionnaire the Caregiver Contribution to Self-Care of Heart Failure Index version 2 (CC-SCHFI v.2), then The Spearman rank correlation test was used for data analysis. The results showed that most participants had high levels of family support (81%), and demonstrated moderate self-care practice (88,6%).  There was a strong and significant positive correlation between family support and self-care behavior (r = 0.680; p < 0.001), suggesting that better family support is associated with improved self-care practices among heart failure patients. Family support significantly enhances self-care behavior in heart failure patients. Involving family members in disease management programs is essential to promote better health outcomes.

Abstract

Chronic kidney disease is a condition when kidney function decreases to the point where the kidneys are unable to filter the body's metabolic waste and maintain the balance of electrolyte fluids such as sodium and potassium. One of the problems that is often found in chronic kidney failure sufferers is non-compliance with diet. The aim of this research is to determine the description of diet compliance in kidney failure patients at Guntur Garut Hospital. The research method used was descriptive, with a purposive sampling technique, the sample size was 25 respondents with the criteria being that the patient had undergone hemodialysis for more than 3 months and had no complications. The data collection technique was carried out using a questionnaire. The results of the study concluded that the majority of respondents (56%) did not comply with the diet. Suggestions from the results of research that have been carried out for health workers in the hemodialysis room at Guntur Hospital, especially nurses, to further improve education about the importance of diet for kidney failure patients.

Abstract

Surgery is a medical procedure that involves invasive actions on certain parts of the body. Fatigue is often experienced by postoperative patients related to changes in hemodynamic status, sedation and so on. It is essential to manage postoperative fatigue in order to prevent delayed recovery and improve quality of life. This literature review aims to identify the nonpharmacological intervention to managing postoperative fatigue. The method use was a narrative review study, The review was conducted several databases ScienceDirect, PubMed, Springer, Sagepub and google scholar, to search data from 2018 to 2024. The keywords used were nonpharmacologic intervention and postoperative fatigue. The results of this study retrieved nine articles reviewed, which were eight article randomized control trials and one article quasi experimental. The review found that the interventions to manage postoperative fatigue are progressive muscle relaxation, Benson relaxation and early exercise. Based on the results of the review, it was found that every specific interventions on specific subject were proven to overcome postoperative fatigue. The study findings provide information for nurses regarding postoperative fatigue management in order to improve the quality of life.
