Abstract

This research aims to analyze the achievement of Universal Health Coverage (UHC) from the perspective of the Policy Analysis Triangle by examining the interconnection of actors, content, context, and process. The study, conducted in the Pelalawan Regency from June to August 2023, employs qualitative research methods with 5 primary informants and 3 supporting informants. The findings reveal that each informant's role aligns with their respective fields, indicating sufficient budgetary allocations. However, not all patients receive counseling about JKN. Regarding content, JKN covers all promotive, preventive, curative, and rehabilitative services. Differences exist in the services provided to general patients and JKN participants, with non-cut-off services functioning well. In the context aspect, all community members are obligated to be covered by JKN, and local governments can budget for JKN financing in accordance with Presidential Regulation number 83 of 2018. Both infectious and non-communicable diseases are covered by JKN, and there is no service discrimination at the first-level health facilities. Regarding the process, the UHC planning initiated in March 2022 was implemented by October 2022. Key drivers for UHC include the integration of (Integrated Social Welfare Data ) DTKS data into JKN and continuous socialization and advocacy efforts.

Abstract

In Indonesia, visits to elderly posyandu (integrated health service posts) are generally below target, particularly in Siak Regency, Riau Province, where elderly health services have been declining annually, especially at Tualang Health Center. This study aims to understand elderly behavior in utilizing health services through knowledge, attitudes, actions, and health center efforts. This qualitative study employs a phenomenological design and was conducted in the working area of Tualang Health Center, Siak Regency, from March to May 2024. The total number of informants is 24, consisting of 9 main informants—3 active elderly (AE) who visit posyandu monthly, 3 inactive elderly (IE) who missed posyandu visits three times a year, and 3 non-attending elderly (NE) who never visit posyandu. Additionally, there are 15 supporting informants, including family members, the head of the health center, elderly program managers, midwives, and 3 cadres. Data processing utilized data triangulation and thematic analysis. The study found that AE and IE have better knowledge compared to NE due to their participation in posyandu activities. All elderly informants had a positive attitude towards the quality of health services at posyandu. AE tend to visit posyandu regularly due to easy access, transportation, and family support, while IE and NE are less active due to distant access, limited transportation, and lack of family support. It is recommended that the health center empower elderly families to increase elderly participation in utilizing health services.

Abstract

Type 2 Diabetes Mellitus (T2DM) is a chronic disease with an increasing prevalence in Indonesia and globally. Complications from T2DM have serious impacts on the quality of life for patients. The increase in the number of complications in T2DM patients at Selasih Regional Hospital, Pelalawan Regency, is the main issue in this research. The study aims to identify risk factors associated with complications in T2DM patients at Selasih Regional Hospital, Pelalawan Regency. The research design used is a case-control study with a sample of 400 patients (200 cases and 200 controls). Cases are T2DM patients with complications, while controls are T2DM patients without complications. Case and control samples were taken using a systematic random sampling method. Data were collected from the medical records of Selasih Regional Hospital in 2023. Data analysis was conducted using chi-square tests and multiple logistic regression. The results showed that high cholesterol (OR=206.46), duration of DM (OR=79.62), high blood pressure (OR=22.54), and irregular check-ups (OR=4.43) were significantly associated with T2DM complications. Interventions to control cholesterol and blood pressure and to increase routine check-ups can reduce the risk of T2DM complications. Therefore, there is a need for increased education and health promotion programs from the hospital regarding diabetes management to prevent the occurrence of T2DM complications.

Abstract

Daycare centres (TPA) play an essential role in supporting mothers in caring for their children, especially for working mothers. Daycare centres are one of the alternatives for mothers to replace their role in caring for their children while they are at work. However, TPA is often a place where the transmission of infectious diseases such as diarrhoea, respiratory infections, and other diseases occurs quickly due to close interaction between children. Therefore, preventing the transmission of infectious diseases in TPA is an important step to prevent stunting. In addition, monitoring of children's growth has yet to be done routinely in the TPA. The purpose of this community service is to increase knowledge for mothers and landfill managers in preventing transmission of infectious diseases and monitoring the growth of toddlers, as well as practical steps in maintaining the personal hygiene of children and the environment and the availability of educational media in the form of posters, leaflets and videos. The targets of the service are mothers of toddlers and landfill managers. The service method provides education, distributes hygiene tools, and provides educational media, namely posters, leaflets, and videos. The evaluation was carried out by distributing questionnaires before and after counselling. The service results showed increased knowledge of mothers of toddlers and landfill managers. The results of growth monitoring show that participating toddlers have good nutritional status and can practice hand washing properly. Although there are obstacles, such as the limited number of participants, this activity positively impacts increasing awareness about stunting prevention. The subsequent development is to expand the reach of education by involving more TPAs and introducing digital monitoring methods through applications to monitor children's growth. Support from relevant agencies is needed to ensure the sustainability of the programme.
