Abstract

Abstract Introduction. Congenital anomaly is one of the leading causes of neonatal death. It is the third leading cause of neonatal death in Indonesia. More than half of all congenital anomalies can’t be related to a specific cause, suggesting multiple risk factors. This study aims to identify patterns and risk factors of congenital anomalies in RSUP Dr. Mohammad Hoesin Palembang. Methods. This observational descriptive study with a cross-sectional design used 100 patients’ medical records, taken at RSUP Dr. Mohammad Hoesin Palembang, as samples. Missing data were complemented with a phone interview. Samples were picked using a proportional random sampling method. Collected data were counted within each category of congenital anomalies. Results. There were 366 neonatal patients diagnosed with congenital anomalies at RSUP Dr. Mohammad Hoesin Palembang in 2015. The most prevalent (44,8%) congenital anomalies belonged to congenital anomalies of the digestive system, followed by congenital anomalies of the circulatory system (18,6%), and cleft lip/palate (11,5%). Following risk factors were found: the history of maternal active or passive smoking (41%), maternal exposure to chemicals or solvents (31%), family member with congenital anomalies of the same category (27%), maternal drug consumption (26%), maternal infection (22%), maternal diabetes mellitus (4%), and maternal hypertension (4%). Conclusion. History of maternal active or passive smoking was the most frequently found risk factors among patients of congenital anomalies.

Abstract

Background: Endometriosis implant has been known to express aromatase enzyme, Cytochrome p450 that catalyzes androgen into estrogen. It causes local estrogen production, leading to increased estrogen level, and subsequently triggers endometriosis lesion. CYP19 gene resided at chromosome 15q21.1 is the biggest component of aromatase coding cytochrome p450 enzyme. Objective: To identify relationship between aromatase gene polymorphism CYP19 Val 80 and the risk of endometriosis. Methods: This is an observational case-control study using frozen DNA sample from women with endometriosis and/ or adenomyosis who had undergone laparotomy/ laparoscopy at Obstetrics and Gynecology Department Dr. Mohammad Hoesin General Hospital Palembang January-November 2013. Samples were amplified and cut by PCR-FRLP using Rsa1 restriction enzyme. Results were divided into A/A genotype (homozygote mutant), G/A (heterozygote mutant), and G/G (homozygote wild type). Data were analyzed by SPSS 21.0 version. Results: PCR-RFLP results for A/A genotype were 20 (21.3%) in endometriosis group and 8 (8.5%) in control group. G/A genotype were 18 (19.1%) in endometriosis group and 22 (23.4%) in control group. G/G genotype were 9 (9.6%) and 17 (18.1%) in endometriosis group and control group, respectively. There was significant increase risk of endometriosis in women carrying genotype A/A to those with genotype G/G with OR 4.722 (p<0.05). Conclusion: Polymorphism on aromatase gene CYP19 Val 80 A/A increases risk of endometriosis.

Abstract

COVID-19 is a respiratory tract infection caused by the novel coronavirus (nCov). Nasal irrigation is one of the efforts to prevent COVID-19. Nasal irrigation is an act of flushing saline solution into the nasal cavity. This research aims to determine level of knowledge, attitudes, and practices of the community regarding nasal irrigation during the COVID-19 pandemic. This study was a descriptive observational study. The sample of this research was the people of Palembang who were eligible for the inclusion and exclusion criteria with the consecutive sampling technique as many as 398 respondents. Data were obtained through primary data in the form of questionnaires, processed using the SPSS program, and analyzed using univariate analysis. The majority of respondents were aged 18-25 years old (65.5%) and had senior high school 58.0%. The level of public knowledge about nasal irrigation during the COVID-19 pandemic was in the moderate category of 36.2%. The level of public attitudes regarding nasal irrigation during the COVID-19 pandemic was in the good category at 83.4%. The level of nasal irrigation practice was included in the bad category of 75.6%. In conclusion, the community of Palembang had moderate level of knowledge, a good level of attitude, and a bad level of practice regarding nasal irrigation during COVID-19 pandemic.

Abstract

Latar belakang. Penyakit ginjal kronis merupakan masalah kesehatan yang merugikan pada anak dengan angka kesakitan dan kematian yang terus meningkat. Continous ambulatory peritoneal dialysis (CAPD)sebagai pilihan terapi pengganti ginjal memberikan manfaat yang lebih baik dibandingkan hemodialisis konvensional pada anak dengan penyakit ginjal terminal. Namun, efektivitas CAPD dalam jangka panjang perlu diantisipasi karena risiko komplikasi infeksi dan non-infeksi.Tujuan. Mengetahui hubungan faktor risiko dengan komplikasi CAPD pada pasien anak di Rumah Sakit Mohammad Hoesin.Metode. Studi cross-sectional ini dilakukan di RSMH. Sampel penelitian adalah pasien anak dengan CAPD pada bulan Juni (2016-2021) dengan rekam medis lengkap. Sampel penelitian dianalisis secara univariat dan bivariat. Hubungan antara faktor risiko dan komplikasi dianalisis menggunakan uji Chi-square.Hasil. Subjek berjumlah 14 orang berjenis kelamin laki-laki, usia>5 tahun, status gizi normal (42,9%), memiliki kelainan anatomi ginjal (57,1%), dan pembedahan secara laparoskopi (71,4%). Komplikasi infeksi terbanyak adalah peritonitis (50%), Komplikasi non infeksi adalah drainase yang buruk (42,9%). Tidak terdapat hubungan yang bermakna antara faktor risiko terhadap komplikasi pada pasien anak yang mengalami CAPD di RSMH (p > 0,05).Kesimpulan. Secara deskriptif setiap faktor berperan terhadap kejadian komplikasi. Namun, tidak ada hubungan yang bermakna antara faktor risiko pada komplikasi infeksi dan non infeksi.

Abstract

Background: Autism Spectrum Disorder (ASD) is a group of nervous system development disorders with polygenic inheritance patterns, characterized by a type of dysfunctional in social communication and limited also repetitive behaviors. Risk factors for ASD can be divided into two categories in general: genetic, and environmental factors. To date, a study about risk factors of ASD in Indonesia, let alone Palembang, is limited. Therefore, this study wished to investigate the risk factors of children with ASD in Dr. Mohammad Hoesin General Hospital, Palembang. Methods: This study is an observational descriptive study. Samples were children with ASD who went to Dr. Mohammad Hoesin General Hospital, Palembang. The primary data obtained from a semi-structured interview with parents/guardians of children with ASD, while secondary data obtained from their medical records. Results: The most common risk factors identified in this study are the paternal age and maternal age ≥ 30 years at the time of conception (59,8% and 40.2%), and the history of cesarean delivery (27,8%). Conclusion: This study concludes that the occurrence of ASD in Palembang is multifactorial, involving both genetic and environmental risk factors.

Abstract

Introduction: Dementia is a chronic progressive syndrome in which there is a decrease in the function of cognitive abilities including memory impairment, thinking ability, orientation, understanding, calculation, language, and assessment but without impaired consciousness. Apart from cognitive impairment, dementia is often accompanied by psychological symptoms and behavioural symptoms so that antipsychotic therapy is needed to overcome this. This study aims to identify the description of the use of antipsychotics in patients with dementia at Dr Mohammad Hoesin General Hospital Palembang in the period 1 January 2014-31 December 2018. Methods: This research was a descriptive study using secondary data in the form of medical records of dementia patients receiving antipsychotic therapy at Dr Mohammad Hoesin General Hospital Palembang. Samples were taken using a total sampling method. Results: There were 29 dementia patients (38.67%) receiving antipsychotic therapy. Most of the dementia patients who received antipsychotic therapy were in the late elderly age (27.59%) and were female (55.17%). The most commonly administered antipsychotic drug is haloperidol from the dopamine receptor antagonist (60%) with the most frequent dose of 0.5 mg (34.48%). Risperidone from the serotonin-dopamine antagonist class is the second most frequently prescribed antipsychotic drug (34.28%) at a dose of 1 mg (17.28%). The mean of haloperidol was 425 days, and risperidone was 295.5 days. Conclusion: Although in theory, psychological and behavioural symptoms are often found in dementia cases, not all dementia patients in Dr Mohammad Hoesin General Hospital Palembang received antipsychotic therapy. People living with dementia who receive antipsychotic treatment get various types of drugs, dosages, and frequencies.

Abstract

Sudden changes that cover all aspects of life in a short time, such as what happened during the current COVID-19 pandemic, can pose challenges to not only the children with special needs, but also their parents / guardians. This community service activity is carried out in the form of online educational seminars on efforts to maintain the health of children with special needs during the pandemic for parents / guardians of children with special needs who attend SLB C Karya Ibu Palembang and also common populace. This activity aims to provide a better understanding to parents / guardians of children with special needs about what children with special needs may face and live with during the pandemic.

Abstract

Changes in all aspects of life due to the pandemic affect all levels of society without exception, including vulnerable populations such as people with mental disorders. Sudden changes in a short time can create challenges for both the patients and the families. A better understanding of the course of their illness and what preventive and promotive efforts can be made by families/caregivers during the pandemic can help improve their quality of life. Online education for caregivers/families aims to shape a better understanding of mental health and mental disorders.

Abstract

According to the Ministry of Health in Indonesia, in 2018, there are about 640 thousand cases of people with HIV within the death rate of 38 thousand people. This number grows annually, in fact, according to WHO, there are approximately 38 million people who are diagnosed with HIV in the world in 2019. Thus, HIV-AIDS is a dangerous health problem and needs to be taken care of as soon as possible. HIV (Human Immunodeficiency Virus) is a retrovirus that got its name from the infecting immune cells in the human body. There are two types of HIV, named HIV-1 and HIV-2. Both have the same basic gene arrangement, transmission process, intracellular replication lane, and both cause AIDS. The differences between them are the HIV-1 spreads globally, while HIV-2 locally happens in West Africa. Currently, ART (Antiretroviral Therapy) is the most commonly used method of treatment for HIV-1. Treatment for HIV-1 with ART is effective in controlling HIV-1 virus replication but has not been able to completely eradicate the latent viral reservoir. In the past few years, it is known that there is a CRISPR (Clustered Regularly Interspaced Short Palindromic Repeat) method that can modify genes (DNA) in the body of an organism. However,apart from its potential in handling HIV-1, there are still obstacles in the mechanism.

Abstract

Selain berdasarkan temuan klinis, penentuan diagnosis Disorders of sex development (DSD) juga didukung oleh ragam pemeriksaan penunjang seperti analisis kromosom, radiologi, dan pemeriksaan laboratorium yang salah satunya berupa pemeriksaan kadar hormon. Pemeriksaan kadar hormon dapat memberikan gambaran fungsi gonadal dan adrenal, yang bermanfaat baik untuk penapisan dan juga penentuan diagnosis DSD. Studi yang bertujuan untuk mengidentifikasi profil hormon pasien DSD di Indonesia masih sangat minim, dan studi ini merupakan studi pertama yang meninjau profil hormon DSD di RSUP Dr. Mohammad Hoesin Palembang. Studi ini merupakan studi deskriptif menggunakan data sekunder dari rekam medik pasien DSD yang menjalani pemeriksaan hormon selama periode 2013-2017. Dari total 173 pasien DSD yang datang berobat ke RSUP Dr. Mohammad Hoesin Palembang selama lima tahun terakhir, hanya 22 (12.72%) pasien yang menjalani pemeriksaan hormon. Ada 13 profil hormon yang diperiksa pada pasien DSD di RSUP Dr. Mohammad Hoesin Palembang dengan hormon testosteron (54.54%) sebagai hormon yang paling sering diperiksa. Pemeriksaan profil hormon sebagai salah satu alat bantu penapisan dan penegakkan diagnosa DSD belum menjadi salah satu pemeriksaan penunjang yang umum dilakukan di RSUP Dr. Mohammad Hoesin Palembang.
