Abstract

World Health Organization (WHO) declared Coronavirus Disease 2019 (COVID-19) a pandemic as of March 9, 2020. Data as of October 8, 2021 recorded a total positive case of COVID-19 in Lampung Province were 49,304 cases with 3,790 deaths. Disease progression in COVID-19 patients was determined based on the results of laboratory tests. Several studies reported that the value of Neutrophil Lymphocytes Ratio (NLR) was strongly associated with clinical worsening and even death in COVID-19 patients. This study was purposed to determine the relationship between NLR with death in COVID-19 patients. This study was an observational analytic study with a cross sectional design. The sample was medical record data taken from 44 COVID-19 patients who survived and 44 COVID-19 patients who died in March 2020 - September 2021 at Dr. RSUD. H. Abdul Moeloek Lampung Province. The sample was selected using purposive sampling technique. Statistical analysis was performed by univariate and bivariate analysis. The result of the bivariate analysis with chi quare test showed the p value was 0.000 (p<0.5), which indicated that there were a relationship between NLR with death in COVID-19 patients at RSUD Dr. H. Abdul Moeloek Lampung Province. There were a statistically significant relationship between NLR with death in COVID-19 patients at RSUD Dr. H. Abdul Moeloek Lampung Province.

Abstract

Thyroid carcinoma is the most common cancer found in the endocrine system that arises from thyroid parenchymal cells.  Its prevalence has increased over the past few decades with a mortality rate of 0.5% per 100,000 cases. The incidence of thyroid carcinoma in 2020 ranks 7th in the world as the 9th most frequent incidence occurs from all types of cancer. Objective: This study aims to determine the characteristics of thyroid carcinoma patients at Dr. H. Abdul Moeloek Regional General Hospital for the period of January 2023 – August 2024. Method: This study used a descriptive observational design and Total Sampling technique with a data source in the form of secondary data on medical records from 82 thyroid carcinoma patients at the Dr. H. Abdul Moeloek Regional General Hospital for the period January 2023 to August 2024.  Results: In this study, thyroid carcinoma patients were mostly female, with an age group in the range of 41-60 years. The most common type of thyroid carcinoma is papillary carcinoma and the least common type is anaplastic carcinoma. Conclusion: In this study, the characteristics of thyroid carcinoma patients were obtained based on gender, age group, and type of histopathological diagnosis.

Abstract

Drug-resistant epilepsy is the failure to administer two or more anti-epileptic drugs to an epileptic child which becomes a challenge in its management. Failure of this therapy can result in a decrease in the child's quality of life, such as decreased cognitive function, personality disorders and sleep quality. This can also have a negative impact on the parents who care for them. Vagus nerve stimulation is a device attached to the child's left chest and neck that can stimulate the vagus nerve. This literature review provides a comprehensive overview of vagus nerve stimulation in the treatment of drug-resistant epilepsy, including a discussion of indications, techniques for use, and scientific evidence for the efficacy of vagus nerve stimulation. We reviewed and analyzed the current scientific evidence to assess the effectiveness of vagus nerve stimulation in the treatment of drug-resistant epilepsy.

Abstract

Acute ischemic stroke is a leading cause of morbidity and mortality worldwide, significantly impacting patients' quality of life and healthcare systems. Understanding prognostic factors that predict clinical outcomes in acute ischemic stroke patients is crucial, especially with the rising prevalence of risk factors such as hypertension, diabetes, and dyslipidemia. Recent studies have focused on hematological biomarkers, particularly the platelet-to-white blood cell ratio (PWR), which reflects the interaction between immune system components and hemostasis. This literature review examines the prognostic value of PWR in acute ischemic stroke, correlating higher PWR with better clinical outcomes measured by the National Institute of Health Stroke Scale (NIHSS). Lower PWR values have been associated with increased mortality risk within 90 days post-stroke. The dynamics of PWR provide insights into the body's response to ischemia and inflammation, highlighting its potential as a valuable prognostic marker. While PWR shows promise as a prognostic tool, limitations such as individual variability in inflammatory response, timing of measurement, and lack of standardization must be considered. Further research is needed to explore PWR's potential across diverse populations and address existing limitations to enhance its clinical application in managing acute ischemic stroke.

Abstract

Pineapple is one of the traditional plants that may be used as an alternative treatment to reduce pathogenic microbes. Its content is rich in chlorine, iodine, phenol and the enzyme bromelain which functions to stop microbial growth. This research aims to determine the potential of pineapple peel extract (Ananas comosus) as antimicrobe agents. This research uses a literature review study method, namely by using research articles obtained through a literature search process related to the potential use of pineapple peel extract on microbe from 2015 to 2025. Pineapple peel extract contains the main compounds, namely flavonoids, polyphenols, bromelain and saponins. This content causes pineapple skin extract (Ananas comosus) to have an effect on microbes.

Abstract

Chronic hypertension in pregnant women is a medical condition that can significantly affect maternal and fetal health. Chronic hypertension is defined as high blood pressure that has been present before pregnancy or diagnosed before 20 weeks of gestation. Risk factors that affect this condition include older maternal age, obesity, family history of hypertension, and an unhealthy lifestyle. The pathophysiology of chronic hypertension that progresses to preeclampsia involves endothelial dysfunction, which causes vasoconstriction and decreased placental perfusion, leading to placental hypoxia and increased oxidative stress, which further triggers activation of systemic inflammatory pathways causing damage to target organs such as the kidneys (proteinuria) and liver (elevated liver enzymes). The prevalence of chronic hypertension in pregnant women in Indonesia is quite high. According to data from the Lampung Provincial Health Office in 2018, hypertension in pregnancy caused 15.16% of maternal deaths. In addition, data from Riskesdas 2018 shows that the prevalence of hypertension in pregnant women in Indonesia is 6.18%, with the highest prevalence in West Java Province at 10.57%. The purpose of this article is to provide a better understanding of the effect of chronic hypertension in pregnant women on the incidence of preeclampsia. This article will discuss the association of chronic hypertension with the risk and underlying mechanisms of preeclampsia.

Abstract

Typhoid fever is a significant health issue worldwide, especially in developing countries. The highest incidence of typhoid fever occurs in the pediatric population, particularly among school-age children. Patients often resort to self-medication with antibiotics, where chloramphenicol is the primary choice for treating typhoid fever in Indonesia. However, reports of resistance to the chloramphenicol group have been documented in some locations. A total of 8 articles are included in this literature, revealing the existence of resistant isolates in the antibiotic susceptibility test for Salmonella thypi during the primary therapy. Several cases demonstrate resistance to various antibiotics such as ceftriaxone, cefuroxime, amoxicillin, ampicillin, ciprofloxacin, augmentin, fluoroquinolones, azithromycin, and nalidixic acid. However, some drugs have proven effective, including Cefixime, ceftriaxone, chloramphenicol, ofloxacin, cefepime, quinolones, and third-generation cephalosporins, which are considered sensitive to S. Typhi.

Abstract

Background: Medical records are an important element in the process of health care in health facilities. Several primary health facilities have used electronic-based medical records to improve service quality. Electronic medical records are patient medical records in electronic form that include health information of an individual that can be accessed using a computer from a network to provide effective, efficient, and integrated health services. The purpose of the study was to determine the development of the use of electronic medical records in primary healthcare facilities in Indonesia with a systematic review before the mandatory use of electronic medical records in Indonesia. Methods: This research is a systematic literature review of the use of electronic medical records in primary health facilities. Researchers searched the literature on google scholar with the keyword electronic medical records and obtained a total of 60 articles, 4 of which were inaccessible, 40 articles were not conducted in primary health facilities, and 3 articles do not contain the aspect we look for so only 13 articles were used in literature studies. Results: The implementation of medical records is carried out by all health workers who provide services from registration, triage, clinical examination, treatment, and drug prescribing. Medical record data is distributed automatically by computer. Health workers have different electronic medical record display menus according to their duties. Conclusions: The implementation of electronic medical records in primary health facilities can be carried out by medical recorders and non-medical health workers who are trained and has a different electronic medical record display menu in each service flow.

Abstract

COVID-19 merupakan pandemi global yang menyebabkan banyak kematian. Baku emas diagnosis COVID-19 adalah RT-PCR. Pemeriksaan hematologi merupakan screening untuk COVID-19. Keparahan pasien COVID berhubungan dengan penurunan CT value dan limfopenia. Penelitian bertujuan untuk mengetahui korelasi ALC dan nilai CT pasien COVID-19 di RSUD Abdul Moeloek, Bandar Lampung. Penelitian ini adalah penelitian analitik dengan metode cross sectional yang menggunakan data sekunder berupa rekam medis periode Juli 2020-Juli 2021. Sampel diambil dengan teknik purposive sampling. Primer yang di gunakan yaitu MBioCoV-19 yang menargetkan gen RdRp dan ORF1b dari virus SARS-CoV-2. Nilai CT dinyatakan positif jika <40. Data di analisis menggunakan uji korelasi pearson. Sampel terdiri dari 40 laki-laki (34,8%) dan 75 perempuan (65,2%) dengan rata rata usia 43,61 tahun. Distribusi penyakit penyerta yaitu 34 orang (29,6%) dengan diabetes melitus (13%), hipertensi (9,6%) dan penyakit ginjal (7%). Rerata ALC yaitu 1516,4 Sel/μL. Rerata CT value untuk gen ORF1b dan RdRp masing-masing yaitu 25,479 dan 27,996. Terdapat korelasi positif antara ALC dan CT value gen ORF1b (p=0.032 r=0.2). Terdapat korelasi positif antara ALC dan CT value gen RdRp (p=0.048 r=0.185). Terdapat korelasi positif secara signifikan dengan kekuatan korelasi lemah (ORF1b) dan sangat lemah (RdRp) antara ALC dengan CT value kedua gen spesifik SARS-CoV-2 penyebab COVID-19.

Abstract

Diabetes Mellitus (DM) is characterized by a state of hyperglycemia due to insulin resistance, decreased insulin production, or both of these conditions which are classified into metabolic diseases. When checking the condition of type 2 DM, HbA1c can be checked as an indicator. High and uncontrolled HbA1c levels will cause disturbances in lipid profiles such as increased triglycerides, LDL, total cholesterol, and decreased HDL. The purpose of this study is to determine whether or not there is an effect of HbA1c on lipid profiles in type 2 DM patients. This method uses scientific publication search media such as Google Scholar, Pubmed, and old medical scientific journals using the keywords “lipid profile, HbA1c, type 2 diabetes mellitus, and dyslipidemia” obtained 15,700 articles and scientific journals. In patients with type 2 diabetes mellitus, glycemic control can be determined by HbA1c examination, high HbA1c levels are associated with disruption of lipid profile levels. The results of the study discuss the effect of HbA1c levels on lipid profiles in patients with type 2 diabetes and determine the relationship between the two.
