Abstract

Advances in digital technology in the healthcare sector are driving increased use of wearable data across various services, including health insurance. Data generated by wearable devices such as physical activity, heart rate, and sleep patterns offer opportunities to support more dynamic health risk assessments, the development of more personalized premiums, and the strengthening of preventive approaches within the health insurance system. This trend indicates a shift in the insurance system from a reactive approach toward a data-driven and preventive approach. This study employs a literature review approach by examining relevant scientific articles from databases such as Google Scholar, PubMed, and Garuda. The analysis focuses on literature regarding the utilization of wearable data in health insurance as well as the accompanying ethical considerations. The findings indicate that wearable data holds significant potential in supporting the transformation of a data-driven health insurance system; however, its implementation still faces limitations regarding privacy, data security, and the readiness of regulations specifically governing the use of digital health data. Additionally, the use of wearable data raises ethical challenges, such as the potential for misuse of health data and the possibility of unequal treatment of insurance participants based on their health conditions.In Indonesia, regulations regarding the protection of digital health data are still evolving and do not yet fully address the technical aspects of using wearable data in health insurance systems. Therefore, there is a need to strengthen regulations and policies grounded in digital ethics to ensure that the use of wearable data is safe, fair, and responsible.

Abstract

Community participation is a crucial element in health development efforts because the success of health programs depends heavily on the active involvement of the community, whether in the form of labor, ideas, or other forms of support. Community participation is necessary to assist the government in achieving its goals of improving health outcomes. Such participation is not merely defined by physical presence but also encompasses the community’s mental and emotional engagement in supporting development activities. Therefore, community involvement needs to be enhanced at various stages of activities, including in the implementation and monitoring of health programs at the local level. A literature review method was employed by searching for scientific articles through the Google Scholar, Garuda, and PubMed databases. Inclusion criteria focused on qualitative and quantitative studies regarding the evaluation of community participation published within the last five years. The selected articles were those relevant to the topic and aligned with the research objectives. An evaluation of community participation policies in the planning, implementation, and monitoring of health programs at the local level indicates that community involvement remains suboptimal. This is due to a lack of knowledge and awareness among the public, limited communication, and a lack of community involvement in health decision-making. During the implementation phase, a participatory approach through health education, outreach, community-based communication, and the empowerment of health volunteers can increase community involvement in health programs. However, during the monitoring phase, the community still does not play a significant role in evaluating health policies.

Abstract

Universal Health Coverage (UHC) is health insurance that ensures that people get justice in promotive, preventive, curative and rehabilitative health services, quality at affordable costs because health services in Indonesia are not evenly distributed, especially in rural areas. The aim of this research is to realize universal access to quality health services in Indonesia. This research uses a literature review method with online searches using scientific databases, such as Pubmed and Google Scholar. Researchers used articles from 2019-2023. The results of this research were obtained from two previous articles, namely regarding the analysis of the principles of financing health services and health services to achieve Universal Health Coverage in Indonesia. So in conclusion, the principle of financing and health services must be carried out fairly in every community, both in urban and rural areas.

Abstract

The National Health Insurance (JKN) programme is a health financing policy designed to achieve universal health coverage by improving access to healthcare services and providing financial protection for the population. It implements a strategic purchasing approach through capitation and INA-CBGs payment mechanisms to promote efficiency and improve the quality of care. This study aims to evaluate JKN financing policies and the implementation of the strategic purchase of healthcare services by conducting a literature review. A literature review was conducted in accordance with PRISMA 2020 guidelines, involving a systematic search across three academic databases. After screening and an eligibility assessment, ten articles met the inclusion criteria. The results reveal that JKN financing has shifted towards a prospective payment system; however, several challenges persist, including discrepancies between tariffs and actual costs, ineffective claims management and limitations in human resources and information system integration. Meanwhile, strengthening information systems, improving human resource competencies and enhancing coordination among stakeholders are recognised as supporting factors. The study concludes that BPJS Kesehatan should periodically adjust INA-CBG tariffs based on case complexity and actual service costs; primary healthcare facilities should strengthen their gatekeeping function through improved capitation incentive design; and hospitals should prioritise diagnostic coding standardisation and internal claims auditing.

Abstract

Nosocomial infections are infections that are acquired or occurred in hospitals and are still a major health problem in the world. Hospitals as complex health units require good human resource management to provide optimal health care and prevent the spread of infections. Nosokomial infections can occur due to a number of factors, one of which is low awareness of the health energy of washing hands. The study aims to find out how high the level of healthcare awareness in Indonesia is in washing hands to prevent nosokomial infections and the factors that influence them. This study uses a literature review method using 11 literature sources from Google Scholar and Garuda. The results of this study are that the majority of health workers in Indonesia have had a good awareness in washing hands with a ratio of 44% – 77.4%. The author suggests to improve and strengthen hand washing behavior for health workers.

Abstract

Although routine health facility accreditation has been implemented in Indonesia, achieving Universal Health Coverage (UHC) remains a significant challenge. In contrast, Thailand stands out as one of the ASEAN countries that has successfully implemented an effective UHC system, despite facing several obstacles. This study aims to explore the strategic role of the Thai government in developing a national health insurance system that may serve as a valuable reference for Indonesia. This research adopts a qualitative approach using literature review and secondary data analysis, with data collected from eight selected journal articles. The findings reveal that the Thai government has effectively strengthened the synergy between participatory and responsive governance in regulating and overseeing health insurance. Public engagement is conducted through annual public consultations and the inclusion of legislators in key bodies such as the Security Council, the Standards and Quality Control Council, and the Social Security Committee. Oversight mechanisms are implemented through regular quality control, public disclosure of reports, periodic financial audits, and by ensuring that the public has easy access to report problems or complaints. Based on these findings, it can be concluded that the strategic role of the Thai government in involving the public in health insurance regulation has not been fully realized in Indonesia. Oversight mechanisms such as financial audit transparency and public access to information on the performance of insurance fund management, as practiced in Thailand, are still lacking in Indonesia. Lessons from Thailand’s approach may provide valuable insights for strengthening oversight and enhancing public involvement in Indonesia's pursuit of UHC.

Abstract

Indonesia continues its efforts to achieve Universal Health Coverage (UHC) through the National Health Insurance (Jaminan Kesehatan Nasional/JKN) program, which faces funding challenges such as financial deficits and reliance on participant contributions. This literature review aims to identify health financing innovations that can support UHC in Indonesia. The study adopts a literature review method, utilizing data sources from Google Scholar covering the period 2016–2025, focusing on relevant studies concerning financing innovations and UHC in Indonesia. The findings indicate that financing innovations, such as increasing excise taxes on unhealthy products and utilizing Corporate Social Responsibility (CSR) funds, hold significant potential. Cross-sector collaboration is essential to expand funding sources and ensure equitable health service delivery. Partnerships between the government, private sector, and communities are crucial for the sustainability of JKN and the achievement of UHC in Indonesia. Further research is recommended to explore the role of non-government sectors in greater depth.

Abstract

Corruption in the health sector undermines efficiency and reduces the quality of public services. This study aims to analyze the current level of transparency and accountability in Indonesia’s health system governance, identify key challenges, and formulate strategies for improvement. A literature review was conducted using five selected scientific articles from national and international databases published between 2016 and 2025. The findings reveal that weak policy implementation, limited cross-sector coordination, and inadequate understanding of accountability frameworks remain significant barriers. Proposed strategies to improve governance include digitalizing procurement through e-Procurement systems and enhancing community participation in monitoring healthcare services. The adoption of technology-based systems is considered effective in promoting transparency, reducing budget inefficiencies, and strengthening public trust. Therefore, sustainable health governance reform must be supported by improving human resource capacity and increasing health literacy at the community level to build a more transparent, accountable, and responsive health system..

Abstract

This study aimed to synthesize the relationship between Public Financial Management (PFM) and Strategic Purchasing in shaping health financing resilience and to develop a conceptual model for strengthening National Health Insurance (JKN) financing in Indonesia. This study used a literature review design with a thematic narrative synthesis approach applied to scientific articles, national official documents, and international policy documents selected through thematic inclusion criteria covering health financing, PFM, strategic purchasing, JKN, and universal health coverage, and analyzed narratively to identify patterns, inter-concept relationships, and policy gaps. The findings indicate that adaptive PFM strengthens health budget planning, allocation, fund disbursement, flexibility, and accountability, while strategic purchasing directs health spending through benefit design, provider contracting, payment methods, and performance and quality monitoring, such that their integration can strengthen allocative efficiency, payment responsiveness, service quality, financial protection, and JKN sustainability. This study concludes that health financing resilience in Indonesia requires the harmonization of fiscal regulation and health policy through the integration of adaptive PFM and effective strategic purchasing as the foundation for a sustainable JKN system.  Keywords: Health Financing Resilience, Indonesia, National Health Insurance, Public Financial Management, Strategic Purchasing, Universal Health Coverage

Abstract

Krisis kesehatan global yang semakin kompleks menuntut sistem kesehatan memiliki kapasitas kesiapsiagaan yang adaptif dan terintegrasi dalam menghadapi berbagai ancaman multi-hazard. Penelitian ini bertujuan untuk mengevaluasi kapasitas kesiapsiagaan krisis dalam kebijakan ketahanan sistem kesehatan di Indonesia melalui pendekatan all-hazards. Metode yang digunakan adalah penelitian kualitatif dengan pendekatan tinjauan pustaka terhadap publikasi ilmiah dan dokumen kebijakan periode 2021–2026 yang diperoleh dari Google Scholar, Garuda, dan PubMed. Sebanyak dua belas literatur dianalisis berdasarkan tujuan, metode, dan temuan utama yang relevan. Hasil kajian menunjukkan bahwa Indonesia telah memiliki kerangka regulasi dan kapasitas awal yang cukup baik dalam kesiapsiagaan krisis, namun masih menghadapi berbagai tantangan, seperti lemahnya koordinasi lintas sektor, kesenjangan kapasitas antar daerah, serta keterbatasan sumber daya. Selain itu, faktor kapasitas komunitas, kolaborasi multi pihak, serta kesiapan fasilitas kesehatan menjadi elemen penting dalam memperkuat ketahanan sistem kesehatan. Ancaman multi-hazard, termasuk perubahan iklim, semakin menuntut sistem yang adaptif dan berbasis teknologi dalam pengambilan keputusan. Kesimpulannya, penguatan ketahanan sistem kesehatan di Indonesia memerlukan integrasi kebijakan, peningkatan koordinasi lintas sektor, pemerataan kapasitas daerah, serta pemanfaatan teknologi untuk mendukung kesiapsiagaan krisis yang lebih efektif dan berkelanjutan.
