Abstract

ABSTRACT. Background. Hypospadias is one of the most common urogenital anomaly in males. The etiology is poorly understood, but is believed to be multifactorial where Intra Uterine Growth Restriction (IUGR) is one of the suspected risk factors, leading to low birth weight and low birth length in affected patients. Objective. This study aims to identify the birth anthropometry characteristics of hypospadias patients who visited RSUP. Dr. Mohammad Hoesin Palembang. Materials and Methods. This was an observational descriptive study with cross sectional design. This study included hypospadias patients with detailed birth characteristics in RSUP Dr. Mohammad Hoesin Palembang during the period of 2012-2015. Results. Out of 106 hypospadias patients, only 19 (17.92%) of them presented with detailed birth characteristics. There were 12 (63,2%) patients with low birth weight and 7 (36,8%) patients with normal birth weight. There were 10 (52,6%) patients with low birth length and 9 (47,4%) patients with normal birth length. The mean birth weight is 2442 grams and the mean birth length is 46,9 centimeters. Conclusion. The majority of hypospadias patients in this study showed a low birth weight and low birth length, signalling an accordance to the existing theory. Abstract

ABSTRACT Background. Disorders of Sex Development (DSD) is a condition where the development of sex chromosomes, gonads, and/or one’s anatomy is atypical. Its causes are often due to genetic mutations, although some are also linked to environmental risk factors. These multiple aetiologies lead to varied clinical findings, ranging from obvious ambiguous genitals to subtle ones in different regions worldwide, signalling a hint of geographical variability. Objective. This study wishes to observe the variations of clinical findings of DSD patients geographically in South Sumatera. Methods. This was an observational study using patients’ medical records in RSUP Dr. Mohammad Hoesin Palembang. Both inpatients and outpatients during five-year period span (2013-2017) with clinical findings suited DSD criteria based on Chicago Consensus in 2006 were included in this study. Results. One hundred and forty nine patients from cities and regencies in South Sumatera province and other provinces like Jambi, Lampung, Bengkulu, Bangka-Belitung, and even Riau were included in this study. Among sixteen clinical findings identified, hypospadias ranked first (59.06%), both in general, and in each regions as well. When set by side with other regions, Palembang city as the capital city of South Sumatera province displays twelve out of sixteen clinical findings documented in this study, showing a lot more variety. Conclusion. Every regions show difference clinical findings. Some regions housed clinical findings that were not found in other regions. However, hypospadias is the most commonly found clinical findings in all regions. It is suspected due to its correlation with certain environmental risks, that the occurence of it becomes rather often, compared to other DSD conditions. Future studies considering risk factors involvement in order to elucidate both differences and similarities found in each regions are strongly suggested. Abstract

ABSTRACT Background. Trinucleotide Repeat Expansion (TRE) in human DNA could lead to various diseases. An expanded CAG repeat (>31 or 37 repeats, depends on the ethnicity) in Androgen Receptor gene is suggested to be associated with the occurrence of isolated hypospadias. In an effort to identify the exact numbers of repeats, sequencing has been the most favored method to be conducted despite its cost. Objective. This study wished to investigate the possibilities of using Polymerase Chain Reaction (PCR) method to screen expanded repeats in isolated hypospadias, as one of the TRE diseases. Materials and Methods. Numbers of CAG repeat in twelve hypospadias patients and one normal male was first predicted from the visualization of PCR products in 3% agarose gel electrophoreses with 20 bp ladder marker before it was finally sequenced. Results. Two samples gave the same exact result, while the rest showed a range of 1-11 bp differences. Statistically, there was a significant difference between the mean of CAG repeats from PCR method (M=26.1667, SD=6.71272) and the mean of CAG repeats from sequencing (M=23.75, SD=5.70685); t(11)= 4.570, p=0.001. Furthermore, the sensitivity of PCR was 100% and the specificity was 83.33%. Conclusion. It can be concluded that PCR method could be used as a screening method in identifying TRE with large numbers of repeats. However, PCR in TRE disease with small numbers of expanded repeats needs to be followed by sequencing in order to obtain the exact numbers of repeats. Abstract

Background. Autism Spectrum Disorder (ASD) is a complex neurodevelopmental condition characterized with disorders in cognitive, behaviour, and social interaction. It is estimated to affect 1:160 children worlwide. Despite its increasing trend, the exact cause of it is yet to be known. Objective. This study wished to identify the geographical distribution of ASD cases in Dr. Mohammad Hoesin Hospital as one of the national reference hospitals in Indonesia for South Sumatera region. Methods. Medical records of the patients who are diagnosed with ASD during 2014-2018 was observed in order to identify the patients’ residential area. Results. There were 107 patients who were clinically diagnosed of having ASD during 2014-2018 in Dr Mohammad Hoesin Hospital, Palembang. Based on its residential area, ASD cases were mostly found in Palembang as the capital city of South Sumatera Province. Conclusion. ASD is found in small and big cities across the South Sumatera province, implying its common occurrence. The varied prevalence could be signifying the role of different related risk factors identified in the area.

Abstract

Abstract Introduction. Congenital anomaly is one of the leading causes of neonatal death. It is the third leading cause of neonatal death in Indonesia. More than half of all congenital anomalies can’t be related to a specific cause, suggesting multiple risk factors. This study aims to identify patterns and risk factors of congenital anomalies in RSUP Dr. Mohammad Hoesin Palembang. Methods. This observational descriptive study with a cross-sectional design used 100 patients’ medical records, taken at RSUP Dr. Mohammad Hoesin Palembang, as samples. Missing data were complemented with a phone interview. Samples were picked using a proportional random sampling method. Collected data were counted within each category of congenital anomalies. Results. There were 366 neonatal patients diagnosed with congenital anomalies at RSUP Dr. Mohammad Hoesin Palembang in 2015. The most prevalent (44,8%) congenital anomalies belonged to congenital anomalies of the digestive system, followed by congenital anomalies of the circulatory system (18,6%), and cleft lip/palate (11,5%). Following risk factors were found: the history of maternal active or passive smoking (41%), maternal exposure to chemicals or solvents (31%), family member with congenital anomalies of the same category (27%), maternal drug consumption (26%), maternal infection (22%), maternal diabetes mellitus (4%), and maternal hypertension (4%). Conclusion. History of maternal active or passive smoking was the most frequently found risk factors among patients of congenital anomalies.

Abstract

Background: Endometriosis implant has been known to express aromatase enzyme, Cytochrome p450 that catalyzes androgen into estrogen. It causes local estrogen production, leading to increased estrogen level, and subsequently triggers endometriosis lesion. CYP19 gene resided at chromosome 15q21.1 is the biggest component of aromatase coding cytochrome p450 enzyme. Objective: To identify relationship between aromatase gene polymorphism CYP19 Val 80 and the risk of endometriosis. Methods: This is an observational case-control study using frozen DNA sample from women with endometriosis and/ or adenomyosis who had undergone laparotomy/ laparoscopy at Obstetrics and Gynecology Department Dr. Mohammad Hoesin General Hospital Palembang January-November 2013. Samples were amplified and cut by PCR-FRLP using Rsa1 restriction enzyme. Results were divided into A/A genotype (homozygote mutant), G/A (heterozygote mutant), and G/G (homozygote wild type). Data were analyzed by SPSS 21.0 version. Results: PCR-RFLP results for A/A genotype were 20 (21.3%) in endometriosis group and 8 (8.5%) in control group. G/A genotype were 18 (19.1%) in endometriosis group and 22 (23.4%) in control group. G/G genotype were 9 (9.6%) and 17 (18.1%) in endometriosis group and control group, respectively. There was significant increase risk of endometriosis in women carrying genotype A/A to those with genotype G/G with OR 4.722 (p<0.05). Conclusion: Polymorphism on aromatase gene CYP19 Val 80 A/A increases risk of endometriosis.

Abstract

Introduction. Sepsis is a group of symptoms of organ dysfunction that can be life-threatening because of dysregulation of body response toward ongoing infection. Organ dysfunction in sepsis can be measured by Sequential Organ Failure Assessment (SOFA) and T3 hormone. The study was aimed to identify the correlation of T3 in predicting mortality of 28 days patients in Intensive Care Unit RSMH Palembang. Method. This study design is cohort prospective. The inclusion criteria consist of a patient diagnosed with sepsis and septic shock in the Intensive Care Unit, 18-64 years old. Patients with a history of thyroid disease, pregnant or post-pregnancy, the patient admitted in referral from other hospitals, and patients with a history of psychiatry medication and thyroid medication were excluded. Data collected is the patient whose stay in Intensive Care Unit RSMH followed in 28 days from January 2021 until the sample was fulfilled (39 samples). Analyzing data was SPSS version 23 with chi-square analysis and Fisher's Exact to identify the relationship. Pearson correlation to identify correlation coefficient, and Medical application to measure AUC, cutoff value, sensitivity, and specificity. Result. The result showed that age (p=0,445). gender (p=1,00), need of ICU (p=0,228), isolation-nonisolation ward (p=0,437) didn't have any significant relationship toward mortality. SOFA score correlate statistically with positive correlation and medium strength (0,633) toward mortality of sepsis patient (p=0,000). T3 hormon correlate positively with medium strength (0,514) toward mortality of sepsis patient (p=0,001). T3 hormone toward SOFA correlate negatively (-0,365) with significant correlation (p=0,22). T3 hormone has AUC 0,291 with sensitivity 3,3% and specificity 67,7%. Conclusion. T3 hormone has a significant negative correlation to mortality in sepsis patients but cannot be used to predict mortality with a low AUC value (0,291).

Abstract

One thing that differentiates one person from another is one’s genetic make-up. Genetic plays a role in every branch of medicine, including anesthesiology. An anesthesiologist must be well familiarized with hereditary (genetic) conditions, chromosomal traits, heredity-familial disorders, and even recessive variants because particular conditions might demand a different anesthetic and perioperative pharmacological management. These circumstances may lead to an opening of a rapidly expanding state of pharmacogenetics/genomics and its relevancy in anesthesia nowadays. This narrative review provides insight into the role of genetics in the field of anesthesiology.Keywords: genetics, anesthesiology, pharmacogenetics

Abstract

The ability to make precise adjustments to the human genome has been a goal of healing in which gene also introduces as the fundamental unit of heredity, in biomolecular technology in genetic diseases have opened new knowledge such as gene therapy. Gene therapy is a technique to repair DNA where its usage is to treat the malignancy and inherited genetic diseases. Gene therapy is a choice to the genetic cloth that goals to remedy a sickness this is hard to deal with or perhaps has no treatment. Currently, gene remedy is done in approaches to patients, specifically embryonic cells and somatic cells, every in vivo and ex vivo. Moral considerations with modification of the difficulty's cells and oversight of regulation and reagents want to be taken into consideration within the gene therapy project. Applications for using gene remedies have begun to be widely used, which include in case of maximum cancers, coronary heart disorder, infectious sicknesses, and others. Gene therapy has spread to a wide range of applications then go beyond the modification of genetic disorders. Advances in genetic modification of cancer cells and immunity and the use of viruses and bacteria to control cancer cells have resulted in many clinical trials and product developments for cancer treatment. The miracles and blessings of gene therapy are might believe, but even though they are being studied and developed now and, in the future, so that the desire for gene therapy may be even better future.Keywords: gene therapy, genetic recombination, gene therapy application

Abstract

Background: Biology is one of the cornerstones of studying medicine. It is imperative for medical students to get a grasp of it before further proceed to advanced medical sciences. Learning and comprehending the subject will help medical students further in their studies. The learning environment is one of the major factors that determine the success of a learning process. This study aimed to assess student’s perception of the learning environment on the biology medicine block, as one of the basic medical sciences blocks given in the first semester in the Faculty of Medicine, Universitas Sriwijaya.Methods: A descriptive observational study with a cross-sectional design on 221 second-year medical students that have undergone the biology medicine block. Assessment is done using Dundee Ready Education Environment Measure (DREEM) questionnaire.Results: A total of 221 respondents, 67.4% have a good perception of learning, a very good perception of lecturers 115 respondents (52%), a good academic achievement of 62%, a perception of a good learning atmosphere is 72% and a perception of a good environment is 73.8%.Conclusion: The majority of respondents stated good perception among all categories.
