Abstract

Highlights Glutamine, an amino acid, can help mitigate the side effects of cisplatin by boosting glutathione (GSH) levels, an antioxidant that counteracts oxidative stress. By reducing AIF protein expression and apoptosis, glutamine may protect kidney cells from cisplatin toxicity, potentially improving the safety of chemotherapy. Background: Cisplatin, a widely used chemotherapy that is effective for many forms of cancer, has nephrotoxic qualities that can reduce renal function. Due to its side effects, the usage of cisplatin is limited by its dose. Glutamine, an amino acid, possesses properties that can counteract the nephrotoxicity of cisplatin. Objective: This study aimed to analyze the effect of intravenous glutamine administration on the decrease of AIF expression and apoptosis in intraglomerular mesangial cells of cisplatin-exposed rats. Material and Method: Samples (n) consisting of 30 rats were divided into 3 groups, with each group containing 10 rats. Group P0 received no treatment, group P1 received an injection of cisplatin on the 7th day, and group P2 received glutamine on the 1st–7th day and an injection of cisplatin on the 7th day. After 72 h, the tissue samples were immunohistochemically processed and then observed under a light microscope with 400x magnification. AIF expression and apoptosis were measured in the Allred/H score. Result: AIF expression results were: P0 = 4,35(2,2-5,4); P1 = 5,9(4,8-6,3); P2 = 5,25(2,4-5,8). The AIF expression of P2 was found to be significantly lower (p = 0.008) than P1. Apoptosis results were: P0 = 5,2(4,7-5,5); P1 = 6,4(6,1-7,1); P2 = 5,7(4,8-5,9). The apoptosis level of P2 was found to be significantly lower (p = <0.0001) than P1. Strong correlations between the decrease in AIF expression and the apoptosis level were found (p = <0.0001). Conclusion: Glutamine contributes to the decreased AIF expression and apoptosis in intraglomerular mesangial cells of cisplatin-exposed rats.

Abstract

Highlights It has been proven that the eucalyptus eco-enzyme from Lamongan does not exhibit strong enough antibacterial activity against E. coli at any concentration. The eucalyptus eco-enzyme method is less effective at extracting active compounds compared to distillation processing.   Abstract Background: Eucalyptus (Melaleuca leucadendra) is known to have antimicrobial potential due to its bioactive terpenoid compounds, including 1,8-cineole. This compound has the potential to inhibit the growth of Escherichia coli, a bacterium responsible for various infectious diseases. The eco-enzyme method, which utilizes fermentation, is simple to perform and does not require complex materials. Objective: This study aimed to assess the antibacterial activity of eucalyptus eco-enzyme against Escherichia coli. Material and Method: The M. leucadendra used in this study was sourced from Candisari Village, Lamongan, Indonesia and E. coli was obtained from laboratory isolates. Antibacterial activity was measured by observing the zone of inhibition in the well diffusion test on Muller-Hinton agar, with chloramphenicol as the positive control and distilled water as the negative control. The incubation period was 24 hours at 36°C. Result: The inhibition zone around the positive control was 25.94±1.1 mm. No inhibition zone (0 mm) was observed around the negative control or the eucalyptus eco-enzyme solution at concentrations ranging from 10% to 100%. However, a clearer zone was observed around the eco-enzyme well. The inability of the eco-enzyme to inhibit the growth of E. coli may be attributed to several factors, including the ingredients, processing method, acidity level, and bacterial resistance. Conclusion: The eucalyptus eco-enzyme did not exhibit sufficient antibacterial activity against E. coli at any of the tested concentrations.

Abstract

Indonesia is the second highest tuberculosis (TB)-burden country in the world. TB affects men and women in productive ages. Early and accurate diagnosis of TB is crucial in TB management because it allows prompt treatment with appropriate regimen. Sputum smear microscopy is the most common method used in resource-limited laboratories. The aim of this study is to assess whether adult learning method with student-centered approach can improve knowledge in laboratory techniques in microscopic TB diagnosis. It was a quasi experimental research with one group pretest and posttest design. Pretest was conducted before the training started. During the training, participants attended lectures, discussion sessions, and hands-on laboratory activities. After training, participants were asked to respond to posttest. There were 33 participants of this training (n=33). Paired t-test analysis showed significant difference between pretest and posttest scores (p=6.5x10-9). Variance between pretest scores was 4.00 and variance between posttest scores were 1.34. Pearson correlation coefficient was 0.45. Regardless their level of knowledge before training, participants could achieve relatively equivalent results after training. Significant difference between pre- and post-training test scores indicated that the learning method was effective and the training could improve participants knowledge in laboratory techniques in microscopic TB diagnosis.

Abstract

Indonesia menghadapi tantangan besar dalam deteksi dini Tuberkulosis, terutama di wilayah dengan tingkat deteksi rendah seperti Kabupaten Sampang. Keterbatasan pengetahuan kader desa dan staf laboratorium menjadi hambatan utama dalam meningkatkan deteksi dini TB. Program pelatihan ini bertujuan untuk meningkatkan kapasitas kader desa dan staf laboratorium melalui pelatihan terstruktur untuk mendukung deteksi dini Tuberkulosis yang lebih efektif. Pelatihan dilakukan dalam dua sesi paralel, yaitu untuk 15 kader desa dan 10 staf laboratorium. Kader desa diberikan pelatihan melalui ceramah dan diskusi kelompok terfokus tentang identifikasi gejala Tuberkulosis dan protokol rujukan. Staf laboratorium dilatih dengan metode ceramah dan praktikum tentang teknik diagnostik menggunakan tes cepat molekuler Tuberkulosis dan pemeriksaan diagnosis mikroskopis Tuberkulosis. Evaluasi pre dan post pelatihan menunjukkan peningkatan signifikan dalam pengetahuan dan keterampilan kader desa serta staf laboratorium. Kader desa mampu mengenali gejala TB lebih dini dan melakukan rujukan yang tepat, sementara staf laboratorium menunjukkan peningkatan keterampilan dalam teknik diagnostik yang lebih akurat. Program ini membuktikan bahwa pemberdayaan komunitas dan pelatihan teknis mampu meningkatkan deteksi dini TB. Model ini dapat diadopsi di wilayah lain yang menghadapi tantangan serupa untuk mendukung eliminasi TB pada tahun 2030.

Abstract

Microbial resistance to antimicrobials, especially antibiotics, has become a global health problem with various adverse impacts. Studies show that resistance to antibiotic drugs increases directly proportional to the number of increases in their consumption. This happens due to the irrational use of antibiotics and inadequate public education. The high incidence of antibiotic use is due to the lack of counseling in the community to increase knowledge and awareness about the wise use of antibiotics. This community service aims to see how the level of knowledge of senior high school students (MAN 3 Banyuwangi) about antibiotic resistance. This community service design is counseling 90 grade XI students and primary data was collected directly from a questionnaire before and after counseling. The evaluation of the questionnaire before and after the counseling showed that educational and counseling activities about antibiotic resistance increased the knowledge of students participating in the counseling at MAN 3 Banyuwangi. It can be concluded that effective educational interventions are essential, including interventions through counseling activities such as community service activities, to improve student understanding so that high school students can play a role in controlling antibiotic resistance.

Abstract

Tuberkulosis (TB) merupakan masalah kesehatan global yang masih menjadi prioritas utama dalam upaya penanggulangan penyakit. Tingginya angka kesakitan dan kematian akibat TB memerlukan langkah-langkah strategis, terutama dalam hal deteksi dini dan pengobatan yang tepat. Diagnosis dini TB paru yang akurat dan cepat sangat bergantung pada deteksi bakteri patogen dalam dahak pasien, yang selanjutnya menentukan strategi pengobatan yang sesuai. Tes Cepat Molekuler (TCM) digunakan untuk diagnosis awal, sedangkan pemantauan hasil pengobatan dilakukan melalui pemeriksaan mikroskopis bakteri tahan asam (BTA). Tujuan kegiatan Pengmas adalah meningkatkan pemahaman dan keterampilan staf teknisi laboratorium dalam mendiagnosis TB sesuai dengan standar kompetensi.  Serta memperkuat peran kader kesehatan dalam penanggulangan TB di masyarakat. Tim Dosen Departemen Mikrobiologi Kedokteran Fakultas Kedokteran Universitas Airlangga bersama mahasiswa dan alumni Program Studi Spesialis Mikrobiologi Klinik melaksanakan kegiatan ceramah mengenai TB paru, meliputi aspek diagnosis, pengobatan, dan pencegahan penularan kepada 17 kader kesehatan serta pelatihan pemeriksaan mikroskopis BTA bagi  16 staf teknisi laboratorium dari berbagai fasilitas kesehatan masyarakat dan RSUD di Kabupaten Pamekasan. Evaluasi pasca pelatihan menunjukkan peningkatan yang signifikan dalam pemahaman dan keterampilan para kader kesehatan terkait temuan kasus TB di masyarakat, diagnostik, serta upaya pencegahan penularan. Seluruh staf laboratorium mengalami peningkatan kompetensi dalam pemeriksaan BTA, termasuk pembuatan dan pembacaan sediaan sesuai standar nasional (IUATLD). Kegiatan pengabdian kepada masyarakat oleh tim akademisi efektif dalam pemberdayaan kader kesehatan sebagai ujung tombak deteksi dini TB serta meningkatkan keterampilan diagnosis mikroskopis staf laboratorium. Pelatihan ini berkontribusi pada peningkatan kualitas diagnosis dan pengobatan TB.

Abstract

Highlights Papaya seeds are a natural and traditionally used remedy in Indonesia, known for their safety and medicinal properties. The 96% ethanol extract of papaya seeds exhibits antibacterial activity against Staphylococcus aureus and Klebsiella pneumoniae.   ABSTRACT Background: Pneumonia is an infectious disease with the highest childhood mortality rate worldwide, commonly caused by Staphylococcus aureus and Klebsiella pneumoniae, which have developed resistance to antibacterial agents. Papaya seeds have traditionally been used in Indonesian medicine due to their bioactive compounds, which are expected to exhibit antibacterial properties. Objective: To analyze the antibacterial effect of 96% ethanol extract of papaya seeds against S. aureus and K. pneumoniae in vitro. Material and Method: This study employed a true experimental design with a posttest-only control group. Ethanol extracts of papaya seeds at concentrations of 10%, 30%, and 50% were tested using the well diffusion method. Erythromycin and chloramphenicol were used as positive controls, and 70% alcohol served as the negative control. Inhibition zone diameters were analyzed using SPSS version 29 with the non-parametric Kruskal-Wallis and Mann-Whitney tests. Results: The ethanol extract of papaya seeds inhibited the growth of S. aureus starting at a concentration of 10%, with the largest inhibition zone observed at 50%. Against K. pneumoniae, the extract demonstrated the lowest effective inhibitory concentration at 30%, which also produced the largest inhibition zone. However, the extract was less effective than the antibiotics used. Conclusion: The ethanol extract of papaya seeds exhibits antibacterial activity against both S. aureus and K. pneumoniae, with a more pronounced effect on S. aureus.

Abstract

Tuberculosis (TB) is a dangerous and highly contagious disease. Global cases have increased since, and Indonesia is one of the countries with the highest number of TB cases. This study aimed to prevent TB transmission by analyzing air contamination. Air samples were taken from three primary health cares (PHCs) in Surabaya City, East Java Province, Indonesia, and cultured in Mycobacterium Growth Indicator Tube (MGIT) media. Of the 108 air samples collected, 36 came from the waiting room, 36 from the examination room, and 36 from the laboratory room. As a result, the waiting room (50%), examination room (16.7%), and laboratory room (2.7%) were declared positive. In short, the air in the three PHCs contained mycobacterium tuberculosis. There is a need for room management to prevent TB transmission.

Abstract

The prevalence of Multidrug antibiotic-resistant Pseudomonas aeruginosa (MDRPA) has been increasing during the decade And has become attention in hospital patients. This retrospective descriptive descriptive aimed to determine the prevalence of MDRPA and its sensitivity patterns. Data were taken from results of bacterial culture and antibiotic resistance tests from various clinical specimens from patients at Dr. Soetomo General Academic Hospital throughout 2022-2023. The resistance test was carried out using a Vitek 2 compact instrument. MDRPA is defined as Pseudomonas aeruginosa that is not sensitive to three or more of the following classes of antibiotics: meropenem or imipenem, ciprofloxacin, gentamicin or amikacin, ceftazidime or cefepime, and piperacillin/ tazobactam. The prevalence of MDRPA was 57.0%. MDRPA isolates were also the most common origin from the burn unit and HCU A (high care unit A), mostly from pus specimens and sputum. Pseudomonas aeruginosa sensitivity was best with piperacillin/tazobactam (55.5%), meropenem (54.8%), amikacin (47.5%), gentamicin (46.5%), cefepime (46.3%), ceftazidime (45.0%), ciprofloxacin (44.7%) and aztreonam (43.2%). The sensitivity of MDRPA to antibiotics is much lower than that of Pseudomonas aeruginosa. This study showed high number of MDRPA specifically in Surabaya and the pattern sensitivity of Pseudomonas aeruginosa can become guidelines in choosing antibiotics treatment for patients.

Abstract

The extended-spectrum b-lactamase (ESBL) producer bacteria until now were mostly identified in hospital environment. The aim of this study was to analyze the prevalence of ESBL-producing gut flora and distribution of ESBL encoding genes between hospitalized patient in Tropical Wards of Dr. Soetomo Hospital and patient from a primary health centre (PHC) as community environment in Surabaya. Thiry rectal swab samples from hospital of Dr. Soetomo patients and from PHC (60 samples in total) were collected for this study. Samples were screened in MacConkey agar supplemented with 2 mg/L of cefotaxim, incubated at 37ºC for 24 hours. Then the growing colony were confirmed with Disk Diffusion Synergy test (DDST) for diagnosis of ESBL producer. The identified ESBL producers were then identified the bacteria species by biochemical method. ESBL gene were detected by PCR with specific primers. The results showed that there was not difference of positif nuber of ESBL-producing bacteria gut floral between patients of Dr.Soetomo Hospital, 25/30 (83.3%) and PHC, 11/30 (36.7%) (p=1). The pattern of ESBL gene distributions among samples from hospital showed that SHV was 12%, TEM was 36%, and CTX-M was 80%, and from PHC were SHV 18.2%, TEM 27,3% and CTX-M 81,8%. Statistical analysis showed that the pattern was not significantly different among hospitals and PHC samples as shown by SHV gene (p=0,631), TEM (p= 0.715), and CTX -M (p=1). From each ESBL gene, the dominant genes that found producing ESBL were the CTX-M genes followed by TEM and SHV genes. The prevalence of ESBL producersin intestinal flora of both the hospital (83,3%) and the PHC (36,7%) was very high. There was not significant difference between the prevalence of ESBL producer in gut flora of hospitalized patients compared to PHC. There was found other patterns of ESBL gene combinations in the hospital of SHV+CTX-M genes, TEM+CTX-M, SHV+TEM+CTX-M genes and PHC, the combination pattern of SHV+CTX-M, TEM+CTX-M.
