Abstract

Hereditary breast cancer, primarily driven by BRCA1 and BRCA2 mutations, presents distinct challenges and prognostic factors compared to sporadic breast cancer. BRCA1-associated breast cancer is often triple-negative (TNBC), which has a more aggressive course and poorer prognosis. Despite this, BRCA mutation carriers exhibit higher sensitivity to platinum-based chemotherapy and PARP inhibitors, potentially improving outcomes. However, the risk of developing other malignancies, such as ovarian cancer and melanoma, remains elevated in BRCA mutation carriers. Studies show a significant variation in survival rates, with BRCA mutation carriers having lower overall survival compared to non-carriers. Loss of heterozygosity (LOH) in BRCA1/2 tumors is frequent but does not significantly alter overall survival rates. Identifying the presence of LOH can guide personalized treatment strategies, particularly the use of PARP inhibitors. The response to chemotherapy, especially platinum-based drugs, is influenced by genetic mutations such as TP53 and PTEN, which are common in TNBC. Surgical choices also impact prognosis; mastectomy may lower ipsilateral breast recurrence but does not affect overall survival. Pathologic complete response (pCR) following neoadjuvant chemotherapy is a critical prognostic marker, with higher rates observed in BRCA mutation carriers, particularly those with TNBC. These factors collectively influence the prognosis and guide treatment strategies for hereditary breast cancer.

Abstract

Luka terjadi ketika integritas jaringan terganggu akibat cedera atau pembedahan dan dapat dikelompokkan berdasarkan struktur, proses penyembuhan, dan waktu sembuh. Saat kulit terluka, tubuh merespons dengan menghentikan pendarahan melalui vasokonstriksi dan pembentukan gumpalan darah. Bunga telang merupakan tanaman obat tradisional yang dapat meningkatkan kesehatan kulit dan mendukung regenerasi jaringan berkat kandungan terpenoid sebagai komponen bioaktif. Terpenoid merupakan senyawa organik dengan berbagai fungsi yang berkontribusi signifikan terhadap khasiat penyembuhan. Senyawa ini memiliki berbagai efek biologis, seperti efek anti-inflamasi, antioksidan, dan antibakteri yang sangat penting untuk penyembuhan luka. Tinjaun literatur pada penelitian ini dengan menggunakan protokol PRISMA-ScR (Scoping Review). Ulasan dilakukan pada artikel yang membahas tentang pengaruh terpenoid dalam proses penyembuhan luka. Pencarian literatur menggunakan database PubMed, ScienceDirect, Google Scholar dan MDPI (Multidisciplinary Digital Publishing Institute).Artikel dalam bahasa Inggris dan Indonesia dengan rentang waktu sepuluh tahun terakhir. Didapatkan hasil empat artikel yang sesuai dengan kriteria penelitian. Simpulan penelitian ini adalah terpenoid dapat mempercepat proses penyembuhan luka dengan mendorong angiogenesis yang membantu aliran oksigen dan nutrisi ke area yang terluka. Selain mengurangi peradangan, terpenoid juga mendukung metabolisme sel yang terlibat dalam penyembuhan luka.The Role of Terpenoids in Butterfly Pea (Clitoria ternatea L.) in the Wound Healing Process: A Literature ReviewAbstractWounds occur when the integrity of the tissue is compromised due to injury or surgery and can be grouped based on structure, healing process, and healing time. When the skin is injured, the body responds by stopping bleeding through vasoconstriction and the formation of blood clots. Telang flower is a traditional medicinal plant that can improve skin health and support tissue regeneration thanks to the content of terpenoids as bioactive components. Terpenoids are organic compounds with various functions that contribute significantly to healing properties. These compounds have various biological effects, such as anti-inflammatory, antioxidant, and antibacterial effects essential for wound healing. The literature review in this study uses the PRISMA-ScR (Scoping Review) protocol. The review was conducted in an article that discusses the influence of terpenoids in the wound-healing process. A literature search was conducted, and articles published in PubMed, ScienceDirect, Google Scholar, and MDPI (Multidisciplinary Digital Publishing Institute) were used. Articles in English and Indonesian have been published for the last ten years. The results of four articles were obtained following the research criteria. This study concludes that terpenoids can speed up the wound-healing process by promoting angiogenesis, which helps flow oxygen and nutrients to the injured area. In addition to reducing inflammation, terpenoids also support the metabolism of cells involved in wound healing.

Abstract

Learning Enrichment to increase knowledge about ambiguous genitalia in students at STIKES Abdurrahman Palembang. Disorders of Sex Development (DSD) is a rare disease in which atypical gonads, chromosomes, and sex anatomy are found. Although it has a significant impact on both the physical and mental health of individuals with DSD, DSD cases are often undetected and underdiagnosed. Efforts to enrich material and learning to training on DSD for prospective midwife health workers are one of the real actions that can be taken to increase knowledge and awareness of DSD which is then expected to increase early identification efforts to improve the quality of life and patient outcomes. This community service activity was carried out for one semester through a series of activities in the form of guest lectures for enrichment of learning which were carried out in stages three times online for 55 students of STIKES Abdurrahman Palembang. Measurement of the level of knowledge of STIKES Abdurrahman students was carried out twice, namely before the series of activities began (pre-test) and after the activities were completed (post-test). From the results of the pre- and post-test, an increase in knowledge was obtained, which was originally only 36.4% with a sufficient level of knowledge at the time of the pre-test, to 69.1% with a sufficient level of knowledge at the time of the post-test. It is hoped that with the enrichment of learning, prospective midwives can know and understand DSD so that in the future they can improve early screening for ambiguous genitalia cases which can lead to improving the quality of life of sufferers.

Abstract

Introduction. Disorders of Sex Development (DSD) is a congenital disorder in which there is atypical chromosomal, gonadal, or anatomical sex development. Children with DSD are often brought to the hospital too late for care due to limited knowledge about this condition among medical practitioners. Early diagnosis is important to improve prognosis and minimize complications and comorbidities of DSD patients. Methods. This study is a descriptive observational study with a cross-sectional approach through interviews and data collection from medical records of Dr. Mohammad Hoesin Palembang Hospital in 2020 - 2023. Results. In this study, it was found that the infancy age group (28 days - 1 year) was the most common age of arrival of patients to the hospital (29%) and the most common main complaint was clitoral enlargement (19.4%). Conclusion. This study found a wide variation in patients’ chief complaints, which makes the age of arrival of DSD patients to the hospital still quite late.

Abstract

Down Syndrome is one of the most commonly found genetic conditions, a chromosomal abnormality that is. It is caused by an extra chromosome 21 in their body. The extra chromosome itself can be a total chromosome 21, or just part of it. Whatever the form is, it leads to distinct physical features along with associated medical conditions. One of the symptoms of people with Down Syndrome is the cognitive problem where all of them would have various levels of intelligence, making the learning and comprehension process challenging. For Muslims all around the world, Ramadhan is the one special month of the year where everyone would fast. Learning about Ramadhan and conducting fasting during Ramadhan could be challenging for children with Down Syndrome. There is no known absolute contraindication for them to fast, but several comorbidities related to the condition might hinder them from doing fasting.

Abstract

The COVID-19 pandemic has changed many things in the medical education system. This change requires adaptation and modification from education providers, teachers, and students. A virtual learning system has been implemented instead of a face-to-face learning system. However, this makes several challenges and issues that must be considered. Virtual learning is considered less attractive and interactive and not effective in teaching clinical skills. For student clerks, the reduced time to undergo education in the hospital leads to fewer case exposures. Another problem is the facilities and infrastructure. Several ways to solve this problem have been implemented while minimizing risk and taking strict precautions. Some institutions modify and innovate to maximize the effectiveness of learning activities. In addition to technical and effectiveness issues, changes in the education system also have an impact on the mental health of medical students, therefore it is important for institutions to be sensitive to changes and risks to students' mental health during the pandemic.

Abstract

Background: Morphometry is a quantitative approach to finding information about variations in the shape of organisms. The maxilla has a central position and plays a vital role in supporting the viscerocranium. Morphometric variations are beneficial in reconstructive procedures and are an essential reference in plastic surgery. However, this study is currently limited. This study aims to determine variations in the maxillary morphometry of patients at Mohammad Hoesin General Hospital that may become a reference for maxillary reconstruction. Method: This descriptive observational study with a cross-sectional approach aims to identify variations in maxillary morphometry in patients treated at RSUP Mohammad Hoesin Palembang. The study is directed at describing a community or society to create an objective picture or description of a condition. It continues by analyzing and comparing maxillary morphometry based on gender. Results: In this study, the results were obtained from as many as 96 samples (male, n = 60, female, n = 36). Variable of male and female were assessed, namely intercanine width (3.58 ± 0.27cm; 3.44 ± 0.18cm), intermolar width (5.64 ± 0.43cm; 5.47 ± 0.27cm), arch length (3.31 ± 0.38cm; 3.32 ± 0.31cm, and palate height (2.46 ± 0.28cm; 2.35 ± 0.22cm) significant results were obtained that the dimensions of the male maxilla were more significant than those of females. Conclusion: There are variations in maxillary morphometry based on gender. Males have larger inter canine width, intermolar width, and palate height morphometry than females.

Abstract

Introduction: Mitochondrial Disorders (1/4,000 patients) are rare and caused by dysfunctional mitochondria. Anesthetic consideration in patients with Mitochondrial Disorders involves careful preoperative and perioperative observations. Objective: To provide a brief insight into how general anesthetics interfere with mitochondrial energy formation pathways and help form precautions for anesthesiologists when managing patients with Mitochondrial Disorder. Review: Mitochondrial Disorder patients would experience various health problems, such as damaged cardiac functions, neurology systems, and musculoskeletal functions due to energy production disruptions by dysfunctional mitochondrial processes. Moreover, patients with Mitochondrial Disorders exhibit hyperreactivity to volatile anesthetics. Conclusion: No anesthetic strategies are found to be safe in patients with Mitochondrial Disorder yet. Therefore, anesthesiologists should remain alert when monitoring fluid choices and managing patient temperature with Mitochondrial Disorders.

Abstract

Kanker payudara merupakan kanker yang paling banyak dijumpai dan menjadi penyebab utama kematian akibat kanker pada wanita. Berbagai fitokimia dari bahan alam berpotensi memberikan efek terapeutik terhadap kanker payudara, beberapa diantaranya adalah genistein, quercetin, dan resveratrol. Penelitian ini bertujuan untuk mengetahui interaksi senyawa bioaktif genistein, quercetin dan resveratrol terhadap reseptor brca1, era dan egfrpada kanker payudara secara in silico dengan molecular docking. Molecular docking dilakukan melalui beberapa tahapan mulai dari preparasi struktur 3D senyawa bioaktif, preparasi protein target, validasi metode molecular docking, dan docking senyawa bioaktif pada protein target. Semakin rendah nilai energi ikatan antara senyawa uji dengan protein target maka ikatan yang terbentuk akan semakin kuat dan stabil. Hasil docking menunjukkan nilai energi pengikatan kompleks genistein

Abstract

Pandemi COVID-19 yang telah berlangsung selama dua tahun terakhir menyebabkan disrupsi layanan kesehatan di berbagai bidang kesehatan tanpa terkecuali termasuk layanan kesehatan jiwa di seluruh negara di dunia. Tinjauan pustaka ini bertujuan untuk mengeksplorasi potensi media sosial sebagai media promosi kesehatan jiwa di masa pandemi COVID-19. Penelusuran literatur dilakukan di 2 situs pencari (Google Scholar dan PubMed) yang memenuhi kriteria inklusi dan ekslusi. Dari hasil penelurusuran diketahui bahwa media sosial sering dijadikan sebagai sarana untuk berbagi informasi dengan banyak orang lainnya. Aksesnya yang relatif mudah menjadikan media sosial sebagai platform berbagi yang cukup efektif dengan jangkauan yang luas. Salah satu platform media sosial yang banyak digunakan oleh masyarakat dari berbagai latar belakang dan usia adalah Instagram. Penggunaan Instagram sebagai salah satu media edukasi atau penyuluhan kesehatan yang murah, dan mudah untuk diakses masyarakat luas merupakan salah satu opsi yang efektif dalam upaya promosi kesehatan di masa pandemi. Berbagai studi menunjukkan bahwa penggunaan media sosial berdampak positif terhadap upaya promosi kesehatan termasuk promosi kesehatan jiwa. Berdasarkan hasil survey WHO di akhir tahun 2021, didapatkan masih banyak masyarakat yang tidak dapat mengakses layanan kesehatan baik di tingkat perawatan primer maupun lanjut di era pandemi COVID-19. Situasi ini dapat berdampak pada information flow di dunia kedokteran dan kesehatan di mana dikhawatirkan dapat menyebabkan tersebarnya informasi kesehatan yang menyesatkan (misleading atau false information/hoax). Karena keterbatasan interaksi sosial akibat situasi pandemi, salah salah satu metode yang dapat digunakan untuk melaksanakan promosi kesehatan sekaligus penyuluhan/edukasi tentang berbagai topik kesehatan yang dapat diakses oleh masyarakat luas adalah dengan menggunakan media sosial.
