Abstract

Perkembangan ilmu pengetahuan dan tehnik operasi bedah saraf ini berbanding lurus dengan kemajuan keilmuan anestesi. Tatalaksana anestesi sangat mempengaruhi kualitas hidup dan kesehatan pasien pascabedah. Seorang laki-laki 58 tahun dengan diagnosis trigeminal neuralgia sinistra, berat badan 60 kg dan tinggi badan 165 cm. Pasien mengeluh nyeri wajah sebelah kiri yang terkadang disertai nyeri kepala. Riwayat hipertensi dan penyakit penyerta lain disangkal. Riwayat konsumsi obat-obatan seperti carbamazepine disangkal. Dilakukan induksi anestesi umum dengan tehnik total intravenous anesthesia (TIVA) menggunakan teknik target controlled infuse (TCI): propofol, dexmetomidine, fentanyl dan rocuronium, sebagai alat pantau/monitoring digunakan index of consciousness (IoC), lama operasi 2 jam dan lama pasien teranestesi 2 jam 30 menit. Pascabedah pasien dirawat di ICU selama 1 hari, lalu dipindahkan ke ruang rawat inap dan pulang ke rumah pada hari ke-6 perawatan. Mikrovaskular dekompresi merupakan operasi bedah otak yang minimal invasif menuntut para ahli anestesi untuk bertanggung jawab menyokong pascabedah yang lebih optimal, sehingga pasien cepat bangun dan penilaian neurokognitif dilakukan sedini mungkin. Penggunaan IoC sebagai alat pantau pasien/monitoring selama diberikan anestesi TIVA sangatlah berguna. Hal ini bertujuan untuk mencegah timbulnya pasien tetap sadar selama operasi berlangsung, dengan melihat kedalaman anestesi yang diberikan, agar tidak terjadi kekurangan atau kelebihan dosis obat-obatan anestesi yang diberikan.The role of index of consciousness (IoC) Total Intravenous Anesthesia Management for Microvascular Decompression SurgeryThe development of science and engineering neurosurgical operation is directly proportional to the scientific advancement of anesthesia. Management of anesthesia greatly affect quality of life and health of patients postoperatively. A man 58 years old with a diagnosis of the left trigeminal neuralgia, weighing 60 kg and height 165 cm. Patients complain of pain left face is sometimes accompanied by headache. A history of hypertension and other comorbidities denied. A history of consumption of drugs such as carbamazepine denied. Induction of general anesthesia with TIVA technique using TCI: propofol, dexmetomidine, fentanyl and rocuronium, as a means of monitoring / monitoring use IoC (index of consciousness), long operating time of 2 hours and anesthetized patients 2 hours 30 minutes. Postoperative patients admitted to the ICU for 1 day, and then transferred to the wards and go home on the 6th day of treatment. Microvascular decompression is a brain surgery less invasive and requires minimal bleeding anesthesiologists responsible for more optimal postoperative support, so patients quickly get up and neurocognitive assessment done as early as possible. The use IoC as a tool to monitor patients during anesthesia TIVA, its very useful. It aims to prevent the patient awareness during surgery, to see the depth of anesthesia is given, in order to avoid under- or overdosing anesthesia agents.

Abstract

Latar Belakang dan Tujuan: Keluhan sakit kepala setelah cedera otak traumatik (COT) disebut sebagai Post Traumatic Headache (PTH), yang dapat terjadi setelah cedera otak ringan, sedang atau berat. Tujuan penelitian ini untuk menemukan korelasi antara tipe hematoma intrakranial dengan kejadian dan beratnya PTH.Subjek dan Metode: Penelitian observasional cohort prospektif pada 31 pasien, umur1359 tahun, laki-laki dan perempuan, yang mengalami COT ringan atau sedang. Pengambilan sampel secara consequetive sampling. Parameter yang dicatat adalah umur, jenis kelamin, berta badan, Glasgow Coma Scale (GCS), tipe hematoma intrakranial, kejadian PTH dan beratnya PTH dengan menggunakan skor numeric rating scale (NRS). Analisis korelasi linier dengan dua variable dengan analisis korelasi Spearman. Korelasi dianggap signifikan bila koefisien korelasi (R) 0,4 dan p0,05.Hasil: Seratus persen pasien subdural hematoma (SDH) dan Intracerebral Hematoma (ICH) mengalami post traumatic headache dan hanya 70,6% pada pasien EDH. Pasien dengan depressed fractur tanpa perdarahan intrakranial mengalami PTH sebanyak 33,3%.Simpulan: Perdarahan yang terjadi dibawah duramater menunjukkan kejadian PTH yang paling tinggi.The Correlation between Type of Intracranial Hematoma with The Incidence and Severity of Post Traumatic Headache (PTH)Background and Objective: Headache occurs after Traumatic Brain Injury (TBI) is known as Post Traumatic Headache (PTH), which could manifest after a mild, moderate, or severe head injury. The aim of this study is to evaluate the correlation between type of intracranial hematoma with the incidence and severity of PTH.Subject and Method: This prospective observational cohort study was performed in 31 patients aged from 1359 years old with mild or moderate TBI usig a consequetive sampling retrieval. Parameters recorded in this study were age, gender, weight, GCS, type of hematoma intracranial, the incidence of PTH, and severity of pain of PTH using the numeric rating score (NRS) score. Linear correlation analysis of two variables was calculated using Spearman correlation analysis. The correlation is significant if the correlation coefficient (R) 0.4 and p 0.05.Result: One hundred percent of subdural hematoma (SDH) and intracerebral hematoma (ICH) patients were experienced PTH and only 70,6% in epidural hematoma (EDH) patients. PTH also found in 33.3% of patient with depressed fracture without intracranial bleeding.Conclusion: Hematoma under duramater causes the highest incidence of PTH

Abstract

Spinal adalah lokasi yang paling umum untuk metastasis tulang. Metastasis spine dapat menyebabkan nyeri, ketidakstabilan tulang belakang dan cedera neurologis lainnya. Pada operasi tumor spinal metastasis pendekatan pembedahan menjadi hal penting baik bagi ahli bedah saraf maupun neuroanestesi. Pada kasus ini laki-laki 60 tahun dengan tumor ekstradura metastasis torakal akan menjalani operasi pengangkatan tumor dan stabilisasi dengan pendekatan posterolateralextracavity untuk mendapat akses yang optimal ke bagian ventral spinal bagian torakal atas. Pasien di induksi dengan fentanil 200 mcg dan propofol 100 mg, fasilitasi intubasi dengan rocuronium 50 mg dan pemasangan double lumen tube kiri, posisi lateral dekubitus. Ketika dilakukan ventilasi satu paru pasien mengalami kejadian desaturasi oksigen sampai 93%, dilakukan penyesuaian mode ventilator sebagai penanganannya. Pascaoperasi pasien tidak diekstubasi dan menjalani perawatan diruangan intensif selama dua hari dan dipulangkan pada hari ke 13. Tatakelola kasus ini difokuskan pada penilaian preoperatif, pengaruh posisi lateral dekubitus dan teknik anestesi satu paru terhadap fungsi respirasi dan kardiovaskular. Risiko hipoksemia akibat ketidaksesuaian ventilasi / perfusi yang menyebabkan gangguan oksigenisasi dan perfusi terhadap otak dan medulla spinalis, sehingga diperlukan tatakelola ventilasi mekanik, monitoring intraoperasi yang berhubungan dengan kaidah-kaidah neuroproteksi terhadap otak dan medulla spinalis.Management of Mechanical Ventilation in the Removal of Thoracal Extradura Metastatic Tumors with One Lung Anesthesia Technique in Decubitus Lateral PositionAbstractThe spine is the most common location for bone metastases. Spine metastases can cause pain, spinal instability and other neurological injuries. In spinal metastatic tumor surgery a surgical approach is important for both neurosurgeons and neuroanesthesiologists. In this case, a 60-year-old man with a thoracic metastatic extradura tumor would undergo tumor removal and stabilization with the posterolateralextracavity approach to obtain optimal access to the ventral spinal ventral region. Patients were induced with 200 mcg fentanyl and propofol 100 mg, facilitation of intubation with 50 mg rocuronium and installation of the left double lumen tube, lateral decubitus position. When one lung is ventilated, the patient experiences an oxygen desaturation event of up to 93%, adjusting the ventilator mode as a treatment. Postoperatively the patient was not extubated and underwent intensive care for two days and was discharged on day 13. The case management focused on preoperative assessment, the influence of lateral decubitus position and one-pulmonary anesthetic technique on respiration and cardiovascular function. The risk of hypoxemia due to ventilation / perfusion mismatches that cause oxygenisation and perfusion disorders of the brain and spinal cord, so that management of mechanical ventilation, intraoperative monitoring associated with neuroprotection rules of the brain and spinal cord.

Abstract

One of the causes of cerebral venous sinus thrombosis (CVST) is platelet hyperactivity. Adhesion and secretion are the beginning of platelet activation, which is indicated by a change in the Platelet-selectin (P-selectin) level. The end result of platelet activation is platelet aggregation. However, it is unknown whether the beginning of platelet activation ends with platelet aggregation. This study aimed to discover the correlation between P-selectin level and platelet aggregation in CVST. This study used a cross-sectional escriptive observational correlative approach. Subjects were the CVST outpatients visiting the Neurology Department of Dr. Hasan Sadikin General Hospital, Bandung, Indonesia, from July to September 2021. A total of 49 subjects met the inclusion and exclusion criteria. This study used citrate plasma samples for platelet aggregation and serum for P-selectin assessment. Platelet aggregation were assessed using the light transmission platelet aggregation method while P-selectin was assessed using Enzyme-linked immunosorbent assay (ELISA). Platelet aggregation median was 10.6% (range 0.2–82.4%), which reflected normoaggregation. Platelet hyperaggregation were seen in 9 samples (8.4%). Median of P-selectin was 2.4 ng/mL (range 0.1–10.1 ng/mL) which were normal. High P-selectin level was observed in 16 (32.7%) with 4/16 (25%) experiencing platelet hyperaggregation. Statistical analysis showed a weak negative correlation between P-selectin and platelet aggregation (r=-0.012; p=0.467). In conclusion, no correlation is seen between P-selectin and platelet aggregation, which may be due to the fact that platelets are influenced by many factors that are not examined in this study.

Abstract

Processed electroencephalography (pEEG) has become an integral tool in modern anesthesia and critical care, enhancing the precision of anesthesia depth monitoring, reducing the risk of accidental awareness under general anesthesia (AAGA), and postoperative cognitive issues. Unlike raw EEG, which records cortical electrical activity directly, pEEG applies mathematical and algorithmic analyses, such as spectral analysis and Fourier transformation, to generate numerical indices that are more interpretable for clinicians. Several commercial systems, including the Bispectral Index (BIS), Entropy, Conox, and SedLine, are widely available. For neuroanesthesiologists, understanding core EEG principles and advanced metrics, such as the Density Spectral Array (DSA), Spectral Edge Frequency (SEF), and Burst Suppression Ratio (BSR), is crucial for accurate interpretation. Moreover, recognizing EEG patterns characteristic of various anesthetic agents, including propofol, inhaled agents, dexmedetomidine, ketamine, and opioids, further refines clinical decision-making. Mastery of EEG interpretation ultimately supports better safety, individualized neuroanesthesia practice aligned with the principles of precision medicine

Abstract

Background: Ischemic stroke in Indonesia is one of the main causes of disability and mortality in elderly. Aging is related with the increase of comorbidities. The comorbidities in elderly increase the potential for polypharmacy prescribing which increases the incidence of drug side effects. This study’s purpose is to know the demographic characteristic, treatment pattern, and side effect of the treatment in elderly patients with ischemic stroke. Methods: This study is a descriptive study with a cross-sectional design which was conducted throughout September–October 2020. This study used secondary data from ischemic stroke patients’ medical records admitted in Dr. Hasan Sadikin Central General Hospital’s in 2015–2019, with simple random sampling.Result: Result of this study shows that most elderly patients with ischemic stroke were young elderly aged 60-69 years, male, with hypertension as the major risk factor. Polytherapy (2-4 drugs) was found in 79 patients and polypharmacy (greater than 5 drugs) was found in 21 patients. Six patients with polytherapy and four patients with polypharmacy experienced drug side effects. Drug side effects of treatment observed were stress ulcer, upper gastrointestinal bleeding, and anemia.Conclusions: The treatment patterns were dominated by polytherapy (2-4 drugs). The percentage of elderly patients with polypharmacy (greater than 5 drugs) experiencing side effects was more than those with polytherapy. Drug side effects were dominated by stress ulcer.

Abstract

Diabetes mellitus (DM) is a major non-communicable disease among older adults and a growing public health problem in Indonesia. Older people often have limited health literacy, which may affect their knowledge and self-management of DM. This study aimed to analyze the relationship between education level and knowledge about DM among the elderly in Bandung City, West Java. A descriptive cross-sectional study was conducted on 150 elderlies with type 2 DM using purposive sampling at primary health care facilities with high DM cases. Data were collected through structured interviews using a demographic questionnaire and a 15-item multiple-choice questionnaire on DM knowledge that had been tested for validity and reliability previously, adapted from an Indonesian diabetes self-management instrument. Knowledge scores were categorized into good and poor. Data were analyzed using the Chi-square test. Most respondents had an elementary or high-school education. The majority of the elderly with higher education levels had good knowledge about DM, while those with lower education levels tended to have poorer knowledge. There was a statistically significant association between education level and elderly knowledge about DM (p<0.001). Education level is significantly associated with DM knowledge among the elderly in Bandung. These findings highlight the need for a tailored continuity of care diabetes program, especially for the elderly with lower educational attainment, in order to improve self-management and glycemic control.

Abstract

Background and Objective: Stroke is an economic and health burden for patients, society, and health services; and one is the length of stay hospitalization. Ischemic stroke patients with hospitalization 7 days indicate the quality of hospital stroke services. This study aimed to compare the clinical characteristics of ischemic stroke patients during hospitalization at Dr. Hasan Sadikin Hospital Bandung, Indonesia.Subject and Method: This study was cross-sectional, with a sample research subjects of all ischemic stroke patients from January to April 2023 at Dr. Hasan Sadikin General Hospital Bandung, Indonesia were divided into 2 groups, namely the ≤ 7 days and the 7 days group. The indicators characteristics were age, onset, National Institute of Health of Stroke Scale (NIHSS), Neutrophyl-to-Lymphocyte Ratio (NLR), and complications obtained through medical records. Statistical analysis was calculated using the Mann-Whitney and Fisher correlation tests.Results: A total of 59 subjects, there are no significance difference (p0.05) on variables of age (median 58 vs 63 years), onset (median 11 vs 7 hours), NIHSS (median 6 vs 8), and NLR (median 3.22 vs 4.41). In-hospital complications such as infection and hyponatremia, was significantly different between two groups (p0.05). Conclusion: In-hospital complications are associated with prolonged length of stay in patients with acute ischemic stroke. This can be the basis for developing strategies to increase the indicators management of stroke services by reducing the complication ischemic stroke during hospitalization.
