Abstract

Background: Polycystic Ovarian Syndrome (PCOS) is an endocrine disorder in women cha­rac­terized by increased levels of androgens, ovu­la­tory dysfunction, and Polycystic Ovarian Mor­p­ho­logy (PCOM), as well as a combination of clas­sic clinical features. Women with PCOS expe­ri­ence changes in the endometrium and endo­met­rial receptivity markers such as VEGF (Vascular En­dothelial Growth Factor). VEGF increases the per­meability and dilation of endometrial blood vessels which play an important role in imp­lan­ta­tion. This study aimed to analyze the difference of VEGF expression between women with PCOS and nor­mal women.Subjects and Method: This study was a case-con­trol study design conducted at the polyclinic of the Department of Obstetrics and Gynecology, Dr. Moewardi Hospital, Surakarta, Central Java, in July 2020. There were 30 infertile women with PCOS (diagnosis based on Rotterdam criteria, con­sensus 2003) and 30 women without PCOS as the sample of this study. The dependent variable was VEGF expression. The independent variables were age, BMI, family history with PCOS, menar­che, occu­pa­tion, and education. VEGF expression was ob­tained from an endometrial biopsy on days 19-24 of periods. The differences in VEGF exp­res­sion (in %) were analyzed by the Mann-Whitney test. VEGF determinants were analyzed by a mul­ti­ple linear regression.Results: The mean of VEGF expression in wo­men with PCOS (Mean=64.33; SD=44.43) was high­er than in women without PCOS (Mean­= 7.90; SD=8.73) and the result was statistically sig­ni­ficant (p<0.001). PCOS (b=42.09; 95%­CI­=­64.­2­6 to -19.93; p<0.001) and obese (b=38.63; 95%­CI=-63.44 to -13.82; p=0.003) increased VEGF expression.Conclusion: VEGF expression in the endo­met­rium of women with PCOS is higher than normal wo­men.Keywords: VEGF, endometrium, Polycystic Ova­rian SyndromeCorrespondence: Ricky Bernadi Saputra. Abstract

Background: Postoperative pain is one of the most common problems in women after cesarean section. Caesarean section is also associated with post-partum depression. Hypno­therapy is one of the complementary therapy options that are effective and efficient in reducing pain symptoms and post-partum depression. This study aimed to analyze the effect of hypnotherapy on the level of pain and post-partum depression in post-cesarean section patients.Subjects and Method: This was an expe­rimental single-blind randomized controlled trial with a post-test group design, which was conducted from January to April 2021, at the Dr. Moewardi and Universitas Sebelas Maret Hospitals, Surakarta. A sample of 80 patients with cesarean section was divided into two groups, namely 40 pati­ents in the hypno­therapy group and 40 patients in the control group. The indepen­dent variable was hypno­therapy. The depen­dent variables were pain and depression. Hypnotherapy was carried out by self-hypnosis using audio recordings of hypnotherapy in 2 sessions before and after the cesarean section procedure. The pain was measured by the Numeric Rating Scale. Post-partum depression was measured by the Edinburgh Postnatal Depression Scale. The data were analyzed by an independent t-test.Results: Post-cesarean section pain in the group of hypnotherapy (Mean= 4.58; SD= 1.20) was lower than without hypnotherapy (Mean= 6.48; SD= 1.01), and it was statis­tically significant (p<0.001). Postpartum de-pression in the group of hypnothera­py (Mean= 6.08; SD= 1.60) was lower than non-hypnotherapy group (Mean= 8.48; SD= 3.78), and it was statis­tically significant (p <0.001).Conclusion: Hypnotherapy has a significant effect on reducing pain and post-partum depression in post-cesarean section patients.Keywords: Hypnotherapy, pain levels, post-partum depression, post-cesarean sectionCorrespondence: Araafi Hariza Mahandaru. Abstract

Latar Belakang Invitro Fertilization merupakan salah satu manajemen infertilitas yang baik, namun masih didapatkan adanya kesamaan peluang kegagalan perkembangan embrio sebanyak 50% dibandingkan dengan pasien yang tidak mengalami endometriosis.Tujuan: untuk mengetahui hubungan endometriosis dan kualitas embrio pada pasien yang menjalani program invitro fertilization.Metode: Data yang dikumpulkan merupakan data pada rekam medis pasien Klinik Fertilitas Sekar yang menjalani invitro fertilization di Klinik Fertilitas Sekar RSUD Dr. Moewardi Surakarta pada Januari 2010 – Desember 2019. Analisis menggunakan uji deskriptif dan Uji T tidak berpasangan. Hasil dianggap signifikan jika p ≤ 0,05.Hasil dan Pembahasan: Dalam penelitian ini, rerata kualitas embrio baik memiliki perbedaan yang signifikan (p<0.05) antara pasien dengan endometriosis dan pasien tanpa endometriosi, sedangkan kualitas embrio buruk tidak memiliki perbedaan yang signifikan (p>0.05).Kesimpulan: Endometriosis memiliki hubungan yang signifikan dengan kualitas embrio pada wanita yang menjalani program IVF.

Abstract

ABSTRACTCervical cancer is the most frequent cause of death related gynecology malignancy in Indonesia. Recent management of advanced-stage cervical cancer has still not been able to improve the prognosis. Chemotherapy and radiation intervention, as well as therapy may resulting pain and cause psychological stress for some patient, furthermore it could effect on the quality of life. Cortisol is a hormone of adrenal cortex, it secretes due to increased production of ACTH by anterior pituitary which is associated with stressful condition. To analyze the effect of Cognitive Behavior Therapy (CBT) intervention on serum cortisol levels and pain score of patients with advanced-stage cervical cancer. This experimental study was a double blind non-randomized clinical trial post-test group design. It was using two groups in this study, each group consisting of 15 subjects, the treatment group were given CBT and standard therapy, while the control group were only given a standard therapy. The study was conducted in the gynecology oncology ward and the gynecology oncology clinic of Dr. Moewardi Hospital Surakarta and Prodia Laboratory, from January - March 2015. Cortisol level of the treatment group was 1.03 ± 0.71 mg / dL, and the control group was 11.41 ± 7.34 mg / dL. Pain score in the treatment group was 4.46 ± 0.83, and the control group was 7.34 ± 0.74. There are significant differences in serum cortisol level decrease (p = 0.00) and pain score (p = 0.00) between the CBT intervention with standard therapy group compared and the standard therapy only group

Abstract

Preeclampsia is one of the main contributors on morbidity and mortality of mother and fetus. Imbalance of body antioxidants and oxidants has a role on endothelial destruction causing endothelial dysfunction which leads to preeclampsia. L-arginine administration is indicated to be able to fix the remodelling process of hepatocytes which was damaged on preeclampsia. The aim of this study was to investigate the effect of L-arginine on hepatocyte cells damage on mouse (Mus musculus) model of preeclampsia. Experimental study was conducted in the Experimental Animal Warehouse and Biomedical Laboratory, Faculty of Veterinary Medicine, Airlangga University, Surabaya. A total of 30 pregnant mice were divided into three groups with 10 mice in each group i.e. normal, preeclampsia model and preeclampsia model with L-arginine therapy pregnant mice. Preparates were taken from dissected mice liver, with prior paraffin block and were then stained with hematoxylin-eosin. Results were analyzed semi quantitatively with Manja Roenigk hepar histology scoring method. Data were analyzed using Kruskal-Wallis and Man-Whitney test. Mean of hepar histopathology score of preeclampsia model with L-arginine therapy (40.80±1.17/µm2) was significantly lower compared with preeclampsia model pregnant mice (58.80±4.23/µm2) (p<0.05), however it was significantly higher compared with normal pregnant mice (18.40±2.41/µm2) (p<0.05). It was indicated that the L-arginine therapy on preeclampsia mouse model can reduce the score of hepar histopathology. In conclusion, L-arginine administration can reduce the hepatocyte cells damage of mouse due to preeclampsia.

Abstract

Preeklampsia masih merupakan penyebab utama morbiditas dan mortalitas maternal maupun perinatal. Pada preeklampsia terjadi gangguan ginjal berupa endoteliosis glomerular. Ketidakseimbangan faktor proangiogenik VEGF dan antiangiogenik sFlt-1 diduga terlibat dalam patogenesis preeklampsia. Pemberian VEGF 121 rekombinan memiliki kemampuan untuk memperbaiki fungsi endotel sehingga dapat memperbaiki endoteliosis glomerular. Penelitian ini bertujuan untuk membuktikan pengaruh VEGF 121 rekombinan terhadap perbaikan endoteliosis glomerular pada mencit model preeklampsia menggunakan metode eksperimental analitik dengan RCT, yang dilakukan pada bulan Januari – Mei 2015 di Kandang Hewan Percobaan dan Laboratorium Biomedik Fakultas Kedokteran Hewan Universitas Airlangga. Sampel dari replikasi Steel dan Torrie adalah 30 mencit bunting yang terbagi dalam 3 kelompok yaitu 10 mencit bunting normal, 10 mencit bunting model preeklampsia dan 10 mencit bunting model preeklampsia dengan terapi VEGF 121 rekombinan. Semua sampel dilakukan pemeriksaan pada endotel ginjalnya dengan metode imunohistokimia. Parameter endoteliosis glomerular menggunakan kriteria Stillman (2007) dan Wide-Swensson (2007). Analisa data menggunakan uji Kruskal Wallis dan Mann Whitney. Rerata endoteliosis gromeruler pada mencit bunting normal 1,60 dan mencit bunting model preeklampsia 1,90 dengan nilai p=0,28 (p>0,05). Rerata endoteliosis gromeruler pada mencit bunting model preeklampsia 1,90 dan mencit bunting model preeklampsia dengan terapi VEGF rekombinan 121 adalah 1,30 dengan nilai p=0,02 (p<0,05). Rerata endoteliosis gromeruler pada mencit bunting normal 1,60 dan mencit bunting model preeklampsia dengan terapi VEGF rekombinan 121 adalah 1,30 dengan nilai p=0,28 (p>0,05). Simpulan, VEGF 121 rekombinan dapat memperbaiki endoteliosis gromerular pada mencit bunting model preeeklampsia.

Abstract

Objectives: This study aims to demonstrate the effect of L-Arginine on pulmonary edema on mice preeclampsia model.Materials and Methods: This experimental analytic study was conducted on 30 pregnant mice divided into 3 groups, namely 10 normal pregnant mice (K1), 10 pregnant mice preeclampsia model (K2), and 10 pregnant mice pre-eclampsia model who received L-Arginine (K3) therapy. All subjects examined the thickness of the alveoli capillary wall in their pulmo using the Immunohisto-chemical method. The results were analyzed using Kruskal Wallis and Mann Whiteney tests.Results:  Average alveoli capillary wall thickness (K1) 5.01±1.76/µm, (K2) 11.57±7.11/µm, with p value=0.00. Average alveoli capillary wall thickness (K3) 7.90±4.10/µm with p value=0.01.Conclusion: L-arginine affects to improve pulmonary edema in preeclampsia by reducing alveoli capillary wall thickness of the pulmonary in mice model.

Abstract

ABSTRACT Introduction: Cervical cancer is cancer occurring in the cervix uteri, which is mostly caused by an  infection by human papillomavirus (HPV). Women  diagnosed with advanced cervical cancer suffer emotional stress that can  lead to depression. This condition causes decreasing quality of life, decreasing level of serotonin, and an increase in depression score. Giving reality therapy to patients is expected to have  positive effects. Methods: The  research was con- ducted through  an experiment with pre-test and post-test design. The research samples were15 subjects taken through consecutive sampling from the Polyclinic of Obstetrics and Gynecol- ogy of Dr. Moewardi Hospital Surakarta starting in March 2015. The analysis of serotonin level was conducted in Prodia Laboratory. The experimental data  was analyzed using a t-test (α =0.05).  Results: The average level of serotonin of the research subjects after receiving reality therapy was higher (223.59 + 41.20)  compared to that  before the  therapy (82.77  + 27.02). From  the t-test analysis with p = 0.00,  it was found that  the  average depression score after receiving  reality therapy is lower (11:40  + 4.80)  compared that  before the  therapy (17:33  +5:52).  Conclusion: There  was a significant increase in the  serotonin level and  a significant decrease in the  depression score as the  effect  of reality  therapy given  to patients with ad- vanced cervical cancer. Keywords: Serotonin, depression score, reality therapy, advanced cervical cancer
