Abstract

Non-communicable diseases (NCDs) are a health development priority at this time. The increase in the prevalence of PTM over the last few years. Hypertension and diabetes mellitus are PTM that have increased cases. Changes in behavior and lifestyle are risk factors for hypertension and diabetes mellitus. The recording and reporting of officers related to PTM still experience difficulties in routine monitoring of treatment and patient visits. There is currently no integrated information system between Hypertension and Diabetes Mellitus to monitor patient compliance for treatment. The purpose of the study was to develop an integrated application system for monitoring hypertension and diabetes mellitus in the city of Semarang. The type of research conducted is rapid and development (RAD), with a qualitative approach. The research stage: is the identification of information system development needs, is database design and application interface, and implementation of android application model development on a small scale (three prolanis officers). The subjects of this study were three prolanis officers and five patients with primary hypertension and diabetes mellitus aged 46 to 55 years. Results: system development by analyzing user data needs of officers and patients include patient identity, patient examination results, follow-up on examination results, patient referrals, education on healthy menus and ways to prevent hypertension and diabetes mellitus, the reminders to take medication, and results of routine examinations. Database design and application interface described through DFD and ERD. Implementation and function tests carried out on fully developed applications.

Abstract

BACKGROUND: Inflammation plays an important role in the atherosclerotic process. High-sensitivity C-reactive-protein (hs-CRP) is commonly used as inflammatory biomarker. It is well known that regular physical activity lowers hs-CRP levels, while prolonged exercise induces hs-CRP elevations. However, the relationship of training and exercise characteristics with hs-CRP levels remains not well elucidated. We evaluated baseline and post-exercise hs-CRP levels and its association with training and exercise characteristics.METHODS: Eighty-eight male endurance cyclists were involved. Demographic data, health condition and training characteristics were collected. Baseline and postexercise blood-samples were collected to determine hsCRP concentrations. A hs-CRP cut-off point of 3 mg/L was used. Blood-cell count and biochemical parameters were measured at baseline. Heart rate (HR) was measured during exercise.RESULTS: Cyclists performed 7.3 hours (interquartilerange (IQR) = 5.4-7.5) of endurance exercise at intensity of 81.8 % (IQR = 74.9-85.8). Cyclists with baseline hsCRP ≥ 3 mg/L reported higher body mass, body mass index (BMI), waist-circumference and total-cholesterol. An increase in hs-CRP was following endurance exercise. Cyclists with any elevation of hs-CRP reported a higher BMI, HR during exercise and exercise intensity. Binary logistic regression analysis showed BMI (OR = 1.24, 95% CI = 1.04-1.48) and cycling distance (OR = 0.22, 95% CI = 0.06-0.76) were associated with post-exercise hs-CRP elevations.CONCLUSION: Body mass, BMI, waist-circumference, total- and HDL-cholesterol are associated with baseline hsCRP, whereas BMI and cycling distance were associated with hs-CRP elevations. These findings suggest that anthropometry parameters and lipid levels attributed to baseline hs-CRP, while anthropometry parameters and cycling intensity attributed to post-exercise hs-CRP elevations.KEYWORDS: C-reactive-protein, exercise, endurancecycling, inflammation, acute-phase-response

Abstract

Background: The assessment from the patient's perspective is the assurance of TB service’ quality to achieve the "End TB" target. The information regarding TB services’ quality assessment in the dimensions of QUOTE TB Light is not widely found.Aims: The study aims to review TB services' quality from the patients’ perspective in the 7 dimensions of QUOTE TB Light.Methods: This literature review study searched from 3 databases, namely PubMed, Science-Direct, and Google Scholar since July - October 2020. The inclusion criteria were research on TB services quality, patients’ perspective, and patient assessment of 7 dimenssion. Articles assessed the other health services’ quality and published before January 2015 was excluded.Results: A total of 7 from 89 articles found were selected for review. The patients’ assessments are based on their experiences or satisfactions. Most of the studies are satisfied with service availability, communication, patient-provider interactions, competence, and affordability. The patients’ dissatisfactions about infrastructure and stigma acquired during treatment.Conclusion: Infrastructure and stigma are assessed negatively. The continously evaluation and training for health workers need to be improved so they can serve patients well. The patients’ perspectives can be the major consideration for providers to enhance the TB services’ quality.Keywords: Quality of TB services, QUOTE TB Light, patients’ perspectives, Tuberculosis

Abstract

Diabetes Mellitus (DM) is a non-communicable disease that contributes to the cause of death. Based on the analysis of the situation in Kawengen Village, Semarang Regency, there were several problems related to the incidence of DM, including the Non-Communicable Disease Integrated Guidance Post Program (Posbindu PTM), which was not running optimally. Based on these problems, it is necessary to form a movement called the Anti-Diabetes Mellitus Community Alert Movement (SIMANIS). Active case finding and detection of pre-DM cases aim to capture people who already have pre-DM symptoms, but they do not know. Furthermore, if caught, they can be followed up so that they are willing to go to the health service unit before complications occur. The implementation of this community service activity is carried out in four stages, namely the formation of SIMANIS cadres, education on prevention and control of DM to SIMANIS cadres and the community, ToT on how to fill in and use the SIDIA Card (pre-diabetes screening) to SIMANIS cadres, and use of the SIDIA Card for early detection active case finding pre-DM. There was an increase in the pre-post education knowledge score from 7.59 + 1.5 to 8.93 + 0.9 and an increase in the pre-post education attitude score from 7.96 + 1.22 to 9.07 + 0.78. SIMANIS through the use of the SIDIA Card can be used to increase public awareness in prevention, early detection, and case finding of DM.

Abstract

Background: Tuberculosis (TBC) treatment regimens require the introduction of PMO (Drugs Swallowing Drug) standardized on International Standards for Tuberculosis Care (ISTC) with DOTS strategy (Directly Observed Treatment Shortcouse). The goal of controlling new challenges such as TB/HIV co-infection, Multi Drug Resistance (MDR), Extensively Drug Resistance (XDR) and Loss to follow-up. Objective: Type of cross sectional study. Population 35 PMO using saturated samples. Methods: The instruments used are questionnaires and in-depth interviews. Data analysis is univariate, bivariate and multivariate. Results: The result of the study was the relationship between the role of PMO from family members in the supervision of taking medicine (p=0,024), there was a relationship between the role of PMO from family members in drug taking assistance in Fasyankes (p=0,002) and there was relationship between the influence of PMO role from family member accompaniment of sputum checks in Fasyankes (p=0,005) to TBC patient compliance. Conclusion: There is a statistical correlation between the roles of PMO from the family towards adherence to tuberculosis patients. Suggestion of research is given to related parties be reformed TBC control policy related to election criteria of PMO coming from family member in Semarang regency.

Abstract

The TB case notification rate (CNR) in Semarang City has decreased from 2019 of 258 per 100,000 population to 155 per 100,000 population in 2020. The main focus in controlling tuberculosis cases is case detection in the community. The purpose was to increase TB knowledge and provide training on TB screening methods to adolescent cadres using the SIKRIBO application. The methods used in this community service are health education and Training of Trainers (ToT). The participants were 27 adolescent representatives from the Adolescent Integrated Healthcare Center working areas of the Sekaran Public Health Center, Semarang City. The results showed that adolescent cadres' average score knowledge about tuberculosis disease and screening improves after receiving TB health education and adolescent cadres can operate the application properly. The SIKRIBO application can be a means to facilitate the discovery of TB suspects in the community and is expected to be applied in various health facilities.   ABSTRAK Angka notifikasi kasus (CNR) TB di Kota Semarang mengalami penurunan dari tahun 2019 sebesar 258 per 100.000 penduduk menjadi 155 per 100.000 penduduk pada tahun 2020. Fokus utama dalam mengendalikan kasus TB merupakan penemuan kasus di masyarakat. Rendahnya penemuan kasus TB berdampak pada tingginya angka penularan TB. Tujuan pengabdian masyarakat ini untuk meningkatkan pengetahuan TB dan memberi pelatihan terkait cara skrining TB melalui aplikasi SIKRIBO pada kader remaja. Metode yang digunakan pada pengabdian masyarakat ini adalah penyuluhan kesehatan dan Training of Trainer (ToT). Jumlah partisipan sebanyak 27 remaja perwakilan dari Posyandu Remaja dan Posyandu Pesantren wilayah Puskesmas Sekaran, Kota Semarang. Hasil kegiatan bahwa terdapat peningkatan rata-rata skor pengetahuan terkait penyakit TB dan skrining TB pada kader remaja setelah diberi penyuluhan kesehatan TB dan kader remaja dapat mengoperasikan aplikasi SIKRIBO dengan baik. Aplikasi SIKRIBO dapat menjadi sarana untuk mempermudah penemuan suspek TB di masyarakat dan diharapkan dapat diterapkan di berbagai fasilitas kesehatan.

Abstract

The increasing number of stroke cases and their risk factors is a crisis that needs to be addressed. This study aimed to determine the number of new stroke cases in Bogor, Indonesia, and its risk factors. We conducted a nested case-control study from the Study of Non-Communicable Disease (NCD) Risk Factors of the National Health Institute of Research and Development, Ministry of Health, Indonesia, in 2018. A deep analysis was conducted on 1.210 respondents based on the subset of baseline this data. Data was collected by interview method on permanent residents in Bogor City, Indonesia in 2018. Diagnosis of stroke was based on the anamnesis of a neurologist and a neurological examination. Independent variables include sociodemographic characteristics, metabolic syndrome, and risky behaviors. The data was analyzed with a dual logistic regression test. The incidence of stroke during 2018 amounted to 48 cases (3.9%). The main determinants of stroke in this population include diabetes mellitus and systolic blood pressure being the main factors for stroke, with each p-value, AOR, and (95% CI) being 0,000, 0.222 (0.122-0.405), 0.003, 0.291 (0.128-0.662). There was no significant difference in metabolic syndrome outcomes one year before the diagnosis of stroke and the year when the stroke was diagnosed. Special attention is needed in DM patients with high blood pressure to prevent stroke.

Abstract

  This study undertakes a comprehensive bibliometric analysis to visualize the knowledge landscape of geriatric rehabilitation. Utilizing the Web of Science and Scopus databases from 1947 to 2023, a meticulous bibliometric analysis was executed. The research was facilitated through ScientoPy and VOSviewer tools, enabling meticulous scrutiny and insightful visualization of the research network. A total of 5120 papers were identified through the search strategy. The outcomes reveal the United States as the foremost contributor to geriatric rehabilitation research, with the Archives of Physical Medicine and Rehabilitation emerging as a highly active journal. Among the most significant author’s keywords, “rehabilitation”, “aged” and “hip fracture” are prominent, underscoring pivotal domains for targeted interventions within geriatric care. Furthermore, this analysis highlights noteworthy keywords such as “sarcopenia,” “physical activity,” and “virtual reality,” offering essential insights into core themes within geriatric rehabilitation. Consequently, this study presents a valuable resource for scholars and researchers alike. It serves as a foundational tool for identifying prevalent research trends, addressing existing gaps, and embarking on new explorations in the dynamic realm of geriatric rehabilitation. The findings provide a holistic understanding of the knowledge landscape, paving the way for informed decision-making, innovative research endeavors, and the enhancement of geriatric rehabilitation practices.

Abstract

Latar Belakang: Data Kementerian Kesehatan Republik Indonesia menyebutkan bahwa tuberkulosis masih menjadi penyebab utama kesakitan dan kematian pada ODHA sebesar 25%. Tujuan penelitian ini adalah untuk mengetahui determinan atau faktor yang mempengaruhi perubahan kadar CD4 pada ODHA koinfeksi TB di RSUD Kota Bandung. Metode: Jenis penelitian yang digunakan merupakan penelitian observasional analitik dengan rancangan atau desain penelitian yang digunakan adalah cohort retrospektif. Jumlah sampel yang ditetapkan sebesar 77 sampel. Instrumen yang digunakan pada penelitian ini adalah register dan rekam medis pasien. Data dianalisis menggunakan uji Chi Square dengan perangkat lunak pengolah data yaitu SPSS. Hasil: Hasil menunjukkan bahwa usia (RR=1,050; CI 95%=0,366-3,016; dan p=0,928), jenis kelamin (RR= 0,810; CI 95%= 0,718-0,913; dan p=0,109), status kerja (RR=0,806; CI 95%=0,714-0,911; dan p=0,109), jumlah CD4 awal (RR=0,240; CI 95%= 0,080-0,723; dan p=0,015), stadium klinis (RR=0,750; CI 95%=0,266-2,117; p=0,586), kepatuhan konsumsi OAT (RR=1,076; CI 95%=0,166-6,979; dan p=1,00), kepatuhan ART (RR=2,000; CI 95%=0,640-6,264, dan p=0,363), lama ART (RR=0,803; CI 95%=0,709-0,909, dan p=0,062). Kesimpulan: Simpulan penelitian ini adalah terdapat hubungan antara jumlah CD4 awal terhadap perubahan kadar CD4 pada ODHA koinfeksi TB di RSUD Kota Bandung. Sementara tidak terdapat hubungan antara usia, jenis kelamin, status kerja, stadium klinis, kepatuhan konsumsi OAT, kepatuhan ART, dan lama ART terhadap perubahan kadar CD4 pada ODHA koinfeksi TB di RSUD Kota Bandung.

Abstract

Tuberculosis (TB) is an infectious disease caused by Mycobacterium Tuberculosis and is the leading cause of health problems and the most common cause of death worldwide. Tuberculosis is the disease that causes the most health problems in the community to date. Based on data from the Semarang City Health Office, it is known that 41.4% of tuberculosis patients will undergo Voluntary Counseling and Testing (VCT) screening in 2022. This is of course still below the screening target, which is the 100% target. Mismatch with VCT screening targets in TB patients can occur due to fear and anxiety if TB patients know their health status related to HIV and AIDS. This study was prepared using a cross-sectional design. The number of respondents included is 107 people diagnosed with TB at Kedungmundu, Bangetayu, and Tlogosari Wetan Health Centers in 2023. Univariate and Bivariate analyses were used to analyze the results of this study. The results of this study found that variables consisting of age, marital status, and education had no relationship with the use of VCT services in tuberculosis (TB) patients in Semarang City.
