Abstract

Background: Ischaemic stroke patients experience an increase in cortisol of 38% over 90 days. The spiritual emotional freedom technique (SEFT) provides manual stimulation of acupuncture points, which makes the muscles feel relaxed so that they can control cortisol. This study set out to assess how SEFT affected the cortisol levels in patients who had suffered an ischaemic stroke. Methods: A control group and pre- and post-test measures were part of the quasi-experimental research design. There were seventy-one patients in the research population of the study. The sample was measured using the Slovin formula, and the sampling strategy used was purposeful sampling. Over the course of three months, the intervention group had 12 sessions of SEFT once a week, whereas the control group received regular spiritual treatment from RSUD Dr. Moewardi Surakarta, an outpatient nurse. The ELISA method was used to measure cortisol levels. The Kolmogorov-Smirnov test was used to assess normality, and the Levene test was used to test for homogeneity in bivariate data. The Mann-Whitney method was used to analyze the data. Results: The mean cortisol before intervention was 119.63, and the mean cortisol after intervention was 99.23. SEFT affects cortisol levels in patients who have had an ischaemic stroke, according to a statistically significant (p<0.05) p-value of zero. Conclusion: Patients with ischaemic stroke experienced a significant drop in cortisol levels after using the SEFT. Furthermore, it is necessary to study the dominant factors that influence the cortisol hormone and the need for a multi-religious approach in an effort to develop spiritual-based interventions.

Abstract

Background: Exercise generally improves the quality of life (QoL) in adult populations especially for patients with chronic health problems such as breast cancer. The purpose of this study was to investigate the effect of physical exercise/activity on the quality of life of breast cancer survivors. Subjects and Method: This was a systematic review study that used articles from online data­bases, such as  PubMed, Science Direct, ProQuest dan Medline, which was published from 2000 to 2021. The author used keywords of “physical exercise”, “physical activity”, “quality of life”, “breast cancer survivor”. The dependent variable was quality of life. The independent variable was physical exercise/ activity. As many as 5 articles were selected for this study from 837 articles located in South Korea, Iran, Malaysia, and China.Results: Moderate or high levels of physical activity had positive relationship on the quality of life related to overall physical health among breast cancer survivors.Conclusion: Moderate to high physical activity improve the quality of life among breast cancer survivors.Keywords: physical exercise, physical activity, quality of life, breast cancer survivor Correspondence: Arief Wahyudi Jadmiko. Abstract

Data from the Ministry of Health states that 23% of people living with HIV/AIDS (PLWHA) experience drug withdrawal. Self-management refers to managing disease needs, activating social support, and living with a chronic disease. Self-management has also been shown to control patients' adherence to medication so that it can increase CD4 counts in PLWHA and improve some clinical symptoms. Specific therapy for HIV cases has not been found to cure HIV infection, but HIV self-management can slow down or prevent the occurrence of more severe conditions. Based on this, self-management is very important for HIV/AIDS patients. This study aims to determine interventions that can improve self-management in HIV/AIDS patients. Methods: Literature sources were obtained through searches using the Scopus, Science Direct, Google Scholar, and PubMed databases with the last 5 years of research (2018-2023). The keywords used in the search were intervention, self-management, and HIV/AIDS. The search equation was applied without limitation, taking into account the title, abstract, interventions, and methods used. Articles that had been obtained were then analyzed for differences and duplication. A chart for visual representation of how the search and selection of articles was made PRISMA-SR flow diagram. Data extraction was done by creating tables. Results: interventions used to improve self-management in HIV/AIDS patients include self-management applications, spiritual counseling, and prayer therapy. Conclusion: The interventions used were proven to improve self-management in HIV/AIDS patients.

Abstract

Nurse service in the world today requires supporting therapy to overcome complex problems including biologically, psychologically, socially, culturally and spiritually. In chronic patients there are many supporting implementation, one of which is motivational interviewing which explains motivational communication as a tool in minimizing the effects of problems that occur in chronic patients. Motivational interviewing can help more patient awareness and compliance leads to more careful responsibilities on treatment and health. Objectives: To determine the effect of motivational interviewing on chronic patients. Literature search using Data Based Sciencedirect, Scopus, Proquest, JStor, and Pubmed. The keywords used are "Interventions" and "Motivational Interviewing" and "Nursing" and "Effective" and "Chronic Diseases". Method: The method used to compile information in this article is by using scoping review, with a span of 2019-2024, screening of articles is guided by the prism. Pubmed (133 articles), Scopus (3 articles), Sciencedirect (125 articles) and proquest (30 articles) from the results of the analysis of articles that can be taken according to criteria are 7. obtained 3 themes, the influence of clinical conditions, on secondary outcomes and health services. Motivational interviewing in chronic patients has a positive impact on patients and health services. So it is hoped that this research will be a reference for patient services with chronic conditions.

Abstract

  Perubahan gaya hidup merupakan elemen penting dalam pengelolaan diabetes mellitus tipe 2. Studi sebelumnya tentang yoga memiliki komponen dan manfaat yang berbeda untuk pasien diabetes tipe 2. Tujuan yaitu untuk mengidentifikasi komponen yoga dan manfaatnya terhadap kondisi kesehatan pasien diabetes tipe 2. Metode pencarian menggunakan database elektronik Scopus, Science Direct, ProQuest, PubMed dan Google Schollar dengan publikasi tahun 2017 hingga 2021. Pencarian termasuk istilah kunci "yoga" dan "diabetes tipe 2". Hasil review dari 12 artikel didapatkan bahwa komponen yoga terdiri dari doa, pemanasan, teknik pernapasan, salam matahari, postur fisik, dan meditasi. Peningkatan kesehatan yang dicapai yaitu penurunan kadar gula darah, profil lipid, tekanan darah, kecemasan, dan depresi. Kesimpulan: yoga terbukti efektif untuk manajemen diabetes tipe 2 dan memberikan manfaat yang komprehensif bagi pasien DM tipe 2 baik secara fisik maupun psikologis.

Abstract

Chronic Kidney Disease (CKD) is a condition in which kidneys don’t function properly resulting in the accumulation of metabolic remnants. Hemodialysis therapy causes common psychological disorders, is anxiety due to changes in health status, self-acceptance, life threats, stress, and depression. Natural Restorative Environment (NRE) is an intervention that can use to reduce anxiety in patients. This study aimed to analyze the effect Natural Restorative Environment on anxiety in CKD patients on hemodialysis. The method in this research was a quasi-experiment with a pretest-posttest with a control group design. The sampling technique was carried out using cross-sectional sampling, namely 48 patients each with an intervention group and control group. Before and after the intervention, patients in the intervention group and the control group pre-test and post-test about the level of anxiety during hemodialysis therapy. Data analysis used Paired t-test, Wilcoxon-test, and Mann-Whitney U-test. The results showed a decrease in anxiety levels intervention group indicated by a p-value<0.001. Abstract

Ischemic stroke is a cause of physical disability. These defects result in impaired biopsychosociospiritual adaptation in stroke patients. The inability to adapt will cause an anxiety reaction which triggers an increase in cortisol levels. The spiritual care model helps patients find meaning and purpose in life as well as an optimistic attitude so that they have better adaptability. This type of research is Quasy Experimental with a pretest-posttest control group design. The study population was ischemic stroke patients in the Outpatient Department of Dr. Regional General Hospital. Moewardi Surakarta. The total sample consisted of 60 respondents, divided into 30 control group respondents and 30 intervention group respondents, who received a spiritual care model intervention in the outpatient room of Dr RSUD. Moewardi Surakarta. The research results show the influence of the Spiritual Care Model Using Callista Roy's Adaptation Theory Approach on Cortisol Levels in Ischemic Stroke Sufferers with a significance value of 0.043 < 0.05. The Spiritual care model influences the amygdala, which stimulates the hypothalamus through the hypothalamic-pituitary-adrenal (HPA) axis, thereby inhibiting the excretion of corticotropin-releasing factor (CRF). The pituitary gland then stimulates adrenocorticotropic hormone (ACTH), which reduces cortisol. This research has a contribution that spiritual care interventions can improve the adaptability of biopsychosocial rehabilitation of stroke patients. Abstract

Background: Speech nerve disorders due to stroke cause slurred speech. Lee Silverman Loud Voice Therapy can be applied using the Swanson technique to help stimulate the speech muscles. Purpose: To determine the effect of "a" vowel phonation training based on Lee Silverman Loud Voice Therapy on speech intelligibility in ischemic stroke patients. Method: This quasi-experimental study used a pretest-posttest non-equivalent control group approach, conducted from September to November 2024 at Dr. Achmad Mochtar Regional General Hospital, Bukittinggi. A purposive sampling method was used to select 22 participants, who were divided into intervention and control groups. Data were analyzed using a paired t-test with α ≤ 0.05. Results: In the intervention group, the average pretest score was 15 and the posttest score was 4.82. In the control group, the average pretest score was 16.91 and the posttest score was 16.45. A decrease in the average score indicates an improvement. The analysis showed an effect of "a" vowel phonation training with p = 0.001 (p ≤ 0.05). Conclusion: There is an effect of "a" vowel phonation training based on the Lee Silverman Voice Treatment Loud on speech intelligibility in ischemic stroke patients. A decrease in the mean speech intelligibility score indicates an increase in speech intelligibility. A lower score indicates better speech intelligibility. Suggestion: Future researchers should conduct further research with a larger sample size and implement the full training procedure, as well as considering other variables, such as severity, initial treatment time, stroke phase, other rehabilitation provided by healthcare facilities, comorbidities, family and friend support, and motivation.   Keywords: Ischemic Stroke; Lee Silverman Voice Treatment Loud; Speech Clarity; Vocal Phonation.   Pendahuluan: Gangguan saraf bicara akibat stroke menyebabkan ketidakjelasan bicara. lee silverman voice treatment loud dapat diterapkan dengan caring Swanson untuk membantu menstimulasi otot bicara. Tujuan: Untuk mengetahui pengaruh latihan fonasi vokal “a” berdasarkan lee silverman voice treatment loud terhadap kejelasan bicara pasien stroke iskemik. Metode: Penelitian quasi-eksperimen menggunakan pendekatan pretest posttest nonequivalent control group, dilakukan pada September-November 2024 di RSUD Dr. Achmad Mochtar Bukittinggi. Sampel diambil menggunakan purposive sampling dengan jumlah 22 partisipan yang terbagi dalam kelompok intervensi dan kontrol. Data dianalisis dengan T-Test (Paired-Test) α≤ 0.05. Hasil: Pada kelompok intervensi rata-rata ketika pre-test 15 dan post-test yaitu 4.82. Pada kelompok kontrol ketika pre-test yaitu 16.91 dan post-test yaitu 16.45. Nilai rata-rata yang menurun, menunjukkan perubahan yang membaik. Analisis menunjukkan terdapat pengaruh latihan fonasi vokal “a” dengan p = 0.001 (p ≤ 0.05). Simpulan: Terdapat pengaruh latihan fonasi vokal “a” berdasarkan Lee Silverman Voice Treatment Loud terhadap kejelasan bicara pasien stroke iskemik. Penurunan rata-rata menunjukkan nilai kejelasan yang membaik. Semakin turun nilai, maka semakin baik kejelasan bicara yang dialami. Saran: Bagi peneliti selanjutnya, dapat melakukan penelitian lebih lanjut dengan sampel yang lebih besar dan melakukan seluruh prosedur latihan serta memperhatikan variabel lain, seperti tingkat keparahan, waktu penanganan awal, fase stroke, rehabilitasi lain yang didapat dari faskes, penyakit komorbid, dukungan keluarga dan teman serta motivasi.   Kata Kunci: Fonasi Vokal; Kejelasan Bicara; Lee Silverman Voice Treatment Loud; Stroke Iskemik.
