Abstract

Background: Cancer diagnosis and therapy that causes chronic stress, its progression to depression  increase cortisol and VEGF-c levels in advanced cervical cancer patientsObjective: To  know the VEGF-c, cortisol and HADS level  in advanced stage cervical cancer patients who get Psychocurative intervention Method : An experimental study of pretest – posttest controle group design in outpatient oncology clinic and ward of Doctor Moewardi General Hospital. Thirty subjects with advanced cervical cancer were randomly divided into 15 patients in the intervention group, and 15 in control. Psycho-curative intervention 4 times a month, 1 time a week, 60 minutes duration. Before and after intervention, each subject of the two groups was examined levels of VEGF-c, cortisol and HADS scores. Data obtained were analyzed by independent t test, Mann Whitney, Pair t and Wilcoxon with the 19th version of SPSS for Windows.Result and Discussion: Average VEGF-c levels before intervention 9006.53 ± 2181.49, after intervention 5631.20 ± 2071.55, p <0.001. The mean cortisol level before intervention was 12.29 ± 4.36, after intervention 6.71 ± 3.88, p <0.001. The mean HADS Anxiety score before intervention was 14.13 ± 3.02, after intervention 8.47 ± 3.07, p <0.001. Mean Depression HADS scores before intervention 13.80 ± 3.21, after intervention 7.20 ± 2.70, p <0.001.Conclusion : VEGF-c levels, cortisol, HADS anxiety scores and depression in patients with advanced cervical cancer decline after psychocurative intervention. Keywords : Psychocurative; VEGF-c; Cortisol; HADS; Cervical cancer

Abstract

Background: Polycystic Ovary Syndrome (PCOS) is an endocrine disorder that most often causes infertility. It occurs due to the failure of folliculogenesis, thus causing non-ovulation. Letrozole is a 3rd generation drug of non-ste­roidal aromatase inhibitors, which can increase ovulation rates. Electroacupuncture is the insert­ion and manipu­lation of needles into acupunc­ture points (acu­points) that are selected speci­fically based on the meridian system and con­nected to the electrode stimulator. This study aimed to determine the differences of the effects of letrozole and letrozoleelectroacupuncture (letrozole-EA) on the improve­ment of folliculo­genesis in women with PCOS.Subjects and Method: This was an analytic observational study with a case-control design. This study was conducted at the Gynecology Polyclinic and Sekar Clinic, Dr. Moewardi Hos­pital, Surakarta. A sample of 30 infertile women aged 20-45 years was selected by fixed disease sampling. The criteria of the PCOS were deter­mined based on Rotterdam criteria. The depen­dent variable was the diameter of the follicle. The independent vari­ables were the administration of letrozole and letrozole-EA. Transvaginal ultra­sound of Voluson P6 was used to measure the folliculo­genesis. The data were analyzed by independent t-test.Results: On the second day after treatment, the diameter of the follicles in the letrozole-EA group (mean= 6.92; SD= 1.60) was bigger than the letrozole group (mean=5.66; SD=1.11), and it was statistically significant (p=0.004).Conclusion: Combination of letrozole and letrozole-EA therapy is more effective in increasing follicular diameter development than letrozole therapy only.Keywords: polycystic ovary syndrome, folli­culogenesis, Letrozole, electro-acupunctureCorrespondence: Yulyanti. Abstract

Background: Placental hypoxia is one of the theories that cause preeclampsia. In placental hypoxia of preeclampsia, there is an increase of Hipoxia Inducible Factor 1 Alpha (HIF-1-A) expression as a response to hypoxia. L-Arginine administration through the L-Arginine-Nitric Oxide pathway is expected to improve placental hypoxia. This study aimed to examine the effect of L- Arginin on placental HIF-1-A expression at preeclampsia mice models.Subjects and Method: Experimental study with parallel group post-test only design. The study was carried out at the Veterinary Medicine Faculty of Airlangga University. Thirty six preeclampsia mice models were made with intravenous injection 10ng anti-Qa-2 on day 1-4 of pregnancy. They were divided into 2 groups: (1)18 mice without treatment (control) and (2) 18 mice were given L-Arginin 200 mg/kgBW/day on day 7-15 of pregnancy. They were terminated on day 16 then immunohistochemistry examination of HIF-1-A expression was done on the placental samples. The dependent variable was placental Hipoxia Inducible Factor-1-Alpha (HIF-1-A) expression. The independent variables were L-Arginin administration. HIF-1-A expression was measured by H-Score method. The data were analyzed by t-test. Results: The mean of placental HIF-1-A expressions of treatment group was lower (Mean= 7.98; SD=9.35) than control group (Mean=41.95; SD= 32.20), and it was statistically significant (p <0.001).Conclusion: L-Arginin reduces the Placental Hypoxia Inducible Factor Alpha (HIF-1-A) expression on preeclampsia mice model.Keywords: L-Arginine, HIF-1-A, PreeclampsiaCorrespondence: Nutria Widya Purna Anggraini. Abstract

Background: Systemic lupus erythematosus (SLE) is one of the pregnancy complications which cause an increase in fetal and maternal com­p­lications which are mainly caused by pla­cen­tal damage due to chronic inflammation and apoptosis. The rate of apoptosis can be assessed by the high expression of caspase-3 which is the ma­jo­rexecutioner caspases of apoptosis. The use of Mesenchymal Secretome Stem Cell for SLE the­rapy has an anti-inflammatory and anti-apo­p­to­tic role. This study aimed to examine the effect of mesenchymal secretome stem cell therapy to­ward caspase-3 expression in pregnant mice with pris­tan-induced lupus model.Subjects and Method: This study was a ran­do­mized controlled trial. This study was con­ducted at the Prodia ProStem Laboratory, La­bo­ra­tory of the Faculty of Veterinary Medicine of Airlangga University, and Anatomical Pathology Laboratory, Faculty of Medicine, Airlangga Uni­ver­sity. A total of 14 healthy BALB/C female mice strain aged 6-8 weeks, with a bodyweight of 20-30 grams, were divided into two groups, (1) 7 preg­nant mice who received pristan therapy only, and (2) 7 pregnant mice who received pristan the­rapy and mesenchymal secretome stem cell.The dependent variable was the caspase-3 exp­res­sion. The independent variable was the ad­mi­ni­s­tration of mesenchymal secretome stem cell the­rapy. The caspase-3 expression was measured by an immunoreactive score. The data were ana­ly­zed using independent t-test.Results: After giving mesenchymal secretome stem cell therapy, the mean of the caspase-3 exp­res­sion in the treatment group (Mean=1.86; SD­=­0.66) was lower than the control group (Me­an­=­3.­30; SD = 1.42), with p=0.031.Conclusion: Mesenchymal secretome stem cell the­rapy is effective for reducing caspase-3 exp­res­sion in the placenta of pregnant mice with pris­tan-induced lupus model.Keywords: Caspase-3 expression, lupus, placen­taCorrespondence: Febrian Andhika Adiyana. Abstract

Background: Polycystic Ovarian Syndrome (PCOS) is an endocrine disorder in women cha­rac­terized by increased levels of androgens, ovu­la­tory dysfunction, and Polycystic Ovarian Mor­p­ho­logy (PCOM), as well as a combination of clas­sic clinical features. Women with PCOS expe­ri­ence changes in the endometrium and endo­met­rial receptivity markers such as VEGF (Vascular En­dothelial Growth Factor). VEGF increases the per­meability and dilation of endometrial blood vessels which play an important role in imp­lan­ta­tion. This study aimed to analyze the difference of VEGF expression between women with PCOS and nor­mal women.Subjects and Method: This study was a case-con­trol study design conducted at the polyclinic of the Department of Obstetrics and Gynecology, Dr. Moewardi Hospital, Surakarta, Central Java, in July 2020. There were 30 infertile women with PCOS (diagnosis based on Rotterdam criteria, con­sensus 2003) and 30 women without PCOS as the sample of this study. The dependent variable was VEGF expression. The independent variables were age, BMI, family history with PCOS, menar­che, occu­pa­tion, and education. VEGF expression was ob­tained from an endometrial biopsy on days 19-24 of periods. The differences in VEGF exp­res­sion (in %) were analyzed by the Mann-Whitney test. VEGF determinants were analyzed by a mul­ti­ple linear regression.Results: The mean of VEGF expression in wo­men with PCOS (Mean=64.33; SD=44.43) was high­er than in women without PCOS (Mean­= 7.90; SD=8.73) and the result was statistically sig­ni­ficant (p<0.001). PCOS (b=42.09; 95%­CI­=­64.­2­6 to -19.93; p<0.001) and obese (b=38.63; 95%­CI=-63.44 to -13.82; p=0.003) increased VEGF expression.Conclusion: VEGF expression in the endo­met­rium of women with PCOS is higher than normal wo­men.Keywords: VEGF, endometrium, Polycystic Ova­rian SyndromeCorrespondence: Ricky Bernadi Saputra. Abstract

Background: Postoperative pain is one of the most common problems in women after cesarean section. Caesarean section is also associated with post-partum depression. Hypno­therapy is one of the complementary therapy options that are effective and efficient in reducing pain symptoms and post-partum depression. This study aimed to analyze the effect of hypnotherapy on the level of pain and post-partum depression in post-cesarean section patients.Subjects and Method: This was an expe­rimental single-blind randomized controlled trial with a post-test group design, which was conducted from January to April 2021, at the Dr. Moewardi and Universitas Sebelas Maret Hospitals, Surakarta. A sample of 80 patients with cesarean section was divided into two groups, namely 40 pati­ents in the hypno­therapy group and 40 patients in the control group. The indepen­dent variable was hypno­therapy. The depen­dent variables were pain and depression. Hypnotherapy was carried out by self-hypnosis using audio recordings of hypnotherapy in 2 sessions before and after the cesarean section procedure. The pain was measured by the Numeric Rating Scale. Post-partum depression was measured by the Edinburgh Postnatal Depression Scale. The data were analyzed by an independent t-test.Results: Post-cesarean section pain in the group of hypnotherapy (Mean= 4.58; SD= 1.20) was lower than without hypnotherapy (Mean= 6.48; SD= 1.01), and it was statis­tically significant (p<0.001). Postpartum de-pression in the group of hypnothera­py (Mean= 6.08; SD= 1.60) was lower than non-hypnotherapy group (Mean= 8.48; SD= 3.78), and it was statis­tically significant (p <0.001).Conclusion: Hypnotherapy has a significant effect on reducing pain and post-partum depression in post-cesarean section patients.Keywords: Hypnotherapy, pain levels, post-partum depression, post-cesarean sectionCorrespondence: Araafi Hariza Mahandaru. Abstract

Latar Belakang Invitro Fertilization merupakan salah satu manajemen infertilitas yang baik, namun masih didapatkan adanya kesamaan peluang kegagalan perkembangan embrio sebanyak 50% dibandingkan dengan pasien yang tidak mengalami endometriosis.Tujuan: untuk mengetahui hubungan endometriosis dan kualitas embrio pada pasien yang menjalani program invitro fertilization.Metode: Data yang dikumpulkan merupakan data pada rekam medis pasien Klinik Fertilitas Sekar yang menjalani invitro fertilization di Klinik Fertilitas Sekar RSUD Dr. Moewardi Surakarta pada Januari 2010 – Desember 2019. Analisis menggunakan uji deskriptif dan Uji T tidak berpasangan. Hasil dianggap signifikan jika p ≤ 0,05.Hasil dan Pembahasan: Dalam penelitian ini, rerata kualitas embrio baik memiliki perbedaan yang signifikan (p<0.05) antara pasien dengan endometriosis dan pasien tanpa endometriosi, sedangkan kualitas embrio buruk tidak memiliki perbedaan yang signifikan (p>0.05).Kesimpulan: Endometriosis memiliki hubungan yang signifikan dengan kualitas embrio pada wanita yang menjalani program IVF.

Abstract

Background: After cesarean section, there is often an increase in stress and anxiety which can have a negative effect on pain perception. Cortisol is the main stress hormone that regulates the duration and intensity of the stress response. Hypnotherapy is a therapy with a hypnotic method to reach someone's subconscious mind. Hypnotherapy is thought to be able to help patients in the post-section coping method because it can cause feelings of pleasure, satisfaction, safety, comfort and can make a person feel they have the strength of themselves to deal with the anxiety they experience. This study aims to analyze the effect of hypnotherapy on serum cortisol levels in postareaal section patients.Subjects and Method: This study was an experimental single-blind randomized controlled trial post-test group design which was conducted in the Obstetrics and Gynecology wards of Dr. Moewardi Hospital Surakarta and Dr. R Soetijono Blora Hospital. The subjects of the study were post-sectional patients who were hospitalized and met the inclusion and exclusion criteria. Subjects were divided into 2 groups, namely the hypnotherapy group which consisted of 15 post-sectional patients who received hypnotherapy and the control group which consisted of 15 postareaal patients who did not receive hypnotherapy. All subjects were examined for cortisol levels in the Prodia laboratory using the Chemiluminescent microparticle immunoassay (CMIA) method. The data of both groups were analyzed using the Mann-Whitney test.Results: The median, minimum, and maxi-mum cortisol levels in the hypnotherapy group were 3.60, 2.10, and 4.40 respectively, while the median, minimum and maximum cortisol values in the control group were 10.50, 8.20 and 32.80 respectively. Effect size= -2,2. The results of the analysis test showed a significant difference in mean values (p <0.001).Conclusion: Hypnotherapy has an effect on decreasing cortisol levels in post-cesarean section patients.Keywords: Hypnotherapy, Cortisol, Post-cesarean sectionCorrespondence: Inne Rizkiani.
