Abstract

Organic mental disorders are psychiatric conditions caused by identifiable brain pathology or specific systemic diseases, characterized by psychotic, affective, anxiety, and cognitive symptoms that can impair daily functioning. Psychotropic medications play a central role in their management; however, data on utilization patterns, particularly among outpatients in regional psychiatric hospitals, remain limited. This study aimed to quantitatively evaluate the use of psychotropic drugs using the Anatomical Therapeutic Chemical/Defined Daily Dose (ATC/DDD) method recommended by the World Health Organization, and to identify drugs within the Drug Utilization 90% (DU90%) segment to support rational drug use. This retrospective descriptive study utilized secondary data from medical records of outpatients diagnosed with organic mental disorders at Kolonel H. M. Syukur Regional Psychiatric Hospital, Jambi, in 2024, with a total sampling of 100 patients. The results showed that the total psychotropic drug utilization was 9.89 DDD per patient per day. Antipsychotics were the most frequently used class, followed by antidepressants and anxiolytics. The DU90% segment consisted of eleven drugs, predominantly atypical antipsychotics, selective serotonin reuptake inhibitors (SSRIs), and benzodiazepines, with olanzapine, escitalopram, fluoxetine, and alprazolam being the most commonly prescribed. Most patients were male, in the productive age group, and diagnosed with unspecified organic mental disorders. Overall, the pattern of psychotropic drug utilization reflects the complexity of clinical manifestations in organic mental disorders and underscores the importance of monitoring drug use to promote rational prescribing practices in clinical settings.

Abstract

Background: Tuberculosis (TB) is an infectious disease caused by infection with the bacterium Mycobacterium tuberculosis (M. tb). This bacterium has characteristics that make it difficult to treat, resulting in prolonged TB therapy. Non-adherence to anti-tuberculosis drugs (ATD) may increase the risk of treatment failure in patients diagnosed with pulmonary TB, which in turn can elevate the risk of multidrug resistance (MDR). Method: This study is a descriptive observational research with a cross-sectional design and consisted of 45 participants. The research instrument used was the MMAS-8 (Morisky Medication Adherence Scale) questionnaire. Univariate and bivariate analyses were performed using the Chi-Square test. Result: The results showed that 34 respondents (75.6%) had a high level of adherence, 9 respondents (20%) had a moderate level of adherence, and 2 respondents (4.44%) had a low level of adherence. According to statistical analysis using the Chi-Square test, the significance value for gender and adherence was 0.822, for age and adherence was 0.031, for educational level and adherence was 0.445, and for occupation and adherence was 0.465. Conclusion: The majority of TB patients at Puskesmas Kenali Besar demonstrated a high level of adherence.

Abstract

Perkembangan sistem penghantaran berbasis nanoteknologi dalam bidang kosmetik semakin meningkat, salah satunya melalui serum nanoekstrak yang mampu meningkatkan stabilitas dan efektivitas bahan aktif terutama yang berasal dari bahan alam seperti Nanoekstrak Daun Pucuk Merah (Syzigium myrtifolium Walp). Senyawa flavonoid dan fenolik pada ekstrak Daun Pucuk Merah memiliki aktivitas antioksidan tinggi, namun rentan mengalami degradasi akibat paparan cahaya dan oksidasi. Penelitian ini bertujuan untuk memperoleh formula serum nanoekstrak daun pucuk merah dengan karakteristik fisik dan aktivitas antioksidan terbaik. Nanoekstrak diformulasikan menggunakan metode gelasi ionik dengan polimer kitosan dan alginat serta natrium tripolifosfat sebagai crosslinker, selanjutnya di formulasi dalam sediaan serum. Hasil karakterisasi menunjukkan ukuran partikel berada pada rentang 135,5-194,4 nm dengan nilai PDI pada rentang 0,463-0,991 dan rentang zeta potensial -23,7 mV s.d -25,6 mV. Formula Nanoekstrak Daun Pucuk Merah (NDPM) menunjukkan aktivitas antioksidan sangat kuat dengan nilai IC50 sebesar 1,86 ppm, lebih baik dibandingkan ekstrak daun pucuk merah tanpa formulasi nanoteknologi yang memilik nilai IC50 sebesar 6,16 ppm. Formula II Serum Nanoekstrak Daun Pucuk Merah (SNDPM) dengan berat ekstrak 0,28 g menghasilkan karakteristik fisik, stabilitas dan aktivitas antioksidan yang terbaik. Serum Nanoekstrak Daun Pucuk Merah (SNDPM) berpotensi dikembangkan sebagai kosmetik alami yang inovatif, aman, dan ramah lingkungan untuk melindungi kulit dari radikal bebas dan paparan sinar ultraviolet.

Abstract

An increase in the ultraviolet (UV) index poses severe health risks by inducing harmful effects on the skin, such as sunburn, erythema, skin darkening, premature ageing, and skin cancer. These risks are mitigated through sunscreens with high Sun Protection Factor (SPF) values (15). Natural sources of high SPF include patchouli leaf essential oil, which contains flavonoids. Flavonoid compounds with double bonds can absorb UV-A and UV-B rays, protecting against their harmful effects. The purpose of this study was to find the best formula and observe its impact on the physical characteristics of the preparation and SPF activity. Formula optimization used Design Expert software 13 with the simplex lattice design method. The optimized variables were TEA and stearic acid as emulsifying agents. The results of the optimum formula composition consisted of 4% TEA and 5% stearic acid. The optimum skin protection lotion preparation has a yellow color, a distinctive smell of patchouli oil, a semi-solid shape and a thick texture. The pH parameters of 7.7±0.09, adhesion of 2.34±0.06 seconds, spreadability of 6.85±0.02 cm and viscosity of 4503.3±219.39 Cp. Patchouli leaf essential oil and lotion have ultra-category SPF values with oil SPF value of 27.86±1.07 and lotion preparation of 27.65±1.75 preparation is also homogeneous with pH parameters of 7.7±0.09, adhesion of 2.34±0.06 seconds, spreadability of 6.85±0.02 cm and viscosity of 4503.3±219.39 Cp. Patchouli leaf essential oil and lotion have ultra-category SPF values with oil SPF value of 27.86±1.07 and lotion preparation of 27.65±1.75.
