Abstract

Introduction: Ferritin is a routine parameter used in the evaluation of iron overload in thalassemia patients. Excess iron causes the formation of ROS, resulting in an oxidative reaction that triggers inflammation. This study aims to determine the correlation of ferritin levels with proinflammatory cytokines in beta thalassemia. Methods: A cross-sectional analytical study was conducted on 33 beta thalassemia patients who received routine blood transfusions at I.A. Moeis Samarinda. Spearman rank correlation test was used to determine the correlation between ferritin and TNF-α and IL-6. Results: The results of this study showed that there was no relationship between ferritin and TNF- (p=0.580) and IL-6 (p=0.924). Meanwhile, there was a significant relationship between TNF- and IL-6 (p less than 0.0001) which had a strong positive correlation (r=0.621). Conclusion: This study showed that there was no correlation between ferritin levels with TNF-α and IL-6. Meanwhile, proinflammatory cytokines showed a correlation between TNF-α and IL-6 in thalassemia patients in Samarinda City.

Abstract

Background: Thalassemia-β patients have erythrocyte damage that requires blood transfusion treatment. Blood transfusion treatment that is undergone has an impact on the condition of iron overload in the body. The condition of iron overload will result in oxidative stress and organ damage that requires additional therapyObjective: aimed to compare the effectiveness of turmeric and kalmegh extract doses against biochemical parameters and histopathological features of iron-induced liver Rattus Norvegicus.Methods: This study used a True-experiment research design with a post-test-only control group. A total of 30 rats were used and divided into six treatment groups, normal, deferiprone drugs, turmeric, and kalmegh extracts 100mg/KgBW, 200mg/KgBW, and 400mg/KgBW. Malondialdehyde, catalase, superoxide dismutase, and ferritin are used as biochemical parameters and hepatic histopathology is documented.Results: The results showed that there were significant improvements and differences in iron-induced rats.Conclusion: The study also showed that turmeric and kalmegh extracts had similar effects to deferiprone.

Abstract

Hepcidin, a key regulator of iron metabolism, interacts with inflammatory cytokines like IL-6 and transcription factors such as NF-κB, which play crucial roles in the body's response to inflammation and iron homeostasis especially in transfusion-dependent Thalassemia-β patient. Their interplay in transfusion-dependent thalassemia-β patients, particularly in the context of iron chelation therapy, remains underexplored. Understanding these dynamics could provide insights into optimizing treatment strategies for better patient outcomes. Objective: Determine the relationship between hepcidin levels and IL-6 and NFκB in β-thalassemia sufferers who underwent blood transfusions and determine the effect of the type of iron chelation, and the regularity of iron chelation consumption with the relationship between hepcidin, IL-6 and NFκB levels in β-thalassemia sufferers who underwent transfusions blood. Method: A cross-sectional, quantitative correlation study involving transfusion-dependent thalassemia-β patients was conducted. Serum levels of hepcidin, IL-6, and NF-κB were measured, and the relationships among these biomarkers were analyzed using Pearson correlation. The impact of iron chelation therapy type and adherence on these relationships was also assessed using stratified statistical analysis. Results: Hepcidin levels with IL-6 were found with p = 0.757. The next analysis is the relationship between Hepcidin levels and NFκB with p = 0.029. Conclusions: The relationship between hepcidin levels and IL-6 did not contribute significantly, while there was a significant relationship between hepcidin levels and NFκB. The relationship was influenced by moderating variables, namely the type of iron chelation and the regularity of iron chelation consumption.

Abstract

Cardiovascular disease is one of the leading causes of death worldwide, especially in developing countries. Combination therapy of ramipril, an ACE inhibitor, and spironolactone, an aldosterone antagonist, is widely used in hypertension and heart failure patients, offering clinical benefits such as blood pressure control and organ protection, but it also poses risks of hyperkalemia and renal dysfunction. This systematic review aimed to evaluate and identify the adverse effects of ramipril–spironolactone combination therapy. A systematic search was conducted in Google Scholar and PubMed NIH Central for articles published between 2020–2024 using the keywords “Ramipril” AND “Spironolactone” AND “Adverse Effect” and “Combination Therapy” AND “Cardiovascular Disease.” From an initial 1,026 articles, 38 were screened, 37 remained after duplicate removal, and 3 studies (2 from Google Scholar and 1 from PubMed) met the inclusion criteria. Article selection followed PRISMA 2020 guidelines, and data were extracted on study design, population, interventions, outcomes, and adverse effects. The results indicate that although ramipril–spironolactone combination therapy provides significant cardiovascular benefits, it is consistently associated with hyperkalemia, hypotension, and renal dysfunction; therefore, close monitoring of potassium levels, renal function, and blood pressure is strongly recommended to minimize adverse outcomes.
