Abstract

Background: The hospital has quite an important role inreducing IMR and MMR because hospitals as providers ofplenary personal health services including maternal and childhealth (MCH). However, until now the IMR and MMR Indonesiais still high compared to other ASEAN countries. The maincauses of maternal mortality are obstetric complications ordisease as a complication that arises during pregnancy, childbirthand postpartum. This factor was experienced by approximately20% of all pregnant women, while complication cases thatwere treated well are less than 10%.Objective: The research aims to identify the effect of hospitalcharacteristics, management of MCH services, humanresources for MCH, MCH services, and MCH equipment on theperformance of MCH services in government hospitals inIndonesia.Methods:Research is using secondary data of Health FacilitiesResearch 2011 (RIFASKES) with a cross sectional study.Population and sample is the entire Indonesian governmenthospitals (685 hospitals). The research variables wereidentified from the available variables in the questionnaireRIFASKES. Performance measurement of the compositevariable proportion of maternal deaths due to hemorhage d”1%, d” 10% pre-eclampsia, sepsis d” 0.2%, d” 20% secariasection, the proportion of stillborn d” 4%, and the proportion ofLBW handling 100% based SPM hospital. Multivariate logisticregression was used to obtain a model determinants ofperformance MCH services.Results: The majority (66.3%) government hospitals inIndonesian has less than optimal performance. As thedeterminant is unaccredited status (OR = 2.99: 1.43 to 6.28),the hospital is not a vehicle of education (OR = 1.78; 1.11 to2.85), team PONEK is incomplete (OR = 1.89; 1.27 to 2.82),there is no PONEK-trained doctor in the ER (OR = 1.89; 1.27 to2.82), there is no team ready to perform the operation or taskthough on call (OR = 2.16; 1.32 to 3.53). The most dominantfactor is the unaccredited status.Conclusions: Suboptimal performances of MCH at Indonesiangovernment hospitals are influenced by the low hospital servicecharacteristics and incomplete of human resources. TheMinistry of Health needs to support improvement in all types ofservices to complete an accredited hospitals (16 types ofservices), not just 5 or 12 services. They also need to makethe government hospital as a vehicle of education, increasethe quantity and quality of human resources are trained inPONEK-skill, ensure availability of PONEK-trained doctor inemergency, provide the team that are ready to perform theoperation or task though on call, and increase organizationalcommitment to overall performance improvement.Keywords: Performance, Maternal and Child Health Services,Government Hospital

Abstract

The  first 1,000 days of life program is a program to improve the quality of children in the future which one of these programs is the provision of exclusive breastfeeding. The decline in infant mortality rates slowed between 2003 to 2012 from 35 to 32 per 1,000 live births, all babies need access to key interventions such as exclusive breastfeeding. The success of these programs are driven by health workers, but the researches that analyze information about exclusive breastfeeding program on health workers, especially in Bengkulu had never been examined. The research design used mixed methods (quantitative and qualitative) on health workers in Bengkulu in 2015 who have infants aged 6-24 months (66 people), which were determined by purposive sampling. The results showed that 69.7% of the mothers did not practice exclusive breastfeeding and 54.5% did not practice early initiation of breastfeeding. Analysis of early initiation of breastfeeding factor as the dominant factor for failure of exclusive breastfeeding. The qualitative analysis showed confidence and perception of little milk production are factor for unsuccessful early initiation of breastfeeding .

Abstract

The high prevalence of stunting is influenced by nutritional intake. Iodine is an important micronutrient for growth. Bengkulu City is located along coastal area, hence it has potentially high iodine food sources. However, stunting prevalence was still high. This was a cross-sectional study conducted from March to July 2016 which aimed to determine the relationship between iodine consumption and linear growth of children under five years old in Malabero coastal area of Bengkulu City. The population were children aged 2-5 years old with a sample of 73 children. Measurement of linear growth was based on AHZ index using microtoice with capacity of 200 cm and accuracy of 0.1 cm. Iodine intake was measured based on semiquantitative Food Frequency Questionnaire (FFQ). The results showed that 71.2% of children under five years old did not had sufficient iodine consumption with average percentage of iodine sufficiency 47.7% ± 0.456. There were 53.5% of children under five years old who experienced stunting. Low iodine consumption was shown to be associated with stunting. There were 73.1% children under five years old with insufficient iodine intake who were stunted while only 4.8% children under five years old with sufficient iodine intake who were stunted.

Abstract

One of the nutritional problems that occur is overweight. The level of knowledge nutrition in an adolescent is one of the factors that can affect the occurrence of obesity in adolescents. Most occurrences of nutritional problems are more or less avoidable if adolescents have sufficient knowledge and attitudes about maintaining nutrition and managing food. This research was a quasi-experimental study with a pre-post test with control group design. The subjects of this study were overweight adolescents totaling 40 people, the intervention group received education through audiovisual media while the control group with leaflets. The results showed the average value of knowledge and attitudes of adolescents after and before being given education for Leaflet groups were the average value of knowledge before 8.60 and after 9.48, attitudes value before 36.58 and after 40.38. While audiovisual media groups, the average value of knowledge before 8.83 and after 9.42, values for attitudes before 36.45 and after 39.65. There was an influence of education by using audiovisual media and leaflets to increase the knowledge and attitudes of overweight adolescents. Suggestions for schools to be able to use audiovisual media and leaflets as a medium to educate nutrition about students in schools so that they can apply daily life and remind each other to lose weight and live healthy lives.

Abstract

The problem of overweight and obesity in various worlds is increasing rapidly. The national prevalence in Indonesia in an adolescent age group is 10.8% (8.3% obesity and 2.5% obesity) caused by inadequate knowledge of nutrition. This study aims to determine the effect of Nutrition Education on changes in body weight of overweight and obese adolescents in SMP Negeri 2 Kota Bengkulu. The type of research is quasi-experimental with pretest and posttest design with control group design. The sample of the study was 11-13 year old teenagers with 44 people using consecutive sampling technique and using Mann-Whitney test and Ancova test. The results showed changes in the mean body weight of adolescents before the intervention of 61.23 kg and the mean body weight of adolescents after being given 61.89 kg of education with the difference in body weight change is only 0.66 kg. There were no significant differences in mean changes in adolescent weight before and after Nutrition Education (p=0.307). Factors that affect Weight Change is pocket money (p=0.044). Parents are expected to manage pocket money so that children can minimize the purchase of snacks that can lead to obesity.

Abstract

The high prevalence of diabetes mellitus (DM) type 2 one risk factor is an unhealthy lifestyle, especially an unhealthy diet and lack of physical activity. Education followed by physical activity required to provide benefits to the body as a way of exercise can lower blood sugar. The recommended physical activity such as gymnastics, Gymnastics is aerobic physical exercise with a series of movements in a way to follow the rhythm of the music. The study aims to determine the effect of diet and exercise education on blood sugar levels in people with type 2 dm in Bengkulu City Puskesmas Lingkar Timur years 2018. This quasi-experimental research design.consists of 30 intervention and 30 control group. Gymnastics held 3 times a week for 1 week and education 1 time for 1 week, with an examination of the blood sugar levels of pretest and posttest. Data collection equipment consists of glucometers and bathroom scales and microtoice. The used statistical test Chi-Square test, t-dependent, and t-independent. The results of the study the characteristics of most women, age range 40-70 years intervention group and a control group 32-79 years, the average BMI in the intervention group is 24.21 m2 and 24.33 m2 in the control group, the intervention group showed that the levels of blood sugar before an average of 271.90 mg/dl, while the results of the blood sugar level after an average of 225.93 mg/dl. in the control group blood sugar levels before an average of 278.27 mg/dl whereas the blood sugar level results after an average of 285.67 mg/dl. Based on the statistical test that no educational influence diet and exercise on blood sugar levels in patients with type 2 dm p-value of 0.002. Expected to Type 2 diabetic patient's diet, increasing exercise and increased activity at home.

Abstract

Prevalence of stunting in Indonesia is a cause for concern. We used IFLS 2007 as secondary data in the cross-sectional study to develop a problem-solving and prevention model of stunting. The study was conducted in 2016 to predict a model from the characteristics, parents, and health care of the child. We recruited 3589 children under-five years from IFLS 2007 data as samples. The inclusion criteria are 1-5 years old children from 15-49 years old pregnant women, living with biological parents, available data of birth weight and gestational age, and do not have chronic disease. We used multiple logistic regression for modeling, and Receiver Operation Characteristic (ROC) Curve as a diagnostic test. We found that 39.5% of children under-five have stunting. Stunting protective factors are: prevention of LBW (Low Birth Weight) in infant, limitation of number of children by three, improved parenting for the boys, prevention of young-age pregnancy and stunting in the female adolescent. Other factors are completing immunization, improving maternal education and the health services of under-five children in rural areas, and improving Fe consumption for pregnant mothers. The model was able to prevent and delay the stunting in toddler by 64%, with 61.9% sensitivity and 60.9% specificity, and AUROC 65.5%. It is necessary to counsel the pregnant women with a low height and young-age pregnancy as a high risk, as well as management of the infant with LBW to prevent stunting.

Abstract

Teenage pregnancy has an impact on the outcomes. Teenage pregnancy is at risk of adverse health, an increased risk of domestic violence, poor nutrition, and sexual and reproductive health problems, lower levels of education, and higher levels of poverty compared to women who postpone marriage age. The study aims to determine the relationship between marriage and Adolescent pregnancy with the incidence of stunting in children under five years in Indoneasia.  The study used a cross-sectional approach from 2014 IFLS (Indonesian Family Life Survey) data. The bivariate analysis used the Chi-Square Test when the multivariate analysis used logistic regression. The results found the prevalence of stunting in Indonesia in 2014 was 36.6%. The stunting prevalence is higher in toddlers of married mothers of adolescents (42.4%) compared to mothers of married mature (35%). The stunting prevalence was also higher in children under five years from adolescent pregnant women (44.4%) compared to mothers who were of sufficient age (35.6%). Teenage pregnancy is associated with the incidence of stunting. A married teenage woman is 1.2 times at risk, and a woman who is less than 20 years pregnant is 1.3 times at risk of having a stunting toddler. Teenage pregnancy increases the prevalence of stunting. Cross-sectoral integrated interventioans are needed to prevent adolescent pregnancy. It is required to decrease the prevalence of stunting. The various risks that occur in teenage pregnancy are the basis for the importance of pregnancy prevention efforts in this age group by involving the related sectors.

Abstract

Prevalensi stunting di Indonesia masih termasuk masalah kesehatan masyarakat yang berat bahkan serius untuk ditanggulangi. Kader kesehatan berperan penting dalam menurunkan prevalensi stunting melalui deteksi risiko stunting dan pendidikan gizi dan kesehatan kepada ibu balita saat kegiatan Posyandu. Hasil beberapa penelitian menemukanpengetahuan, sikap dan keterampilan kader dalam deteksi risiko stunting masih kurang, sehingga perlu dilakukukan pemberdayaan kader melalui kegiatan pelatihan dan pendampingan. Tujuan Pengabdian kepada Masyarakat (PKM) ini adalah meningkatkan kemampuan kader keehatan dalam deteksi stunting memalui pelatihan penggunaan meteran deteksi stunting yang dilaksanakan di Kabupaten Timur Tengah Selatan (TTS) Propinsi NTT dan Bengkulu Utara (BU) Propinsi Bengkulu. PKM menggunakan metode pelatihan selama 3 hari pada 20 kader. 30 anak yang terdeksi berisiko stunting kemudian dilakukan pendampingan gizi dan kesehatan selama 5 bulan oleh kader bersama tenaga kesehatan puskesmas. Pelatihan dilaksanakan tanggal 15-17 Juni 2019 di Kabupaten BU dan tanggal 17-19 Mei 2019 di Kabupaten TTS. Evaluasi keberhasilan pelatihan dengan melakukan pengukuran pengetahuan dan sikap kader menggunakan uji statistik independent t test dan paired t test, pengukuran kemampuan deteksi dengan observasi. Hasil pelatihan menunjukan terjadi peningkatan skor pengetahuan 19,36 point  (p=0,0001), peningkatan skor sikap sebesar 20,17 point  (p=0,0001) dan peningkatan kemampuan kader mendeteksi stunting pada anak 6-24 bulan. Kader terlatih perlu terus diberdayakan dengan melibatkan dalam kegiatan kunjungan rumah dan kegiatan posyandu untuk aktif melakukan deteksi risiko stunting serta mengikutsertakan dalam kegiatan pembinaan dan pelatihan-pelatihan terkait pencegatan stunting.Kata kunci: Kader, pelatihan pengetahuan, sikap, keterampilan, stunting.
