Abstract

Objective : This article objective is to describe cytology diagnosis difficulties of yolk sac tumors of the ovary.Method : Case reports and literature review.Case : The author reports the case of a 24 year old woman who complained of an enlarged stomach. Serum AFP increased to 16,519.7 U/mL. Ultrasound examination revealed solid and irregular mass of ovarian, so the conclusion was suspect ovarian carcinoma. Conclusion of CT scan examination was a solid ovarian tumor. The working diagnosis was suspect ovarian carcinoma. Optimal debulking was performed, accompanied by taking a sample from the peritoneal rinse fluid. Microscopic examination of peritoneal fluid showed the distribution and group of cells with pleomorphic nuclei, partly hyperchromatic, partly vesicular with coarse chromatin and prominent nucleoli. There were also cells with polygonal nuclei, small nuclei, basophilic and vacuole cytoplasm with a mucoid background. These cells formed a solid arrangement. Conclusion from these features was carcinoma metastases to the peritoneal fluid. Microscopic examination from tumor tissue sample showed an ovarian yolk sac tumor appearance.Conclusion : Cytologic examination of peritoneal fluid in cases of ovarian yolk sac tumor is quite difficult to determine the diagnosis. This is due to the microscopic appearance of tumor cells which often looks like a carcinoma and limited literature about this tumors in the peritoneal fluid.Keywords: Yolk sac tumor, ovary,

Abstract

Introduction: Pelvic exenteration (PE) is an ultra-radical surgical procedure introduced by Alexander Brunschwig in 1948, which a way for symptomatic palliative care in subjects with advanced gynecological malignancies that do not respond to radiation therapy, who experience subsequent complications including fistula, infection, or pain. Objective: The aim of this case report is to share our first experience of pelvic exenteration in patient with recurrent cervical cancer in West Sumatera. Case Report: We report the case of a 42-year-old woman, who was diagnosed with a progressive recurrent cervical cancer that had been treated with a radiotheraphy for cervical cancer stage IIIB and adjuvant chemotherapy. The patient was planned for pelvic exenteration. Prior to surgery, patient had done several preparations such as ultrasound, CT scan, laboratory tests, urologist and colorectal surgeon consult. The patient had undergone a supralevator pelvic exenteration by gynecologic oncologist, urologist and colorectal surgeon. The supralevator exenteration pelvic included total hysterectomy, bilateral salpingo-oophorectomy, cystectomy, and Miles procedure. The reconstruction process included ileal conduit and colostomy. Conclusions: Pelvic exenteration can be done with good teamwork between gynecologic oncologist, urologist, colorectal surgeon with postoperative complications (hypoalbuminemia and low intake) can be tolerated.Keywords: supralevator pelvic exenteration, cervical cancer

Abstract

This case report aims to report and discuss cases of ovarian fibrosarcoma. Ovarian fibrosarcoma is a rare casethat generally occurs in women who have menopause. In the case of ovarian fibrosarcoma, the patient was 43years old, had two children, and had no complaints during menstruation. The patient has lost weight in the lastsix months. The patient has performed debulking surgery optimally. On the PA results, it looks macroscopic, apiece of brownish-white tissue, densely rubbery, with a size of 15x12x5 cm, a solid white part is visible, and thereis a yellowish-white part and anger, there is a 0.5-1.5 cm cavity filled with a yellowish liquid. On microscopicexamination of the tumor, the tissue section appears to consist of a diffuse proliferation of tumor cells withhypercellular and hypocellular areas. Based on the results of macroscopic and microscopic examination of thetumor tissue, a skewed sarcoma is established in fibrosarcoma. The patient underwent chemotherapy after tumorsurgery. Although there is no clear consensus on the administration of fibrosarcoma chemotherapy, severalpublished case reports have demonstrated a lower probability of recurrence and a better prognosis in patientsundergoing chemotherapy.

Abstract

Background : Peritoneal tuberculosis is one of the most challenging forms of extrapulmonary tuberculosis to diagnose. Extrapulmonary TB is very difficult to diagnose because of its non-specific signs and symptoms, thus it sometimes leads to a diagnosis of gynaecological malignancy such as advanced ovarian carcinoma. Case Report : A teenage girl, 12 years old, with a chief complaint of intermittent abdominal pain since the last 1 month. There was no history of an enlarged abdomen, fever and weight loss. Ultrasound examination revealed an irregular adnexal mass suggesting an ovarian malignancy with other possibilities of an infectious process. Diagnostic laparoscopy and peritoneal biopsy were performed on the patient. Intraoperatively, multiple friable nodular-mylar peritoneal masses with abdominopelvic attachments and fibrin fibres and white vesicles were found on the entire surface of the genitalia, peritoneum, and intestines. A mass biopsy was performed with histopathological results showing peritoneal tuberculosis. Currently, the patient is undergoing anti-tuberculosis drugs (OAT) treatment. Discussion : The diagnosis of this disease is difficult to establish. Symptoms are highly variable and non-specific and can lead to the wrong tumour pathology. The clinical characteristics are dominated by changes in general condition, abdominal pain and transit disorders, as well as masses (20 to 25%). The gold standard for definitive PTB diagnosis remains laparoscopy with peritoneal biopsy and subsequent pathological or microbiological confirmation. Yellow/white nodules in the peritoneum observed on the patient's laparoscopic images are the hallmark of wet peritoneal TB. Omental thickening and abdominal cocoon with matted small bowel are other classic laparoscopic findings for peritoneal TB. Conclusion: PTB has similar characteristics to peritoneal carcinoma, which makes diagnosis difficult for clinicians. Early and correct diagnosis of PT

Abstract

Method Contraception Long Term (MKJP) is possible contraception used for a period long time, more than two years, effectively and efficiently For objective space birth is more than three years or end pregnancy in couples age already fertile no want to add child Again. Based on the data obtained found that active KB acceptors that is not 51.7 % used MKJP and only 48.3% used MKJP. Analysis is done in a way univariate, bivariate, and multivariate. Research results obtained that amount children (p-value 0.016), access service health (p-value 0.329), knowledge (p value 0.003), attitude (p value 0.014), support family (p value 0.048), and source information (p value 0.009). Based on the results Multivariate obtained the most dominant factor related to behavior use of MKJP, namely knowledge with the highest OR value was 1.897.

Abstract

Cervical carcinoma is a malignant tumor of cervical epithelial cells which is more than 90% caused by the human papillomavirus. Clinicopathology factors in cervical carcinoma such as age, history of parity, history of sexual intercourse, history of clinical manifestations, histopathology type and degree of differentiation are known to affect the prognosis. This study aims to determine the clinicopathological characteristics of cervical carcinoma at Dr. M. Djamil Hospital Padang in 2018-2021. This research is a descriptive research with the number of samples, 108 patients with confirmed cervical carcinoma. Research was conducted from March 2022 to March 2023 at the Laboratory of Anatomical Pathology, Dr. M. Djamil Padang. The results found that majority of cervical carcinoma patients is ≥45 years old (74,1%) with all patients are married (100%). The clinical manifestation that most patients complained about was vaginal bleeding 99.1% and 91,7% patients of cervical carcinoma were multiparity. The most common histopathological type of cervical carcinoma is squamous cell carcinoma (77.8%) and the highest degree of differentiation is moderate (47.2%). The cervical carcinoma patients at the Laboratory of Anatomical Pathology Dr. M. Djamil Hospital in 2018-2021 were found in the early elderly age group and are in advanced stage so it is necessary to carry out early detection and prevention of cervical carcinoma.

Abstract

Ovarian cancer is the most lethal disease among gynecologic malignancies; about 1/4 of ovarian cancer patients are in stage IV when diagnosed. Based on the data of Globocan, there were 295,414 newly diagnosed ovarian cancer patients and 184,799 deaths registered in 2018. For ovarian epithelial cancer patients with stage IV disease, the mean overall survival (OS) is around 20 months, and the 5 year-OS is about 20%. About 1 in 5 patients with advanced-stage ovarian cancer have hepatobiliary involvement. The study on HGSOC aims to understand the molecular and genetic features, diagnosis, and prognosis of HGSOC, as well as its response to therapy. The method involves analyzing the genetic traits, chromosomal instability, and dysregulation of signaling pathways associated with HGSOC, as well as the detection of stemness and EMT levels in response to chemotherapy and radiation therapy. The results highlight the unique characteristics of HGSOC, such as nearly universal mutation in and dysfunction of p53, genomic instability, and the role of cancer stem cells and EMT in chemo- and radiation resistance. The implication of the study is the need for a better understanding of the molecular basis of HGSOC for improved diagnosis, prognosis, and treatment strategies.

Abstract

Latar Belakang: Kanker ovarium merupakan kanker pada perempuan yang cukup sering terjadi dan mematikan. Penggunaan kemoterapi dalam pengobatan dapat menyebabkan mual muntah sehingga diperlukan penggunaan obat yang efektif dan rasional. Objektif: Untuk mengevaluasi penggunaan obat antiemetik pada pasien kanker ovarium pasca kemoterapi di RSUP Dr. M. Djamil Padang tahun 2020-2022. Metode: Penelitian ini merupakan penelitian deskriptif dengan pendekatan retrospektif. Data rekam medis dikumpulkan dengan teknik total sampling. Sampel yang memenuhi inklusi berjumlah 45 sampel. Hasil: Hasil penelitia­­n didapatkan karakteristik terbanyak pada kelompok usia 41-60 tahun (48,9%), berpendidikan SLTU (75,6%), dan pekerjaan ibu rumah tangga (55,6%). Jenis obat kemoterapi yang paling banyak digunakan adalah paklitaksel-karboplatin (42,2%). Sebagian besar pasien mendapatkan antiemetik pra-kemoterapi berupa deksametason-difenhidramin-ondansetron-ranitidin (71,1%) serta pasca kemoterapi berupa metoklopramid-ranitidin dalam 3 siklus (74,8%). Rasionalitas penggunaan obat antiemetik pada pasien kanker ovarium pasca kemoterapi yaitu kesesuaian indikasi 93,3%, kesesuaian obat 62,2%, kesesuaian dosis 91,1%, kesesuaian pasien 97,8% dan potensi interaksi obat 2,2%. Kesimpulan: Karakteristik pasien kanker ovarium di RSUP Dr. M. Djamil Padang tahun 2020-2022 sering terjadi pada kelompok usia 40-60 tahun, pendidikan SLTU, dan pekerjaan ibu rumah tangga. Sebagian besar pasien mendapatkan obat kemoterapi paklitaksel-karboplatin dan obat antiemetik pra-kemoterapi deksametason-domperidon-ondansetron-ranitidin serta pasca kemoterapi metoklopramid-ranitidin. Rasionalitas penggunaan obat antiemetik pada pasien kanker ovarium pasca kemoterapi sebagian besar sudah sesuai. Kata kunci: antiemetik, kanker ovarium, kemoterapi, rasionalitas

Abstract

Introduction : High Grade Serous Ovarian Carcinoma (HGSOC) been the dominant subtype and accounted for the highest mortality rate of epithelial ovarian cancer. Therefore, knowledge of the risk factors for epithelial ovarian cancer was needed for early detection. Age and menopausal status were examples of risk factors. This study aimed to determine the relationship between age and menopausal status with the incidence of epithelial ovarian cancer subtype of HGSOC at M Djamil Central Hospital Padang. Method : This type of research is an observational analytic research design cross-sectional. The form sample is secondary data used in the total sampling method, which comes from 90 patients of ovarian cancer. Research in analysis univariate and bivariate presented in form table frequency. Result : In this study, it was found that epithelial ovarian cancers subtype HGSOC was found mostly in women aged > 50 years (68,9%) and who had experienced menopause (80%). In the analytic test, the p value for age and menopausal status was 0.382 and 0.104. Conclusion: Concluded there is no relationship between age and menopausal status with the incidence of epithelial ovarian cancer subtype HGSOC at M Djamil Central Hospital Padang

Abstract

Introduction: Pseudomyxoma Peritonei (PMP), also known as "jelly-belly," is a rare condition characterized by the accumulation of mucinous ascites in the peritoneal cavity. It often originates from mucinous neoplasms of the appendix and presents with non-specific symptoms like abdominal bloating and discomfort. Diagnosis is often incidental through imaging, surgery, or histopathology. Objective: To report the diagnosis and management of PMP in a 53-year-old woman presenting with abdominal enlargement. Case Report: A 53-year-old woman was referred with a two-month history of abdominal enlargement. Initial ultrasound findings suggested ascites due to cirrhosis or ovarian carcinoma. However, further evaluation with abdominal CT revealed ascites and cholelithiasis without signs of cirrhosis or ovarian carcinoma. Diagnostic laparoscopy showed peritoneal carcinomatosis, and 8000 ml of hemorrhagic ascites was drained. Histopathological examination confirmed the diagnosis of PMP, and tissue resection was performed. Conclusion: The management of PMP requires a comprehensive diagnostic approach, incorporating clinical history, physical examination, imaging, and histopathological evaluation. This systematic approach is crucial for accurate diagnosis and effective treatment of PMP.
