Abstract

Introduction: Carpal Tunnel Syndrome (CTS) is a compressive neuropathy, caused by mechanical distortion produced by a compressive force of the median nerve at the level of the wrist. Primary sign is pain in the wrist, tingling sensation, pain or numbness in thumb, index finger, middle finger, and radial side of the ring finger, also there is a reduction of the grip strength and function of the affected hand. This sign tend to be worse at night and clumsiness during the activities that requiring wrist flexion. This syndrome is well-known and frequent accounts for 90% of all entrapment neuropathies. Incidence rates up to 276:100.000 per year. More common in females than in males, its occurrence is commonly bilaterally with a peak age range of 40 to 60 years. This is the productive age, which is often reported that the Carpal Tunnel Syndrome are work-related musculoskeletal disorders caused by strain and repeated movements. Disscussion: Dentist are high risks to go through musculoskeletal disorders covering wrist joint. The condition may happen because of the dentist position while handling patients is not in ergonomic position. These include repetitive prolonged hand activities, forceful static posture of wrist, vibration, and localized mechanical stress. To reduce the symptoms of musculoskeletal disorders since the beginning, has developed an integrated concept of teamwork in a modern dental practice. This concept is known as the four-handed dentistry which consists of dentists and assistants with their respective skills. Conclusion: The four-handed treatment techniques has been developed that is now largely acceptable. The concept of four-handed dentistry is expected to prevent the movement that makes the muscles tense, especially in the area around the wrist. And may ultimately reduce the incidence of Carpal Tunnel Syndrome

Abstract

Introduction: Implementing informed consent in medical practice in Indonesia is fundamental to protecting patients' rights, but it is not quite enough to answer the law’s power. Research This analyzes the legal and ethical aspects of the application of informed consent in medical practice in Indonesia, with a review of applicable laws and regulations and principles of the underlying bioethics.Methods: This study used a normative juridical and qualitative descriptive approach to analyze legal and ethical aspects of informed consent in medical practice. Data were obtained from 15 secondary sources, including laws, academic literature, and court decisions, and analyzed qualitatively. Source triangulation linking legal, health, and bioethical perspectives was applied to ensure validity. The study aims to contribute theoretically and practically to improving informed consent policy and practice.Results: Research results show that although the regulation related to informed consent is sufficiently comprehensive, there are still challenges in its implementation, such as a lack of understanding of the patient, limitations in the time doctors have to give adequate information, and aspects of culture that influence the decision-making process in medical care.Conclusion: Improvement in socialization, education for power medical and patient, and strengthening of the regulations are required to ensure informed consent can be obtained and applied effectively and fairly in medical practice in Indonesia.Keywords: word; another word; lower case except names

Abstract

Background: Regular exercise has been postulated as beneficial to prevent, treat, and manage a chronic disease that nowadays is caused by a sedentary life. However, its prescription and mechanism of how exercise positively affects health conditions is requiring further research. Objective: To investigate the types of regular exercise and exercise prescription and mechanisms in preventing or treating a chronic disease. Methods: This study reviewed articles from ProQuest, Science Direct, Wiley, and PubMed using the terms 'chronic disease', 'exercise', 'physical activity', and 'cytokine'. Results: Forty-six articles were reviewed in this study which originated from different regions spanning Asia, Europe, America, and Africa. The articles were categorized based on sample characteristics, which were; elderly, healthy adults, athletes, mice, and others. Each article describes an aerobic or anaerobic exercise performed under various interventions ranging from low, moderate, to high and comprehensively explains its effect on human biology, including on the body system, tissue adaptation, muscle hypertrophy, and neuroplasticity, by examining its effect on proteins, hormones, enzymes, microRNAs, as well as functional metrics pertaining to physical capacity enhancement. Conclusion: This review concludes that exercise will elicit effects on the immune and metabolic systems of people with chronic disease.

Abstract

Limited hamstring flexibility and decreased knee extension range of motion (ROM) are risk factors for musculoskeletal injuries in recreational female runners. This study aimed to compare the effectiveness of proprioceptive neuromuscular facilitation (PNF) with nerve glide in improving hamstring flexibility and knee extension ROM. This quasi-experimental pre-test–post-test study involved 20 female runners from the Solo Runners community, who were randomly assigned to two groups (PNF and nerve glide, n=10 each). Both groups underwent 12 intervention sessions over 4 weeks (3 times/week). Flexibility was measured using the Sit and Reach Test (SRT), while knee extension ROM was measured using the Active Knee Extension (AKE) test. Data were analyzed using the Wilcoxon test, paired t-test, and Mann–Whitney U test. Both interventions significantly improved hamstring flexibility (PNF Δ=+7.1 cm, p=0.004; nerve glide Δ=+5.4 cm, p=0.004) and knee extension ROM (PNF Δ=−13.2°, p<0.001; nerve glide Δ=−10.1°, p=0.005). The improvement in flexibility was greater in the PNF group than in the nerve glide group (p=0.001), whereas the between-group difference in Δ ROM was not statistically significant (p=0.057). PNF is more effective than nerve glide in improving hamstring flexibility, whereas both techniques yield similar results in improving knee extension ROM. The selection of technique should be tailored to the primary source of movement limitation, whether muscle-dominant or nerve-related.

Abstract

The public is increasingly aware of the importance of exercise for the elderly because exercise is an option to reduce complaints due to setbacks and improve the health of the elderly. In fact the implementation in the field is often wrong as in Banjar Benaya which implements routine exercises only once a week with a duration less than 30 minutes, does not pay attention to nutritional needs, does not use sports clothing, and displays that are not clear. The measurement of elderly physical fitness is still relatively low. Based on these problems, improvements were made to the application of ergonomic elderly gymnastics due to improve elderly’s physical fitness and increase the bone mass.  This study used a treatment by subject design conducted in April 2017-August 2018. The target population was all elderly in Denpasar and the affordable population was determined by multistage random sampling then selected Banjaya Benaya Peguyangan Village with a sample of 20 people who met the inclusion criteria. The data collection were physical fitness and bone mass. The difference in treatment effects were analyzed using a Paired Sample t-Test with α = 0.05 for data with normal distribution and Wilcoxon test, α = 0.05 for data with abnormal distribution. Data showed that there were differences in physical fitness improvement in Period I and Period II after 8 weeks of gymnastics at 43.43% with very bad categories being bad, the difference in increase in whole body bone mass was 16.76% and leg bone mass 68.67%. Analysis of Paired Sample-t-Test physical fitness data and Wilcoxon test for bone mass data in Period I and Period II after gymnastics for 8 weeks, found that there was a significant difference (p <0.05). It can be concluded that ergonomic elderly gym can significantly improve physical fitness and increase bone mass of the elderly. It is recommended that the elderly continue to exercise with a duration of 30-45 minutes, the frequency of 3 times a week to maintain elderly’s physical fitness.

Abstract

Overtime work condition can create work related fatigue caused by stress. Work condition im- provement could decrease quality of fatigue of the employees. A total ergonomic approach (TEA) was conducted to decrease the quality of fatigue of metal crafters at Kediri Tabanan. Samples (N = 23) were chosen randomly from the population. Fatigue was measured through a modification of 30 items questionnaire from the Industrial Fatigue Research Committee. Results show a 9.22% (p < 0.05) decrease in fatigue, revealing that decrease of motivational fatigue is higher than decrease in fatigue due to physical activity, due to the type of work: manual, repetitive, and continuous. It was suggested to use TEA continuously to decrease the fatigue and maintain motivation. Kondisi kerja lembur dapat menimbulkan kelelahan sebagai salah satu akibat stres. Perbaikan kondisi kerja diharapkan menurunkan kualitas kelelahan karyawan. Sebuah pendekatan ergonomi total telah dilaksanakan untuk mengetahui perubahan kualitas kelelahan perajin logam di Kediri Tabanan. Sampel sejumlah 23 orang dipilih secara acak dari populasi. Kelelahan diukur dengan modifikasi 30 butir kuesioner model Industrial Fatigue Research Committee. Hasil-hasil menunjukkan penurunan kelelahan sebanyak 9.22% (p < 0.05) dan menunjukkan bahwa penurunan kelelahan motivasi lebih banyak daripada kelelahan aktivitas fisik, mengingat jenis pekerjaan yang manual, repetitif, dan berkelanjutan. Disarankan pendekatan ergonomi total diteruskan untuk menurunkan kelelahan dan mempertahankan motivasi kerja karyawan.
